Background/Objectives: The evaluation of the differences in corneal epithelial thickness profiles in healthy eyes and eyes with mild and moderate stages of keratoconus, using optical coherence tomography (OCT). Methods: Fifty-two healthy eyes (group 0), forty-one eyes with mild keratoconus (group I), and thirty eyes with moderate keratoconus (group II) were included in this study. Only one of the patient's eyes was enrolled, and they were divided into groups using the Amsler–Krumeich (A–K) classification—stage I and II. All patients underwent a visual acuity assessment, slit-lamp examination, corneal tomography, and automatic mapping of corneal thickness and epithelial thickness on a diameter of 9 mm. Corneal tomography with a Placido/Scheimpflug instrument (Sirius, CSO, Florence, Italy) and OCT with a corneal adaptor module (Avanti RTVue XR, Optovue, Lombard, IL, USA) were used. Results: Minimum corneal epithelium thickness was 49.5, 43, and 40 µm in groups 0, I, and II, respectively (Kruskal–Wallis test, p < 0.001). A moderate correlation was found between minimum epithelial thickness and the apex curvature (Pearsons's coefficient r = −0.62, p < 0.001) and posterior radius of central corneal curvature (Pearsons's coefficient r = 0.62, p < 0.001). The difference between minimum and maximum epithelial thickness showed a high correlation (r = −0.770, p < 0.001). In groups I and II, on corneal epithelial thickness maps the thinnest sector, located inferiorly and temporally to the center, was surrounded by sectors with increased thickness. Conclusions: At the apex of the cone, the corneal epithelium becomes thinner, and a thicker ring forms around the cone. Although there is a moderate-to-strong correlation to parameters linked with the severity of keratoconus, and minimum epithelial thickness as well as the minimum–maximum difference, it is not possible to establish cut-off values for stages I and II in the Amsler–Krumeich (A–K) classification. Introduction: Glaucoma is a leading cause of blindness, often progressing asymptomatically until significant vision loss occurs. Early detection is crucial for preventing irreversible damage. The pupillary light reflex (PLR) has proven useful in glaucoma diagnosis, and mobile technologies like the AI-based smartphone pupillometer (AI Pupillometer) offer a promising solution for accessible screening. This study assesses the reliability of the AI Pupillometer in detecting glaucoma. Methods: In Experiment 1, 20 healthy participants were assessed using both the AI Pupillometer and the NPi-200 device to evaluate equivalence in measuring PLR. Each eye underwent three trials. Experiment 2 included 46 participants, 24 with primary open-angle glaucoma (POAG) and 22 healthy controls. PLR measurements from the AI Pupillometer were correlated with structural and functional ocular parameters. An additional study expanded the sample to 387 participants (103 glaucoma patients, 284 controls), focusing on differential pupillometry parameters to minimize ambient light interference. Results: In Experiment 1, the AI Pupillometer demonstrated strong correlations with the NPi-200 in key parameters like initial pupil size (r = 0.700), constricted pupil size (r = 0.755), and constriction velocity (r = 0.541), confirming its reliability. In Experiment 2, although no statistically significant differences in light-corrected PLR parameters were found between groups, glaucoma patients had a marginally higher constricted pupil size (p = 0.1632). Significant correlations were observed between pupillometry and advanced ocular imaging results, notably between constriction amplitude and visual field loss. The additional study revealed significant differences in constriction amplitude (p = 0.014) and relative pupil size change (p = 0.0072) between glaucoma patients and controls, reinforcing the AI Pupillometer's diagnostic potential. Conclusion: This study confirms the AI Pupillometer as a reliable, accessible tool for glaucoma screening. Mobile diagnostics could enhance early detection, improving outcomes for glaucoma patients. Aim: To compare total corneal power (TCP) values obtained from optical coherence tomography with conventional keratometry in intraocular lens (IOL) power calculation methods in corneas after laser correction of myopia. Material and methods: The retrospective analysis included 64 eyes of patients after laser vision correction (LVC) of myopia. The results of optical coherence tomography of the anterior segment of the eye (Avanti RTVue XR) - central corneal thickness (CCT) and TCP - parameters from the optical biometry of IOLMaster 500 and the corneal tomography of Sirius were analyzed. Based on the obtained measurements, the IOL power was calculated using Haigis-L, Barrett True K No History and OCT formulas as well as the average from the ASCRS IOL Calculator for Eyes with Prior Myopic LASIK/PRK. Results: The average spherical equivalent value before LVC was -3.99 ±2.13 D. The average decrease in mean keratometry after keratorefractive surgery measured with IOLMaster 500 and Sirius was 3.34 ±1.89 D and 3.14 ±1.83 D, respectively. Mean difference between TCP after LVC and keratometry measured by two conventional keratometers was 1.344 ±0.5 D for IOLMaster 500 and 1.267 ±0.82 D for Sirius. We did not observe a statistically significant difference in the IOL power using different calculation formulas for post-myopic eyes, but we found the highest agreement between the OCT formula and ASCRS average. Conclusions: Optical coherence tomography of the anterior segment of the eye (Avanti RTVue XR) allows measurement of the TCP in patients after LVC, which makes the calculation of the IOL power in these patients more accurate. Aim To evaluate refractive and visual outcomes of photorefractive keratectomy (PRK) to treat high hyperopia using an aberration-neutral profile and large ablation zone. Methods This was a retrospective, consecutive observational case series at the Oftalmika Eye Hospital, Bydgoszcz, Poland. We included 51 consecutive eyes of 34 patients who underwent alcohol-assisted PRK to correct hyperopia within the range of +3.6 to +6.15 D (mean+4.61±0.67 D). Procedures were performed with an Amaris 750S excimer laser (Schwind eye-techsolutions GmbH, Kleinostheim, Germany) using an aberration-neutral profile and a 10 mm total ablation zone. Refractive results, predictability, safety and efficacy were evaluated 3 years postoperatively. Results At 1-year postsurgery, the mean manifest refraction spherical equivalent (MRSE) was .0.002±0.43 D and mean cylinder was .0.181±0.31 D, while the values were +0.09±0.46 D and .0.15±0.26 D, respectively, at 2 years (MRSE p<0.001) and +0.15±0.44 D and .0.15±0.26 D, respectively, at 3 years (MRSE p<0.001). 78% of eyes were within ±0.50 D of the attempted spherical equivalent correction. Three years postoperatively, 22% of eyes lost one line of corrected distance visual acuity and 27% gained a line or two. The change in the mean corneal spherical aberrations for the 6 mm zone was from 0.27±0.07 to 0.08±0.13 ìm. Conclusions High hyperopia correction with PRK using an aberration-neutral profile and large ablation zone provides good efficacy, safety, predictability and visual outcomes. Relatively low change of corneal spherical aberrations and low increase of hyperopia in the first three postoperative years were observed. [ABSTRACT FROM AUTHOR]

Background: To evaluate functional and anatomical outcomesof cataract surgery in neovascular age-related macular neovascularization (nAMD) eyes receiving anti-vascular endothelial growth factor (anti-VEGF) intravitreal injections in modified pro re nata (PRN) regimen. Materials and Methods: Sixty eyes of 60 nAMD patients, including 41 women (68.3%) and 19 men (31.7%) in an average age of 77.35 ± 6.41 years, under treatment with intravitreal aflibercept injections in modified PRN regimen with no signs of macular neovascularization (MNV) activity during two consecutive visits were included in this prospective, observational study. Best-corrected visual acuity (BCVA), central retinal thickness (CRT), as well as the number of anti-VEGF injections were monitored six months before and after phacoemulsification with intraocular lens (IOL) implantation. Further, the change of the abovementioned parameters was assessed during the six-month follow-up period for CRT and the number of injections, while the BCVA was monitored for 54 months. Results: BCVA measured on the first day after surgery (0.17 ± 0.19 logMAR) as well as in the six-month post-surgery (0.13 ± 0.16 logMAR) significantly improved compared to preop values (0.42 ± 0.20 logMAR). BCVA remains stable during the observational period. We found that both differences were statistically significant ( p < 0.01). The mean CRT and the mean number of injections did not differ between the six-month pre- and post-surgical periods. Conclusions: We showed the beneficial effect of phacoemulsification in nAMD patients treated with anti-VEGF agents on visual outcomes in the short and long term. Cataract surgery in nAMD eyes treated with anti-VEGF injections does not increase the frequency of anti-VEGF injections and does not cause deterioration of the macular status.
