Trauma is the leading cause of death in individuals under 44 years of age. Thorax trauma (TxT) is strongly associated with trauma‐related death, an unbalanced innate immune response, sepsis, acute respiratory distress syndrome, and multiple organ dysfunction. It is shown that different in vivo traumata, such as TxT or an in vitro polytrauma cytokine cocktail trigger secretion of small extracellular nanovesicles (sEVs) from endothelial cells with pro‐inflammatory cargo. These sEVs transfer transcripts for ICAM‐1, VCAM‐1, E‐selectin, and cytokines to systemically activate the endothelium, facilitate neutrophil‐endothelium interactions, and destabilize barrier integrity. Inhibition of sEV‐release after TxT in mice ameliorates local as well as systemic inflammation, neutrophil infiltration, and distant organ damage in kidneys (acute kidney injury, AKI). Vice versa, injection of TxT‐plasma‐sEVs into healthy animals is sufficient to trigger pulmonary and systemic inflammation as well as AKI. Accordingly, increased sEV concentrations and transfer of similar cargos are observed in polytrauma patients, suggesting a fundamental pathophysiological mechanism., It is shown that thorax trauma in mice facilitates enhanced release of small extracellular nanovesicles (sEVs) with proinflammatory features, which originate from endothelial cells. Injection of TxT‐plasma‐sEVs into healthy animals triggers local and systemic inflammation. Mechanistically, endothelial‐derived sEVs transfer transcripts for cytokines and adhesion molecules to propagate the inflammatory response. Similar sEVs are detected in polytrauma patients suggesting a fundamental pathophysiological mechanism.

Initially underestimated as platelet dust, extracellular vesicles are continuously gaining interest in the field of inflammation. Various studies addressing inflammatory diseases have shown that microvesicles (MVs) originating from different cell types are systemic transport vehicles carrying distinct cargoes to modulate immune responses. In this study, we focused on the clinical setting of multiple trauma, which is characterized by activation and dysfunction of both, the fluid-phase and the cellular component of innate immunity. Given the sensitivity of neutrophils for the complement anaphylatoxin C5a, we hypothesized that increased C5a production induces alterations in MV shedding of neutrophils resulting in neutrophil dysfunction that fuels posttraumatic inflammation. In a mono-centered prospective clinical study with polytraumatized patients, we found significantly increased granulocyte-derived MVs containing the C5a receptor (C5aR1, CD88) on their surface. This finding was accompanied by a concomitant loss of C5aR1 on granulocytes indicative of an impaired cellular chemotactic and pro-inflammatory neutrophil functions. Furthermore, in vitro exposure of human neutrophils (from healthy volunteers) to C5a significantly increased MV shedding and C5aR1 loss on neutrophils, which could be blocked using the C5aR1 antagonist PMX53. Mechanistic analyses revealed that the interaction between C5aR1 signaling and the small GTPase Arf6 acts as a molecular switch for MV shedding. When neutrophil derived, C5a-induced MV were exposed to a complex ex vivo whole blood model significant pro-inflammatory properties (NADPH activity, ROS and MPO generation) of the MVs became evident. C5a-induced MVs activated resting neutrophils and significantly induced IL-6 secretion. These data suggest a novel role of the C5a-C5aR1 axis: C5a-induced MV shedding from neutrophils results in decreased C5aR1 surface expression on the one hand, on the other hand it leads to profound inflammatory signals which likely are both key drivers of the neutrophil dysfunction which is regularly observed in patients suffering frommultiple traumatic injuries., publishedVersion

Cells communicate with other cells in their microenvironment by transferring lipids, peptides, RNA, and sugars in extracellular vesicles (EVs), thereby also influencing recipient cell functions. Several studies indicate that these vesicles are involved in a variety of critical cellular processes including immune, metabolic, and coagulatory responses and are thereby associated with several inflammatory diseases. Furthermore, EVs also possess anti-inflammatory properties and contribute to immune regulation, thus encouraging an emerging interest in investigating and clarifying mechanistic links between EVs and innate immunity. Current studies indicate complex interactions of the complement system with EVs, with a dramatic influence on local and systemic inflammation. During inflammatory conditions with highly activated complement, including after severe tissue trauma and during sepsis, elevated numbers of EVs were found in the circulation of patients. There is increasing evidence that these shed vesicles contain key complement factors as well as complement regulators on their surface, affecting inflammation and the course of disease. Taken together, interaction of EVs regulates complement activity and contributes to the pro- and anti-inflammatory immune balance. However, the molecular mechanisms behind this interaction remain elusive and require further investigation. The aim of this review is to summarize the limited current knowledge on the crosstalk between complement and EVs. A further aspect is the clinical relevance of EVs with an emphasis on their capacity as potential therapeutic vehicles in the field of translational medicine.

Bu kadar geniş bir fonksiyon alanı üstlenen ambalajın tasarımı da o derece önem kazanmaktadır. Tüm bu beklentiler göz önünde bulundurularak yapılan tasarımlar başarılı olurken, bu beklenti ve ihtiyaçlardan bir veya birkaçı tasarım aşamasında göz ardı edilirse bu durumda ürünün de ekonomik olarak başarısız olmasına sebep olacak sorunlar ortaya çıkması riski bulunmaktadır. Günümüzde her yıl binlerce yeni ürünün raflarda yerini aldığını ve ürün çeşitliliğinin arttığını düşünürsek bu noktada benzer nitelik ve fonksiyonellikteki ürünlerin tüketici nezdinde farklı algılanması için de ambalaj etkeni oldukça önem kazanmaktadır. Bu nedenle de ürünün piyasada tutuna bilmesi ürüne bağlı olduğu kadar, yukarıda sayılan beklenti ve ihtiyaçların tamamını karşılayacak bir ambalaj tasarımına da bağlıdır. Bu çalışmada ambalaj kavramı, çeşitleri ve ambalajın önemi incelenirken, özellikle yukarıda belirtilen çerçevede ambalajın üstlenmiş olduğu fonksiyonlar açısından başarılı bir ambalaj tasarımının nasıl olması gerektiği gıda sektörü özelinde detaylı bir şekilde ele alınmıştır. Çalışmanın ilk bölümünde ambalaj tanımı, dünyada ve Türkiye’de tarihçesi, ambalaj tipleri ve ambalajın fonksiyonları ele alınırken ambalaj kavramına açıklık getirilmeye çalışılmıştır. İkinci bölümde ambalajın kullanıldığı sektörler arasında “gıda sektörü” detaylı incelenmiş, gıda sektöründe kullanılan ambalaj çeşitlerine ve gıda ambalajında öne çıkan önemli bir unsur olan hijyene gıda yönetmelikleri çerçevesinde değinilmiştir. Son bölümde ise tasarım konusu kapsamlı bir şekilde ele alınmıştır. Ürünün başarılı olması ürünü piyasaya süren her firmanın, iyi ve doğru tasarımı yapmak da her ambalaj tasarımcısının amacıdır. Bu çalışma ile bir ürünün yoğun rekabet şartlarında başarılı olabilmesi için ambalajın üstlendiği fonksiyonlar çerçevesinde ambalaj tasarımında nelere dikkat edilmesi gerektiğini açıklığa kavuşturmaya çalışırken, hem ürün üreticisine hem de ambalaj tasarımcısına yol gösteren bir kaynak ortaya koymayı hedefledim. Çünkü büyük emek, özveri ve kısıtlı sermaye ve kaynak yapısı ile ortaya konulan ürünlerin başarısının bağlı olduğu en önemli unsurlardan biri olan ambalajın tasarımında yapılan hatalar ürünün başarısızlığı ile şirket karlılığını olumsuz etkilemekle birlikte bu aynı zamanda milli servet ve kaynak kaybı ve israfı olmaktadır. Dileğim ve amacım bu çalışma ile Türk sanayi ve tarım işletmeleri üreticileri ile ambalaj tasarımcılarına ambalajın anlam ve önemini ortaya koymak; özellikle gelişmekte olan ekonomilerde, kıt olan kaynaklarla meydana getirilen ürünlerin değer ve miktar kaybına uğramadan başarılı ürün geliştirme çalışmaları çerçevesinde en doğru ürünlerin tüketici yararına sunulmasını sağlamaktır. Packaging is an indispensable part of almost every industry in the current world economy and although the primary function of a package was to preserve its contents when it first appeared five thousand years ago, it has gained a wider use in time and the package of a product became almost as important as, and sometimes even more important than the product itself. Today, for producers a package must be economic, compact and occupy little space in the warehouse, be filled easily and quickly, and help to carry products undamaged to consumers, whereas for marketers it has to differentiate the product from other products on the market and help to promote sales, for package producers it has to be easy-to-produce, suitable to its existing machinery and made of raw materials available on the market, for distributors it has to protect the goods in the best possible way and be suitable for storing and transportation, for retailers it has to facilitate sales, preserve and display its contents, and for consumers it has to be attractive, give information on its contents, and be easy to use, carry, store and even to destroy. This multi-functionality makes the design of packaging very important. Successful designs meet all of these expectations, and if one or more of these expectations and needs are ignored during designing stage, problems may occur which will then lead to economic failure of the product. Every year, thousands of new products are launched to the market and product diversity is displaying an ever-increasing trend, therefore, packaging is also important to differentiate a product among others with similar qualities and functions. Thus, success of a product on the market depends not only on the quality of such product but also on the design of its package, which meets all of the expectations and needs listed above. This study takes a close look at the concept and different types of packaging, and the role it plays today, and discusses in detail how a successful package design should be in the food industry, taking into account all the functions attributed to packaging. In the first part of this study, definition and history of packaging in the world and in Turkey, and different types and functions of packages are discussed to clarify the concept of packaging. In the second part, “food industry”, which is one of the heavy-users of packaging is examined in detail, and different types of packages used in the food industry as well as hygienic food regulations that are important for food packaging are presented. The last part of this study deals with design issues in detail. Every company launching a product aims to achieve success with such product, whereas, every package designer aims to make a good and functional package. With this study, I aim to provide guidelines to both product producers and package designers by explaining what are important when designing packages in line with the functions of packaging, so that products in those packages can be successful in today’s market where there is fierce competition. Products are produced with a substantial amount of labor, considerable sacrifice, and limited funds and resources, and packaging constitutes one of the most important factors in the success of such products. Therefore, any failure in the design of packaging results in the failure of the product, has an adverse effect on the profitability of the company and wastes national wealth and resources. In this study, I am targeting to explain the significance of packaging to Turkish industrial and agricultural producers, and to packaging designers so that product development studies are carried out successfully, and products produced with limited resources, especially in developing countries, are offered to consumers without any degradation in their quality and quantity. Bu çalışma ile amaçlanan düşük maliyetli taşıyıcı modelinin uzun mesafeli hatlarauygulanabilip uygulanamayacağını değerlendirmektir. Düşük maliyetli taşıyıcılar kısamesafeli hatlarda fiyat üzerinden rekabet ederek, ?en düşük maliyet?le rekabet avantajıyakalayan taşıyıcılardır. İş modelleri kısıtlı ikram ile yüksek frekans birleşiminedayanmaktadır.Düşük maliyetli taşıyıcı modelinin başarısı için dört koşul sağlanmalıdır. Pazar homojenolmalı ve serbestleşmiş olmalıdır. Uçakların günlük kullanımını optimize etmek için, hattınmesafesi 4 saatten kısa olmalıdır. Toplumun ikramsız yolculuğu kabul etmeme olasılığıbulunduğundan zihniyet değişimi bir önkoşuldur. Son olarak, deneyimler yeni bir başlangıçyaparak şirketin kurulum aşamasından itibaren maliyet düşürmeye odaklanarak sektördebenzer çalışmalardaki hataları tekrarlamamanın başarının sırrı olduğunu göstermektedir.Diğer yandan düşük maliyetli taşıyıcı modelinin uzun mesafeli hatlara uygulanması sırasındasorunlar belirmektedir. İkincil havaalanlarında var olmayan besleyici trafik, kısıtlayıcı havatrafik anlaşmalarının pazara hükmetmesi, yolcu konforu, iş modeline eklenen karmaşıklık yada kontrol edilemez maliyetlerin kontrol edilebilir maliyetleri bastırması başlıca problemlerolarak sayılabilir.İki farklı düşük maliyetli taşıyıcı modeli üstün gelmektedir. Bunlardan ilki uçuş sıklığı vegünlük uçuş saati olarak uçakların azami kullanımına dayanmaktadır. İkincisiyse benzer birbasit ürün yaklaşımı sunarak, geliri uzun mesafeli hatlarda tek sınıf düzenlemesiyle azamikılmaktadır. İkinci modelin dikkatle seçilmiş uzun mesafeli hatlarda uygulanabileceğigörülmektedir.Anahtar Kelimeler: Havayolu Taşımacılığı, Havayolu İşletmeleri, Liberalleşme,Maliyet The objective of this project is to assess if the l ow c ost carriers? business modelcould be applied to long haul operations. Low cost/low fare carriers which compete on priceon short haul destination, the ?lowest cost? providing a key competitive advantage. Thebusiness model is based on a no frills/high volume combination.Four conditions must be present for the success of the low cost carrier?s business model.The market must be homogeneous and deregulated. Sector length should be less than fourhours to enable an optimized daily utilisation of the airliners. A change of mentality isprerequisite as no frills travel may not be accepted by the society. Finally, experience showsthat a ?green field? approach to avoid the burden of past decisions is key to success. Howeverproblems appear when low cost carriers are moving to long haul operations. Absence offeeding traffic at secondary airports, markets which are ruled by restrictive bilateral airservice agreements, passenger comfort, complexity added to the model or controllable coststhat are swamped by uncontrollable ones are the major problems.Two different low cost carrier models prevail. The first is based on a maximum dailyutilisation of the aircraft. The second offer a similar basic cabin service approach, revenuebeing maximised by operating a bigger airliner on longer routes. Background: Beta-thalassemia major (beta-TM) patients need blood transfusions, which result in iron deposition. To regulate chelation therapy, iron load has to be measured. With MRI, the amount of signal loss and T2* decay time shortening are used for iron quantification. Objectives: The aim was to measure adrenal iron load with T2* relaxometry using MRI, and to compare it with liver and cardiac iron and serum ferritin, and to find out whether adrenal iron could be predicted from those parameters. Patients and Methods: Between October 2014 and March 2015, MRI was performed in 21 patients with beta-TM, recieving blood transfusions and chelation therapy. The control group (n = 11) included healthy volunteers with no known history of adrenal, hematologic, chronic disease, and blood transfusion. Results: Among patients, there was no significant correlation between plasma ferritin and adrenal T2*. Significant difference was detected among T2* values of adrenals between the patient and control groups. There was no significant correlation between adrenal gland and liver T2* in beta-TM patients, moderate correlation was detected between adrenal T2* and cardiac T2*. Conclusion: Adrenal iron in beta-TM can be reliably measured in 3 Tesla MRI.
