Background: Patients with chronic diseases such as chronic kidney disease (CKD) have been reported to have more adverse outcomes during the coronavirus disease 2019 (COVID-19) pandemic. There are insuficient data on the outcomes of patients with primary glomerular diseases (PGD) afer COVID-19. Methods: We designed a national multicenter observational study that included adult patients with biopsy-proven PGD who survived COVID-19. A control group was created from the same centers, including PGD patients without COVID-19. The clinical and laboratory data of the patients at baseline, frst, and third months afer COVID-19 were recorded. Results: A total of 129 patients from 21 centers were included (COVID-19 group, n = 77). Baseline characteristics were almost similar except the ratio of active disease in the non-COVID-19 group was signifcantly higher than in the COVID-19 group. No patients died during the frst and third months. Respiratory symptoms were signifcantly higher in the COVID-19 group than in the non-COVID-19 group in the frst month (7.8% vs. 0%, P = .039). All other follow-up outcomes, including initiation of chronic kidney replacement therapy and initiation of new immunosuppressive treatment, and the laboratory data were not diferent between the groups in the frst and third months. Conclusion: Primary glomerular disease patients in the post-COVID-19 period had more respiratory symptoms than non-COVID-19 PGD patients, but outcomes, including death and initiation of kidney replacement therapy, were not diferent in the frst and third months post COVID-19. - Abstract
Delivery of care for kidney failure (KF) globally has a significant disparity; even in some countries, it means end of life for the person. The International Society of Nephrology Global Kidney Health Atlas (ISN-GKHA) tries to address gaps in KF care and standardize global nephrology care. From the third iteration of the ISN-GKHA, we present data for countries in the ISN Eastern and Central Europe region. The median prevalences of chronic kidney disease (12.8%) and treated KF (873.5 pmp) were higher than the global rates, respectively. Hemodialysis was the most preferred modality for KF in adults, whereas kidney replacement therapy was more balanced in children. Although most of the countries in the region had lower-middle–income and upper-middle–income levels, health expenditures for kidney health care were almost generally covered publicly. Nephrologists were responsible for the medical kidney care of people with KF in all countries. There was adequate infrastructure to provide all kinds of treatment for kidney care in the region. Regional characteristics such as high levels of obesity, smoking, and Balkan nephropathy as an endemic disease coupled with a shortage of workforce and finance continued to affect kidney care in the region negatively. By making organizational and legislative arrangements, partnerships with national authorities and societies may accelerate the improvement of kidney health care in the region.

Disasters and crisis situations are unforeseen events. When a disaster occurs, the most critical step after the intervention at the scene is the health and treatment services provided in hospitals. Since it is of vital importance that hospitals, where health services are provided, are accessible and operational when faced with natural and man-made disasters such as earthquakes, fires, epidemics, CBRN events, wars, and crises such as cyber-attacks, economic problems, hospitals must protect themselves against a disaster hazard and plan what to do during and after the disaster. This review was written to emphasize the importance of hospitals and their resilience in times of crisis and disaster.Hospitals can enhance their resilience by strengthening both their physical and social aspects. It is essential to create resistance in hospitals not against specific dangers such as fire and earthquake, but against all crises that may occur in the system. A hospital must first identify its structural and non-structural risks to enhance its physical resilience. To enhance social resilience, a hospital should plan its organisations and human resources, establish accurate information communication, and engage in logistics and financial planning. It is crucial to guarantee uninterrupted patient care and all supportive services. Measures should be taken for decontamination and evacuation of patients when necessary while also ensuring the overall security of the hospital. As a result, hospital resilience plays a critical role in maintaining healthcare services, effectively managing emergencies, and generally protecting public health. Although collecting scientific data during disaster conditions may be very challenging and not considered a priority, it is crucial since it may serve to shed light on similar future disaster conditions. The primary obstacles in data collection fall into 2 main categories: (i) the recording process itself and (ii) the reliability of the test results. During and after crisis periods, data collection is complicated by various factors. These include a sudden and significant surge in patient numbers, disruptions in hospital operational systems, health-care professionals experiencing burnout, inadequate examination of incoming disaster victims, rapid patient transfers to other health-care facilities, and infrastructural damage affecting the operation of hospital health information systems. These challenges collectively lead to inaccuracies in hospital data collection and storage processes. The reliability and interpretation of even the basic test results may be hampered by functional disruption of the laboratory systems, mixing of patients’ lab results due to overburden of patients, and lack of identification information as well as incorrectly abnormal values due to inadequate preanalytical and analytical processes. This opinion article discusses health-care professionals’ data collection challenges regarding kidney problems during large-scale disasters and suggests solutions to overcome these problems. Objectives: There is limited data about coronavirus disease-19 (COVID-19) characteristics and results in peritoneal dialysis (PD) patients. This study aimed to investigate the characteristics and outcomes among PD patients and compare them with matched hemodialysis (HD) patients and a control group without kidney disease. Methods: We included 18 PD patients and consecutive age- and gender-matched 18 HD and 18 patients without kidney disease (control group) registered into the Turkish Society of Nephrology database including 1301 COVID-19 patients. We compared demographic, clinical, radiological, laboratory data, and outcomes namely intensive care unit (ICU) admission, mechanical ventilation, mortality, and composite outcome (death and/or ICU admission). Results: ICU admission, mechanical ventilation, and mortality rates in PD patients (27.8%, 22.2%, and 22.2%, respectively) and the HD group (16.7%, 11.1%, and 16.7%, respectively) were higher than the control group (11.1%, 11.1%, and 5.6%, respectively), but intergroup comparison did not reveal difference. A total of 11 (20.3%) patients had composite outcome (6 PD patients, 3 HD patients, and 2 patients in the control group). In Cox regression analysis, higher age and higher CRP level were related to increased risk of composite outcome. Adjusted rate of composite outcome in PD group was significantly higher than the control group (P =.050). This rate was similar in HD and control groups (P =.30). Conclusions: Combined in-hospital mortality and/or ICU admission of PD patients with COVID-19 was significantly higher than the control patients. There is a need for careful surveillance of PD patients for infection signs and prompt treatment of COVID-19.

Background Galactose-deficient IgA1 (Gd-IgA1) has an increased tendency to form immunocomplexes with IgG in the serum, contributing to IgAN pathogenesis by accumulating in the glomerular mesangium. Several studies showed that glomerular IgG deposition in IgAN is an important cause of mesangial proliferation and glomerular damage. This study aims to determine the association of the positivity of IgG and the intensity of IgG staining with a poor renal prognosis. Methods A total of 943 IgAN patients were included in the study. Glomerular IgG staining negative and positive patients were compared using Oxford classification scores, histopathological evaluations, proteinuria, eGFR, albumin, blood pressures. IgG positive patients were classified as (+), (++), (+++) based on their staining intensity, and the association with the prognostic criteria was also evaluated. Results 81% (n = 764) of the patients were detected as IgG negative, while 19% (n = 179) were positive. Age, gender, body mass index, blood pressure, proteinuria, eGFR, uric acid values were similar in IgG positive and negative patients who underwent biopsy (p > 0.05). Intensity of glomerular IgG positivity was not found to be associated with diastolic and systolic blood pressure, urea, uric acid, age, eGFR, albumin, proteinuria (p > 0.05 for all, r = − 0.084, r = − 0.102, r = − 0.006, r = 0.062, r = 0.014, r = − 0.044, r = − 0.061, r = − 0.066, r = 0.150, respectively). There was no difference for histopathological findings between IgG (+), IgG (++), IgG (+++) groups (for all, p > 0.05). Conclusion Glomerular IgG negativity and positivity detected by routine IFM in IgAN patients is not associated with poor renal prognostic risk factors.

Objectives: Geriatric syndromes are common in elderly subjects with chronic kidney disease. An age-adapted definition of chronic kidney disease has recently been proposed. This study aimed to investigate the effects of this change in the definition of chronic kidney disease (from an estimated glomerular filtration rate of <60 to <45 mL/min/1.73 m² ) on geriatric assessments. Methods: Records of an elderly outpatient population were retrospectively reviewed. Subjects underwent comprehensive geriatric assessment including the Basic and Instrumental Activities of Daily Living, Mini-Mental State Examination, Geriatric Depression Score, Tinetti Mobility test, the Timed Up and Go test, the Mini Nutritional Assessment, the handgrip test, and the Insomnia Severity Index. Logistic regression analysis was performed in order to determine the odds ratio of each chronic kidney disease definition on geriatric syndromes. Results: Of the 1222 patients, 832 (68.1%) were women and the median age was 73 (interquartile range, 67-80) years. Patients with an estimated glomerular filtration rate of <45 and <60 mL/min/1.73 m² comprised 8.3% (n = 101) and 21.6% (n = 264) of the cohort, respectively. Both estimated glomerular filtration rates of <45 and <60 mL/min/1.73 m² were significantly associated with more unfavorable geriatric assessment scores in univariate analysis. After adjustments, associations of an estimated glomerular filtration rate <60 mL/min/1.73 m² with the Timed Up and Go test and polypharmacy remained significant; however, none of the geriatric assessment measures remained significantly associated with an estimated glomerular filtration rate of <45 mL/min/1.73 m² . Conclusion: Chronic kidney disease was more significantly associated with impairments in geriatric assessment parameters when the cut-off of estimated glomerular filtration rate for the definition of chronic kidney disease was kept as <60 mL/min/1.73 m² in comparison to modification of <45 mL/min/1.73 m². - Abstract
Background: Chronic kidney disease (CKD) and immunosuppression, such as in renal transplantation (RT), stand as one of the established potential risk factors for severe coronavirus disease 2019 (COVID-19). Case morbidity and mortality rates for any type of infection have always been much higher in CKD, haemodialysis (HD) and RT patients than in the general population. A large study comparing COVID-19 outcome in moderate to advanced CKD (Stages 3-5), HD and RT patients with a control group of patients is still lacking. Methods: We conducted a multicentre, retrospective, observational study, involving hospitalized adult patients with COVID-19 from 47 centres in Turkey. Patients with CKD Stages 3-5, chronic HD and RT were compared with patients who had COVID-19 but no kidney disease. Demographics, comorbidities, medications, laboratory tests, COVID-19 treatments and outcome [in-hospital mortality and combined in-hospital outcome mortality or admission to the intensive care unit (ICU)] were compared. Results: A total of 1210 patients were included [median age, 61 (quartile 1-quartile 3 48-71) years, female 551 (45.5%)] composed of four groups: control (n = 450), HD (n = 390), RT (n = 81) and CKD (n = 289). The ICU admission rate was 266/1210 (22.0%). A total of 172/1210 (14.2%) patients died. The ICU admission and in-hospital mortality rates in the CKD group [114/289 (39.4%); 95% confidence interval (CI) 33.9-45.2; and 82/289 (28.4%); 95% CI 23.9-34.5)] were significantly higher than the other groups: HD = 99/390 (25.4%; 95% CI 21.3-29.9; P < 0.001) and 63/390 (16.2%; 95% CI 13.0-20.4; P < 0.001); RT = 17/81 (21.0%; 95% CI 13.2-30.8; P = 0.002) and 9/81 (11.1%; 95% CI 5.7-19.5; P = 0.001); and control = 36/450 (8.0%; 95% CI 5.8-10.8; P < 0.001) and 18/450 (4%; 95% CI 2.5-6.2; P < 0.001). - Abstract
The global nephrology community recognizes the need for a cohesive strategy to address the growing problem of end-stage kidney disease (ESKD). In March 2018, the International Society of Nephrology hosted a summit on integrated ESKD care, including 92 individuals from around the globe with diverse expertise and professional backgrounds. The attendees were from 41 countries, including 16 participants from 11 low- and lower-middle-income countries. The purpose was to develop a strategic plan to improve worldwide access to integrated ESKD care, by identifying and prioritizing key activities across 8 themes: (i) estimates of ESKD burden and treatment coverage, (ii) advocacy, (iii) education and training/workforce, (iv) financing/funding models, (v) ethics, (vi) dialysis, (vii) transplantation, and (viii) conservative care. Action plans with prioritized lists of goals, activities, and key deliverables, and an overarching performance framework were developed for each theme. Examples of these key deliverables include improved data availability, integration of core registry measures and analysis to inform development of health care policy; a framework for advocacy; improved and continued stakeholder engagement; improved workforce training; equitable, efficient, and cost-effective funding models; greater understanding and greater application of ethical principles in practice and policy; definition and application of standards for safe and sustainable dialysis treatment and a set of measurable quality parameters; and integration of dialysis, transplantation, and comprehensive conservative care as ESKD treatment options within the context of overall health priorities. Intended users of the action plans include clinicians, patients and their families, scientists, industry partners, government decision makers, and advocacy organizations. Implementation of this integrated and comprehensive plan is intended to improve quality and access to care and thereby reduce serious health-related suffering of adults and children affected by ESKD worldwide.

Objective: Studies on predictors of mortality among elderly chronic kidney disease (CKD) patients have conflicting results. We aimed to assess the factors related to mortality in CKD versus non-CKD elderly subjects. Methods: Medical records of consecutive elderly subjects presented to geriatrics outpatient clinics were retrospectively searched. Logistic regression models were set in order to determine independent predictors of mortality. Results: The median age was 73 (67-80) years, and 837 (67.9%) were female. CKD constituted 21.9% of the cohort. During the follow-up of 3 to 4 years, 7.2% of the patients died. In the CKD cohort, older age (per year, OR 1.12, 95% CI 1.01-1.25, P = .040) and serum uric acid levels (per 1 mg/dL increase, OR 1.74, 95% CI 1.12-2.69, P = .013) were associated with a higher risk of mortality while serum albumin (per 1 g/dL increase, OR 0.08, 95% CI 0.01-0.52, P = .008) and vitamin D levels (per 1 ng/mL increase, OR 0.77, 95% CI 0.62-0.96, P = .019) were associated with a lower risk of mortality in the multivariate regression model. Conclusion: Older age, lower serum albumin and vitamin D levels, and higher serum uric acid levels are independent predictors of mortality in outpatient elderly subjects with CKD.
