Background Atrioventricular block (AVB) is associated with adverse outcomes in patients with cardiovascular diseases, and there is a relative scarcity of epidemiological data in China. Identifying and eliminating risk factors for AVB to prevent its occurrence is of great importance. Objective To explore the prevalence and risk factors of AVB in the Kailuan study population. Methods Participants who underwent health examinations from 2018 to 2020, consisting of employees and retirees of the Kailuan Group, were included as subjects of observation. General information, medical history, and medication history of the participants were collected. On the day of the physical examination, 5 mL of blood was drawn from the elbow vein after fasting for 8 hours for biochemical tests, and an electrocardiogram was conducted simultaneously. Multivariate Logistic regression analysis was used to explore the risk factors affecting AVB and to perform sensitivity analysis. Results A total of 80 859 subjects were included in the final observation, with an average age of (53.8±14.9) years. The prevalence of AVB was 0.97% (786/80 859), with a first-degree AVB prevalence of 0.91% (732/80 859) and a second-degree or higher AVB prevalence of 0.07% (54/80 859). The proportion of male, age, systolic pressure, fasting blood glucose, low-density lipoprotein cholesterol, BMI, hypertension, diabetes, alcohol consumption, blood uric acid, medication use, and atherosclerotic cardiovascular diseases (ASCVD) was higher in AVB patients than in those without AVB, while diastolic pressure and heart rate was lower (P

ObjectiveWe aimed to characterize the relationship of a combination of circulating non-high-density lipoprotein-cholesterol (non-HDL-C) concentration and brachial-ankle pulse wave velocity (baPWV) with cardiovascular disease (CVD).MethodsWe performed a prospective cohort study of the residents of the Kailuan community, with data from a total of 45,051 participants being included in the final analysis. The participants were allocated to four groups according to their non-HDL-C and baPWV status, each of which was categorized as high or normal. Cox proportional hazards models were used to explore the relationships of non-HDL-C and baPWV, individually and in combination, with the incidence of CVD.ResultsDuring the 5.04-year follow-up period, 830 participants developed CVD. Compared with the Normal non-HDL-C group independently, the multivariable adjusted hazard ratios (HRs) and 95% confidence intervals (CIs) for CVD in the High non-HDL-C was 1.25 (1.08–1.46). Compared with the Normal baPWV group independently, the HRs and 95% CIs for CVD in the High baPWV was 1.51 (1.29–1.76). In addition, compared with the Normal both non-HDL-C and baPWV group, the HRs and 95% CIs for CVD in the High non-HDL-C and normal baPWV, Normal non-HDL-C and high baPWV, and High both non-HDL-C and baPWV groups were 1.40 (1.07–1.82), 1.56 (1.30–1.88), and 1.89 (1.53–2.35), respectively.ConclusionHigh non-HDL-C concentration and high baPWV are independently associated with a higher risk of CVD, and individuals with high both non-HDL-C and baPWV are at a still higher risk of CVD.

The association of atherosclerotic cardiovascular disease (ASCVD) with cancer occurrence is not well examined, and the impact of common risk factors on the risk of cancer in ASCVD patients is not known. This study aimed to explore the effect and possible causes of ASCVD on cancer risk through a cohort study. A total of 14,665 age- and sex-matched pairs of participants were recruited from the Kailuan cohort (ASCVD vs non-ASCVD). A competing risk model was used to calculate the risk of cancer after ASCVD. A total of 1124 cancers occurred after 5.80 (3.05-9.44) years of follow-up. The ASCVD group had a reduced risk of cancer (hazard ratio 0.74; 95% confidence interval, 0.65-0.85). Also, the risk of cancer in the digestive system, respiratory system, urinary system, and reproductive system was reduced by 17%, 16%, 14%, and 52%, respectively. According to the status of systolic and diastolic blood pressure, fasting blood glucose, high-sensitivity C-reactive protein and body mass index after ASCVD, the risk of overall cancer and digestive system cancer decreased with the increase in the number of ideal indicators (P for trend <.01). With the increase of follow-up time, the risk of cancer and the 5 site-specific cancers gradually decreased. Cancer risk can be reduced by controlling for common risk factors after ASCVD event.
