A series of 2substituted43Hquinazolinones 1320has been synthesized in good yields using the reaction of double lithiated 2methylquinazolinone4 with a variety of aromatic aldehydes. They have been easily transformed in high yields into the corresponding 2substituted conjugated derivatives 2128bearing terminal aryl groups by F3CCOOH mediated dehydration.

Abstract

The possibility of using elevated Chlamydia trachomatis-specific serum IgG and IgA as a screening test for Chlamydia-associated epididymitis was analyzed in 28 acute epididymitis patients and 42 apparently healthy men by the single antigen (L2) immunoperoxidase assay. The prevalence rates of C. trachomatis IgG antibody titer greater than or equal to 64 and elevated C. trachomatis IgG titers (greater than or equal to 128) were significantly higher in the epididymitis patients (75 vs. 40%, p less than 0.01, and 39 vs. 14%, p less than 0.025, respectively) than in controls. The prevalence rate of C. trachomatis IgA antibodies (titer greater than or equal to 8) was significantly higher (p less than 0.001) in epididymitis patients as compared to controls (46 vs. 10%, respectively). The potential application of elevated serum C. trachomatis IgG and IgA antibodies as a noninvasive screening marker in epididymitis patients is discussed.

32. Computational Probes into the Basis of Silver Ion Chromatography. II. Silver(I)−Olefin Complexes

Abstract

Alkene complexes of silver(I) are studied by four computational methodologies:  ab initio RHF, MP2, and MP4 computations, and density functional B3LYP computations, with a variety of all-electron and effective core potential basis sets. Methodological studies indicate that MP2/SBK(d) computations can adequately represent Ag(I)−alkene complexes, and this method is used to study Ag(I) complexes with one to six ethene ligands as well as with methyl-substituted ethene derivatives. The first two alkene ligands enter the coordination sphere of Ag(I) with approximately the same energy, in line with ICR experiments. The association enthalpies of additional complexing ethene ligands diminish progressively. Computations on methyl-substituted alkenes show that the observed trends in argentation chromatography are opposite to those observed and computed for the gas phase, and suggest strong medium effects in Ag(I) chromatography.

33. Prostaglandin E2 Affects the Tumor Immune Response in Prostatic Carcinoma

Abstract

Antitumor immunity to prostatic carcinoma was assayed by the tube leukocyte adherence inhibition assay. Peripheral blood leukocytes from 37 patients with prostatic carcinoma and 128 patients without prostatic carcinoma were incubated separately with extracts from prostatic carcinoma and from an unrelated control cancer, and the non-adherence index was calculated. The number of patients with prostatic carcinoma who had a positive response (non-adherence index greater or equal to 30) was small. Nevertheless, the mean non-adherence index (11) of the patients with prostatic carcinoma was statistically different (p less than 0.02) from the mean non-adherence index (2) of the subjects without prostatic carcinoma. Of patients with prostatic carcinoma 14 per cent (5 of 37) had positive leukocyte adherence inhibition responses, compared to 1.5 per cent of control subjects (2 of 128) (p less than 0.05). Sixty patients diagnosed clinically as having benign prostatic hyperplasia were screened by the leukocyte adherence inhibition test and the non-adherence index results then were evaluated with the histopathological diagnosis of the surgical specimen. Of the patients with benign prostatic hyperplasia 3 (5 per cent) had stage A prostatic carcinoma, and 1 of these 3 patients (33 per cent) had a positive leukocyte adherence inhibition test.

Abstract

The nonneurogenic neurogenic bladder syndrome which actually comprises a large spectrum of clinical radiological, psychological and behavioral disorders, is manifested by bladder-sphincter incoordination and constitutes a very complex problem of overall management. The condition refers to infravesical functional obstruction in children in whom no anatomic or neurological defect is demonstrated. Although the etiologic bases are often unclear, successful management of urinary infection and reflux and prevention of upper tract deterioration are achievable by multidisciplinary conservative measures, sometimes reinforced by ureteric reimplantation. End-stage renal failure due to this syndrome is a rare complication. The presented case demonstrates well all the clinical features of this syndrome, which unfortunately ended in hemodialysis.

Abstract

Bladder contracture following cutaneous loop ureterostomy is a known but sometimes neglected serious complication. Recently we treated a boy successfully by effecting an increase in bladder capacity by long-term hydraulic dilatation prior to performing colonic augmentation. The severity of the contracture seems to be the worst ever published in the literature.
