Gossypiboma is used to describe the forgotten cotton/gauze piece in the body after the surgical procedure. It is formed by the combination of the words “gossypium”, a Latin word for cotton, and the word “boma” in Swahili, which means the place of hiding1 . Although many different materials have been reported in the literature, cotton materials are among the most forgotten objects. Though there is no consensus, the incidence is given as 0.01-0.001%. Gossypiboma, which is more common after abdominal and pelvic surgery, has also been reported after thoracic surgery, spinal, orthopedic, and breast surgery2,3. Introduction: Today, laparoscopic cholecystectomy is essential in the treatment of acute cholecystitis, particularly in all patients who have applied in the early period and can tolerate surgery. Although the rates of complication and conversion to open surgery tend to decrease over time with the increasing experience, they are still among the important problems today. The most important complications of laparoscopic cholecystectomy in acute cholecystitis are biliary tract injuries and bleeding. In this article, we aimed to retrospectively present the complications that we encountered in laparoscopic cholecystectomy due to acute cholecystitis. Materials and Methods: The patients, who underwent laparoscopic cholecystectomy with the diagnosis of acute cholecystitis between March 2016 and December 2019, were retrospectively examined. The patients’ age, gender, symptoms–examination findings, laboratory findings, ultrasonography findings and pathology findings were analyzed. Complications and the rate of conversion to open in surgical treatment were evaluated. Results: Of 70 the patients included in this study, 22 (31%) were male and 48 (69%) were female. The ages of the patients were between 25–70 and the mean age was 46.08±11.8. Open surgery was performed in six patients (9%). Perop–postop complications developed in three (4%) patients in total. Conversion to open was required in one of these patients. Biliary tract injury occurred in one patient, cystic duct stump leak occurred in one patient, and choledochal stenosis occurred in one patient. Conclusion: As a result, early laparoscopic cholecystectomy in acute cholecystitis is an acceptable treatment option with these complications and rates of conversion to open. It should be considered that complications may occur in the interim period and patients will be at a second risk due to the postponed operation. Minimal invasive methods should be preferred as much as possible with developing complications, and if necessary, support should be obtained from more experienced teams

Cerrahikliniklerde sık rastlanılan durumların başında Akut apandisit gelmekle birlikteKlasik tedavisi cerrahidir. Bu klasik tedavi son yıllara damgasını vuranlaparoskopik cerrahiden de etkilenmiştir. Laparoskopik apendektominin açıkapendektomiye göre en önemli avantajı ‘tanısal laparoskopi’ yapılarak tanısıarada kalan hastalara doğru yaklaşım sağlanmış olunur. Bunun yanı sıra diğeravantajları olarak da; yara enfeksiyonu ve fıtık riskinin daha düşük olması,kozmetik olarak daha iyi yara yeri görünümü elde edilmesi, ameliyat sonrasıağrının daha az olması, günlük hayat aktivitelerine ve işe daha hızlı dönüşünsağlanabilmesi olarak sıralanabilir. Biz de bu yazımızda iki adet 5 mm'lik port kullanaraklaparoskopik yardımlı minimal invaziv apendektomiyi sunmayı amaçladık., Although acute appendicitis is one ofthe leading conditions faced in surgery clinics, its classical treatment issurgery. This classical treatment was also influenced by the laparoscopic surgerywhich left its mark in recent years. The most important advantage oflaparoscopic appendectomy compared to open appendectomy is that it provides thecorrect approach for the patients who could not be diagnosed definitely byperforming 'diagnostic laparoscopy'. In addition to this, its other advantagesinclude lower risk of wound infection and hernia, a better cosmetic appearanceof the wound, lesser post-operative pain, ability to return faster to dailyactivities and work. In the present article, we aimed to presentthe laparoscopy-assisted minimally invasive appendectomy using two 5 mm ports.

Purpose: The aim of our studyis to evaluate patients who have undergone the Rubber Band Ligation (RBL)procedure for hemorrhoids in our clinic between January 2015 and January 2016retrospectively.Methods: We retrospectivelyreviewed the records of patients who underwent LBL for hemorrhoid complaints inour clinic between January 2015 and January 2016. Patients who were missing or whose recordswere not available were not taken to work.Findings: Twenty-five patientsunderwent a rubber band ligation in our polyclinic. Because the data of the twopatients were not fully accessible, the study was performed on 23 patients. Ofthe patients, 10 were female and 13 were male. The average age was 33.4(18-67). LBL was applied to patients with symptomatic 2nd and 3rd degreehemorrhoidal disease who wanted to be treated with non-surgical methods. 15patients had grade 2 and 8 patients had grade 3 hemorrhoids. There werebleeding in 16 patients (69%), prolapse in 6 patients (14%), difficulty indefecation in 8 patients (53%), pain in 14 patients (61%) and pruritus in 11patients (49%). There was pain in 1 case and minimal bleeding in 1 case whomwas treated with RBL. Results: In conclusion, LBL method is thesafest, effective, and easily applicable method and it is the most frequentlyused treatment method of non-surgical treatments for hemorrhoids. However, thismethod can be seen as a disadvantage that the microscopic examination offalling hemorrhoid pakes can not be performed., Amaç: Çalışmamızın amacı Ocak2015 – Ocak 2016 tarihleri arasında kliniğimizde hemoroit şikayeti nedeniyleLastik Band Ligasyon (LBL) yöntemi uygulanan hastaları retrospektif olaraktaramaktır. Materiyal Metot: Geriye dönükolarak Ocak 2015 – Ocak 2016 tarihleri arasında kliniğimizde hemoroid şikayetinedeniyle LBL uygulanan hastaların kayıtları incelendi. Verileri eksik olanveya tam olarak kayıtlarına ulaşılamayan hastalar çalışmaya alınmadı. Bulgular: Polikliniğimizde 25hastaya lastik band ligasyonu uygulandı. İki hastanın verilerine tamulaşılamadığı için çalışma 23 hasta üzerinden yapıldı. Hastalardan 10’ u kadın, 13’ ü erkekti. Yaş ortalaması33,4 (18-67) idi. LBL semptomatik 2. ve 3. derece hemoroidal hastalığı olan vecerrahi dışı yöntemlerle tedavi olmak isteyen hastalara uygulandı. 15 hastadagrade 2, 8 hastamızda grade 3 hemoroit pakesine işlem yapıldı. 16 hastada ( %69) kanama, 6 hastada (% 14) prolapsus,8 hastada (% 53) dışkılama zorluğu, 14 hastada (% 61 ) ağrı ve 11 hastada da (%49) kaşıntı şikayetleri vardı. LBL uyguladığımız hastalarımızdan 1 olguda ağrıve 1 olguda da minimal kanama gözlendi.Sonuç: Sonuç olarak LBLyönteminin güvenli, etkili, kolay uygulanabilen bir yöntem olması ve ayaktantedavi protokolü olduğu için hemoroitin cerrahi dışı tedavilerinden en sıkkullanılan tedavi yöntemidir. Fakat bu yöntemin uygulanma sonrasında düşen hemoroitpakesinin mikroskopik tetkikinin yapılamaması bir dezavantaj olarak görülebilir.

Pilonidalsinüs hastalığı daha sıklıkla genç-erişkin erkeklerde görülür. Tedavisindeuygulanan birçok cerrahi tedavi yöntemi ve bunların modifiye edilmiş uygulamalarımevcuttur. Ayrıca hastalığın cerrahidışı yöntemlerle tedavisi de sürekli gündemdedir. There are many surgicaltreatment modalities and their modifications applied in the treatment. Inaddition, non-surgical treatment of the disease is always on the agenda. Inthis article we aimed to present briefly in recent years.

Objective: The basis of damage control surgery rests on quick control of life-threatening bleeding, injuries, and septic sources in the appropriate patients before restoring their physiological reserves as a first step followed by ensuring of the physiological reserves and control of acidosis, coagulopathy, and hypothermia prior to complementary surgery.

Kist hidatik hastalığı, Türkiye'nin de aralarında bulunduğu pek çok ülkede önemli bir sağlık sorunudur. Daha çok karaciğer ve akciğerde yerleşim gösterse de vücudun tüm dokularında görülebilir. Karaciğer kist hidatiklerinin çeşitli komplikasyonları vardır ve perforasyon bunlardan biridir. Perforasyon kendiliğinden olabileceği gibi travma sonucu da gelişebilir. Bu olgu sunumunda travma sonucu subkapsüler alana perfore olmuş karaciğer kist hidatiğini sunuyoruz., Hydatid cyst disease is an important public health problem in some countries including Turkey. The disease localized in liver and lungs in most of the cases, but also could be exists everywhere in the body. There are various complications of liver's hydatid cyst disease and perforation is one of them. Perforation can occur spontaneously or as a result of trauma. We are presenting; a liver hydatid cyst which was perforated as a result of trauma in the subcapsular area.

Giriş: Gastrointestinal sistem kanserlerinde özellikle CEA, CA-19.9 ve AFP düzeyleri yardımcı olan tümör belirteçleridir.Yöntem ve Gereç: Kliniğimizde Ocak 2008–Aralık 2013 yılları arasında mide kanseri tanısı ile opere edilen 54 hastanın preoperatif serum tümör belirteci düzeyleri, cinsiyet, yaş, evre ve tümör yayılım göstergeleri (karaciğer metastazı, omental tutulum, serozal tutulum, lenf nodu tutulumu) incelendi.Bulgular: TNM evrelemesine göre evre-I 9 (%16.7), evre-II 6 (%11.1), evre-III 35 (%64.8) ve evre-IV 4 (%7.4) hasta izlendi. CEA, 37’sinde (%68.5) normal, 17’sinde (%31.5) yüksekti. Tümör evresi arterken CEA düzeylerindeki artışın anlamlı olmadığı (p=0.161) ancak erken evre mide kanseri (Evre-I) hastalarında ileri evre mide kanseri (Evre-IV) hastalarına oranla preoperatif CEA düzeylerinin anlamlı derecede düşük olduğu (p=0.027) izlendi. Karaciğer metastazı, omental tutulum, lenf nodu tutulumu ve serozal invazyon ile CEA yüksekliği arasında ilişki izlenmedi.CA-19.9,43 hastada (%80) normal, 11 hastada (%20) yüksekti. Tümör evresi ile serum CA-19.9 arasında anlamlı bir ilişki görülmedi (p=0.88). Evre-I hastalar ile Evre-IV hastalar arasında preoperatif CA19.9 düzeyleri açısından anlamlı farklılık izlenmedi (p=0.538). Lenf nodu tutulumu, karaciğer metastazı, omental tutulum ve serozal invazyon açısından CA19.9 yüksekliği anlamlı değildi.AFP, 41 hastada (%76) normal, 13 hastada (%24) yüksek izlendi. Tümör evresi ile anlamlı bir ilişki göstermediği izlendi (p=0.478). Evre-I hastalar ile Evre-IV hastalar arasında preoperatif AFP düzeyleri açısından anlamlı farklılık izlenmedi (p=0.14). Karaciğer metastazı, lenf nodu tutulumu, omental tutulum ve serozal invazyon ile serum AFP düzeyleri arasından anlamlı bir ilişki izlenmemiştir. Cinsiyet ve yaş açısından CEA, CA19.9 ve AFP yüksekliğinde anlamlı farklılık izlenmedi.Sonuç: CEA düzeyleri ileri evre mide kanseri hastalarında anlamlı derecede yüksekti. Bu nedenle CEA düzeylerindeki yüksekliğin mide kanserlerinde ileri tümör evresinin habercisi olabileceğini ancak düşük değerlerde olmasının tümörün olmadığını ya da erken evre olduğunu düşündürmemesi gerektiği kanısındayız.

Yabancı cisim yutulması pediatrik yaş grubunda erişkin yaş grubuna göre daha sık görülmektedir. Yutulan yabancı cisim tıbbi komplikasyonlara neden olabilmektedir. Tedavi yaklaşımları çeşitlilik gösterse de yabancı cismin çoğunlukla gastrointestinal sistem aracılığı kendiliğinden atılmaktadır. Yabancı cisim yutan hastaya uygulanan tedavi yaklaşımı hastada görülen klinik tabloya göre değişkenlik göstermektedir. Bu olgu sunumunda üç adet toplu iğne yutan hastaya uygulanan üç farklı tedavi yaklaşımı bildirildi. Foreign body swallowing in pediatric age group are seen more frequently than adult age group. Engulfed foreign bodies exhibits clinical medical complications. Although treatment modalities may vary, the foreign body is mostly spontaneously excreted through the gastrointestinal tract. Treatment approach against swallowed foreign bodies is performed according to the clinical presentation in patient. In this case report three different treatment approaches were reported in patient who swallowed three needle.

Bu çalışmanın amacı obstrüksiyon yapmış kolorektal kanserler nedeniyle kliniğimizde acil şartlarda ameliyat edilen hastalardaki morbidite ve mortalite oranlarını ve bunlara etki eden faktörleri sunmaktır. Materyal ve Metod Ocak 2008 - Temmuz 2012 yılları arasında mekanik bağırsak tıkanıklığına neden olmuş kolorektal kanser nedeniyle ameliyat edilen 10 erkek ve 8 kadın olmak üzere 18 hasta retrospektif olarak değerlendirildi. Hastalar yaş, cinsiyet, komplikasyon, yapılan ameliyat, morbidite, mortalite açısından incelendi. Yaşın, cinsiyetin ve tümör yerleşiminin morbidite üzerine olan etkileri incelendi. Bulgular Hastaların yaş ortalaması 66 ± 8,6 idi. 70 yaş üzerinde 10 (% 56,6) vardı. Hastaların 16’sında komplet bir obstrüksiyon izlendi (% 88,8). Bir hastada obstrüksiyonla beraber perforasyon mevcuttu (% 5,6). On iki hastaya Hartmann prosedürü (% 66,7) uygulanırken, 2 hastaya loop kolostomi (% 11,2), 2 hastaya sağ hemikolektomi (% 11,2), 1 hastaya total kolektomi ileorektal anastomoz (% 5,6), 1 hastaya sağ hemikolektomi uç ileostomi (% 5,6) uygulandı. Beş hastada morbidite izlendi (% 27,8). İki hastada mortalite izlendi (% 11,2). Morbidite görülen tüm hastalar 70 yaş üzerindeydi (p=0.21). Cinsiyete göre ve tümörün yerleşim yerine göre ise morbidite oranları açısından anlamlı fark yoktu. Sonuç Obstrüktif kolorektal kanser nedeniyle acil operasyona alınan hastalarda mortalite ve morbidite izlenme olasılığı artmaktadır. Bunun nedenlerinden birisi hastaların daha çok ileri yaşlarda olmalarıdır. Ayrıca hastalarda perforasyon gibi eşlik eden bir komplikasyon varlığı mortalite ve morbiditeyi ciddi ölçüde arttırmaktadır. Aim The aim of this study is sharing the morbidity and mortality rates and risc factors efecting the morbidity and mortality rates of patients underwent emergency operation in our clinic. Materials and Methods Between January 2008 - July 2012 eighteen patients, 10 men and 8 women, who operated because of obstructive colorectal cancer, were reviewed. Patients were examined fora ge, sex, complication, operation type, morbidity and mortality. The effects of age, sex and tumor location on morbidity and mortality were examined. Results Mean age of patients was 66 ± 8,6. Ten patients were elder than 70 (% 56,6). Complet obstruction was seen in 16 patients (% 88,8). In one patient perforation was seen wtih obstrucion (% 5,6). Hartmann’s procedure was performed for 12 patients (% 66,7), loop colostomy was performed for two patients (% 11,2), right hemicolectomy was performed for two patients (% 11,2), total colectomy with ileorectal anastomosis was performed for 1 patient (% 5,6), right hemicolectomy with end ileostomy was performed for 1 patient (% 5,6). Morbidity was seen in 5 patients (% 27,8). Mortality was seen in two patients (% 11,2). Both patients with mortality were elder than 70 (p=0.21). There weren’t any significant difference for sex and tumor’s location. Conclusion Morbidity and mortality rates increases in patients whom underwent emergency surgery for obstructive colorectal cancer. Being elder patient is one of the reasons. And also accompanied complications like perforation searously increase mortality and morbidity.

Amaç: Bu çalışmadaki amacımız kolorektal kanserli hastalarımızda prognostik faktörlerin sağ kalım üzerine etkilerini retrospektif olarak incelemektir. Yöntemler: Fırat Üniversitesi Hastanesi Genel Cerrahi Kliniğinde 1994 ve 2005 yılları arasında kolorektal kanser tanısıyla cerrahi tedavi uygulanan 154 hasta içerisinden 126 hastanın klinik kayıtları, patoloji ve ameliyat raporları geriye dönük olarak incelendi. Hastalarda belirlenen prognostik faktörlerin 2 yıllık, 5 yıllık ve 10 yıllık sağ kalım, genel sağ kalım (GSK) ve hastalıksız sağ kalım (HSK) üzerine etkileri araştırıldı. Sağ kalım süreleri ay olarak hesaplandı. Bulgular: Hastaların 72 (%57,1)’si erkek, 54 (%42,9)’ü kadın olup yaş ortalaması 57,5 ± 15,2 idi. Ortanca takip süresi 44,5 ay idi. Hastalarda ortanca GSK 60 ay idi. 2,5 ve 10 yıllık GSK oranları sırasıyla %59,5, %46,0 ve %42,9 idi. Ortanca HSK evre I-III kanserli hastalarda 62 ay ve toplamda 40 ay olarak bulundu. Tek değişkenli analizde patolojik evre, kanserin evresi, T evresi, lenf nodu tutulumu, metastaz, cerrahi sınır, preoperatif CEA düzeyi ve uygulanan cerrahi tedavi hem GSK hem de HSK ile ilişkili bulundu. Yaş ve histolojik tip sadece GSK ile ilişkili bulundu. Çok değişkenli analizde kanserin evresi, T evresi ve metastaz GSK ve HSK açısından anlamlı bulunan bağımsız prognostik parametreler olarak değerlendirildi. Sonuç: Kolorektal kanserlerde cerrahi tedavi sonrası sağ kalımı etkileyen çok sayıda prognostik faktör mevcuttur. Sağ kalımı daha isabetli tahmin etmek amacıyla, bağımsız prognostik faktörlerin yanında, diğer prognostik faktörleri de içeren bir skorlama sistemi daha geniş serilerle oluşturulabilir. Objective: The aim of this study is to investigate retrospectively the impact of the various prognostic factors on survival in patients with colorectal cancer. Methods: The clinical records, pathology and operation reports of 126 patients in 154 patients operated for colorectal cancer General Surgery Clinic of Fırat University Hospital between 1994 and 2005 years. The effects of the prognostic factors on 2-years, 5-years and 10-years survival, overall survival (OS) and disease-free survival (DFS) were evaluated. The survival durations are presented as months. Results: The mean age was 57.5 ± 15.2. The median duration of follow-up was 44,5 month. The median OS was 60 months. The median DFS in stage I-III patients was 62 months. According to the univariate analysis, the factors found significant in both OS and DFS rates were the following; stage of the cancer, extent of bowel wall penetration, lymph nodes metastasis, distant metastasis, pathologic stage, surgical margin, pretreatment CEA levels and type of surgery. The age and histologic type of tumor were related to only OS. The following variables were independent prognostic factors for both OS and DFS as determined by multivariate analysis; stage of the cancer, extent of bowel wall penetration and distant metastasis. Conclusion: A large number of prognostic factors are available to help predict the outcome of patients with colorectal cancer after surgery. In order to predict survival more accurately, a scoring system can be created that involving not only prognostic factors determined by multivariate analysis, but also prognostic factors determined by univariate analysis.
