PURPOSE: To characterize the recovery of diagnostic cardiovascular procedure volumes in U.S. and non-U.S. facilities in the year following the initial COVID-19 outbreak. MATERIALS AND METHODS: The International Atomic Energy Agency (IAEA) coordinated a worldwide study called the IAEA Noninvasive Cardiology Protocols Study of COVID-19 2 (INCAPS COVID 2), collecting data from 669 facilities in 107 countries, including 93 facilities in 34 U.S. states, to determine the impact of the pandemic on diagnostic cardiovascular procedure volumes. Participants reported volumes for each diagnostic imaging modality used at their facility for March 2019 (baseline), April 2020, and April 2021. This secondary analysis of INCAPS COVID 2 evaluated differences in changes in procedure volume between U.S. and non-U.S. facilities and among U.S. regions. Factors associated with return to prepandemic volumes in the United States were also analyzed in a multivariable regression analysis. RESULTS: Reduction in procedure volumes in April 2020 compared with baseline was similar for U.S. and non-U.S. facilities (-66% vs -71%, P = .27). U.S. facilities reported greater return to baseline in April 2021 than did all non-U.S. facilities (4% vs -6%, P = .008), but there was no evidence of a difference when comparing U.S. facilities with non-U.S. high-income country (NUHIC) facilities (4% vs 0%, P = .18). U.S. regional differences in return to baseline were observed between the Midwest (11%), Northeast (9%), South (1%), and West (-7%, P = .03), but no studied factors were significant predictors of 2021 change from prepandemic baseline. A hybrid Photovoltaic/Thermal(PV/T) approach is proposed in this study based on extensive research and a comparative analysis of several hyperparameter tuning methods. The models analyzed are Linear Regression (LR), Random Forest (RF), XGBoost Regression, AdaBoost Regression, Edge Regression, Support Vector Regression (SVR), elastic net, and lasso (L) models. Grid search optimisation approach was used to maximise all of the model's hyperparameters. A detailed analysis is presented as well as the strategies for tweaking the positive and negative hyperparameters. The suggested hybrid PV/T approach is evaluated in two ways. First, the cumulative yield of solar still was obtained. Second, support vector regression, followed by the hyperparameter tuning function was used to provide the maximum accuracy of the PV output. The findings show that RF and SVR achieved the uttermost precision both before and after the use of the hyperparameter tuning approach, with r2 scores of 0.9952, 0.9935, Root Mean Squared Error values of 0.2583 and 0.5087 while utilising grid search optimisation.

BACKGROUND: The extent to which health care systems have adapted to the COVID-19 pandemic to provide necessary cardiac diagnostic services is unknown. OBJECTIVES: The aim of this study was to determine the impact of the pandemic on cardiac testing practices, volumes and types of diagnostic services, and perceived psychological stress to health care providers worldwide. METHODS: The International Atomic Energy Agency conducted a worldwide survey assessing alterations from baseline in cardiovascular diagnostic care at the pandemics onset and 1 year later. Multivariable regression was used to determine factors associated with procedure volume recovery. RESULTS: Surveys were submitted from 669 centers in 107 countries. Worldwide reduction in cardiac procedure volumes of 64% from March 2019 to April 2020 recovered by April 2021 in high- and upper middle-income countries (recovery rates of 108% and 99%) but remained depressed in lower middle- and low-income countries (46% and 30% recovery). Although stress testing was used 12% less frequently in 2021 than in 2019, coronary computed tomographic angiography was used 14% more, a trend also seen for other advanced cardiac imaging modalities (positron emission tomography and magnetic resonance; 22%-25% increases). Pandemic-related psychological stress was estimated to have affected nearly 40% of staff, impacting patient care at 78% of sites. In multivariable regression, only lower-income status and physicians psychological stress were significant in predicting recovery of cardiac testing. CONCLUSIONS: Cardiac diagnostic testing has yet to recover to prepandemic levels in lower-income countries. Worldwide, the decrease in standard stress testing is offset by greater use of advanced cardiac imaging modalities. Pandemic-related psychological stress among providers is widespread and associated with poor recovery of cardiac testing.

BackgroundThe coronavirus disease-2019 (COVID-19) pandemic significantly affected management of cardiovascular disease around the world. The effect of the pandemic on volume of cardiovascular diagnostic procedures is not known.ObjectivesThis study sought to evaluate the effects of the early phase of the COVID-19 pandemic on cardiovascular diagnostic procedures and safety practices in Asia.MethodsThe International Atomic Energy Agency conducted a worldwide survey to assess changes in cardiovascular procedure volume and safety practices caused by COVID-19. Testing volumes were reported for March 2020 and April 2020 and were compared to those from March 2019. Data from 180 centers across 33 Asian countries were grouped into 4 subregions for comparison.ResultsProcedure volumes decreased by 47% from March 2019 to March 2020, showing recovery from March 2020 to April 2020 in Eastern Asia, particularly in China. The majority of centers cancelled outpatient activities and increased time per study. Practice changes included implementing physical distancing and restricting visitors. Although COVID testing was not commonly performed, it was conducted in one-third of facilities in Eastern Asia. The most severe reductions in procedure volumes were observed in lower-income countries, where volumes decreased 81% from March 2019 to April 2020.ConclusionsThe COVID-19 pandemic in Asia caused significant reductions in cardiovascular diagnostic procedures, particularly in low-income countries. Further studies on effects of COVID-19 on cardiovascular outcomes and changes in care delivery are warranted.

Key Points: Questions: What is the risk of major clinical outcomes in patients with rheumatic heart disease (RHD) and what are the risk predictors in endemic countries? Findings: In this prospective observational study including 13 696 patients enrolled from 24 low- and middle-income countries, nearly 15% of patients died at 3 years, mostly due to heart failure or sudden death. Corrective valve surgery or valvuloplasty was independently associated with a reduced risk of death. However, although most patients were symptomatic, only about 5% underwent valve surgery in 3 years. Meaning: Improved availability and access to surgical and interventional care for patients with RHD in endemic countries are needed. Importance: Rheumatic heart disease (RHD) remains a public health issue in low- and middle-income countries (LMICs). However, there are few large studies enrolling individuals from multiple endemic countries. Objective: To assess the risk and predictors of major patient-important clinical outcomes in patients with clinical RHD. Design, Setting, and Participants: Multicenter, hospital-based, prospective observational study including 138 sites in 24 RHD-endemic LMICs. Main Outcomes and Measures: The primary outcome was all-cause mortality. Secondary outcomes were cause-specific mortality, heart failure (HF) hospitalization, stroke, recurrent rheumatic fever, and infective endocarditis. This study analyzed event rates by World Bank country income groups and determined the predictors of mortality using multivariable Cox models. Results: Between August 2016 and May 2022, a total of 13 696 patients were enrolled. The mean age was 43.2 years and 72% were women. Data on vital status were available for 12 967 participants (94.7%) at the end of follow-up. Over a median duration of 3.2 years (41 478 patient-years), 1943 patients died (15% overall; 4.7% per patient-year). Most deaths were due to vascular causes (1312 [67.5%]), mainly HF or sudden cardiac death. The number of patients undergoing valve surgery (604 [4.4%]) and HF hospitalization (2% per year) was low. Strokes were infrequent (0.6% per year) and recurrent rheumatic fever was rare. Markers of severe valve disease, such as congestive HF (HR, 1.58 [95% CI, 1.50-1.87]; P <.001), pulmonary hypertension (HR, 1.52 [95% CI, 1.37-1.69]; P <.001), and atrial fibrillation (HR, 1.30 [95% CI, 1.15-1.46]; P <.001) were associated with increased mortality. Treatment with surgery (HR, 0.23 [95% CI, 0.12-0.44]; P <.001) or valvuloplasty (HR, 0.24 [95% CI, 0.06-0.95]; P =.042) were associated with lower mortality. Higher country income level was associated with lower mortality after adjustment for patient-level factors. Conclusions and Relevance: Mortality in RHD is high and is correlated with the severity of valve disease. Valve surgery and valvuloplasty were associated with substantially lower mortality. Study findings suggest a greater need to improve access to surgical and interventional care, in addition to the current approaches focused on antibiotic prophylaxis and anticoagulation. This prospective observational study investigates mortality rates and predictive risk factors for rheumatic heart disease in 24 endemic low- and middle-income countries and evaluates interventions like corrective valve surgery and valvuloplasty. Diabetic Retinopathy (DR), a common diabetes complication affecting retinal blood vessels, may result in vision damage if not addressed promptly. Early and accurate detection is crucial for effective management, and Deep Learning (DL) techniques offer promising tools for the automated screening of Retinal Fundus Images (RFIs). This approach enhances objectivity, reduces inter-observer variability, and has the potential to extend the DR diagnoses to regions with limited access to specialized medical professionals. This manuscript presents the design of the Beluga Whale Optimizer (BWO) with Deep Learning (DL)-assisted DR Diagnosis on RFIs (BWODL-DRDRFI) technique in the Internet of Things (IoT) platform. The proposed technique automatically examines the RFIs for identifying and classifying DR. During the IoT-based data-gathering procedure the patient utilizes a head-mounted camera for capturing the RFI and sends it to a cloud server. Median Filtering (MF)-based image preprocessing is performed to eradicate noise. Next, the BWODL-DRDRFI technique exploits the ShuffleNet-v2 approach to derive feature vectors. For DR recognition, the BWODL-DRDRFI technique applies a deep Stacked AutoEncoder (SAE) model. Finally, the BWO model optimally adjusts the hyperparameter values of the DSAE model for greater classification performance. The simulation output of the BWODL-DRDRFI approach can be examined on a standard image dataset and the outputs are computed on discrete measures. The simulation result highlighted the enhanced performance of the BWODL-DRDRFI approach in the DR diagnosis process. [ABSTRACT FROM AUTHOR]

The Photovoltaic (PV) cell converts a portion of incident irradiation of visible wavelength into electricity and the remaining into heat, thereby decreasing the electrical efficiency. Hence, it is necessary to transfer the heat generated in the module by a cooling medium, thereby maintaining the operating temperature within the operating limit. This study discusses the feasibility of cooling the monocrystalline and polycrystalline modules by using rehashed edible oil (coconut oil, sesame oil, and peanut oil) with an integrated oil tank attached to the backside of the module. This study has used the environmentally friendly rehashed edible oil as a coolant, which can be used as an alternate to the toxic mineral oil. Hence, this study can prevent the consumption of reused edible oil along with the food, which causes harmful effects on human health. The rehashed edible oil flows from the storage tank through the backside of the module and is collected in another storage tank, which can be reused. Also, the performance of the monocrystalline and polycrystalline modules under different rehashed edible oil shows a significant reduction in module temperature and improvement in the efficiency of the module. The performance of the peanut oil is found to be superior in improving the performance by 14.0 % and 16.8 % on monocrystalline and polycrystalline modules respectively.

- Abstract
The inherently low sensitivity of nuclear magnetic resonance (NMR) spectroscopy is the major limiting factor for its application to elucidate structure and dynamics in solids. In the solid state, nuclear spin hyperpolarization methods based on microwave-induced dynamic nuclear polarization (DNP) provide a versatile platform to enhance the bulk NMR signal of many different sample formulations, leading to significant sensitivity improvements. Here we show that 1H NMR hyperpolarization can also be generated in solids at high magnetic fields by optical irradiation of the sample. We achieved this by exploiting a donor–chromophore–acceptor molecule with an excited state electron–electron interaction similar to the nuclear Larmor frequency, enabling solid-state 1H photochemically induced DNP (photo-CIDNP) at high magnetic fields. Through hyperpolarization relay, we obtained bulk NMR signal enhancements εHby factors of ∼100 at both 9.4 and 21.1 T for the 1H signal of o-terphenyl in magic angle spinning (MAS) NMR experiments at 100 K. These findings open a pathway toward a general light-induced hyperpolarization approach for dye-sensitized high-field NMR in solids.

Abstract Introduction Phenomena related to reward responsiveness have been extensively studied in their associations with substance use and socioemotional functioning. One important task in this literature is the Monetary Incentive Delay (MID) task. By cueing and delivering performance‐contingent reward, the MID task has been demonstrated to elicit robust activation of neural circuits involved in different phases of reward responsiveness. However, systematic evaluations of common MID task contrasts have been limited to between‐study comparisons of group‐level activation maps, limiting their ability to directly evaluate how researchers’ choice of contrasts impacts conclusions about individual differences in reward responsiveness or brain‐behavior associations. Methods In a sample of 104 participants (Age Mean = 19.3, SD = 1.3), we evaluate similarities and differences between contrasts in: group‐ and individual‐level activation maps using Jaccard's similarity index, region of interest (ROI) mean signal intensities using Pearson's r, and associations between ROI mean signal intensity and psychological measures using Bayesian correlation. Results Our findings demonstrate more similarities than differences between win and loss cues during the anticipation contrast, dissimilarity between some win anticipation contrasts, an apparent deactivation effect in the outcome phase, likely stemming from the blood oxygen level‐dependent undershoot, and behavioral associations that are less robust than previously reported. Conclusion Consistent with recent empirical findings, this work has practical implications for helping researchers interpret prior MID studies and make more informed a priori decisions about how their contrast choices may modify results.
