Abstract

Introduction: This prospective controlled study evaluated and compared health-related quality of life (HRQOL), after transoral robotic supraglottic laryngectomy (TORSGL) versus transcervical open supraglottic laryngectomy (TCOSGL) in patients with T1 and T2 supraglottic laryngeal carcinoma (SLC). Methods: The TORSGL group comprised 14 patients, and the TCOSGL group comprised 13 patients. All 27 patients completed the European Organization for Research and Treatment of Cancer 30-item core quality-of-life questionnaire version 3.0 (EORTC QLQ-C30), the European Organization for Research and Treatment of Cancer head-and-neck cancer-specific module (EORTC QLQ-H&N35), before treatment and during the early and the late postoperative periods. Results: The present prospective study demonstrated the near-term postoperative HRQOL of patients with T1 or T2 SLC treated with TORSGL (Group A) or TCOSGL (Group B). On comparison of EORTC QLQ-C30 data for the two groups in the early postoperative period, all functional subscale scores and global health status scores were statistically significantly lower (all p < 0.05) in Group A than in Group B and in the late postoperative period, other than the cognitive function score (p = 0.450), all functional subscale scores and global health status scores were statistically significantly lower (all p < 0.05) in Group A than in Group B. On comparison of the EORTC QLQ-H&N35 data for the two groups in the early postoperative period, except for the teeth problems scale score (p = 0.061), all symptom scale scores were statistically significantly lower in Group A than in Group B (all p < 0.05) and in the late postoperative period, the speech, social eating, social contact, and coughing scale scores were statistically significantly lower (p = 0,0215, p = 0.021, p = 0.01, p = 0.011, respectively) in Group A than in Group B. HRQOL parameters recovered in the late postoperative period in both groups; recovery was better in Group A. Discussion/Conclusion: This study suggested that TORSGL may provide patients with a better HRQOL than those TCOSGL, especially in the early period, but also in the late period. Abstract

Introduction: Sialadenoma papilliferum (SP) is a rare benign tumor, which originates from the minor salivary gland. It occurs at sites that have minor salivary glands, such as the palate, retromolar pads, buccal mucosa, and lips. The most common location for tumor development is on the hard palate. A differential diagnosis consists of ruling out other salivary gland tumors. Transoral robotic surgery (TORS) is a new technology used in head and neck surgery within certain centers around the world. Case Report: Herein, we present the first successful case of SP tumor removal by TORS. Conclusion: This particular case highlights the identification of this rare tumor in an unusual location. Furthermore, it demonstrates the utilization of TORS, leveraging the superior visualization to obtain excellent local control with minimal acute and long-term morbidity, in comparison to conventional transoral surgical approaches.

25. Typical carcinoid tumor of the larynx in a woman: a case report

Abstract Introduction Neuroendocrine tumors are the second most common neoplasms of the larynx. Histopathologically, neuroendocrine tumors can be classified into four types: typical carcinoid tumors, atypical carcinoid tumors, small cell neuroendocrine tumors and paragangliomas. Typical carcinoid tumor of the larynx is a particularly rare occurrence. We present a case of this rare disease, and review and discuss its diagnosis and treatment. Case presentation A 55-year-old Turkish woman presented with a two-year history of persistent hoarseness. Endoscopic laryngeal examination and computed tomography revealed a supraglottic mass. Direct laryngoscopy was performed and a biopsy taken. Results of the histopathologic examination and immunohistochemical analysis were consistent with typical carcinoid tumor of the larynx. A supraglottic laryngectomy was performed. There was no recurrence during a follow-up period of three years. Conclusion Carcinoid tumors require an accurate diagnosis because of their varied clinical behavior and prognosis. A correct pathologic diagnosis is essential, differentiating the tumors from other neuroendocrine neoplasms and medullary cancer of the thyroid gland. Immunohistochemical analysis is supplementary to a standard histopathologic evaluation. Currently, conservative surgical resection without elective neck dissection is the recommended treatment for typical carcinoid tumor of the larynx. Additional cases and case series with long-term follow-up will be useful for understanding the nature of this tumor and should clarify the prognosis.

Abstract

Objective: There are many factors considered to be related with the etiology of sudden sensorineural hearing loss (SSNHL) such as viral infections, microvascular diseases and inflammation. Increased mean platelet volume (MPV) levels are assumed to represent more active enzymatic process and thrombotic predisposition. Our aim in this study is to show whether MPV could be a predictive value in SSNHL or not.Methods: The medical records of a total of 93 patients with SSNHL and 93 healthy controls were reviewed retrospectively in this study. Peripheral blood MPV values of both groups were compared statistically.Results: Mean platelet volume levels did not show a statistically significant difference between these groups.Conclusion: As a result of our study, we consider MPV is not a predictive parameter in idiopathic SSNHL.

Abstract

GİRİŞ ve AMAÇ: Dış kulak yolunda yabancı cisim olan olguların demografik özellikleri, tedavi yönetimi ve çıkarılan yabancı cisimlerin özellikleri değerlendirmek.YÖNTEM ve GEREÇLER: Çalışmaya dış kulak yolunda yabancı cisim olan 233 olgu (91 kadın 142 erkek; ort.yaş 13.04±15.48 yıl,dağılım: 3 ay- 72 yıl) dahil edildi. Tüm olguların dosyaları, yaş, cinsiyet, yabancı cismin cinsi, şekli, rengi, hangi tarafta olduğu, uygulanan tedavi ve komplikasyonlar açısından değerlendirildi.BULGULAR: Yabancı cisimler 104 olguda(%44.6) sol kulak, 118 olguda (%50.6) sağ kulak, 11 olguda (%4.8) ise her iki kulakta saptandı. En sık saptanan yabancı cisimlerin %38.6’sı (n=90) boncuk, %16.7’si (n=39) kulak çöpü pamuğu ve %15’i (n=36) oyuncak idi. Yabancı cisimlerin %91.8' i renkli ve %8.2'si renksiz idi. Yabancı cisimler 15 olguda (%6.4) ameliyathane şartlarında genel anestezi altında çıkartıldı. TARTIŞMA ve SONUÇ: Dış kulak yolu yabancı cisimleri, kulak burun boğaz (KBB) acilleri arasında en sık görülen acillerdendir. Dış kulak yolu yabancı cisimleri, komplikasyonlardan korunmak için uygun teknik ve aletlerle deneyimli KBB hekimi tarafından çıkarılmalıdır. Eğitim almamış ve bu konuda deneyimi eksik kişilerce yapılan çıkarma denemeleri komplikasyonlara yol açmaktadır., INTRODUCTION: To evaluate the demographic data and treatment methods of patients with external auditory canal and the proporties of foreign bodies. METHODS: In total, 233 patients (91 females 142 males; average age 13.04±15.48; range 3 months –72 years) who were diagnosed with foreign bodies in external auditory canal. All patients files were evaluated in terms of age, gender, the type, shape and color of foreign bodies, side of presentation, treatment protocols and complications.RESULTS: Foreign bodies were detected in left ear in 104 patients (%44.6), in right ear in 118 patients (%50.6) and in bilateral in 11 patients (%4.8). The most common foreign bodies in external auditory canal are beads in 90 patients (%38.6), cotton buds in 39 patients (%16.7) and plastic toy parts in 36 patients (%15). Majority (%91.8) of foreign bodies has a color while %8.2 of foreign bodies are colorless (transparent). Fifteen (%6.4) of all patients required general anaesthesia for foreign body extraction in operating condition. DISCUSSION AND CONCLUSION: Foreign bodies in external auditory canal are common among emergency problem in the field of otorhinolaryngology. Aural foreign bodies should be extracted by an experienced otorhinolaryngologist using suitable techniques and tools to prevent complications. Removal attemps by untrained health professionals and lack of experience in foreign body management induce to complications.

30. Impact of the Angles of the Septal Deviation on the Degree of the Mastoid Pneumatization.

Abstract

Introduction: The aim was to explore the developmental relationships between the angles of septal deviations and the degree of the mastoid pneumatization. Materials and Methods: In total, 143 patients with a diagnosis of septal deviation who underwent septoplasty were included. The patients were divided into three groups in terms of the angles of the septal deviation. The angle of the septal deviations was defined as mild (<9 degrees), moderate (between 9 and 15 degrees) and severe (15 degrees and above). The degree of the mastoid pneumatization of each groups were compared. Results: In right-sided septal deviation subjects, the right mastoid air cell volumes of group mild, moderate and severe were 6,31±2,33 cm³, 5,20± 1,51 cm³, and 5,31±1,57 cm³, respectively. The mean right mastoid volumes of each groups did not differ in right-sided deviations subjects (P>0.05). The mean left mastoid volumes of each groups did not differ in right-sided deviations subjects (P>0.05). In right-sided septal deviation subjects, the mean volume of the right and left-sided mastoid air cells of each groups did not differ (P>0.05). In left-sided septal deviation subjects, the mean volume of the right and left-sided mastoid air cells of each groups did not differ (P>0.05). Conclusions: No developmental relationships between the angles of septal deviations and the degree of the mastoid pneumatization was observed in the study. [ABSTRACT FROM AUTHOR]

The hard and soft palate are the most common sites of PA of the minor salivary glands. PA of the buccal minor salivary gland is seen rarely. A differential diagnosis consists of ruling out other salivary gland tumors. The typical treatment for PA is complete excision with clear surgical margins. Abstract

Frontal sinüs mukoselleri, yavaş büyüyen ve frontal sinuüste lokal agresif davranan lezyonlardır. Büyük frontal mukoseller, sinüs duvarında destrüksiyon yaparak çevre anatomik yapılara yayılırlar. Mukosel tedavisi cerrahidir. Cerrahi yönetiminde farklı yaklaşımlar bulunmaktadır. Bu makalede kliniğimizde tedavi edilen üç dev frontal mukosel olgusu tanı, tedavi ve takip aşamaları güncel literatür bilgileri ışığında sunuldu., Frontal sinus mucoceles are slow-growing and locally aggressive lesion of the frontal sinuses. A gradually enlarging lesion results in destruction of the wall of the sinus and extends to the encircling anatomic structures. Treatment of mucocele is surgical. There are different surgical approaches in frontal mucocele. Hereby we reported the diagnosis, treatment and follow-up of three cases of giant frontal mucocele and reviewed the relevant literature.
