Abstract

Aim: To investigate the prevalence of gingivitis and periodontitis, and the oral hygiene status of adults with cystic fibrosis (CF) in the Republic of Ireland. Materials and Methods: A case–control study in the form of a clinical examination of 92 adults with a diagnosis of CF was carried out in the adult CF unit in Cork University Hospital. A 40‐item questionnaire was used to capture socio‐demographic variables and medical and dental information. Two calibrated examiners carried out a periodontal assessment on participants, using the WHO‐recommended CPI‐modified index, and oral hygiene status was measured using the Greene–Vermillion index. The results were compared with a population‐based control group of similar socio‐demographic profile. Results: Oral hygiene levels (plaque and calculus) were significantly worse in people with CF, with a median plaque index of 0.83 (interquartile range [IQR] 0.333–1.542) in the CF group compared with 0.5 (IQR 0.167–0.667) in the non‐CF group. Calculus index in the CF group was 0.33 (IQR 0.17–0.83) compared with 0.33 (IQR 0.125–0.33) in the non‐CF group. However, periodontal disease levels were significantly lower in the CF group. Gingivitis (bleeding on probing ≥ 10% sites) was seen in 67.4% of the CF group, compared with 83.7% of the non‐CF group, OR 0.365 (95% confidence interval [CI] 0.181–0.736), relative risk (RR) 0.779 (95% CI 0.655–0.928). Mild periodontitis (periodontal probing depth [PPD] < 5 mm) was seen in 15.2% of the CF group, compared with 31.5% of the non‐CF group, OR 0.390 (CI 0.190–0.800), RR 0.483 (95% CI 0.273–0.852). Severe periodontitis (PPD ≥ 6 mm) was seen in 0% of the CF group, compared with 9.8% of the non‐CF group. There was a tendency, albeit non‐significant, towards reduced periodontitis in PWCF who regularly took antibiotics, particularly azithromycin. Conclusions: In this study, adults with CF had poor oral hygiene practices, with high levels of plaque and calculus. Despite this finding, adults with CF had lower levels of clinical gingivitis and periodontitis than seen in a non‐CF control group. Further study is required to examine the causes of this phenomenon. 17. Building an evidence base towards sustainable diets in the Irish population

Abstract

Background: Climate change, food demand and diet-related disease underline an urgent need for more sustainable and healthy food practices. Dietary guidance in the form of Sustainable Food Based Dietary Guidelines have been recommended at a national level to mitigate the environmental impact of food consumption, while improving health. For the Irish population to transition to sustainable diets, an improved knowledge base is required on the quality of current dietary patterns and the environmental impact of such. Objective: This thesis was designed to analyse diets reported in the Irish Population from a nutrition, environment, and cost perspective, and to complete preliminary modelling of healthier and more sustainable diets for sub-groups of the population. Methods: Food intakes on children, teenagers, and adults were taken from IUNA food consumption databases (FCDB). Nutrient based diet quality scores were calculated and evaluated as a metric of diet quality. Environmental data on greenhouse gas emissions and water use were taken from a UK life cycle assessment study, and agricultural impacts (cropland, nitrogen, and phosphorous use) from the FAOSTAT database to adults. A food price database was compiled, and dietary optimisation was completed using FICO Xpress software. Results: An evidence base was created by assigning three nutrient based scoring systems and linking data on environmental factors and food price to the FCDBs. In the nutritional analysis, teenagers and young adult females had significantly lower nutritional adequacy than the rest of the Irish population (58% probability of meeting nutrient recommendations). Lower nutritional adequacy was highlighted in females, those with a lower education level, and those from a more urban location. Abstract

Background Interactive health literacy (HL) skills enable individuals to participate more fully in healthcare activities and play a role in improving their outcomes. We examine the associations between HL and cystic fibrosis (CF) outcomes and compare HL in a sample from both the Irish CF and general populations. Methods A total of251 CF Registry participants aged 13–30 years completed the HLS-EU-Q16 survey and a disease-specific instrument for measuring quality of life (QoL) in CF. Health outcome and healthcare resource utilization (HCRU) data were sourced from the registry. CF patient outcomes were examined using generalized linear models (GLMs) with interactive HL categorization included as a factor. General population interactive HL data are from the 2011 European HL Survey (HLS-EU). Interactive HL in 180 age-sex matched CF and general population individuals was examined using a GLM with study population, sex and educational level included as factors and age as a covariate. Results Sufficient interactive HL (total sum score ≥13) was self-reported by 81.7% of individuals with CF. Sufficient HL was associated with fewer outpatient visits [7.02(SD = 6.7: 7.4) vs. 8.74(SD = 7.9: 9.6), P < 0.001], days hospitalized [10.25(SD = 9.8: 10.7) vs. 12.8(SD = 11.8: 13.9), P < 0.001], days on intravenous antibiotics [15.3(SD = 14.7: 15.8) vs. 19.7(18.5: 21.1), P < 0.001], days on oral antibiotics [27.4(SD = 26.7: 28.1) vs. 48.48(38.7: 42.4), P < 0.001] and better QoL [77.1(SD = 75.4–78.9) vs. 64.6(60.8–68.3), P < 0.001]. Mean HL scores in CF and general populations were sufficient, although higher among individuals with CF (14.3 vs. 13.1, P < 0.01). Abstract

peer-reviewed 1. A co-ordinated continental-scale field experiment across 31 sites was used to compare the biomass yield of monocultures and four-species mixtures associated with intensively managed agricultural grassland systems. To increase complementarity in resource use, each of the four species in the experimental design represented a distinct functional type derived from two levels of each of two functional traits, nitrogen acquisition (N2-fixing legume or non-fixing grass) crossed with temporal development (fast-establishing or temporally persistent). Relative abundances of the four functional types in mixtures were systematically varied at sowing to vary the evenness of the same four species in mixture communities at each site, and sown at two levels of seed density. 2. Across multiple years, the total yield (including weed biomass) of the mixtures exceeded that of the average monoculture in >97% of comparisons. It also exceeded that of the best monoculture (transgressive overyielding) in about 60% of sites, with a mean yield ratio of mixture to best-performing monoculture of 1.07 across all sites. Analyses based on yield of sown species only (excluding weed biomass) demonstrated considerably greater transgressive overyielding (significant at about 70% of sites, ratio of mixture to best-performing monoculture = 1.18). 3. Abstract

To estimate biodiversity it may be important to establish whether increased data can be obtained from repeated surveys. Bird species richness was determined after repeated surveys in winter and in the breeding season. Data were collected over two breeding seasons and two winter seasons at nine sites, with three surveys in each winter and four surveys in each breeding season using a standard method. Poisson models were used to determine differences in total cumulative species richness recorded after each additional survey. There were significant increases in species richness in winter between survey one and survey two and between survey two and survey three across nine sites. There was a significant increase between survey one and survey two for resident breeding species richness. Based on pure species richness, three surveys recorded at least 95% of the total observed species richness recorded after four surveys in the breeding season within the agricultural survey area. ObjectiveNo up-to-date data on the dietary intake of Irish adolescents are available. The aim of the present pilot study was to obtain and compare cross-sectional information on habitual adolescent beverage consumption between four distinct post-primary schools in the Republic of Ireland, in 2014–2015.DesignA cross-sectional observation study. A beverage consumption questionnaire was used to obtain data on beverage intake and influences on consumption.SettingFour post-primary mixed-sex schools in Ireland representing the following school classifications were selected for the study: urban fee-paying, urban disadvantaged, rural fee-paying and rural disadvantaged.SubjectsStudents (n 761) aged 12–18 years.ResultsData were analysed by Kruskal–Wallis (non-parametric) ANOVA to compare the distribution of beverage consumption across the schools. Water was the most highly consumed beverage among students from all four schools (median 1425 ml/d). Students from urban and rural disadvantaged schools reported a significantly higher volume of carbonated beverage intake than students from fee-paying schools. Students from an urban disadvantaged school also reported a significantly higher volume of carbonated beverage and energy drink intake compared with the other three schools. Students from an urban fee-paying school reported the highest consumption of water, while rural disadvantaged school students were the biggest consumers of tea and milk.ConclusionsSignificant differences in beverage consumption (ml/d) were reported by adolescents from four schools in Ireland. Abstract

Field boundaries are important habitats for birds within the agricultural landscape. In this study, bird surveys were carried out during the winter and breeding season on nine farms in the east and south-east of Ireland and field boundaries at each site were assessed using the Field Boundary Evaluation and Grading System (FBEGS). Poisson regression demonstrated that FBEGS scores were a good predictor of both winter and breeding bird species richness and diversity within the field boundaries studied. We interpret these preliminary results with caution since our sample size was relatively small (compared to the wide variety of field boundary types found in Ireland) and no single combination of field boundary attributes is likely to be optimum for all bird species. However, our results suggest that FBEGS may be a useful surrogate indicator of overall field boundary bird diversity and we discuss the consequent implications for agri-environmental policy, and for the possible adaptation and use of FBEGS as a tool to monitor the impact of changing farm management practice.
