This study was designed to investigate the effects of vitamin D and mannitol in an experimental rat ovarian torsion model. Thirty-two female Wistar albino rats were randomly classified as group 1: (sham), group 2: (detorsion), group 3: (detorsion + mannitol), group 4: (detorsion + vitamin D) and group 5: (detorsion + mannitol + vitamin D) (for each group n = 8). All groups were subjected to bilateral adnexal torsion for 2 h except for group 1. Bilateral adnexal detorsion was performed in all groups except for group 1. Groups 3 and 5 intraperitoneally received the injection of mannitol at a dose of 0.3 mg/kg 30 min before detorsion. Also, the group’s 4 and 5 orally received vitamin D in a dose of 500 IU/kg/day for two weeks before torsion. Total oxidant status (TOS), total antioxidant status (TAS), oxidative stress index (OSI) and proliferating cell nuclear antigen (PCNA) levels were analyzed. According to the histopathological analyses, ovarian tissue damage and follicle counting were evaluated. TOS, OSI and histopathologic score values of ovarian tissue were significantly lower in group 5 than groups 2, 3 and 4 (p < 0.05). The PCNA level was significantly higher in group 5 than in groups 2, 3 and 4 (p < 0.05). A strong negative correlation was found between OSI and PCNA in groups 2, 3, 4 and 5 (r = −0.92, p = 0.01; r = −0.98, p < 0.0001; r = −0.98, p < 0.0001 and r = −0.96, p = 0.0002, respectively). The numbers of primordial follicles in group 5 (p < 0.001) and primary follicles in group 4 (p < 0.001) were significantly higher when compared to group 2. Based on the results of this study, it could be suggested that combination treatment of mannitol with vitamin D is more effective in reversing tissue damage induced by ischemia–reperfusion (I/R) injury in the ovarian torsion model than administration of only an agent.

This paper deals with a family of normalized multivariate neural network (MNN) operators of complex-valued continuous functions for a multivariate context on a box of R N ¯ , N ¯ ∈ N . Moreover, we consider the case of approximation employing iterated MNN operators. In addition, pointwise and uniform convergence results are obtained in Banach spaces thanks to the multivariate versions of trigonometric and hyperbolic-type Taylor formulae on the corresponding feed-forward neural networks (FNNs) based on one or more hidden layers. Management of melanoma has changed significantly with the discovery of targeted therapies and immune checkpoint inhibitors (ICI). Our aim in the study is to determine which treatment alternatives, specifically dabrafenib plus trametinib and ICIs, are effective in adjuvant therapy and which treatment is effective as first-line metastatic therapy. This retrospective, multicenter study included 120 patients diagnosed with stage IIIB-IIID melanoma receiving both adjuvant and first-line metastatic treatment between 2007 and 2023. Data on clinicopathologic characteristics, treatment regimens and outcomes were collected. Objective response rate (ORR), disease control rate (DCR), progression-free survival (PFS) and overall survival (OS) were analyzed. Patients treated with dabrafenib plus trametinib as adjuvant therapy had the longest relapse-free survival (RFS) (median: 8.3 months), followed by those treated with interferon (4.1 months) and nivolumab (1.9 months) (p = 0.002). Metastatically, the highest ORR was observed in patients treated with dabrafenib plus trametinib (54.5%), followed by ICI (52.0%) and chemotherapy (33.3%). Similarly, DCR was superior for dabrafenib plus trametinib (86.3%) compared to ICI (70.8%) and chemotherapy (66.6%). Median PFS was 9.7 months (95% CI 7.2–12.2 months) in the whole group. This was 14.3 months (95% CI 9.6–19.0 months) with ICI, 10.3 months (95% CI 4.2–16.4 months) with BRAF/MEK inhibitors and 6.3 months (95% CI 4.7–7.9 months) with chemotherapy, which was statistically significant (p < 0.001). Dabrafenib plus trametinib showed the longest median OS (53.5 months) in metastatic patients and was significantly better than chemotherapy (33.6 months) (p < 0.001). BRAF V600E mutation, RFS > 6 months, ORR are all independent factors for OS prognosis. In our study, dabrafenib plus trametinib combination was more effective in adjuvant treatment of melanoma, while immunotherapy was more effective in metastatic first-line treatment. Aim: Although most defects can close by primary suturing after radical surgery of gynecological malignancies, different reconstruction options are available when large defects that require reconstruction occur. In this study, we present the treatment strategy and results for patients who underwent reconstruction after resection for gynecological cancer in the vulva and perineum. Material and Methods: A total of 18 patients who underwent reconstruction between May 2018 and July 2020 were included in this retrospective study. Demographics and clinical data, the resection operation, characteristics of the defect, and the reconstruction methods applied were evaluated. Postoperative treatment strategy and complication rates were evaluated. Results: The mean age was 62.3±13.2 (42-83) years. 88.9% of the patients had additional diseases. Pelvic exentration was performed in 5 (27.8%) patients, anterior resection in 2 (11.1%) patients and vulvectomy in 11 (61.1%) patients. The most common malignancy was squamous cell carcinoma, and mean defect size was 106±97 (12-476) cm2. Reconstruction was performed with a local fasciocutaneous flap in 16 (88.9%) patients, pedicled rectus myocutaneous flap in one (5.6%) patient, and skin graft in one (5.6%) patient. Wound complications occurred in 5 (27.8%) patients, partial flap necrosis in one (5.6%) patient, and recurrence in one (5.6%) patient in the long term. Conclusion: It is possible to reconstruction most of the vulva and perineal defects with local flaps after oncological resections, Considering the characteristics of the area and patient comorbidities, it should be kept in mind that prolonged wound problems may be seen, especially in vulvectomy patients.

Aim: Gestational diabetes mellitus (GDM) is a condition that can cause serious maternal and fetal complications. Platelets are one of the cells that play an important role in inflammation in our body. In this study, we aimed to investigate whether there is a relationship between platelet values obtained by a simple blood count and GDM. Materials and Methods: Two groups consisting of 60 individuals with GDM and 250 healthy populations were created. Demographic characteristics, the sugar loading test results performed between 24th-28th weeks and simple complete blood count results including platelets, plateletcrit (PCT), platelet distribution width (PDW), and mean platelet volume (MPV)) were analyzed with SPSS. Results: When comparing the ages, BMI, average blood pressure, heights, and gestational weeks of both groups, there was no any significant differences (p>0.05). In terms of the hemoglobin, platelet, and white blood cell counts, there wasn't any significant differences between the two groups (p>0.05). MPV, PDW and PCT values were significantly increased in patients with GDM compared to healthy pregnant women (p<0.02, p<0.009, and p<0.001, respectively). Specifically, there was a significant positive correlation was found between PCT and the 1st hour glucose level (r: 0.585, p=0.001). Conclusion: We predict that a simple blood count (especially MPV, PDW and PCT values) performed during the first routine visit of pregnant women may be useful in predicting the risk of GDM. Bu çalışmanın amacı, İmam Hatip Lisesi öğrencilerinin İmam Hatip Lisesi, İmam Hatip Lisesi meslek dersi öğretmeni ve İmam Hatip Lisesi idaresine ilişkin algılarını metaforlar yoluyla tespit etmektir. Araştırma, nitel araştırma desenlerinden fenomenolojik (olgubilim) desende tasarlanmıştır. Amaçlı örnekleme yöntemlerinden ölçüt örnekleme yoluyla belirlenen çalışma grubu, 2018-2019 eğitim öğretim yılında 5 farklı İmam Hatip Lisesinde 9. ve 12. sınıfta öğrenim gören 412 öğrenciden oluşmaktadır. Araştırmanın verileri katılımcıların, “İmam Hatip Lisesi…gibidir; çünkü…”, “İmam Hatip Lisesi Meslek dersi öğretmeni… gibidir; çünkü…”, “İmam Hatip Lisesi idaresi…gibidir; çünkü…” cümlelerini tamamlamaları yoluyla toplanmıştır. Elde edilen verilerin analizinde tümevarımcı içerik analizi yöntemi kullanılmıştır. İçerik analizi sonucu oluşturulan temalar/kategoriler SPSS programında frekans, yüzde ve çapraz tablo olarak analiz edilmiştir. Araştırma sonucuna göre, öğrencilerin “imam hatip lisesi” ile “imam hatip lisesi meslek dersleri öğretmenlerine” ilişkin algılarının olumlu olduğu, fakat İmam Hatip Lisesi idarecilerine ilişkin algılarının ise olumsuz olduğu belirlenmiştir. Bunun yanında erkek öğrencilerin, imam hatip ortaokulundan mezun öğrencilerin ve 9. Sınıf öğrencilerinin metaforik algılarının daha olumlu olduğu görülmüştür.Özet: Bu çalışmanın temel amacı, İmam Hatip Lisesi öğrencilerinin İmam Hatip Lisesi, İmam Hatip Lisesi meslek dersi öğretmeni ve İmam Hatip Lisesi idaresine ilişkin algılarını metaforlar yoluyla tespit etmektir. Türk eğitim sistemi içerisinde yer alan İmam Hatip Liseleri 1924 yılında açılmış, hem mesleğe hem de yükseköğretime öğrenci yetiştiren bir ortaöğretim kurumudur. Öğretim programında yer alan derslerin ortalama %40’ı dini mesleki dersler, %60’ı ise kültür derslerinden oluşmaktadır. Meslek derslerini meslek dersleri öğretmenleri vermektedir. Bu okulların yönetiminde, meslek dersi öğretmenlerinin yanında farklı branşlara ait idareciler de bulunmaktadır. Türkiye’de 2018-2019 eğitim öğretim yılı itibariyle ortaöğretime devam eden öğrencilerin %11’i bu okulları tercih etmektedir. Bu araştırma, nitel araştırma desenlerinden fenomenolojik (olgubilim) desende tasarlanmıştır. Araştırmanın verileri katılımcıların, “İmam Hatip Lisesi…gibidir; çünkü…”, “İmam Hatip Lisesi Meslek dersi öğretmeni… gibidir; çünkü…”, “İmam Hatip Lisesi idaresi…gibidir; çünkü…” cümlelerini tamamlamaları yoluyla elde edilmiştir. Elde edilen verilerin analizinde tümevarımcı içerik analizi yöntemi kullanılmıştır. İçerik analizi sonucu oluşturulan temalar/kategoriler (olumlu-olumsuz) SPSS programında frekans, yüzde ve çapraz tablo olarak analiz edilmiştir. Araştırmada, öğrencilerin, İmam Hatip Lisesine ilişkin 140, meslek dersi öğretmenlerine ilişkin 147, idarecilere ilişkin 161 metafor ürettikleri görülmüştür. Ürettikleri metaforların analiz edilmesi sonucunda, temelde olumlu ve olumsuz olmak üzere iki kategoride toplandığı anlaşılmıştır.Araştırmaya katılan öğrencilerin ortalama üçte ikisi (%63,3) İmam Hatip Lisesine ilişkin olumlu metafor (93) üretirken, üçte biri (%36,7) ise olumsuz (47) metafor üretmiştir. Olumlu olarak üretilen metaforlar içerisinde en sık tekrarlanan metaforların “Evimiz (33/%8,01)”, “Aile ( 24/% 5,83) ve “Kur’an Kursu (14/%3,40)” metaforlarının olduğu; olumsuz olarak en sık tekrarlanan metaforların ise “Cezaevi/hapishane (76/%18,45)”, “Düz lise (12/%18,45)”, “Kötü bir yer (6/%6)” şeklinde olduğu görülmüştür. Bu bulgulara göre öğrencilerin yarıdan fazlasının İmam Hatip Lisesine ilişkin olumlu metaforik algıya sahip oldukları söylenebilir. Araştırmada kız öğrencilerin erkek öğrencilere göre İmam Hatip Lisesini daha fazla olumsuz algıladıkları görülmüştür. Araştırmaya katılan öğrencilerin dörtte üçü (%77,4) meslek dersi öğretmenlerine ilişkin olumlu (98) metafor üretirken, beşte birlik kısmın (%22,6) ise olumsuz (49) metafor ürettikleri belirlenmiştir. Olumlu olarak üretilen metaforlar içerisinde en sık tekrarlanan metaforların “İyi biri (34/%8,25)”, “Abla-abi (32/%7,77)”, “Aile (31/%7,52)”, metaforlarının olduğu; olumsuz olarak en sık tekrarlanan metaforların ise “Kötü biri (10/%2,42)”, “Diğer öğretmen (7/%1,70)”, “Sıkıcı (6/%1,46)” şeklinde olduğu görülmüştür. Bu bulgu öğrencilerin büyük çoğunluğunun meslek dersi öğretmenlerine ilişkin olumlu metaforik algıya sahip olduğunu göstermektedir. Araştırmaya katılan öğrencilerin üçte biri (%35) idarecilere ilişkin olumlu (67) metafor üretirken, üçte ikisinin (%65) ise olumsuz (94) metafor ürettikleri belirlenmiştir. Olumlu olarak üretilen metaforlar içerisinde en sık tekrarlanan metaforun “Aile (22/%5,34)”, “İyi biri (14/%3,40))”, “Disiplinli (7/%1,70)” metaforlarının olduğu; olumsuz olarak en sık tekrarlanan metaforların ise “Gardiyan (32/%7,77)”, “Aşırı disiplinli (30/%7,28)”, “Kötü (30/%7,28)” şeklinde olduğu görülmüştür. Öğrencilerin oluşturduğu diğer metaforların “diktatör, baskıcı, polis, hapishane, hapishane müdürü, sert, baskıcı baba, mahkeme salonu, komutan, dedektif, askerlik” gibi baskı ifade eden metaforlar olması dikkat çekicidir. Bu veriler idarecilerin öğrencilerin yarıdan fazlası tarafından olumsuz olarak algılandığı; idarecilerin öğrencilerin zihninde baskı yapan eğitimci imajı bıraktığı söylenebilir. Araştırmada kız öğrencilerin (%77) erkek öğrencilere (%41,3) göre okul idaresini daha fazla olumsuz algıladıkları görülmüştür. Sonuç olarak, İHL öğrencilerinin okullarına ve meslek dersi öğretmenlerine ilişkin metaforik algılarının olumlu yönde olduğu, fakat okul idarecilerine karşın olumsuz olduğu görülmektedir. Bu bağlamda öğrencilerin idareci algıları üzerine farklı çalışmaların yapılması gerektiği söylenebilir.

Amaç: Plasenta akreata spektrumu (PAS), özellikle gelişmekte olan ülkelerde fetomaternal morbidite ve mortalitenin ana sebebi olan obstetrik kanama için önemli bir risk faktörüdür. Bu çalışmada; PAS olan ve olmayan plasenta previa olgularının özellikleri, insidansı, maternal ve fetal sonuçlarının araştırılması amaçlanmıştır. Ek olarak, PAS ile başvuran plasenta previa vakalarının klinik özellikleri ve risk faktörlerinin analiz edilmesi amaçlanmıştır.Gereç ve Yöntemler: Plasenta previa ile birlikte PAS olan ve olmayan gebelikleri analiz etmek amacıyla retrospektif kohort bir çalışma planlandı. Ocak 2005 ile Aralık 2014 arasında yazılı ve elektronik olarak tıbbi kayıtlar gözden geçirildi. Plasenta previa tanısı ultrasonografik olarak, PAS ise klinik olarak plasentanın uterustan ayrılma aşamasında zorluk olarak tanımlandı.Bulgular: Ocak 2005 ile Aralık 2014 arasında toplam 11351 doğumun retrospektif analizi yapıldı. 387 plasenta previa tanısı konulmuş olgu izlendi. Bu olguların 11'inde çoğul gebelik saptandı ve bu vakalar çalışma dışı bırakıldı. Kurumumuzda plasenta previa insidansı ‰3.41 idi. PAS riskini ≥4 gebelik sayısı 1,56 kat, ≥2 geçirilmiş sezeryan sayısı 9,74 kat, ≥3 abort sayısı 3,83 kat ve gestasyonel hipertansiyon varlığı 29,72 kat, gestasyonel diabet varlığı 49 kat arttırmıştır. Risk faktörlerinin lojistik regresyon analizinde ≥3 abort sayısı, ≥2 geçirilmiş sezaryen sayısı ve gestasyonel hipertansiyon varlığı anlamlı olarak değerlendirilmiştir.Sonuç: Plasenta previa vakalarında; ≥3 abort sayısı, geçirilmiş sezaryen sayısının ≥2 ve gestasyonel hipertansiyon varlığı PAS gelişimi açısından en önemli risk faktörleridir. Bu risk faktörlerine sahip plasenta previa olgularının gebeliği boyunca PAS gelişimi açısından sıkı takibi maternal-neonatal morbidite ve mortalite üzerine azalan bir etkiye sahip olabileceğini düşünmekteyiz.

In this article, some mathematical properties of (i, x0)-generalized logistic-type function are presented. This four-parameter generalized function can be considered as a statistical phenomenon enhancing more vigorous survival analysis models. The aim of this work is to consider bicomplex Bernstein polynomials attached to analytic functions on a compact C2-disk and to present some approximation properties extending known approximation results for the complex Bernstein polynomials. Furthermore, we obtain and present quantitative estimate inequalities and the Voronovskaja-type result for analytic functions by bicomplex Bernstein polynomials.
