Aim: We aimed to prospectively interpret the cardiac repolarization changes with 12-lead electrocardiography (ECG) in children with cancer who were treated with anthracycline drugs.Materials and Methods: A total of 53 patients with cancer treated with anthracycline were enrolled in the study. During 6 months follow-up, standard 12-lead ECG was performed at basal, 1st, 4th, and 24th hours after first dose of anthracycline treatment, at the time of 120mg/m2 cumulative anthracycline dose and 240mg/m2 of cumulative anthracycline dose in same patients, respectively. P dispersion(PWd), QT dispersion(QTd), corrected QT dispersion(QTcd), Tp-e interval, Tp-e/QT and Tp-e/QTc ratio were obtained from 12-lead ECG. Inhibitor development is the most frequent and serious complication of the treatment in patients with hemophilia. Immune tolerance induction (ITI) is the only option of treatment for the eradication of factor VIII (FVIII) inhibitor. We would like to present our case with hemophilia whose FVIII inhibitor eradication was done by a low-dose ITI regimen. Our patient has been applied on-demand therapy until 8 years of age. Secondary prophylaxis was began because of having hemophilic arthropathy. A low titer of FVIII inhibitor (4.2 BU/ml) was detected in the fifth month of the prophylaxis. The peak inhibitor titer of patient was 4.6 BU/ml, and there was no decrease in inhibitor titer in the follow-up duration. The low-dose ITI (50 IU/kg, 3 days a week) was started. His inhibitor level was detected negative and the recovery test was ameliorated in the 15th of the ITI therapy. High-dose regimen ITI could not be given particularly in developing countries such as Turkey in view of the high cost of treatment. Ağır hemofili A hastalarında erken yaşta başlanan profilaktik Faktör VIII konsantresi replasmanına rağmen eklem içi kanamalar tam olarak önlenememekte, kalıcı eklem hasarları oluşmaya devam etmektedir. Kalıcı eklem hasarlarının azaltılması için kanamanın erken teşhis ve tedavisi önemlidir. Günümüzde, klinik bulgular ve mevcut laboratuvar ve radyolojik görüntüleme yöntemleri ile eklem içi kanamayı ilk başladığında saptamak henüz mümkün değildir. Son dönemde yapılan çalışmalarda hemofili hastalarında tekrarlayan eklem içi kanamalar sonrası sinovit gelişiminde neoanjiyogenezisin önemli rol oynadığı, hemofilik artropatide sinovyada saptanan histolojik anormalliklerin ve neoanjiyogenezisin diğer eklem hastalıklarındakilerle benzer olduğu gösterilmiştir. Bu çalışmada ağır hemofili A hastalarında eklem bulguları ile plazma anjiogenik ve inflamatuvar faktör düzeyleri arasındaki ilişkinin araştırılması amaçlanmıştır.Çalışmaya hemofili takibi yapılan dört merkezden inhibitörü negatif olan, ağır hemofili A'lı olgular alındı. Akut eklem içine kanama ile gelen ve eklem hasarı olan 10 ağır Hemofili A hastası, akut eklem kanaması olmayan (mevcut eklem hasarı olan veya olmayan) 25 ağır hemofili A hastası ve kontrol grubu olarak da 22 sağlıklı erkek çocuk alındı. Hastalar eklem hasarı olanlar ve ileri eklem hasarı olanlar olmak üzere 2 alt gruba ayrıldı. Tüm hastalardan ve kontrol grubundan periferik kandan tam kan sayımı, C-reaktif protein (CRP), serum ferritin, laktik asit ve ELISA yöntemiyle vasküler endotelyal büyüme faktörü ( VEGF),intersellüler adezyon molekülü, trombomodulin (TM), makrofaj migrasyon inhibitör faktör (MIF) ve endostatin bakıldı. Hemofili hastalarının eklem bulguları klinik olarak Hemofilik eklem sağlığı skoru, radyolojik olarak Petterson radyolojik skorlaması ve Denver manyetik rezonans görüntüleme skorlaması ile değerlendirildi.Hemofili hastalarında serum ferritin, CRP ve laktik asit düzeylerinin sağlıklı kontrollere göre yüksek olduğu, eklem hasarı olan hemofili hastalarında ise CRP'nin eklem hasarı olmayanlara göre daha yüksek olduğu bulundu. Sadece akut eklem kanaması sırasında CRP ve MIF düzeylerinin belirgin arttığı saptandı. Serum TM ile VEGF ve MIF değerleri atasında pozitif korelasyon saptandı. Çalışmamız sonuçlarına göre serum CRP ve MIF düzeylerinin akut kanama sırasında yüksek olarak bulunması eklem hasarının derecesine bağlı olmadan kanama varlığını desteklemektedir. Mevcut bulgular, bu anjiyogenik faktörlerin düzeylerinin daha fazla sayıda akut eklem kanamalı ağır Hemofili A hastasında incelenmesinin erken eklem hasarı konusunda bilgi verebileceğini düşündürmektedir.Anahtar kelimeler: Anjiyogenezis, hemartroz, hemofili Despite the use of early prophylactic Factor VIII replacement in severe hemophilia A patients, bleeding into joints cannot be prevented completely and permanent joint damage remains to occur as a result. Early diagnosis and treatment of the joint bleeedings are important for prevention of permanent joint damage. Today, the early detection of the joint bleeding at the beginning of the episode is not possible with clinical signs and current labarotory and radiologic imaging methods. Recent studies have shown that neoangiogenesis plays important role in development of synovitis after recurrent joint bleedings and the histological abnormalities and findings of neoangiogenesis in hemophiliac arthropathy are similar to those in other joint diseases. This study aimed to investigate the relationship between joİnt findings and levels of plasma angiogenic and inflammatory factors in severe hemophilia A patients.Severe, inhibitor negative, hemophilia A patients from four hemophilia centers were included into the study. The patient groups consisted of 10 severe hemophilia A patients withacute joint bleeding and joint damage and 25 severe hemophilia A patients with no acute joint bleeding (with joint damage or not). The control group consisted of 22 healthy male children.Each group of patients were further subdivided into two groups with respect to degree of joint damage as presence of joint damage and presence of severe joint damage. Complete bloodcount analysis, C-reactive protein (CRP), serum ferritin, lactic acid and ELISA based detection of vascular endhotelial growth factor (VEGF), intercellular adhesion molecule-1, thrombomodulin (TM), macrophage inhibitory factor (MIF), and endostatin were performed from peripheral blood of patient and the control groups. The joint findings of hemophilia A patients were evaluated clinically by hemophilia joint health score and radiologically by Petterson scoring and Denver magnetic resonance imaging scoring methods.Serum ferritin, CRP and lactic acid levels were found to be increased in hemophilia A patients compared to the control group. CRP was significantly increased in hemophilia patients with joint damage compared to hemophiliacs with no joint damage. The increase in levels of CRP and MIF were detected significantly higher in patients with acute joint bleeding. There was a positive correlation between serum TM and VEGF and MIF levels.The finding of increased levels of CRP and MIF in acute bleeding supports the presence of bleeding regardless of the degree of the joint damage. The present findings show that investigation of the levels of these angiogenic factors in large number of severe hemophilia A patients with acute bleeding episodes may give further information about early joint damage.Key words: Angiogenesis, hemarthrosis, hemophilia 83
