Amaç: Postoperatif atriyal fibrilasyon, koroner arter bypass cerrahisi sonrası morbidite ve mortaliteyi etkileyen en yaygın komplikasyondur. Bu komplikasyonun önlenmesi için kullanılan çeşitli profilaktik ajanlar mevcuttur. Bunlardan en yaygın kullanılanı bir beta blokör olan metoprololdür. Bu tezin amacı, yeni bir beta blokör olan ve beta blokör etkinliklerinin yanında farklı ve pozitif farmakolojik etkileri de olan nebivololün bu konudaki etkinliğini metoprolol ile karşılaştırmaktır.Metot: Bu tez, İnönü Üniversitesi Tıp Fakültesi kurulunun 2009/145 sayılı onayı alındıktan sonra, İnönü Üniversitesi Turgut Özal Tıp Merkezi Kalp ve Damar Cerrahisi Kliniğinde Ekim 2009 ile Haziran 2010 tarihleri arasında 60 yaş ve üzeri izole koroner bypass ameliyatı yapılan ardışık 200 hastaya ait veriler prospektif randomize olarak kaydedilerek yapıldı. 100 er hastadan oluşan iki grup yapılarak, gruplardan birine nebivolol (grup 1), diğerine de metoprolol (grup 2) başlandı ve preop, postop ve taburculuk dönemlerine ait veriler toplandı.Bulgular: Her iki grup arasında demografik özellikler açısından istatistiksel fark yoktu. Nebivolol kullanan postoperatif atriyal fibrilasyon sıklığı (%20), metoprolol kullanan grupta ise %18 idi ve istatistiksel olarak fark yoktu (p>0,05). Her iki grupta da preop, postop ve taburcu dönemlerindeki kan basıncı ve kalp hızı değişiklikleri de kendi içlerinde istatistiksel olarak anlamlı çıktı ve bu değişiklikler bakımından iki grup arasında istatistiksel fark yoktu.Sonuç: Çalışmamızda nebivolol postoperatif atriyal fibrilasyon profilaksisinde klasik ve kabul görmüş bir ajan olan metoprolol kadar etkili bulundu. Nebivololün kan basıncı ve kalp hızı kontrolü konusunda da metoprolole benzer etkinlik gösterdiği görüldü. Sonuç olarak beta blokaj yanında aterosklerozu önleme, antioksidan özellikler ve NO üzerinden damar dilatasyonu yapıcı etkileri yönünden ve ayrıca yan etki konusundaki plaseboya benzer profil bakımından nebivolol kalp cerrahisinde hem postoperatif atriyal fibrilasyon profilaksisi için hem de diğer olumlu etkileri için kullanım alanında göz önüne alınması gereken bir ajandır. Objective: Postoperative atrial fibrillation is the most common complication affecting morbidity and mortality after coronary bypass grafting. To prevent this complication there are several medical agents in use. Metoprolol is a beta blocker which is commonly used for this purpose. The aim of this study is to compare the effectiveness of nebivolol, a new beta blocker with different and positive effects beside its beta blocker effects, with metoprolol.Method: Data of 200 patients over 60 years old was collected prospectively who underwent isolated coronary bypass surgery at Turgut Ozal Medical Center between October 2009 and June 2010. Two study groups were designed, each consisted of 100 patients. Nebivolol was administered for one (group 1) and metoprolol for other (group 2). Data were collected preoperatively, postoperatively and at the time of discharge.Results: the demographic properties of two groups were similar statistically. Atrial fibrillation incidence was 20% at nebivolol group and 18% at metoprolol group. There was no difference statistically. Blood pressure and heart rate differences between preoperative, postoperative and at the time of discharge were statistically significant in two groups and these changes are similar between two groups statistically.Conclusion: Nebivolol was found as effective as metoprolol for preventing postoperative atrial fibrillation. It was also as effective as metoprolol for regulation of blood pressure and heart rates. The anti-atherosclerotic effects, antioxidant properties and vasodilatator characteristics are other positive effects of nebivolol beside its beta blocker properties. For this reason nebivolol is a new beta blocker and must be concerned for rutin use in preventing postoperative atrial fibrillation and for other indications in cardiovascular surgery. 76

ÖZET Enflasyon olgusunu incelediğimiz zaman, para değerinin düşmesi ve fiyatların yükselmesi ile ortaya çıkan bir tablo sergilenmektedir. Enflasyon olgusunu incelediğimiz zaman, genellikle az gelişmiş ve gelişmekte olan ülkelerin, yüksek oranlarda gerçekleşen enflasyonlara maruz kaldığını görmekteyiz. Çünkü az gelişmiş ve gelişmekte olan ülkelerde enflasyonun nedenlerini araştırdığımız zaman, ulusal ve uluslar arası ekonomik ilişkilerin yapısını incelemek gerekmektedir. Uygulanan iktisadi plan ve programlarının amacı ekonomik istikrar olan ülkeler, ekonominin çeşitli yapısal nedenleri ve dış bağımlılığın getirdiği iktisadi sorunlardan dolayı enflasyonla mücadelede başarılı olamamaktadır. Enflasyonla mücadelede uygulanacak programlar incelendiği zaman, klasiklerin denk bütçe ve ekonominin kendi serbest kuralları içinde ücret, faiz esnekliği gibi araçlarla ekonominin dengeye geleceğini savunurken, Keynescil görüş, klasiklerin bu görüşlerini kabul etmeyen, Keynes ve taraftarlarının, ekonominin istikrara kavuşabilmesi için, kamu harcamaları ve diğer faktörlerle beraber, iktisadi refahın gerçekleşmesi için devlet müdahalesini savunmuşlardır. Moneteristler ise, para politikalarının önemini savunarak, iktisadi istikrar için maliye politikalarının yanında para politikalarının uygulanmasını savunmuşlardır. Bugün için çağdaş devlet anlayışında, ekonomik istikrar için istihdam gelir dağılımı, tabana yayılmış vergi politikaları, bölgelerarası gelişmişlik farkı, sanayileşme vs. ekonomik ve sosyal sorunların enflasyonla birlikte ele alındığı zaman, enflasyonla mücadelenin, makro düzeyde istikrar programlarına ihtiyaç duyduğugörülmektedir. Bu bakımdan enflasyonla mücadelede, para ve maliye politikalarının uyumlu bir şekilde yürütülmesi siyasi kaygılardan uzak, parlamento desteği ile kararlı bir şekilde uygulanmasının gerektiği ortaya çıkmaktadır. Enflasyonun yüksek oranlarda yaşanması, toplumda sosyal ve ekonomik çıkmazların yaşanmasına neden olur. Bu açıdan gerek kamu, gerek özel sektörün bu olumsuz gelişmelerden etkilenmesini önlemek için, çağdaş parlamenter, demokratik sistemlerin hakim olduğu günümüz ekonomilerinin, toplumsal refahı gerçekleştirmek için enflasyonla mücadelede, kamu ve özel sektörün mücadelesinin hükümetlerin objektif desteğiyle gerçekleşebileceği kabul edilmektedir. When we do research on the reasons of inflation in developing and underdeveloped countries we need examine (search) characteristics of national and international relations. The countries which aim of the applying economic projects and programmes is economic stability, haven't succeeded in struggling with inflation because of the economic problems brought by importing (external dependence) and different characteristic factors of economy. When the programmes which will be applied in struggling with inflation are examined, we face to face (we have seen) two ideas. While the classics claiming (accepting) that economy will be stabilized by such means as wage and flexible interest in its free rules, Keynecians don't agree with them. They have accepted (defended) the Keynes and his followers ideas. In order to stabilize the economy and to realize economic prosperity (order) they have definded interference of government with the other factors and public expenditures.IV On the other hand, Monetariests defending the importance of monetary policy for economic stability besides fiscal policy they have defended (claimed) applying monetary policy. For today, in contemporary government understanding (sense) to get economic stability employment, income distribution, minimum tax policy, the difference of regional development becoming and social problems, struggling with inflation needs highly developed stability programmes. In this respect (from this point of view) monetary and fiscal policies should be executed in conformity far from the political anxiety with the support of parliament applying in determination have been shown (occured) in struggling with inflation. Living with highly inflation rate causes appearing social and economic crises (depressions) in the society. From this point of view, in order to prevent effecting public and private sectors from these bad conditions today's economy that dominated by contemporary parliamentary democratic systems for getting (realizing) social comfort (prosperity) in struggling inflation, the struggling (contention) of public and private sectors only can be realized by the objective supports of governments have been accepted. 88

We have used the single and dual color fluorescence in situ hybridization (FISH) technique combined with a new detection system, tyramide signal amplification (TSA), by using the multiple endocrine neoplasm type I (MEN1) gene and chromosome I I specific alpha satellite DNA probes for the study of the allelic deletion of the MEN1 gene. The MEN1 gene is a new tumor supressor gene and has been recently cloned on chromosome 11q13. FISH combined with the TSA detection system was performed on bone marrow interphase nuclei of 22 patients with acute myeloid leukemia (AML). The FISH-TSA analysis revealed the mono allelic deletion of the MEN1 gene in 4 out of 22 patients (18.18%), 2 of 9 AML-M2 patients (22.2%), 1 of 6 AML-M4 patients (16.6%), and I of 4 AML-M5 patients (25.0%). Our study indicates that allelic deletion of the MEN1 gene is not a major cause or a primary event in tumorigenesis of AML, although the long arm (q13 region) of chromosome 11 involves a chromosomal rearrangement in AML.

Biomimetic retinas with a wide field of view and high resolution are in demand for neuroprosthetics and robot vision. Conventional neural prostheses are manufactured outside the application area and implanted as a complete device using invasive surgery. Here, a minimally invasive strategy based on in situ self-assembly of photovoltaic microdevices (PVMs) is presented. The photoelectricity transduced by PVMs upon visible light illumination reaches the intensity levels that could effectively activate the retinal ganglion cell layers. The geometry and multilayered architecture of the PVMs along with the tunability of their physical properties such as size and stiffness allow several routes for initiating a self-assembly process. The spatial distribution and packing density of the PVMs within the assembled device are modulated through concentration, liquid discharge speed, and coordinated self-assembly steps. Subsequent injection of a photocurable and transparent polymer facilitates tissue integration and reinforces the cohesion of the device. Taken together, the presented methodology introduces three unique features: minimally invasive implantation, personalized visual field and acuity, and a device geometry adaptable to retina topography
