© Copyright 2021 by Gazi University Medical FacultyObjective: The aim of the study is to compare the differences between COVID-19 pneumonia and other viral pneumonia (OVP) in terms of demographic, clinical and radiological features. Methods: This retrospective cohort study was conducted in Gazi University Hospital between 11 March and 24 May 2020. Patients, admitted to the hospital with suspected COVID-19 infection aged >18 years and those who had pneumonia on chest computed tomography (CT) scan were evaluated. SARSCoV-2 RT-PCR and multiplex PCR, for other respiratory viruses, were performed. Patients with a positive SARS-CoV-2 PCR were included in “COVID-19 pneumonia” group and those who had a positive result for any other respiratory viruses and two consecutive negative results for SARS-CoV-2 were included in the “OVP” group. Two groups were compared in terms of clinical, laboratory and chest CT findings. Results: Of the 63 patients included in the study, 45 had COVID-19 pneumonia and 18 had OVP. Çalışmamızın amacı, mide tümörü tanısı ile BT çekilmiş hastaların görüntülerini tekstür analizi yöntemiyle inceleyerek, bu yöntemin ayırıcı tanıdaki yerini araştırmaktır.Çalışmamıza Ocak 2014 ile Ağustos 2019 tarihleri arasında mide tümörü ön tanısıyla BT çekilen 2038 hasta dahil edilmiş olup, hasta kayıtları ve görüntüleri retrospektif olarak değerlendirilmiştir. Dosya kayıtları veya patoloji sonuçları eksik olan, görüntüleme öncesinde cerrahi veya medikal tedavi görmüş hastalar, görüntü özellikleri inceleme için elverişli bulunmayan hastalar çalışma dışı bırakılmıştır. Çalışmaya dahil edilen 204 hastanın görüntüleri, açık kaynak kodlu LIFEx yazılımına aktarılıp, piksel boyutu ayarlanması, gri seviye ayrıklaştırması ve gri seviye normalizasyonu işlemleri uygulanmıştır. Ardından lezyonun en geniş olduğu aksiyel kesitten ROI belirlenerek, bu ROI üzerinden her hasta için 40 adet tekstür özelliği hesaplanmıştır. İstatistiksel analiz aşamasında yüksek korelasyon gösteren tekstür özelliklerinden sadece biri kullanılacak şekilde eleme yapılmıştır. Ardından tümör alt tipleri tekstür özellikleri açısından karşılaştırılarak, anlamlı farklılık gösteren özelliklerden lojistik regresyon analiziyle birleştirilmiş parametre elde edilmiştir. Bu özelliklerin ve birleştirilmiş parametrenin tümör alt tiplerini ayırt etmedeki duyarlılığı ve özgüllüğü ROC analizi ile ölçülmüştür.62Çalışmamızda tüm tümör alt tiplerini başarılı şekilde ayıran bir parametre bulunamamasına rağmen, ikili gruplar birbiriyle karşılaştırıldığında adenokanser-lenfoma, adenokanser-GIST ve iyi diferansiye adenokanser-az diferansiye adenokanser arasında yüksek duyarlılık (sırasıyla %98, %91, %96) ve özgüllükle (sırasıyla %75, %77, %80) ayrım yapılabilmiştir.Tekstür analizi halen gelişmekte olan bir yöntem olup, teknik uygulamaları konusunda henüz bir fikir birliği mevcut değildir. Ancak bu araştırma ve son dönemde yayınlanmış birçok çalışmada ayırıcı tanı, prognoz gibi konularda umut vaat edici sonuçlar raporlanmıştır. The aim of this study was to examine the CT images of patients with a diagnosis of gastric tumor by texture analysis and to investigate its place in differential diagnosis.Between January 2014 and August 2019, 2038 patients who had undergone CT imaging with a pre-diagnosis of gastric tumor were included in the study. Patient records and images were evaluated retrospectively. Patients with missing file or pathology records, who had undergone surgical or medical treatment prior to imaging, and those whose CT images were not suitable for analysis were excluded. Images of 204 patients included in the study were transferred to the open source LIFEx software and pixel size adjustment, gray level discretization and gray level normalization procedures were applied. Then, ROI was determined from the axial slice where the lesion was the widest and a total of 40 texture features were calculated for each patient. In the statistical analysis, only one of the highly correlated texture features was selected, and these features were compared between the tumor subtypes. Subsequently, a combined parameter was obtained by logistic regression analysis using the texture features that showed significant differences between the tumor groups. The sensitivity and specificity of these features and the combined parameter to differentiate tumor subtypes were measured by ROC analysis.64Although we could not find a parameter that successfully differentiated all tumor subtypes in our study, pairwise comparisons yielded better results. Adenocarcinoma vs. lymphoma, adenocarcinoma vs. GIST and well-differentiated adenocarcinoma vs. poorly differentiated adenocarcinoma could be distinguished by high sensitivity (%98, %91, %96 respectively) and specificity (%75, %77, %80 respectively).Texture analysis is still a developing method and there is no consensus on its technical procedures. However, this study as well as some recent publications reported promising results in differential diagnosis and prognosis.
