The paper presents a case of 25 female patient, who is a clinical example of a triple comorbidity – she was a preterm child in which at the age of 2 autism spectrum disorder (ASD) was diagnosed, and at the age of 3 type 1 diabetes mellitus (T1DM). The described case is special because autism coincides not only with diabetes, but also with blindness due to retinopathy of prematurity. In such a situation, the choice of the right treatment method has become a clinical challenge. Considering the patients’ health status and life conditions (comorbidity of T1DM, autism and blindness, abusive, not engaged father, difficult financial situation) very good treatment results as far as T1DM is concerned were achieved: her HbA1c values available from last 3 years ranged from 6.5-7.3% Also, regardless of many obstacles and life crises, the patient maintains possibly the best in her conditions, psychological well-being. The paper can be a source of knowledge and inspiration, presenting challenges and possibilities in treatment of people with T1DM, autism, and other comorbidities. From the therapeutic point of view, it indicates that well planned, individualized therapy may be the basis for creating optimal conditions for the development and improvement of the quality of life of patients.

Cel pracyWyniki analizy zaprezentowane w artykule są doniesieniem z badań poświęconych samobójstwom na torach kolejowych.MetodaPrzeprowadzono analizę epidemiologiczną serii przypadków z wykorzystaniem danych pochodzących z Komendy Głównej Policji. Wskaźniki samobójstw na kolei w Polsce w latach 2007-2018 porównano z ogólną liczbą samobójstw w Polsce i krajach UE.WynikiWyniki: W latach 2007-2018 średni odsetek zgonów w wyniku samobójstwa na kolei w ogólnej liczbie samobójstw popełnionych w Polsce wyniósł 2,06%. W latach: 1999–2018 zwiększała się liczba prób samobójczych na kolei w ogóle oraz prób samobójczych nie zakończonych zgonem. Od roku 2012 obserwuje się wzrost liczby prób samobójczych na torach kolejowych oraz utrzymującą się liczbę zgonów samobójczych. W latach 2017-2018 rzucenie się pod pociąg, aby zakończyć życie, było trzecią najczęściej wybieraną metodą popełnienia samobójstwa w Polsce. Chociaż stanowi jedynie niewielki procent wśród wszystkich metod samobójczych, w Polsce była wybierana stosunkowo częściej w porównaniu do innych krajów europejskich.WnioskiPolska należy do niewielkiej grupy krajów europejskich, w których liczba zgonów samobójczych, w ciągu ostatnich 20 lat, cechowała się tendencję wzrostową. Rośnie liczba nieudanych prób samobójczych na kolei. Wskaźnik zgonów na torach kolejowych w Polsce był znacznie wyższy niż średni wskaźnik dla krajów UE.

The aim of the study was to check the prevalence of unipolarity (depression), bipolarity, as well as the quality of sleep and temperament traits in patients with type 1 diabetes (T1DM) who are provided with optimal conditions of diabetes care and to identify possible risk factors connected with affective traits.Out of the 107 T1DM patients, 78 (54 females, 24 males) were included for the analysis (HbA1c [%] 7.11 ± 1.0, BMI [kg/mIn QIDS-S (full explanation and abbreviation 26 patients (33.8%) were screened positive for depression, in PHQ (full explanation and ab 57.7% of the patients (45 patients) had symptoms of depression (age was negatively correlated with PHQ score [r = -0.26; p = 0.023]). In CES-D 16 (20%) of the patients assessed their present affect as depressed. None of the analysed clinical variables correlated with depression scores. In the Mood Disorder Questionnaire (MDQ), 16 patients reported having symptoms of bipolarity (20.5% vs. 79.5%). Hypomania Checklist (HCL) analysis indicated 10 patients with bipolar traits (14) (14.9% vs. 85.1%). None of the analysed clinical variables correlated with HCL results. 11.5% of patients were indicated to be of morning type. Morningness was more often seen in younger patients (r = 0.39; p = 0.001). As many as 46.6% declared that they had poor sleep quality. The temperament traits analysis correlated with clinical parameters: Cyclothymic temperament trait was negatively correlated with age (r = -0.30; p = 0.007) and positively with HbA1c level (r = 0.30; p = 0.025). Hyperthymic temperament was positively correlated with (BMI r = 0.28; p = 0.016). Quality of sleep was highly correlated with depressive symptoms CESD (r = 0.61, p = 0.001), PHQ Score (r = 0.62; p = 0.001), QISD (r = 0.68; p = 0.001) and bipolarity MDQ (p = 0.50, p = 0.001) and HCL (r = 0.42, p = 0.001). In addition, QIDS was shown to be correlated with the following features of temperament: depressive factor (r = 0.41; p = 0.001), irritable factor (r = 0.53; p = 0.001), cyclothymic factor (r = 0.59; p = 0.001), anxious factor (r = 0.58, p = 0.001).The prevalence of affective disorders and poor sleep quality in the examined T1DM patients was much higher than in the general population. Even if the patients have in general good glycaemic control, their mental health condition should not be neglected. Well organised cooperation between patients, diabetologists, psychiatrists and psychotherapists is needed (Clinical Trials Identifier: NCT04616391).

Cel pracyCelem pracy było sprawdzenie czy poszczególne skutki uboczne leczenia inhibitorami kinaz tyrozynowych u pacjentów chorych na przewlekłą białaczkę szpikową mogą sprzyjać występowaniu objawów lękowych. Dodatkowo postanowiono sprawdzić, czy występują zależności pomiędzy wiekiem, płcią oraz czasem trwania leczenia a nasileniem lęku w podziale na występowanie poszczególnych skutków ubocznych.MetodaBadanie zostało przeprowadzone w Poradni Hematologicznej Szpitala Uniwersyteckiego w Krakowie, objęto nim 91 chorych. Do analizy występowania objawów lękowych zastosowano ankietę własną, Kwestionariusz Davida Goldberga GHQ-28 oraz czterowymiarowy Kwestionariusz 4DSQ.WynikiNajczęściej wymienianymi skutkami ubocznymi leczenia były: bóle kości i stawów oraz skurcze i bóle mięśni, zatrzymanie wody oraz męczliwość. Średni czas trwania choroby wyniósł 10 lat. Męczliwość, nudności/niestrawność, częste infekcje, bóle kości i stawów, ból brzucha oraz utrata apetytu były skutkami ubocznymi leczenia IKT, które wpływały na zwiększony poziom objawów lękowych. Wiek, długość trwania leczenia oraz płeć w minimalnym zakresie wpływały na występowanie objawów lękowych.WnioskiPraca wskazuje na duże znaczenie występowania skutków ubocznych leczenia IKT dla pojawiania się objawów lękowych. Praca lekarzy, psychologów oraz firm farmaceutycznych nad zmniejszeniem/ złagodzeniem skutków ubocznych może wpłynąć na lepszą jakość życia tych pacjentów. Dodatkowo, prowadzone od kilku lat próby odstawienia leczenia niektórymi IKT (imatynibem i nilotynibem) u pacjentów uzyskujących głęboką odpowiedź molekularną może sprzyjać poprawie ich stanu psychicznego.

Cel pracyW artykule przedstawiono wyniki prac nad polską adaptacją Samoopisowej Skali Mądrości (SAWS). Przedstawiono właściwości psychometryczne polskiej wersji narzędzia.MetodaBadanie miało charakter ilościowy i zostało przeprowadzone w schemacie korelacyjnym. Respondenci wypełnili zestaw kwestionariuszy. W badaniu wzięło udział 880 osób w wieku od 60 do 80 lat (M = 68,15, SD = 5,96). Oprócz SAWS zastosowano sześć innych metod psychologicznych. Dobór narzędzi pomiarowych był celowy.WynikiOstateczna polska wersja SAWS zawiera z 40 pozycji (w tym 36 diagnostycznych), które składają się na 5 wymiarów mądrości: (1) Doświadczenia życiowe, (2) Regulacja emocjonalna, (3) Reminiscencja i refleksyjność, (4) Otwartość i (5) Humor. Współczynnik rzetelności dla całej skali (36 pozycji) wyniósł a = 0,92 (bardzo wysoki). Rzetelności (α-Cronbach) dla poszczególnych skal waha się od α=0,60 do α=0,84. Cel pracyCelem naszego badania obserwacyjnego było określenie , czy wybrane cechy osobowości są skorelowane ze stopniem wyrównania metabolicznego/czasem trwania cukrzycy u dorosłych pacjentów z T1DM.MetodaDane zebrano od 56 dorosłych pacjentów (40 mężczyzn) z T1DM leczonych w ośrodku o trzecim stopniu referencyjności. Cechy osobowości „Wielkiej Piątki” oceniano za pomocą kwestionariusza “NEO-Five Factor Inventory”. Kilka zmiennych uzyskano po sczytaniu osobistych pomp insulinowych, glukometrów i zaślepionego systemu ciągłego monitorowania glikemii (CGM).WynikiWszystkie cechy osobowości oprócz neurotyczności (niski poziom cechy) wykazywały przeciętne nasilenie. Ugodowość była skorelowana z większością zmiennych uzyskanych z systemu CGM. Większa sumienność wiązała się z dłuższym czasem trwania cukrzycy. Większa neurotyczność korelowała z większą zmiennością glikemii (GV), podczas gdy wysoka ekstrawersja wiązała się z niższym wskaźnikiem GV. Niższa Otwartość była związana z wydłużeniem czasu klinicznie istotnej hipoglikemiiWnioskiNasze badanie sugeruje, że cechy osobowości przejawiają się w indywidualnym podejściu do leczenia cukrzycy i regulacji emocji, co przekłada się również na stosunek do leczenia. BACKGROUND While introducing new technologies and methods of treatment of T1DM it seems essential to monitor whether modern technologies in diabetes treatment may improve the psychological, emotional status of the patients. OBJECTIVE To assess the baseline psychological parameters of the T1DM patients included into the study of the direct transition from Multiple Daily Injections (MDI) and Self-Monitoring of Blood Glucose (BGM) to the MiniMed™ 780G Advanced Hybrid Closed Loop (AHCL) and to evaluate changes in the psychological well-being and quality of life after the transition in these individuals versus control group. METHODS The trial was a two-center, randomized controlled, parallel group study. 41 T1DM patients managed with MDI/BGM were enrolled and randomized either to AHCL or MDI/BGM groups. 37 participants (age 40.3±8.0 years; duration of diabetes 17.3 ± 12.1 years; HbA1c 7.2 ± 1.0%) completed the study (20 in AHCL and 17 in the MDI/BGM groups). Psychological parameters (the level of stress, coping mechanisms, level of anxiety, self-efficacy level, acceptance of illness, locus of control of the illness, life satisfaction, quality of life) were measured at baseline and at the end of the study using 10 psychological questionnaires. RESULTS Study participants’ general level of stress was higher than in general population (p=0.001), their coping strategies were significantly more effective and they had significantly higher level of self-efficacy (p CONCLUSIONS The results indicate that transitioning from MDI/BGM treatment to AHCL in patients naïve to technology may significantly improve psychological well-being and quality of life of the patients within 3 months. The rapidity of these changes suggests that they may be related to the significant improvement in glycemic outcomes.

The analysis presented in the article is the first study of suicide on railway tracks in Poland.An epidemiological assessment was conducted, study type: case series, population-based with use of the data from the National Police Headquarters. The rates of railway suicides in Poland in the years 2007-2018 were compared to the overall number of suicides in Poland and in the Europe.In 2007-2018, the average share of deaths by railway suicide in the overall number of suicides committed in Poland amounted to 2.06%. Between the years: 1999 -2018 there is a growing number of railway suicide attempts and railway suicide attempts without deaths. Since 2012, an increase of suicide attempts on railway tracks, along with a constant number of suicide deaths, has been observed. In 2017 - 2018, throwing oneself under a train to end one's lifewas the third most commonly chosen method in Poland. Although it accounted for only a small percentage of all methods of suicide, it was chosen relatively more often than in other European countries.Poland belongs to a small group of European countries where the number of deaths by suicide has displayed an upward tendency in the course of 20 years. There is a growing number of unsuccessful railway suicide attempts. The index of deaths on railway tracks in Poland, was considerably higher than the average index for the EU.

Objective: The aim of this study was to evaluate the outcomes of transitioning to the MiniMed™ 780G advanced hybrid closed-loop (AHCL) system in adult individuals with type 1 diabetes (T1DM) naïve to CSII and CGM technologies. Research Design and Methods: The trial was a two-center, randomized controlled, parallel group study evaluating individuals with T1DM aged 26-60 years managed with Multiple Daily Injection (MDI) and self-monitoring of blood glucose (BGM), with an HbA1c Results: 41 participants were recruited and randomized to either the AHCL (20) or the MDI+BGM (21) group. 37 participants (age 40.3±8.0 years; duration of diabetes 17.3 ± 12.1 years; BMI 25.1±3.1 kg/m2; HbA1c 7.2% ± 1.0) completed the study. Time spent in target (TIR) increased from 69.3±12.3% at baseline to 85.0±6.3% at 3 months in the AHCL group, while remaining unchanged in the control group: treatment effect 21.5% [95% CI 15.7, 27.3%]; p < 0.001. The Time Below Range ( Conclusions: People with T1DM naïve to CSII and CGM technologies initiating AHCL significantly and safely improved their glycemic control, as well as their quality of life and psychological well-being.

The end of the year is a time for some summaries, and 2022 was a year of many events and crises affecting our mental health: the ongoing pandemic, the war in Ukraine, the economic crisis with rocketing fuel prices or inflation to name but a few. But after the shortest day of the year, the longer ones come ahead, and just as light finally triumphs over darkness, so are we looking forward to the upcoming 2023 with hope. This issue of the APP opens with the paper by Magdalena Chęć et al., reflecting on the relationship between temperament and addiction to psychoactive substances in young adults.

Cel pracyCelem pracy była ocena nasilenia lęku i depresji u kobiet leczonych z powodu niepłodności w porównaniu z nasileniem tych parametrów klinicznych u kobiet bez problemów z rozrodem oraz ocena wpływu zaburzeń emocjonalnych na cykl owulacyjny.MetodaOwulację oceniano za pomocą badań hormonalnych i ultrasonograficznych. Stan emocjonalny został zbadany za pomocą Inwentarza Stanu i Cechu Lęku STAI oraz skali lęku i depresji HADSWynikiNasilenie lęku jako cechy i jako stanu oceniane przy pomocy STAI było znacznie wyższe u kobiet leczonych z powodu niepłodności niż u kobiet z potwierdzoną płodnością, a także wyższe u kobiet leczonych z powodu niepłodności, które nie zaszły w ciążę, niż u kobiet, które zaszły w ciążę. Nasilenie objawów lękowych i depresyjnych według HADS było znacznie wyższe u kobiet leczonych z powodu niepłodności niż u kobiet z potwierdzoną płodnością. Kobiety leczone z powodu niepłodności, które nie zaszły w ciążę, osiągnęły znacznie wyższe nasilenie objawów depresyjnych niż kobiety w ciąży. W surowicy kobiet leczonych z powodu niepłodności poziomy kortyzolu, zarówno podczas owulacji, jak i po niej, dodatnio korelowały z nasileniem lęku jako cechy i stanu, a także z depresją Te same korelacje stwierdzono w grupie kobiet z potwierdzoną płodnością (z wyjątkiem objawów lękowych).Wnioski1. Lęk i depresja związane są z problemami reprodukcyjnymi. Należy zbadać naturę i kierunek tego związku. 2. Zaburzenia emocjonalne niekorzystnie wpływają na wydzielanie hormonów cyklu płciowego i wzrost endometrium. 3. Pary z problemami reprodukcyjnymi wymagają wsparcia psychologicznego w celu poprawy efektów leczenia niepłodności.

The subject of the analysis addresses seniors. Social, medical and psychological support can be classified as particularly important considering problems seniors face in their everyday life. This implies the need to narrow down the field of analysis to those aspects that the authors are able to responsibly undertake based on their professional experiences and education. Hence, the idea of this article is to explain and justify the need to develop tridisciplinary cooperation: sociology, medicine and psychology. The analysis of issues related to supporting seniors shows also the need to stimulate the processes of the seniors activation and counteracting social marginalization.

There is ample evidence that sport is a protective factor against a number of health risks, across all ages, in the general population. An in-depth understanding of energy metabolism has reasonably entailed exercise as a cornerstone in the lifestyle of almost all subjects with type 1 diabetes mellitus (T1DM). Nevertheless, individuals with T1DM often fail in accomplishing exercise guidelines and they are often less active than their peer without diabetes. Two major obstacles are feared: management of blood glucose control and hypoglycemia. Nowadays, strategies, including glucose monitoring technology and insulin pump therapy, have significantly contributed to the participation in regular physical activity, and even in competitive sports, for T1DM people. The case report presents an analysis of specific personality traits and psychological parameters of 20 years old female Polish multiple champion in weightlifting with excellent T1DM control.

In total, 41 patients with T1DM managed with MDI or SMBG were enrolled and randomized either to the AHCL or the MDI+SMBG group. Of these, 37 (90%) participants (mean age 40.3 years, SD 8.0 years; mean duration of diabetes 17.3, SD 12.1 years; mean hemoglobin A1c [HbA1c] 7.2%, SD 1.0%) completed the study (AHCL: n=20, 54%; MDI+SMBG: n=17, 46%). Psychological parameters (level of stress, coping mechanisms, level of anxiety, self-efficacy level, acceptance of illness, locus of control of illness, life satisfaction, QoL) were measured at baseline and at the end of the study using 10 psychological questionnaires. Results At baseline, the general level of stress of the examined patients was higher than in the general healthy Polish population (P=.001), but coping strategies used in stressful situations were significantly more effective and the level of self-efficacy (P Conclusions The results indicate that the patients who decided to take part in the transition study were characterized by higher levels of stress than the general healthy population but had better coping strategies and self-efficacy. Furthermore, transitioning from MDI+SMBG treatment to the AHCL in patients naive to technology may significantly improve psychological well-being and QoL within 3 months. The rapidity of these changes suggests that they may be related to the significant improvement in glycemic outcomes but also significantly less burdened diabetes self-management. Aim of the studyCommunication competencies are crucial in healthcare. Medical staff is exposed to increased emotional stress. It is still unclear, whether the level of communication competencies correlates with stress associated with professional work in healthcare providers. The aim of this study was to determine this relationship and investigate the models of coping with stress among different professional groups in the hospital wards.Subject or material and methodsMethods: The study involved interviewing medical personnel (N=93, 69 female, doctors, nurses, psychologists) to collect the psychological parameters including stress, anxiety, coping strategies and communication skills.ResultsResults: Nurses use less often task oriented coping with stress than doctors, while psychologists have higher level of avoidance-oriented style than doctors and nurses. Important associations between readiness for cooperation and the stress levels in doctors (positive) and psychologists (negative) were detected.DiscussionThe level of communication barriers correlated strongly negatively with the level of stress in psychologists. There are important relationships between cooperation and stress levels, individual communication style and anxiety levels.ConclusionsHealthcare professionals use various coping strategies when exposed to stressful situations which depend partly on their profession. The coping strategies used correlate with the communication strategies. Moreover, the level of anxiety is associated with communication competence.

Współczesny pracownik staje w obliczu pojawiających się coraz to nowych zagrożeń, uciążliwości oraz trudności. Istnienie stresogennych warunków pracy oraz zjawisk patologicznych (mobbing, pracoholizm) generuje psychologiczne koszty, obniżając tym samym efektywność i możliwości adaptacyjne pracownika. Jedną ze składowych pracy zawodowej jest syndrom wypalenia zawodowego. Zjawisko to analizowane jest w kontekście różnych zawodów. Pracownicy służby zdrowia zajmujący się zdrowiem psychicznym są na nie szczególnie narażeni. Celem niniejszej pracy jest przedstawienie wyników badań dotyczących wypalenia zawodowego w grupie lekarzy psychiatrów. Przeszukano elektroniczne bazy danych oraz publikacje empiryczne z lat 2000-2017. Aby zidentyfikować doniesienia spełniające wybrane kryteria selekcji, prace poddano krytycznej ocenie. W prezentowanych badaniach wykazano , iż syndrom wypalenia zawodowego najczęściej prezentowany jest zgodnie z modelem teoretycznym zaproponowanym przez Christinę Maslach (obejmujący wymiar depersonalizacji, emocjonalnego wyczerpania oraz poczucia braku osobistych osiągnięć). Wśród czynników ryzyka wymienia się m.in.: zbyt długi czas pracy , niskie zarobki, negatywny obraz siebie, specyfikę leczonych pacjentów, konflikty ze współpracownikami, poziom satysfakcji z wykonywanej pracy, niewystarczająca liczba superwizji swojej pracy. Otrzymane rezultaty pokazały, iż wypalenie zawodowe stanowi istotną składową życia zawodowego lekarzy psychiatrów. Zatem warto byłoby objąć szczególną uwagą już studentów medycyny, aby zapoznać ich z tematyką wypalenia zawodowego. Wymagana byłaby troska o ich przygotowanie merytoryczne, kompetencje interpersonalne, kształtowanie realistycznych przekonań zawodowych oraz wyposażenie w konstruktywne umiejętności radzenia sobie z sytuacjami stresogennymi.

Happiness and hope are essential parts of human health. One of the main purposes of health care, including diabetes care, are to achieve happiness and a sense of purpose in life. Material and methodDuring educational workshops a short survey concerning the level of happiness and hope for success in the group of Type 1 Diabetes Mellitus (T1DM) patients was carried out. 120 patients anonymously filled in Subjective Happiness Scale (SHS) and a Hope for Success Questionnaire (KNS).Results and conclusionsThe level of subjective happiness in T1DM patients was lower than in general population for both sexes, for all age categories apart from people older than 50, who seem to be happy and satisfied with their life with no differences compared to the general population. In terms of hope for success, T1DM teenagers and adults aged 27-50 did not differ from the general population. T1DM patients older than 50 turned out to have higher hope for success in life that other T1DM patients. Special attention should be paid to patients in young adulthood (18-26), who seem to be the most pessimistic group of T1DM patients, with low self-esteem and low believe in their possibilities.

The eleventh revision of ICD - ICD-11 - was released by WHO in June 2018 and approved by the World Health Assembly (WHA) in May 2019. The ICD-11 is an open source for the global community of clinicians in various settings, researchers, consumers, administrators, policy makers and governments. Aim: The aim of the study was to investigate the correlation between symptoms of overactive bladder (OAB) and severity of depressive symptoms in patients treated for depression. Method: 102 patients (43 males, 59 females) aged 20–67 (M = 46.1±11,3) treated for depression were included in this cross-sectional analysis. OAB symptoms were examined with the International Prostate Symptom Score (IPSS) and International Consultation on Incontinence Questionnaire Overactive Bladder Module (ICIQOAB). OAB-related quality of life was assessed with the International Consultation on Incontinence Questionnaire Overactive Bladder Quality of Life Module (ICIQ-OABqol). Depressive symptoms were assessed with the 17-item Hamilton Depression Rating Scale (HDRS) and Quick Inventory of Depressive Symptomatology - Self Report (QIDS-SR). Results: Symptoms of OAB assessed with IPSS and ICIQ-OAB were more severe in women than in men. In the entire group highest scores in IPSS of assessed OAB symptoms were observed in nocturia (1,53±1,64) followed by urinary frequency (1,19±1,48) and lowest in urgency (0,80±1,28). Symptoms of OAB (based on the ICIQOABqol total score) were more bothersome in women (48,74±29,18), than in men (39,18±17,70). In the whole group and in women the QIDS-SR total score correlated with the ICIQ-OAB score ( p < 0,05). The total QIDS-SR score correlated with the ICIQ-OABqol score in entire group (p < 0,05), in women (p < 0,05) and in men (p < 0,05). Patients suffering from at least moderate depression assessed with QIDS-SR had significantly more pronounced symptoms of urinary frequency (p = 0,005) and urgency (p = 0,039). A number of other significant (p < 0.05) correlations were observed between the total ICIQ-OAB score and certain items’ scores in the GHQ-30 both in women. Conclusions: OAB symptoms are common among patients treated for depressive disorders. There is a correlation between severity of depressive symptoms and OAB. OAB is bothersome and affects the quality of life in patients treated for depression. Comorbidity of OAB and depression has clinical significance and that should enhance interdisciplinary treatment approaches. Due to limitations of this study, further researches are essential to reveal more details of the correlation between OAB and depression.

Suicide prevention is an important issue. Every year almost 800,000 people die by suicide worldwide, while alarmingly, many more individuals attempt to take their own life. However, despite the existence of evidence-based suicide preventive strategies, suicidal behaviours remain difficult to prevent. This overview will address some of the obstacles that arise in preventing suicide. Despite the existing evidence about effective prevention methods, there are still many obstacles in preventing suicide. Nonetheless, these are obstacles which can be overcome. A continued effort needs to be made to ensure that evidence-based suicide prevention methods are being implemented amongst all levels of society, particularly by healthcare professionals and policymakers alike.

BACKGROUND The COVID-19 pandemic has challenged the worldviews of most people. Social isolation after the COVID-19 lockdown has not only led to economic difficulties but also resulted in adverse psychological reactions. As in most countries, including Poland, this situation has been very challenging for patients with type 1 diabetes mellitus (T1DM). In Poland, a crisis intervention team for patients with T1DM was established. The goal of the team was to provide psychological support for these patients, if needed, and to present information concerning how these patients may obtain medical consultations and prescriptions. OBJECTIVE We aimed to analyze the psychological parameters and main emotional reactions of patients with T1DM during the COVID-19 lockdown. METHODS An email with information concerning the possibility of having a web-based consultation with psychologists and psychiatrists and an attached set of psychological tests was sent to all patients with T1DM who were under the care of an outpatient diabetes clinic. The consultations were performed by licensed clinical psychologists and psychologists. This study was approved by the Bioethics Committee of the Jagiellonian University in Krakow, Poland. RESULTS The patients who decided to use psychological support had statistically higher levels of anxiety (state P=.043; trait P=.022), stress (P=.001) than those of patients from the group who did not seek support. CONCLUSIONS The presented intervention team may be perceived as an example of important and successful cooperation and communication between specialists of different fields of medicine (diabetology, psychiatry, and psychology) in a crisis situation.

Altered methylation of the FKBP5 gene has been observed in various mental disorders and attributed to the effects of adverse childhood experiences (ACEs). However, the level of FKBP5 methylation has not been investigated in patients with psychotic disorders. Therefore, in this study we aimed to determine the FKBP5 methylation in patients with psychosis and controls, taking into account the effects of ACEs. Participants were 85 patients with psychotic disorders, including first-episode psychosis (FEP) patients and acutely relapsed schizophrenia (SCZ-AR) patients, as well as 56 controls. The level of four CpG sites at the FKBP5 gene was determined in the peripheral blood leukocytes using pyrosequencing. After controlling for potential confounding factors, the level of FKBP5 methylation at one out of four tested CpG sites was significantly lower in FEP patients compared to other groups of participants. Significant main effects of parental antipathy and sexual abuse on the level of FKBP5 methylation were observed at the differentially methylated CpG site. Participants reporting this category of ACEs had significantly lower levels of FKBP5 methylation at this CpG site. Lower levels of FKBP5 methylation were associated with better cognitive performance and higher functional capacity in patients with psychosis. In controls, lower methylation of FKBP5 was related to worse performance of immediate memory and language skills. Our findings suggest that hypomethylation of the FKBP5 appears at early stages of psychosis and might be associated with a history of ACEs as well as less severe clinical manifestation.

The current issue, which we are handing over to you, covers a very wide spectrum of topics close to the heart of psychiatrists and psychologists. Undoubtedly, the article by Henryk Axer is supporting the currently developing model of community treatment in mental disorders. Such small community – based residential programs offer mental health treatment and 24/7 personal care support for adults experiencing severe mental illness. The principles of dealing with residents are of key importance and, according to the author, can gradually improve the independent functioning and coping mechanisms of patients. What are these methods? Take a look at the article, it is definitely worth it, especially at a time when we are debating about the reform of psychiatry in Poland.

In the time this issue of Archives of Psychiatry and Psychotherapy (APP) is released, the whole world is striving against the third wave of the SARS-CoV-2 pandemic. COVID-19 with its largely unpredictable outcomes became a hard challenge for professionals representing nearly all medical specialities, including psychiatry. The psychological burden of a new disease that took so many lives and left so many people in mourning is not to be underestimated. Scientific literature focused on the recent pandemic is successively growing (e.g., 23,634 unique published articles have been indexed on Web of Science and Scopus between 1 January and 30 June 2020! [1]) and few related manuscripts were already presented in APP last year [2,3].

Analizie statystycznej poddano wyodrębnioną grupę 466 pacjentów nie zgłaszających wyjściowo MS, z czego u 4% wystąpiły MS pod koniec leczenia.WynikiStwierdzono szereg postaw deklarowanych przez pacjentów, które w istotnym stopniu predysponowały do wystąpienia MS pod koniec cyklu intensywnej psychoterapii. Należały do nich: agresja fizyczna wobec bliskich (pWnioskiU pacjentów deklarujących wymienione postawy, wyższe niż u pozostałych ryzyko wystąpienia MS pod koniec cyklu psychoterapii (10–30% vs 4%) wskazuje na obecność określonych dysfunkcji osobowości, nad którymi intensywna praca psychoterapeutyczna wymaga szczególnej ostrożności – może wyzwalać napięcia emocjonalne prowadzące do wystąpienia MS. Można przypuszczać, że wystąpienie MS jest następstwem przebudowy mechanizmów obronnych podczas pracy nad wglądem, konfrontacją z przyczynami i konsekwencjami własnej agresywności fizycznej wobec bliskich, urealnianiem wielkościowego myślenia na swój temat i wglądem w jego funkcję oraz wglądem w przyczyny postaw rezygnacyjnych.

Cel pracyCelem badań była ocena nasilenia objawów z dolnego odcinka układu moczowego (LUTS) w zależności od nasilenia objawów psychopatologicznych u pacjentów leczonych z powodu zaburzeń depresyjnych.MetodaZbadano 102 pacjentów (43 mężczyzn, 59 kobiet) w wieku 20-67 lat (M = 46,1) leczonych z powodu depresji. Objawy depresyjne oceniano za pomocą Skali Oceny Depresji Hamiltona (HDRS) oraz Inwentarza Samooceny (QIDS-SR). W celu analizy wpływu prezentowanych objawów, zarówno urologicznych, jak i psychiatrycznych, na jakość życia badanych osób badanych wykorzystano 30-itemowy Kwestionariusz Ogólnego Zdrowia (GHQ-30).WynikiŚredni wynik IPSS u kobiet był istotnie wyższy niż u mężczyzn (9,59 vs 6; p = 0,04). Pacjenci cierpiący na depresję ocenianą za pomocą QIDS-SR mieli istotnie wyższe wyniki w IPSS (9,76 vs 4,1; p = 0,002). Nasilenie wszystkich LUTS ocenianych za pomocą IPSS korelowało z wynikiem QIDS-SR u badanych mężczyzn (p WnioskiLUTS występują powszechnie wśród pacjentów z depresją. Istnieje korelacja między nasileniem objawów depresyjnych a LUTS. LUTS wpływa na jakość życia i dobrostan oraz powoduje znaczne cierpienie u pacjentów z depresją. Współwystępowanie LUTS i depresji powinno zwrócić uwagę zarówno psychiatrów, jak i urologów, skłaniając do wzmocnienia interdyscyplinarnego podejścia do leczenia. Dalsze badania prospektywne i kohortowe są niezbędne, aby poszerzyć wiedzę na temat zależności między LUTS a depresją.

Skrajne nasilenie częstomoczu wiązało się z wieloma wymienionymi skalami silniej, oraz nieco inaczej z innymi np. u mężczyzn z Poczuciem przeciążenia i Wyobraźnią, fantazjowaniem.WnioskiNeurotyczne cechy osobowości opisywane przez nieprawidłowe wartości skal kwestionariusza KON-2006 wiążą się z obecnością (a także do pewnego stopnia z nasileniem) psychogennych objawów z dolnego odcinka dróg moczowych. Powiązania mogą być dwukierunkowe - w niektórych przypadkach przeżywanie i samoopis cech osobowości mogą być wtórne do cierpienia związanego z objawami częstomoczu i moczenia.

The article is an attempt to look at the issue of functioning of social services in Poland from an interdisciplinary perspective, presenting common areas of activity of professionals with socio-pedagogical affiliation with representatives of psychological, psychotherapeutic and medical environments. The considerations relate not only to the literature of the subject, but also to the research of the authors in this regard. The purpose of the presented reflection is also to indicate the areas of interdisciplinary scientific cooperation, including research. Its effect could be generating new research problems and solving them in the course of the work of interdisciplinary teams.

Psychiatric disorders are more common in patients with epilepsy than in the general population. The aims of the study were to assess the frequency and type of psychotropic drug usage in patients with epilepsy, to assess the risk factors for their use, and to assess their proconvulsive potential and the risk of interactions with antiepileptic drugs. This 20-month prospective study included patients treated at the university hospital outpatient clinic. Psychotropic drugs have been classified according to the Anatomical Therapeutic Chemical Classification System. Of the 621 patients (with a mean age of 35.4years), 60% were women, and 37.5% were in remission; 54.8% of the patients used antiepileptic drug monotherapy. The most commonly used antiepileptic drugs were valproate, levetiracetam, lamotrigine, and carbamazepine. Eighty-nine (14.3%) patients received psychiatric comedication. Sertraline, perazine, and hydroxyzine were the predominantly used psychotropic drugs. Independent variables associated with psychotropic drug usage in the logistic regression model included age, active epilepsy, combined focal and generalized epilepsy type, use of somatic comedication, and phenobarbital. Over one-third of the patients simultaneously received antiepileptic drugs and psychotropic drugs, between which clinically significant interactions may occur, 10% of patients used psychotropic drugs to lower the seizure threshold. The results of the study indicate the need for closer cooperation between doctors of various specialties when caring for patients with epilepsy.

Aim of the studyThe study aimed to determine the relationship between the intensity of anxiety, neurotic symptoms and perceived control using Bryant’s model - A Four-Factor Model of Perceived Control. The goal of the study was also to describe the structure of perceived control among patients with neurotic and personality disorders.Subject or material and methodsThe study included 49 patients with neurotic disorders, personality disorders or coexistence of neurotic and personality disorders from the Department of Psychotherapy University Hospital in Krakow. Used the following tools: the Symptom Checklist KO “O”, the Neurotic Personality Questionnaire KON-2006, the Polish adaptation of the State-Trait Anxiety Inventory STAI, the Bryant’s Questionnaire of Perceived Control.ResultsThe intensity of neurotic personality and anxiety-trait are significantly negatively associated with most indicators of perceived control.DiscussionIn the literature there are some findings showing association of perceived control with anxiety disorders, depressive disorders and even psychotic states. To date, Rotter’s one-dimensional Scale has been used in most studies (internal versus external Locus of Control). The conducted research enables researchers to broaden the perspective of the perceived control, adding cognitive control and emotional control over positive events as well as aversive one.ConclusionsMost of the various factors of perceived control are related to some extent to the personality structure, which is responsible for occurrence of neurotic disorders, and to some extent to anxiety-trait – an important predictor for neurotic disorders. The results presented are relevant to patients suffer from neurotic symptoms and diagnosed with neurotic disorders and personality disorders according to ICD-10 criteria.

Mental health disorders and addictions affect no less than 40% of children and adolescents worldwide. One out of every ten youths is estimated to require intensive psychiatric treatment [1]. Mental disorders of the developmental period are an increasingly growing phenomenon. According to WHO, neuro-psychiatric disorders are to be the first of the 5 leading medical conditions, causes of death and disability among children in 2020 [2]. Previously observed in other countries and cultural contexts, these trends are increasingly reflected not only in Poland and Europe, but also in various Asian and American regions. The problem of addiction to social media is discussed in the first article entitled Comorbidity of social media addiction and other mental disorders - an overview, by Krzysztof Szczygieł.

There is still deficit in the number of health care specialists dealing with child and adolescent psychiatry and in the subject literature, that could be a solid ground for the practitioners. Opening this Spring issue of archives of Psychiatry and Psychotherapy with a Thirty years’ follow-up of depressive adolescents: a preliminary report, we warmly encourage to get acquainted with all the papers included in the 1/2017 issue, but also to refresh information from papers already published in Psychiatria Polska and Archives of Psychiatry and Psychotherapy concerning mental health of children and adolescents [1-7] kkk kkk

As we were faced with the unprecedented Coronavirus Pandemic, health services around the world: doctors, nurses, laboratory staff and public health workers have been working hard and uninterrupted to serve the official institutions and organizations with which they are affiliated. Mentally ill patients with suspected coronavirus infection or comorbidities with COVID-19 are treated in specially prepared hospital wards, primarily psychiatric units. While all routine on-site operations of psychiatric day care facilities were suspended, the necessary mental healthcare services are available online. Stable outpatients are managed via live video or teleconferencing, but those with unstable mental state are scheduled in-person appointments. There are no delays concerning administration of depot/LAI antipsychotic medication.

The year 2020 marks the 100th anniversary of the Polish Psychiatric Association. We would therefore like to begin this issue with a review by D. Palumbo and S. Galderisi- (Past President of the European Psychiatric Association) from Naples, Italy, on “Controversial issues in current definitions of mental health”. The paper focuses on recently proposed mental health definitions, with a particular emphasis on mental health normative criteria involved in each of them. The next paper, “War trauma and self-help” by P. Piotrowski et al., is an attempt to analyze the frontline soldier drawings, inscriptions and the author’s text entitled “In the time of war”, as a form of self-help.

Znaczna uciążliwość częstego oddawania moczu dotyczyła 9-14% pacjentów a bezwiednego oddawania moczu tylko 0,6-1% badanych.WnioskiUzyskane wyniki pozwalają uznać objawy ‘urologiczne’ za istotną część spektrum dolegliwości odczuwanych przez pacjentów z zaburzeniami nerwicowymi i osobowości, w znacznym stopniu niezależnie od ich rozpoznania szczegółowego oraz płci, oraz przy możliwym do przewidzenia związku ich występowania z wiekiem. Ich współwystępowanie z innymi symptomami zaburzeń nerwicowych zgłaszanych przez pacjentów wskazuje przede wszystkim (choć nie wyłącznie) na ich najsilniejsze powiązanie z zespołami objawów somatyzacyjnych, dysocjacyjnych/konwersyjnych, seksualnych oraz agorafobicznych.
