Abstract

BACKGROUND-: A comparison between the clinical features of tetracycline-induced pseudotumor cerebri (PTC-T) and those of idiopathic intracranial hypertension (IIH) is absent in the literature. We hypothesized that significant clinical differences between these two etiologies do exist and could be better understood by thoroughly analyzing signs, symptoms, and patient demographics. METHODS-: We retrospectively reviewed charts of patients who were evaluated for pseudotumor cerebri syndrome (PTCS) at a single neuro-ophthalmic referral center. We identified patients who developed PTC-T after therapy with minocycline, doxycycline, and tetracycline, as well as patients with IIH. All patients met the recently revised diagnostic criteria for PTCS. Patients with PTC-T and IIH were compared by demographics, body mass index (BMI), visual exam at presentation, treatment, course of illness, and visual outcomes. RESULTS-: We identified 52 cases of PTC-T and 302 cases of IIH. Minocycline was identified as the causative tetracycline antibiotic in 41 of 52 cases of PTC-T (78.8%). Obesity rates among the two groups (PTC-T vs. IIH) were 43.8% vs. 79.2% (P < 0.001). Mean age at diagnosis was 19.8 vs. 28.1 years (P < 0.001). Diplopia occurred at a rate of 40.4% vs. 20.1% (P = 0.001). Mean duration of illness was found to be 18.3 vs. 62.9 weeks (P < 0.0001). Recurrence rates among the two groups were 4.0% vs. 16.5% (P < 0.001). CONCLUSION-: We identified several significant clinical differences between PTC-T and IIH. PTC-T patients tend to be younger, often non-obese, and more likely to experience diplopia. Disease recurrence is less frequently seen with PTC-T than IIH. The frequency of aggressive treatments between the two groups (e.g. shunting procedures and optic nerve sheath fenestrations) was found to be similar. Vision loss, while uncommon, occurs with similar frequency, despite a significantly shorter duration of illness in PTC-T. Recognition of these important clinical differences may help guide further investigation into the poorly-understood pathogenesis of IIH and PTC-T.

Abstract

Background. Giant cell arteritis (GCA) is a systemic vasculitis of elderly individuals associated with significant morbidity, including blindness, stroke, and myocardial infarction. Previous studies have investigated whether GCA is associated with increased mortality, with conflicting results. The objective of this study is to determine whether GCA, is associated with increased mortality. Methods. Forty-four cases with GCA were identified from the University of Utah Health Sciences Center, the major tertiary care center for the Intermountain West. The Utah Population Database, a unique biomedical information resource, selected cases and age- and gender-matched controls. Cases were defined as patients with a temporal artery biopsy-proven diagnosis of GCA (international classification of diseases [ICD]-9 code 446.5) between 1991 and 2005. Exclusion criteria included a negative biopsy, alternative diagnoses, or insufficient clinical data. For each of the 44 cases, 100 controls were identified; thus, 4,400 controls were included in the data analysis. Median survival time and 5-year cumulative survival were measured for cases and controls. Results. The median survival time for the 44 GCA cases was 1,357 days (3.71 years) after diagnosis compared with 3,044 days (8.34 years) for the 4,400 controls (p = 0.04). Five-year cumulative survival was 67% for the control group versus 35% for the cases (p < .001). Survival rates for cases and controls converged at approximately 11.12 years. Conclusions. Patients with GCA were more likely than age- and gender-matched controls to die within the first 5 years following diagnosis. Abstract

Non-arteritic anterior ischaemic optic neuropathy (NAION) and optic neuritis (ON) may be difficult to distinguish early in their disease courses. Our goal was to determine if specific magnetic resonance imaging characteristics differentiate acute NAION from ON. Neuroradiologists, masked to diagnosis, reviewed the diffusion-weighted imaging (DWI) and post-contrast enhancement (PCE) characteristics of the optic nerve in 140 eyes. PCE and DWI signals of the optic disc alone did not discriminate between NAION and ON. After taking age and sex into consideration, only DWI and PCE of the intraorbital segment of the optic nerve differentiated the two, with ON having the increased likelihood of these findings. Isolated PCE without DWI signal at the optic disc, however, was 100% specific for NAION. This may be the most specific way to radiographically differentiate between NAION and ON in the acute setting.
