9. Subhepatic perforated subhepatic appendicitis versus acute cholecystitis: a diagnostic dilemma

Abstract

Appendicitis, being one of the most common surgical problems, is often diagnosed clinically. The typical clinical sign elicited from a patient suffering from acute or perforated appendicitis includes: Rebound tenderness, Rovsing sign, Obturator sign, Psoas sign, and tenderness over the right iliac fossa. Cholecystitis, another common surgical problem, typically presents with tenderness over the right hypochondrium, and the typical sign associated with this condition is positive Murphy’s sign. In this report, we describe how a subhepatic perforated appendicitis can mimic acute cholecytstitis, both clinically and radiologically in an adult patient.

10. Exposure to perfluorodecanoic acid impairs follicular development via inducing granulosa cell necroptosis

Abstract

Per- and polyfluoroalkyl substances (PFAS) have attracted significant attention due to their environmental toxicity. However, the detrimental impact of PFAS on the development of the female reproductive system remains controversial. In this study, we investigated the effects of three specific PFAS compounds perfluorooctanoic acid (PFOA), perfluorononanoic acid (PFNA), and perfluorodecanoic acid (PFDA) on ovarian development. Among these compounds, PFDA demonstrated the most pronounced cytotoxic effect on ovarian granulosa cells. The results showed that a 200 μM concentration of PFDA induced cell apoptosis via the intrinsic pathway by elevating reactive oxygen species (ROS) levels and activating Caspase-9 and Caspase-3. Furthermore, 200 μM PFDA triggered necroptosis, a form of regulated cell death (RCD), through the receptor-interacting serine/threonine kinase 1 (RIPK1), receptor interacting protein kinase 3 (RIPK3), and mixed-lineage kinase domain-like protein (MLKL) axis, mediated by inhibition of the canonical apoptosis proteolytic enzyme Caspase-8. In vivo experiments confirmed that mice exposed to PFDA displayed a significantly reduced ovarian index compared to the control group, accompanied by evident follicular atresia. Ovarian tissues from the PFDA-exposed group showed upregulated necroptosis markers, which were effectively mitigated by inhibiting the phosphorylation of RIPK1 at Ser166. Importantly, this study provides the first evidence that PFDA disrupts ovarian development through a novel mechanism involving the RIPK1-mediated necroptosis pathway, alongside the detection of the intrinsic apoptosis pathway. This greatly expands our insight into the effects of PFDA on cell death. This finding highlights the potential public health hazards associated with PFDA exposure and emphasizes the need for further research to fully understand its broader implications.

Abstract

Nowadays, sustaining the atmosphere and protecting it from global warming are recognized as the main concerns of the planet. This problem has been addressed through the development of a number of different carbon capture and sequestration approaches. Lately, metal‐organic framework (MOF) has emerged as the most auspicious CO2 adsorption and conversion material. This novel class of porous material has demonstrated tremendous potential for gas separation technologies, particularly CO2 adsorption and conversion due to their remarkable characteristics. Therefore, the present review attempted to provide current understanding of the CO2 adsorption and conversion performance over MOF‐based materials. This review paper discussed the type and properties of the MOF, effect of metal and ligand on MOF's properties, and various applications of MOF. Abstract

The PD-1/PD-L1 pathway plays an important role in regulation of alloimmune responses and in induction and maintenance of peripheral tolerance. Because GVHD is driven by donor T cells and PD-L1 expression can be markedly elevated on T cells during activation, we investigated the functional significance of PD-L1 expressed by donor T cells in regulating murine models of acute GVHD. PD-L1 expression was up-regulated on donor CD4 and CD8 T cells during GVHD. We considered the possibility that PD-L1 expression on activated donor T cells might inhibit GVHD by down regulating donor anti-host T cell responses, consistent with PD-L1 co-inhibitory activity when expressed on host parenchymal cells during GVHD. 46. Urine neopterin: a new parameter for serial monitoring of disease activity in patients with systemic lupus erythematosus

Abstract

Changing levels of urine neopterin may indicate fluctuations in disease among patients with systemic lupus erythematosus (SLE). Urine neopterin is a byproduct of upregulation of the immune system and thus potentially provides a means of evaluating flare ups and remissions of an autoimmune disease. Researchers examine 68 patients with SLE regularly for up to 18 months and took regular urine samples. Urine neopterin levels rose in 14 patients who had disease flare-ups and declined among 30 patients who had active disease and went into remission. Neopterin levels were significantly correlated with disease severity. Neopterin measurements may prove to be a useful way of monitoring disease activity.

47. Improved hydrogen storage performances of LiAlH4 + Mg(BH4)2 composite with TiF3 addition.

Abstract

Summary: This paper studies on the preparation of the alanate‐borohydride combined system, LiAlH4 + Mg(BH4)2 with diverse molar ratios (1:1, 1:2, and 2:1) using the ball milling technique. The findings show that there is a mutual destabilization between the hydrides where the newly combined system has superior hydrogen storage performances as opposed to the unary components (LiAlH4 and Mg[BH4]2). Analysis on the initial decomposition temperature and isothermal de/hydrogenation kinetics has proven that the 2LiAlH4 + Mg(BH4)2 system possesses better performance. In an endeavor to ameliorate the performances of the hydrogen storage of 2LiAlH4 + Mg(BH4)2, the destabilized system was doped with TiF3. Three major steps of desorption were detected during the heating process in the 2LiAlH4 + Mg(BH4)2 system with and without the addition of TiF3, which are correlated to the decomposition of Mg(AlH4)2, MgH2 and LiBH4. It is found that 2LiAlH4 + Mg(BH4)2 + 5 wt.% TiF3 has decreased the initial decomposition temperature at about 60°C, which is 55°C lower than the non‐catalyzed 2LiAlH4 + Mg(BH4)2 system. The isothermal absorption/desorption kinetics of the 2LiAlH4 + Mg(BH4)2 system have also been enhanced by the addition of TiF3. The activation energy for Mg(AlH4)2‐, MgH2‐, and LiBH4‐relevant decomposition after doped 2LiAlH4 + Mg(BH4)2 with TiF3 are reduced to 36.5, 23.3, and 17.6 kJ mol−1. Studies on the structural characteristics analysis of the 2LiAlH4 + Mg(BH4)2 + TiF3 sample hint to the fact that the formation of the MgF2, LiF, TiH2 and Ti‐Al species, during desorption, is the main responsible factor for the observed thermodynamics change of the reactions by modifying the dehydrogenation and hydrogenation pathway. The catalytic role played by TiF3 may have encouraged the interaction between Mg(AlH4)2, MgH2, and LiBH4, further ameliorate the de/hydrogenation of the 2LiAlH4 + Mg(BH4)2 + TiF3 destabilized system. Abstract

Background: This study assessed the potential cost-effectiveness of high (80–100%) vs low (21–35%) fraction of inspired oxygen (FiO2) at preventing surgical site infections (SSIs) after abdominal surgery in Nigeria, India, and South Africa. Methods: Decision-analytic models were constructed using best available evidence sourced from unbundled data of an ongoing pilot trial assessing the effectiveness of high FiO2, published literature, and a cost survey in Nigeria, India, and South Africa. Effectiveness was measured as percentage of SSIs at 30 days after surgery, a healthcare perspective was adopted, and costs were reported in US dollars ($). Results: High FiO2 may be cost-effective (cheaper and effective). In Nigeria, the average cost for high FiO2 was $216 compared with $222 for low FiO2 leading to a −$6 (95% confidence interval [CI]: −$13 to −$1) difference in costs. In India, the average cost for high FiO2 was $184 compared with $195 for low FiO2 leading to a −$11 (95% CI: −$15 to −$6) difference in costs. In South Africa, the average cost for high FiO2 was $1164 compared with $1257 for low FiO2 leading to a −$93 (95% CI: −$132 to −$65) difference in costs. The high FiO2 arm had few SSIs, 7.33% compared with 8.38% for low FiO2, leading to a −1.05 (95% CI: −1.14 to −0.90) percentage point reduction in SSIs. Conclusion: High FiO2 could be cost-effective at preventing SSIs in the three countries but further data from large clinical trials are required to confirm this.
