Purpose This study compares doctor staffing level and the scale of medical education in China with those of other countries and proposes policy recommendations for future adjustments to the scale of China's medical education. Design/Approach/Methods This study employs a literature review and descriptive analysis. Findings China had 1.98 medical doctors per 1,000 people in 2018, ranking 85th out of the 193 member-states of the World Health Organization (WHO). In 2017, China had 1.99 practicing doctors per 1,000 people, only ranking above Turkey (1.88) in Organisation for Economic Co-operation and Development (OECD) countries. China had only 10.28 medical graduates per 100,000 people—placing in the bottom third of OECD countries. China's provision of 1.4 medical schools per 10 million people was also significantly lower than the global average (3.9). However, the average number of students enrolled in medical schools (509) in China was significantly higher than the global average (160). Originality/Value Although the scale of admission in undergraduate medical education must be expanded in China, this needs to be achieved while controlling the average number of medical students per school and reducing enrollment in low-quality medical schools. Furthermore, it is necessary to establish new medical schools while improving the operating level of existing ones.

Linear oscillatory machines are becoming more widely used in refrigeration compressors due to advantages of compact volume, little maintenance, and so on. In order to further increase the efficiency and reduce the cost of the whole system, both resonant frequency tracking control and sensorless control strategies should be implemented simultaneously. In this article, an adaptive full-order observer with simple structure and less computation is proposed, which can not only provide accurate displacement estimation signals for the system piston stroke control, but also achieve resonant frequency tracking control through the estimation of machine parameters. The system stability analysis proves the quick convergence of the proposed observer, and comprehensive simulation and experimental results demonstrate the effectiveness of the proposed algorithm.

In order to solve the constrained-input problem and reduce the computing resources, a novel event-triggered optimal control method is proposed for a class of discrete-time nonlinear systems. In the proposed method, the event-triggered control policy is applied to the globalized dual heuristic dynamic programming (GDHP) algorithm. Compared with the traditional adaptive dynamic programming (ADP) control, the event-triggered GDHP control can reduce the computation while ensuring the system performance. In this paper, a non-quadratic function is given to code the control constraints and the trigger condition with the stability analysis is provided. Neural networks (NN) are constructed in the GDHP structure, where the model network is designed to identify the unknown nonlinear system, the critic network is used to learn the cost function and its partial derivative, and the action network is designed to obtain the approximate optimal control law. Three simulation examples are presented to demonstrate the performance of the proposed event-triggered design for constrained discrete-time nonlinear systems.

For the linear compressor system driven by linear oscillatory machine (LOM), the piston stroke signal is very important to realize the efficient control of the motor. Since it is difficult to install a position sensor in the LOM, and the position sensor will increase the difficulty of system control, it is necessary to use an accurate and efficient position sensor-less method to estimate the piston stroke signal. In this paper, a novel position sensor-less piston stroke estimation method based on sliding-mode observer (SMO) is proposed. Besides, a self-adaptive band-pass filter (SABPF) is designed to filter the estimated piston stroke signal, which not only overcomes the pure integration problem, but also solves the chattering problem of SMO. Simulation results show that the proposed SMO has a good performance in estimation accuracy, and the chattering problem is well solved.

Global DNA hypomethylation is commonly observed in benzene-exposed workers, but the underlying mechanisms remain unclear. We sought to discover the relationships among reduced white blood cell (WBC) counts, micronuclear (MN) frequency, and global DNA methylation to determine whether there were associations with mutations in DNMT3A/3B. Therefore, we recruited 410 shoe factory workers and 102 controls from Wenzhou in Zhenjiang Province. A Methylated DNA Quantification Kit was used to quantify global DNA methylation, and single nucleotide polymorphisms (SNPs) in DNMT3A (rs36012910, rs1550117, and R882) and DNMT3B (rs1569686, rs2424909, and rs2424913) were identified using the restriction fragment length polymorphism method. A multilinear regression analysis demonstrated that the benzene-exposed workers experienced significant global DNA hypomethylation compared with the controls (β = -0.51, 95% CI: -0.69 to -0.32, P < 0.001). The DNMT3A R882 mutant allele (R882H and R882C) (β = -0.25, 95% CI: -0.54 to 0.04, P = 0.094) and the DNMT3B rs2424909 GG allele (β = -0.37, 95% CI: -0.70 to -0.03, P = 0.031) were significantly associated with global DNA hypomethylation compared with the wild-type genotype after adjusting for confounding factors. Furthermore, the MN frequency in the R882 mutant allele (R882H and R882C) (FR = 1.18, 95% CI: 0.99 to 1.40, P = 0.054) was higher than that of the wild-type. The results imply that hypomethylation occurs due to benzene exposure and that mutations in DNMTs are significantly associated with global DNA methylation, which might have influenced the induction of MN following exposure to benzene. A Cu-0.7Fe-0.15P (wt.%) alloy was designed, and its comprehensive properties, especially electrical conductivity and temper-softening resistance of the designed alloy, were higher than those of traditional Cu-Fe-P alloys. The microstructure of this alloy was investigated with optical microscopy, scanning electron microscopy, and transmission electron microscopy. The particle of secondary phase was confirmed to be Fe2P with x-ray spectroscopy and digital diffractogram. By virtue of precipitation hardening and work hardening, the tensile strength and electrical conductivity of Cu-0.7Fe-0.15P alloy were 498 MPa and 62% IACS, respectively. The electrical conductivity of this alloy can be up to 92% IACS due to the complete precipitation of Fe2P. After repeating the cold rolling and aging process for three times, the tensile strength, elongation, and conductivity of this alloy were 467 MPa, 22%, and 78% IACS, respectively. Due to the low driving force of recrystallization and the pinning effect of fine dispersed Fe2P, the alloy with low deformation rate showed excellent softening resistance. The designed alloy can be used as a high-strength, high-electrical-conductivity lead-frame alloy.

Currently, there are many 3D modeling softwares, like 3DMAX, AUTOCAD, and more populous BIM softwares represented by REVIT. CityEngine modeling software introduced in this paper can fully utilize the existing GIS data and combine other built models to make 3D modeling on internal and external part of buildings in a rapid and batch manner, so as to improve the 3D modeling efficiency.

BACKGROUND The majority of human cases of novel avian influenza A(H7N9), which emerged in China in spring 2013, include reported exposure to poultry. However, specific host and exposure risk factors for disease are unknown, yet critical to design prevention measures. METHODS In April-June 2013, we conducted a case-control study in 8 Chinese provinces. Patients with laboratory-confirmed A(H7N9) (n = 89) were matched by age, sex, and neighborhood to controls (n = 339). Subjects completed a questionnaire on medical history and potential exposures, including poultry markets and other poultry exposure. We used conditional logistic regression to calculate matched and adjusted odds ratios (ORs) for the association of A(H7N9) virus infection with potential risk factors. RESULTS Fifty-five percent of patients compared with 31% of controls reported any contact with poultry (matched OR [mOR], 7.8; 95% confidence interval [CI], 3.3-18.8). Sixty-seven percent of patients compared with 35% of controls visited a live poultry market (mOR, 5.4; CI, 3.0-9.7). Visiting live poultry markets increased risk of infection even after adjusting for poultry contact and other confounders (adjusted OR, 3.4; CI, 1.8-6.7). Backyard poultry were not associated with increased risk; 14% of cases did not report any poultry exposure or market visit. Obesity (mOR, 4.7; CI, 1.8-12.4), chronic obstructive pulmonary disease (mOR, 2.7; CI, 1.1-6.9), and immunosuppressive medications (mOR, 9.0; CI, 1.7-47.2) were associated with A(H7N9) disease. CONCLUSION Exposures to poultry in markets were associated with A(H7N9) virus infection, even without poultry contact. China should consider permanently closing live poultry markets or aggressively pursuing control measures to prevent spread of this emerging pathogen.

In mainland China, most avian influenza A(H7N9) cases in the spring of 2013 were reported through the pneumonia of unknown etiology (PUE) surveillance system. To understand the role of possible underreporting and surveillance bias in assessing the epidemiology of subtype H7N9 cases and the effect of live-poultry market closures, we examined all PUE cases reported from 2004 through May 3, 2013. Historically, the PUE system was underused, reporting was inconsistent, and PUE reporting was biased toward A(H7N9)-affected provinces, with sparse data from unaffected provinces; however, we found no evidence that the older ages of persons with A(H7N9) resulted from surveillance bias.
