Abstract Background Understanding antimicrobial prescribing (AMP) practices and their prudent use in livestock can support the implementation of stewardship programs in veterinary medicine. Empiric therapy using antimicrobials is widely practiced in resource-poor settings, including Ethiopia. This could significantly contribute to the global burden of antimicrobial resistance (AMR) and the potential accumulation of residues in food. This study assessed AMP practices in Ethiopian veterinary clinics located in different agroecological zones (AEZs) as well as adherence to antimicrobial stewardship principles (ASPs). Methods Data were collected from case book records from four district veterinary clinics located in different AEZs in Ethiopia from 2015 to 2019. To identify factors associated with non-prudent AMP practices (i.e., the use of antimicrobials without therapeutic effects or benefits to the animal), data from registered clinical veterinary cases were selected using a systematic random sampling method. This led to the inclusion of approximately one-third of all records, including animal characteristics, disease symptoms and diagnosis, and details about the prescribed drugs. Descriptive statistics were used to report the proportions of drugs prescribed and adherence to the ASPs. The Chi-square test was used to establish an association between the drugs prescribed and the disease diagnoses and the districts. The factors associated with cases receiving antimicrobials and non-prudent AMP practices were also assessed using a multivariable logistic regression analysis. Results In total, 12,438 clinical case records were considered. Approximately 97% of these cases received treatment without laboratory confirmation, and 15,243 drugs were prescribed. Among these prescriptions, 75% were for antimicrobials, with oxytetracycline (29%) and a combined formulation of penicillin-dihydrostreptomycin (19%) being the most often prescribed drugs. Abstract Background Persistent challenges of fragmented, food safety management in low- and middle-income countries underscore the need for more robustly coordinated mechanisms. National food safety technical working groups, operating under a One Health framework, offer potential in streamlining coordination efforts to effectively address these challenges. However, more clarity regarding their formation and functioning is important for understanding how to best establish and support such groups. The aim of this study is to systematically document the development process of established groups in Vietnam and Ethiopia. Methods We assess the process used to establish and support the technical working groups against six critical success factors for multisectoral collaboration: drive change, define, design, realise, relate, and capture success. To do so, we review meeting minutes, Terms of Reference, and other related publications. Results The analysis underscores the importance of financial and technical support by development partners in initiating working groups while also highlighting the challenge posed by the absence of legal frameworks to secure government commitment. Embedding the technical working groups within existing government structures – such as One Health platforms – from the outset could help to ensure the active participation and sustainability of such groups. Conclusion Both Vietnam and Ethiopia have established operational and institutionalized technical working groups to bolster national food safety efforts under a One Health framework. The approaches employed in these countries could serve as valuable models for others seeking to establish comparable multisectoral collaborative mechanisms to address emerging health risks.

Abstract Background In Ethiopia, milk production and handling practices often lack proper hygiene measures, leading to the potential contamination of milk and milk products with Staphylococcus aureus (S. aureus), including methicillin-resistant strains, posing significant public health concerns. This study aimed to investigate the occurrence, antimicrobial susceptibility profiles and presence of resistance genes in S. aureus strains isolated from milk and milk products. Methods A cross-sectional study was conducted in the Arsi highlands, Oromia, Ethiopia from March 2022 to February 2023. A total of 503 milk and milk product samples were collected, comprising 259 raw milk, 219 cottage cheese, and 25 traditional yogurt samples. S. aureus isolation and coagulase-positive staphylococci enumeration were performed using Baird-Parker agar supplemented with tellurite and egg yolk. S. aureus was further characterized based on colony morphology, Gram stain, mannitol fermentation, catalase test, and coagulase test. Phenotypic antimicrobial resistance was assessed using the Kirby–Bauer disc diffusion method, while the polymerase chain reaction (PCR) was employed for confirming the presence of S. aureus and detecting antimicrobial resistance genes. Results S. aureus was detected in 24.9% of the milk and milk products, with the highest occurrence in raw milk (40.9%), followed by yogurt (20%), and cottage cheese (6.4%). The geometric mean for coagulase-positive staphylococci counts in raw milk, yogurt, and cottage cheese was 4.6, 3.8, and 3.2 log10 CFU/mL, respectively. Antimicrobial resistance analysis revealed high levels of resistance to ampicillin (89.7%) and penicillin G (87.2%), with 71.8% of the isolates demonstrating multidrug resistance. Of the 16 S. aureus isolates analyzed using PCR, all were found to carry the nuc gene, with the mecA and blaZ genes detected in 50% of these isolates each. Conclusion This study revealed the widespread distribution of S. aureus in milk and milk products in the Arsi highlands of Ethiopia. The isolates displayed high resistance to ampicillin and penicillin, with a concerning level of multidrug resistance. The detection of the mecA and blaZ genes in selected isolates is of particular concern, highlighting a potential public health hazard and posing a challenge to effective antimicrobial treatment. These findings highlight the urgent need to enhance hygiene standards in milk and milk product handling and promote the rational use of antimicrobial drugs. Provision of adequate training for all individuals involved in the dairy sector can help minimize contamination. These measures are crucial in addressing the threats posed by S. aureus, including methicillin-resistant strains, and ensuring the safety of milk and its products for consumers.

Abstract Milk is a nutritious food that plays a great role in the diets of a society largely dependent on livestock production. On the other hand, contaminants can enter milk naturally or intentionally, causing a negative impact on the health of consumers. Milk adulteration is a wide concern in the dairy industry in many countries, including Ethiopia, with a subsequent negative impact on its nutritive value and potentially affecting the health of consumers. This study was designed to assess the perceptions of rural and urban residents in Borana pastoral and agro-pastoral areas in Ethiopia related to milk adulteration. It was also aimed at identifying the potential reasons for milk adulteration in the area. A semi-structured questionnaire and focus group discussions (FGDs) were used to collect quantitative and qualitative data, respectively, focusing on the types of substances added to milk and the reasons for the addition of the substances. In rural and urban areas, 73.1% and 91.7% of respondents reported suspicion of the addition of nonmilk substances or milk of other animal species to cow’s milk before selling, respectively. According to the qualitative data, most reported adulterants were water and ‘pasta or rice water’ (a murky fluid left after boiling rice or pasta). Respondents mentioned that they identify adulterated milk by observation or tasting. Economic gain was the primary perceived reason to adulterate the milk according to the study participants. The respondents had concerns about the quality and safety of milk associated with adulteration in the area. The weak enforcement of regulations related to milk quality and marketing as well as the inadequacy of capacity for the detection of adulteration were mentioned as gaps toward mitigating the problems. Awareness creation about the negative impacts of milk adulteration among the community supported by strategies for regulation, such as improving regular testing of milk and taking actions on adulterated milk, is recommended to tackle consumer concerns around milk adulteration in the area.

BackgroundThe value chains of tomatoes in Ethiopia are largely informal which may pose potential food safety risks. Value chains (VCs) mapping was carried out with emphasis on tomatoes in two major cities in eastern Ethiopia—Harar and Dire Dawa—which were assessed to identify practices likely to result in unsafe food, considering production, transportation, retail, preparation, and consumption.MethodsQualitative methods were used to map the VCs to understand the flows, actors, and practices. Group discussions and key informant interviews were performed to better understand the processes, practices, beliefs, and food safety risks in these VCs.ResultsThe two cities are supplied by two vegetable VCs: the first and larger being from distant producers in central Ethiopia, and the second from surrounding rural and peri-urban producers. The long-distance VCs involve producers, brokers, transporters, wholesalers, retailers, and consumers. The local producers, however, bring their fresh products directly to the cities without the involvement of VC actors other than final retailers and consumers. The study suggests microbiological contamination risks along tomato VCs, potentially including soil contamination, use of sewerage-contaminated irrigation water, untreated manure, unhygienic handling and storage conditions, and dirty contact surfaces during transportation and retailing. Tomatoes are mostly harvested by hand picking, collected, and sorted on the ground exposing to sunlight and physical bruising with potential contamination. More importantly, tomatoes are widely consumed raw without “a kill-step” that certainly contributes to food-borne infections. Suggestions by study participants for improving food safety and hygiene include funding toward improved infrastructure and facilities in the sectors, supporting VC actors with improved technology for quality production, and increasing awareness of good and hygienic practices. Consumers were particularly concerned about contamination with agrochemicals without much emphasis on the potential microbial contaminants. Fresh tomatoes are prepared in a variety of ways and are often consumed raw or slightly cooked. Further recommendations included using health extension workers to conduct awareness campaigns on improved food safety and hygienic practices.ConclusionThe qualitative VC mapping generated useful information for designing intervention strategies, especially targeting developing food safety interventions and an awareness communication campaign.

Abstract Background The determination of the microbiological quality and safety of raw milk and the associated influencing factors at the farm level is very critical given that the quality or safety of subsequent products that are further produced depends on this. Therefore, this study aimed to determine the microbiological quality and safety of bulk milk and identify associated risk factors, and assess the presence/absence of S. aureus in bulk milk with potential contaminating sources in dairy farms in Asella, Ethiopia. Results The geometric means of bacterial counts in farm bulk milk were 5.25 log cfu/ml, 3.1 log cfu/ml and 2.97 log cfu/ml for total bacterial count (TBC), coliform count (CC) and coagulase-positive staphylococci count (CPS), respectively. Of the 50 dairy farms, 66, 88, and 32% had TBC, CC and CPS counts, respectively, that exceeded the standard international limits for raw cow’s milk intended for direct human consumption. TBC tended to increase as CC increased in bulk milk (r = 0.5). In the final regression model, increased TBC, CC and the contamination of farm bulk milk by S. aureus were significantly associated with dirty barns, dirty cows and soiled udder and teats. Abstract Background Food-borne pathogens are the foremost causes of food-borne human illness in the world. Escherichia coli O157:H7 (E. coli O157:H7) is one of the major food-borne pathogenic bacteria around the world. Though evidence is lacking; especially in developing countries like Ethiopia, the potential health impact of E. coli O157:H7 can be high where food production, handling and consumption is often taking place under unhygienic conditions. In Ethiopia, studies reported E. coli and E. coli O157: H7 from food of animal origin, mainly meat and milk, and also animal surfaces and feces. The objective of the present study was to investigate the occurrence of E. coli O157:H7 in raw milk and the dairy production farm environment and further assess the antimicrobial resistance pattern of the bacterium. Methods Samples of milk from individual lactating cows’ and dairy farm environmental samples (feces, water and manure) were collected at Adami Tulu Jido Kombolcha district (ATJKD) and analyzed for the presence of E. coli O157:H7. Standard microbiological techniques including culture, biochemical testing and serological test were performed to isolate and identify the bacterium. The bacterial isolates were evaluated for antimicrobial susceptibility patterns using disk diffusion method. A questionnaire was used to collect possible factors affecting E. coli O157:H7 occurrence. Results The overall prevalence of E. coli O157:H7 was 4.7% (19/408) (95% CI: 2.6; 6.7). Out of 19 E. coli O157:H7 isolates, 4/50, 7/154, 2/50, and 6/154 were from water, milk, manure, and feces samples, respectively. From potential risk factors considered in this study area, floor type, cleaning of pens, milking location and hand washing during the time of milking were significantly associated with the occurrence of E. coli O157:H7. The antimicrobial susceptibility pattern indicated varying degrees of resistance. All of the isolates were found to be resistant ampicillin, cephalothin, and rifampin, and 100% susceptibility was observed against the drugs: chloramphenicol, ciprofloxacin, gentamicin, nalidixic acid, kanamycin, and tetracycline. Concerning streptomycin, 63.15% of the isolates were susceptible and 36.8% showed intermediate susceptibility. Conclusions The occurrence of multi-drug resistance E. coli O157:H7 observed both in lactating cows and in dairy farm environments can sustain a continuous transmission of the bacteria. The occurrence of multidrug-resistant E. coli o157:H7could hamper the control and prevention efforts.

Abstract Background Respiratory disease is the most common presenting complaint at veterinary clinics and a priority concern for equid owners and veterinary practitioners in Ethiopia. Objectives This study aimed to report the molecular detection of EHV‐2 and EHV‐5 and to assess the risk factors associated with infection in working equids in central Ethiopia. Methods Nasopharyngeal swabs were collected from 58 horses and donkeys to detect EHV‐2 and EHV‐5 using PCR targeting the conserved region of glycoprotein B (gB) genes. Results From 58 equids, EHV‐5 and EHV‐2 were detected in 20 (34.5%) and 19 (32.8%) equids, respectively. Concurrent infection with EHV‐2 and EHV‐5 was found in 6 (10.3%) equids who exhibited respiratory clinical signs. EHV‐2 was detected in a significantly higher (p = 0.002) proportion of horses (54.5%; n = 18) than donkeys (4%; n = 1). In contrast, EHV‐5 was detected in a significantly higher (p = 0.004) proportion of donkeys (56%; n = 14) compared to horses (18.2% n = 6). EHV‐2‐positive equids were seven times more likely to display clinical signs of respiratory disease than EHV‐2‐negative equids (Odds ratio (OR) = 6.9; 95%CI: 1.72‐27.60). However, statistically significant (p = 0.832) difference was not observed for EHV‐5. EHV‐2 was detected in a significantly higher (p = 0.004) proportion of female (50%; n = 16) compared to male equids (11.5%; n = 3). Conclusions This study revealed the molecular detection of EHV‐2 and EHV‐5 in horses and donkeys residing in central Ethiopia. The association between EHV‐2‐test‐positive equids and displaying of clinical signs of respiratory disease was observed, which suggests EHV‐2 involvement in the development of respiratory disease; however, it deserves further investigation.

Abstract Background Rift Valley fever virus (RVFV) is the cause of one of the most important mosquito‐borne emerging diseases negatively affecting the health of humans and animals, particularly in Africa. In Ethiopia, the status of RVFV and the existence of potential vectors are unknown. Objectives This study aimed to survey the mosquito vectors of RVFV and the detection of the virus in selected sites (Batu, Hawassa, Arba Minch and Borana) in Ethiopia. Methods CDC light traps baited with the sugar‐yeast solution were set up at various locations for a total of 29 trap nights. Mosquitoes identification were made morphologically using a stereomicroscope and for RVFV detection by reverse transcriptase‐polymerase chain reaction (RT‐PCR). Results Among a total of 132 trap efforts conducted, 60 (45%) captured the mosquitoes. A total of 1576 adult mosquitoes were collected and identified. Including Aedes (n = 407; 25.8%), Anopheles (n = 493; 32.3%), Culex (n = 466; 29.6%) and Mansonia (n = 210; 13.32%). The genome material of RVFV was not detected by RT‐PCR. Conclusions The existence of a potential Aedes species may pose a risk for the occurrence of the RVF outbreak in Ethiopia. Based on the current study, we recommend further monitoring for potential mosquito vectors of RVFV, particularly with a view to targeting the seasons during which the mosquitoes can be abundant along with a serological survey of susceptible hosts.

This scoping review identifies and describes the methods used to prioritize diseases for resource allocation across disease control, surveillance, and research and the methods used generally in decision-making on animal health policy. Three electronic databases (Medline/PubMed, Embase, and CAB Abstracts) were searched for articles from 2000 to 2021. Searches identified 6, 395 articles after de-duplication, with an additional 64 articles added manually. A total of 6, 460 articles were imported to online document review management software (sysrev.com) for screening. Based on inclusion and exclusion criteria, 532 articles passed the first screening, and after a second round of screening, 336 articles were recommended for full review. A total of 40 articles were removed after data extraction. Another 11 articles were added, having been obtained from cross-citations of already identified articles, providing a total of 307 articles to be considered in the scoping review. The results show that the main methods used for disease prioritization were based on economic analysis, multi-criteria evaluation, risk assessment, simple ranking, spatial risk mapping, and simulation modeling. Disease prioritization was performed to aid in decision-making related to various categories: (1) disease control, prevention, or eradication strategies, (2) general organizational strategy, (3) identification of high-risk areas or populations, (4) assessment of risk of disease introduction or occurrence, (5) disease surveillance, and (6) research priority setting. Of the articles included in data extraction, 50.5% had a national focus, 12.3% were local, 11.9% were regional, 6.5% were sub-national, and 3.9% were global. In 15.2% of the articles, the geographic focus was not specified. The scoping review revealed the lack of comprehensive, integrated, and mutually compatible approaches to disease prioritization and decision support tools for animal health. We recommend that future studies should focus on creating comprehensive and harmonized frameworks describing methods for disease prioritization and decision-making tools in animal health.

BackgroundFoodborne diseases impose substantial public health burden and jeopardize socio-economic development worldwide. While accurate information on foodborne hazards is needed for informed decision in food safety interventions, such information is scarce in developing countries such as Burkina Faso. We conducted a systematic review and meta-analysis of studies reporting foodborne hazards in foods in Burkina Faso to describe the present knowledge of the situation.MethodsPreferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guideline was used to conduct this review. Abstracts were searched in PubMed and CAB direct between 1 January 1990 to 30 September 2019. We used random-effects models to estimate pooled prevalence and I2 values to measure heterogeneity between studies.Results188 articles were identified, of which 14 are included in this review: 12 were on bacterial hazards (Salmonella, Campylobacter, Staphylococcus, E. coli, Shigella), three on fungal hazards and one on parasitic hazards (Toxoplasma gondii). The overall pooled prevalence of Salmonella spp. was 13% (95% CI: 8–21), the highest in lettuce: 50% (95% CI: 30–70) and the lowest in milk: 1.2% (95% CI: 0–5), demonstrating substantial variation among the studies (I2 = 85, 95% CI: 79–90%, p

A cross-sectional study was conducted to assess the prevalence, molecular detection, and antimicrobial resistance of Salmonella isolates within 162 poultry farms in selected urban and peri-urban areas of central Ethiopia. A total of 1515 samples, including cloacal swabs (n = 763), fresh fecal droppings (n = 188), litter (n = 188), feed (n = 188), and water (n = 188), were bacteriologically tested. The molecular detection of some culture-positive isolates was performed via polymerase chain reaction (PCR) by targeting spy and sdfl genes for Salmonella Typhimurium and Salmonella Enteritidis, respectively. Risk factors for the occurrence of the bacterial isolates were assessed. Antimicrobial susceptibility testing of PCR-confirmed Salmonella isolates was conducted using 12 antibiotics. In this study, it was observed that 50.6% of the farms were positive for Salmonella. The overall sample-level prevalence of Salmonella was 14.4%. Among the analyzed risk factors, the type of production, breed, and sample type demonstrated a statistically significant association (p < 0.05) with the bacteriological prevalence of Salmonella. The PCR test disclosed that 45.5% (15/33) and 23.3% (10/43) of the isolates were positive for genes of Salmonella Typhimurium and Salmonella Enteritidis, respectively. The antimicrobial susceptibility test disclosed multi-drug resistance to ten of the tested antibiotics that belong to different classes. Substantial isolation of Salmonella Typhimurium and Salmonella Enteritidis in poultry and on poultry farms, along with the existence of multi-drug resistant isolates, poses an alarming risk of zoonotic and food safety issues. Hence, routine flock testing, farm surveillance, biosecurity intervention, stringent antimicrobial use regulations, and policy support for the sector are highly needed.

Abstract Background Staphylococcus aureus is an important opportunistic pathogen of raw milk and milk products, and the enterotoxins cause food poisoning. Milk and milk products are important reservoirs of enterotoxin-producing S. aureus. The aims of this systematic review were to estimate the pooled prevalence of S. aureus, including methicillin-resistant Staphylococcus aureus (MRSA), and to summarize their molecular characteristics, assess the potential sources of S. aureus contamination in bulk milk and analyse the antimicrobial resistance patterns of the isolates. Methods Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we searched publicly available scientific online databases and search engines: PubMed, Research for Life, African Journal Online (AJOL), and Google Scholar. In addition, the reference lists of the identified studies were manually checked for relevant literature. A random effects model using the DerSimonian method was used to compute pooled prevalence estimates, and the data were transformed using variance stabilizing Freeman-Tukey double arcsine transformation. Results A total of 38 studies were included in this systematic review. The pooled prevalence of S. aureus was highest in raw cow milk (30.7%), followed by camel milk (19.3%), goat milk (13.6%) and pasteurized milk (3.8%). The pooled prevalence of S. aureus in locally produced soft cheese (ayib) and traditional fermented milk (ergo) was 18.6% and 14.9%, respectively. The pooled prevalence of MRSA in milk and milk products was 0.73%. In this study, 58.9% of S. aureus isolates recovered from milk and milk products harbored at least one type of enterotoxin gene. Raw milk of the three species (cow, goat and camel) showed the highest S. aureus pooled prevalence rate at processing plants (50.3%), followed by milk collection centers (MCCs) (47.1%), selling points (34.5%), farm bulk milk (25.8%), milking buckets (24.8%) and udder milk (20.3%). Water for washing milking utensils (39.3%) was more contaminated than swab samples from farm workers’ nares (31.5%), milkers’ hands (25.9%), MCCs containers (23.8%), bulk tanks (20.4%), udders (15.6%), milking buckets (14.2%) and towels (10%). S. aureus isolates were highly resistant to penicillin G (92%), followed by ampicillin (82%) and amoxicillin (62.6%). The pooled multidrug resistance (MDR) was high (62.1%). Conclusion This systematic review revealed a high and increasing level of S. aureus contamination of raw milk from udder to MCCs or processing plants. Enterotoxin genes and MRSA were reported in milk, milk products and samples from farm workers. S. aureus showed resistance to different antimicrobial agents, with β-lactams showing the highest pooled antimicrobial resistance and the level of MDR was high. The results of this study indicated that the consumption of raw milk and milk products may predispose consumers to staphylococcal food poisoning. Application of good hygiene and handling practices across the dairy value chain starting from farm, udder health, milk cooling, heat treatment of milk before drinking and rational use of antibiotics in veterinary medicine can reduce the potential health risks from S. aureus and MRSA contamination of milk and milk products.

IntroductionInappropriate antimicrobial use (AMU) in livestock production is an important aspect of the global burden of antimicrobial resistance (AMR). In Ethiopia, a low-income country with a large and increasing livestock population, AMU in food animals is not properly regulated. Hence, farmers are fully free to use antimicrobials to their (perceived) benefit. Therefore, understanding farmers' mindsets is important to improve antimicrobial stewardship in the livestock sector.MethodsThis cross-sectional study was conducted to assess livestock disease management practices and knowledge, attitude, and behavior (KAB) among livestock producers regarding AMU, residues, and resistance, as well as factors potentially explaining differences in KAB. We determined the KAB of livestock owners of three selected districts of central and western Ethiopia (n = 457), using a pretested questionnaire administered through face-to-face interviews. Logistic regression was used to evaluate the association between potential explanatory variables and the KAB scores of the respondents.ResultsThe results showed that 44% of the farmers used antimicrobials in the past few years, where antibiotics (21%) and trypanocides (11%) were most widely used to manage livestock diseases. Escherichia coli O157:H7, Salmonella and Staphylococcus aureus are common foodborne pathogens. We determined the prevalence of E. coli O157:H7 and Salmonella in feces and milk and the prevalence of S. aureus in milk from dairy cattle and camels in the Borana pastoral community in the Southern Oromia Region of Ethiopia. Paired individual cow composite (pooled from all quarters in equal proportions) milk and fecal samples were collected from cows (n = 154) and camels (n = 158). Samples were cultured on bacterial isolation and identification media. E. coli O157:H7 and Salmonella isolates were further tested for susceptibility against nine antimicrobial drugs. Different risk factors associated with hygienic milking practices were recorded and analyzed for their influence on the prevalence of these bacteria in milk and feces. The prevalence of E. coli O157:H7 and Salmonella in feces were 3.9% and 8.4%, respectively, in cows, and 0.6% and 2.5%, respectively, in camels. E. coli O157:H7 and Salmonella were detected in the composite milk samples of 2.6% and 3.9% of the cows, respectively, and 0% and 1.3% of the camels, respectively. S. aureus was detected in composite milk samples of 33.4% of the cows and 41.7% of the camels. All E. coli O157:H7 (n = 11) and Salmonella (n = 25) isolates from both animal species and sample types were resistant to at least one antimicrobial drug. Multidrug resistance was observed in 70% (7/10) of the E. coli O157:H7 fecal and milk isolates from cows and 33.3% (2/6) of the Salmonella fecal and milk isolates from camels. The prevalence of these bacteria in feces and milk was not affected by risk factors associated with milking practices. Given the very close contact between herders and their animals and the limited availability of water for hand washing and udder cleaning, these bacteria are most likely present in all niches in the community. Improving community awareness of the need to boil milk before consumption is a realistic public health approach to reducing the risk of these bacteria.

The present study aims to assess the level of implementation of biosecurity practices of small-, medium- and large-scale poultry farms in central Ethiopia. A cross-sectional study design was implemented involving 226 poultry farms (153 small-, 53 medium- and 20 large-scale farms) in three selected areas of central Ethiopia, including Addis Ababa, Bishoftu and West of Shaggar. The results revealed a very low overall biosecurity score of 43.1% for central Ethiopia compared to the global average score of 64.3%. No significant difference (p > 0.05) in the overall biosecurity practices among the three study sites (36.1% for Addis Ababa, 49% for Bishoftu and 34.9% for West of Shaggar) was observed. Six of the eight external biosecurity components with an overall score of 40.7% as compared to the global average of 64.6% were highly disrupted and scored lower values. With regard to internal biosecurity disease management, cleaning and disinfection practices were found to be 52.6% as opposed to the global average of 64%. The poor biosecurity level among poultry farms of central Ethiopia clearly demonstrates the urgent need for the implementation of appropriate biosecurity practices through the active involvement of all stakeholders to combat the impact of various diseases and boost the productivity of the sector.

Ethiopian dairy farming has many constraints including disease and lack of appropriate biosecurity measures. With this into consideration, a cross-sectional survey was carried out from November 2021 to April 2022 to determine the animal health biosecurity status of dairy farms and investigate the sociodemographic characteristics of livestock keepers on dairy farm management. A face-to-face questionnaire survey using an online application was used to collect data. The interview involved a total of 380 dairy farms located in six towns in central Ethiopia. The results showed that out of the surveyed farms, 97.6% missed footbaths at their gate points, 87.4% lacked isolation areas for either sick or newly introduced cattle, and 83.4% did not check the health status or quarantine newly introduced cattle. Furthermore, written formal record-keepings on animal health was uncommon, except for a few farms (7.9%). However, nearly all of the respondents (97.9%) gave medical treatments for sick cattle, and 57.1% of them vaccinated their herds regularly during the past 12 months before the survey. Hygienic aspects of the farms showed that 77.4% of the dairy farms appeared to clean the barn on a daily basis. However, 53.2% of respondents did not utilize personal protective equipment while cleaning their farms. A quarter of the dairy farmer (25.8%) avoided mixing their cattle with other herds, and 32.9% of them have implemented isolation of sick animals. In general, the animal health biosecurity assessment of the farms showed that most of the dairy farms (79.5%) earned unacceptable biosecurity levels (score of ≤ 50%), whereas the remaining 20.5% of dairy farms had received a score of >50% (“acceptable level”). The gender of dairy farmers (χ2 value = 7.61; p = 0.006), education level (χ2 value = 12.04; p = 0.007), dairy farm ownership (χ2 value = 41.6; p < 0.001), training on dairy farm management (χ2 value = 37.1; p < 0.001), towns (χ2 value = 31.69; p < 0.001), farm size (χ2 value = 7.7; p = 0.006), and herd size (χ2 value = 28.2; p < 0.001) showed a significant statistical association with biosecurity status. Finally, the study revealed that the level of biosecurity adoption of dairy farms in central Ethiopia is mostly unsatisfactory and calls for designing and implementing intervention measures toward improved animal health in dairy farms and further public health.

BackgroundFoodborne disease (FBD) affects millions of people each year, posing a health burden similar to malaria, tuberculosis or HIV. A recent World Bank study estimated the productivity losses alone attributed to unsafe food within Africa at $20 billion in 2016, and the cost of treating these illnesses at an additional $3.5 billion. Ethiopia faces multiple food safety challenges due to lack of infrastructure and basic pre-requisites for food safety such as clean water and environment, washing facilities, compounded by limited implementation of food safety regulations, and a lack of incentives for producers to improve food safety. A consolidation of our understanding and evidence of the source, nature and scale of FBD in Ethiopia is needed to inform policy and future research. We performed a Systematic Literature Review (SLR) of publications on FBD occurrence in Ethiopia including hazard presence and impact.MethodThe SLR followed Cochrane and PRISMA guidelines. We searched PubMed and CAB-Direct for relevant publications between 1990 and 2019 (inclusive). Observational studies and reviews were included. Two reviewers screened titles and abstracts, and retained publications were reviewed in full for quality and data extraction.ResultIn total 128 articles met the inclusion criteria. Most articles focused on the identification of biological hazards in food. High levels of microbial contamination in different food value chains were often found in the small, ad hoc, observational studies that dominated the literature. Raw milk (22/128, 17.0%) and raw beef (21/128, 16.4%) were the most studied food products. Foodborne (FB) parasites were often found at higher rates in food than bacterial and viral pathogens, possibly due to differences in ease of identification. High levels of bacterial contamination on the hands of food handlers were widely reported. There were no reports on the incidence of human FBDs or resulting health and economic impacts.ConclusionOur findings reflect existing concerns around food safety in Ethiopia. A lack of substantial, coordinated studies with robust methodologies means fundamental gaps remain in our knowledge of FBD in Ethiopia, particularly regarding FBD burden and impact. Greater investment in food safety is needed, with enhanced and coordinated research and interventions.

Reproductive disorders in cows are crucial as reproduction and fertility are the two key pillars of dairy production profitability. The study was conducted to identify and estimate the prevalence of clinically manifested postpartum reproductive disorders, to identify the associated risk factors and to estimate the sero-prevalence of brucellosis in dairy cows kept under intensive and semi-intensive dairy farms in Wondo Genet district from November 2019 to September 2020. Out of the total 205 cows examined, 90 cows (43.90%) had at least one of the reproductive problems identified by either questionnaire interview or regular follow up of individual cows. Out of 146 cows retrospectively and 59 cows prospectively assessed for the presence of any reproductive disorder, 62 (42.47%) and 28 (47.46%) cows had at least one reproductive disorders, respectively. The objective of this study was to characterize the distribution of antimicrobial resistance (AMR) of Escherichia coli (E. coli) isolated from livestock feces and soil in smallholder livestock systems. A cross-sectional study was carried out sampling 77 randomly selected households in four districts representing two agroecologies and production systems. E. coli was isolated and the susceptibility to 15 antimicrobials was assessed. Of 462 E. coli isolates tested, resistance to at least one antimicrobial was detected in 52% (43.7–60.8) of isolates from cattle fecal samples, 34% (95% CI, 26.2–41.8) from sheep samples, 58% (95% CI, 47.9–68.2) from goat samples and 53% (95% CI, 43.2–62.4) from soil samples. AMR patterns for E. coli from livestock and soil showed some similarities, with the highest prevalence of resistance detected against streptomycin (33%), followed by amoxycillin/clavulanate (23%) and tetracycline (8%). The odds of detecting E. coli resistance to ≥2 antimicrobials in livestock fecal samples were nearly three times (Odd Ratio—OR: 2.9; 95% CI, 1.72–5.17; p = 0.000) higher in lowland pastoral than in highland mixed crop–livestock production systems. These findings provide insights into the status of resistance in livestock and soil, and associated risk factors in low-resource settings in Ethiopia.

Antimicrobial resistance (AMR) has been recognized as one of the greatest global threats for human and animal health. The present review retrieved up to date information on the epidemiology of AMR in the animal-source food chain in Ethiopia focusing on AMR in bacterial species isolated from food handlers, live animals, foods (animal origin and non-animal origin), and in environmental samples. Accordingly, pooled prevalence of AMR in the different sources was estimated. For data analysis, we used random effect meta-analysis and in order to avoid exclusion of studies with zero prevalence of antimicrobial resistance, Freeman-Tukey double arcsine transformation was applied. We identified 152 eligible studies and retrieved 4097 data records (183 in food handlers, 2055 in foods, 1040 in live animals and 819 for environmental samples) which together reported a total of 86,813 AMR tests with 64 different antimicrobial disks for 81 bacteria species. We present the pooled prevalence of AMR for major bacterium-antibiotic combination in different sample types. The pooled prevalence of AMR in bacteria from food producing live animals was 20%. High estimates of AMR pooled prevalence were found in bacteria identified from milk, food handlers and the environmental samples with 29%, and 28% in meat. In foods of non-animal origin, the prevalence was lower with 13%. In milk, the highest AMR estimate was found for penicillin (69%) followed by amoxicillin (51%). Regarding multi-drug resistance (MDR), the overall pooled prevalence was 74% among AMR positive samples. Microbes reported having a higher MDR pattern were: Staphylococcus spp. (96%), Salmonella spp. (81%) and Escherichia coli (77%). The present review revealed a high resistance against commonly used drugs for animal and human treatments and/or prophylaxis. In conclusion, the high estimate of prevalence of AMR observed in bacteria recovered from different sample sources related to the animal-source food chain (food, live animal and environment) can highlight the possible linkage among them. The MDR levels in several bacteria species are a clear indication that the threat is directed to many antimicrobials. Our review demonstrated that the high overall AMR resistance levels call for effective policy and intervention measures, which best address the problem along the food chain through a One Health approach.

Abstract Background Safety and wholesomeness of milk intended for human consumption are influenced by various interlinked factors. However, information on what these factors are, especially in the pastoral traditional communities of Ethiopia, is largely lacking. The objective of this study is to assess the hygienic milk production, processing and consumption practices, and behaviors of Borana pastoralists. Methods The study used qualitative participatory research methods. Individual semi-structured interviews, focus group discussions, informal discussions, and observations were carried out on (1) milk handling practices, (2) perceptions of quality and safety of milk, including perceived criteria for good milk, (3) awareness of milk-borne diseases, and (4) perception towards milk boiling practices. The interviews and discussions were audio recorded, transcribed, and analyzed by identifying themes. Results Some risky behaviors related to milk handling and consumption were identified. These include unhygienic conditions in handling milk and milk products, consumption behaviors such as consuming raw milk purchased from markets, and children directly consuming milk from the udder of animals (e.g., goats). There was a very strong reluctance to boil milk before consumption mainly because of the misconception that nutrients in the milk are destroyed when milk is boiled and “boiled milk is dead”. On the other hand, potential risk mitigation practices were identified such as smoking of milk containers (which may help reduce microbial growth), processing milk through fermentation, consuming milk in boiled tea, and a recent trend towards boiling milk for babies. However, the latter was not motivated by concern over microbial hazards but the belief that raw milk could form curds in children’s stomach which might then suffocate them. Conclusion The findings highlight the need to promote hygienic handling practices of milk and closely engage with local communities to improve their understanding of milk safety to facilitate change in practices. Educating pastoralists on good milk production practices should be given priority. One of the ways to do this could be by strengthening the integration of milk hygiene in research and development programs as an entry point for behavioral change towards the safe handling and consumption of milk and milk products.

Brucellosis is an important neglected zoonotic disease caused by infection with bacteria of the genus Brucella affecting different mammalian species including humans. A cross-sectional study was conducted to estimate the seroprevalence of brucellosis in camels and humans and its associated risk factors in Amibara District of Afar Region in Northeast Ethiopia, from October 2019 to May 2020. A total of 250 camel and 120 human sera were serially tested using the Rose Bengal plate test (RBPT) and complement fixation test (CFT). The overall seroprevalence of camel brucellosis in this study was 7.6% (95% CI: 4.9–11.56) by RBPT and 3.2% (95% CI: 1.63–6.2) by combined RBPT and CFT. In humans, twelve (10%) of the collected sera were positive by RBPT among which only four of them (3.33%) were positive by CFT. The risk factor analysis indicated that age, body condition, number of parity, and abortion history were significantly associated with Brucella seropositivity in camel (P ≤ 0.05). In humans, occupation and nonprotective handling of dystocia cases showed an apparent association with Brucella seropositivity. The results of this study indicated that brucellosis is a common health problem in camels and humans in Amibara District of Afar Region. The public health importance of this disease is associated with raw milk consumption and close contact with the animals having history of recent abortion. Therefore, controlling the risk factors, establishing Brucella diagnostic service in human clinics and hospitals, continuous social training with feedback assessments, and overall implementing of One Health approach framework to attain optimal health for people and domestic animals in area are recommended to safeguard the health of society.

Antimicrobial resistance (AMR) is a major public health threat, and inappropriate antimicrobial use (AMU) in food animal production can contribute to the global burden of AMR in humans. This study was conducted to understand knowledge, attitude, and practice (KAP) of smallholder livestock owners regarding antimicrobial use, residue, and resistance in three agro-ecological zones and production systems in Ethiopia. A cross-sectional study based on structured interviews was conducted. Twenty-one items were used to assess farmers' KAP. Item response theory (IRT) model and Cronbach's alpha were used to assess the KAP measurement scales. Inferential analyses were used to compare the differences in the practices in terms of the farm and socio-economic characteristics. There was a difference in the type of antimicrobials reported use between agro-ecological zones and production systems. Pastoralists most commonly used antibiotics (86.7%) followed by anthelminthics (70.8%). Overall, tetracyclines (36.4%), aminoglycosides (31.3%), and trimethoprim-sulfonamides (6.2%) were the most frequently used classes of antibiotics across the study sites. Human preparation antibiotics (tetracyclines) were also being used for veterinary purposes by 18.5% of pastoralist households. About 81.6% of livestock owners surveyed reported to have access to veterinary drugs although access varied between agro-ecological zones and production system. About 72.3% of pastoralists administered antibiotics by not following through the full treatment course. Moreover, 70% of respondents were not aware of the recommended withdrawal periods of milk and meat after antibiotic treatment. It was noticed that around 80 and 70% of respondents had a tendency to give doses higher or lower than recommended of antimicrobials, respectively. The study confirms the need for interventions to increase knowledge among smallholder farmers to improve the way antimicrobials in general and antibiotics in particular are used in these settings. In addition, professional involvement, supervision, and guidance can also lead to more efficient antimicrobial use by smallholder livestock owners. The study also highlights the need for research into the development of usable tools that measure antibiotic knowledge and attitudes.

Objectives: The objective of this study was to map the global risk of the major arboviral diseases transmitted by Aedes aegypti and Aedes albopictus by identifying areas where the diseases are reported, either through active transmission or travel-related outbreaks, as well as areas where the diseases are not currently reported but are nonetheless suitable for the vector. Methods: Data relating to five arboviral diseases (Zika, dengue fever, chikungunya, yellow fever, and Rift Valley fever (RVF)) were extracted from some of the largest contemporary databases and paired with data on the known distribution of their vectors, A. aegypti and A. albopictus. The disease occurrence data for the selected diseases were compiled from literature dating as far back as 1952 to as recent as 2017. The resulting datasets were aggregated at the country level, except in the case of the USA, where state-level data were used. Spatial analysis was used to process the data and to develop risk maps. Results: Out of the 250 countries/territories considered, 215 (86%) are potentially suitable for the survival and establishment of A. aegypti and/or A. albopictus. A. albopictus has suitability foci in 197 countries/territories, while there are 188 that are suitable for A. aegypti. There is considerable variation in the suitability range among countries/territories, but many of the tropical regions of the world provide high suitability over extensive areas. Globally, 146 (58.4%) countries/territories reported at least one arboviral disease, while 123 (49.2%) reported more than one of the above diseases. The overall numbers of countries/territories reporting autochthonous vector-borne occurrences of Zika, dengue, chikungunya, yellow fever, and RVF, were 85, 111, 106, 43, and 39, respectively. Conclusions: With 215 countries/territories potentially suitable for the most important arboviral disease vectors and more than half of these reporting cases, arboviral diseases are indeed a global public health threat. The increasing proportion of reports that include multiple arboviral diseases highlights the expanding range of their common transmission vectors. The shared features of these arboviral diseases should motivate efforts to combine interventions against these diseases.

Abstract Background Ethiopia has high prevalences of udder health problems including clinical and subclinical mastitis across production systems in different livestock species. Previous studies on udder health problems have largely focused on identification of mastitis causing microbial pathogens and associated risk factors. However, relatively little is known about the knowledge and beliefs of livestock keepers regarding udder health problems. An understanding of the beliefs on the other hand would facilitate effective communication between livestock keepers and animal health professionals. Therefore, this study aimed at exploring the knowledge and belief surrounding the causes, clinical signs and treatments for udder health problems in (agro-) pastoral communities in southern Ethiopia using qualitative investigation. Results The result showed that udder health problem, locally known as ‘dhukkuba muchaa’, which translates to ‘disease of teats’, was classified into three main types: (1) tick infestation (dirandisa), (2) swelling of udder often with pus discharge (nyaqarsa) and (3) acute mastitis caused by evil eye (buda) with ‘bloody milk’. Tick infestation was perceived to directly cause mechanical damage to udder tissue or to resulting in swelling leading to nyaqarsa. Our analysis also revealed the strong misperception that acute and severe swelling of udder was caused by evil eye. According to the pastoralists, cows with large udders in the late pregnancy are prone to evil eye infliction upon giving birth. The pastoralists often treat udder health problems by combining both modern and traditional methods. Removal of ticks by hand and acarcide application were the preferred methods for limiting tick infestation while swelling and evil eye cases were treated with antibiotics (e.g. oxytetracycline). The study also revealed that specific herbs, only known by the herbalists, were used for traditional treatment of udder health. Although this information could not be divulged at the time, it should form the subject of further investigation. Traditional treatment for evil eye was often administered through nostrils, raising questions about its effectiveness. Conclusion The narration given by the pastoralists in associating tick infestation with udder health problems was compatible with existing scientific evidences. In this respect, such local knowledge can be better utilized for the educational messages targeting control and management of tick infestation in livestock. However, the misperception of causes for acute udder swelling as evil eye can be problematic as far as the application of appropriate treatment and management of the problem is concerned. The misperception can significantly impact the welfare of animals and highlights the need for capacity building of the pastoralists on the causes and treatment of udder health problems.

Fisheries play a significant role in food security, livelihood, and source of income in developing countries. Although fish are a healthy source of protein, they can also spread diseases caused by pathogenic micro-organisms they may harbor. Epidemiology of foodborne pathogens is not well studied in Ethiopia. To address this issue to some extent, a cross-sectional study with a simple random sampling approach was conducted from October 2017 to May 2018 with the objectives of to isolate and estimate the prevalence of Escherichia coli O157: H7 in fish, and to evaluate the antimicrobial susceptibility pattern of the isolates in selected Lakes of Northern Ethiopia. All the microbial identification and isolation procedures were conducted based on ISO 6887-3:2017 recommendations. Antimicrobial susceptibility test was also performed following the standard procedure of Kirby-Bauer disk diffusion protocol. From the total 410 fish samples examined, six (1.46%) of them were found contaminated with Shiga toxin-producing Escherichia coli O157: H7 strain. The organism was isolated from landing sites (5/293) and local retail markets (1/75). Besides, Escherichia coli O157: H7 was isolated from filleted fish (5/214) and whole fish (1/125); however, it was not isolated from samples of ready to eat fish and working environments of restaurants. The antibiotic susceptibility test revealed that the isolates were resistant to Ampicillin and Streptomycin disks. However, Ciprofloxacin, Gentamicin and Nalidixic acid were found effective in inhibiting the growth of all of the isolates. Since pathogenic Escherichia coli strain was detected from fish, raw and undercooked fish consumption in Ethiopia may result in contracting infections. The occurrence of such pathogenic organisms in fish indicates the need for intervention by stakeholders. Supports like freezers, generators, the establishment of fish processing plants and on job training about proper fish handling practices may play a tremendous role in decreasing the level of contamination of fish in Ethiopia.

A study was carried out from August 2017 to February 2018 on lactating dairy cows, one-humped dromedary camels, and goats to determine mastitis in the Bule Hora and Dugda Dawa districts of in Southern Ethiopia. Milk samples from 564 udder quarters and udder halves from 171 animals consisting of 60 dairy cows, 51 camels, and 60 goats were tested for mastitis. Sixty-four positive udder milk samples were cultured, and bacterial mastitis pathogens were isolated and identified. The antibiotic resistance of bacterial isolates from milk with mastitis was tested against nine antimicrobials commonly used in the study area. Cow- and quarter-level prevalence of mastitis in dairy cows, camels, and goats was 33.3%, 26.3%, and 25% and 17.6%, 14.5%, and 20%, respectively. In cattle, the prevalence was significantly higher in Dugda Dawa than in Bule Hora. Major bacterial isolates were coagulase-negative Staphylococcus species (39.1%), S. aureus (17.2%), S. hyicus (14.1%), and S. intermedius and Escherichia coli (9.4% each). In camels, udder abnormality and mastitis were significantly higher in late lactation than in early lactation. Mastitis tends to increase with parity in camels. E. coli isolates were highly resistant to spectinomycin, vancomycin, and doxycycline, whereas most S. aureus isolates were multidrug-resistant. Most of the rural and periurban communities in this area consume raw milk, which indicates a high risk of infection with multidrug-resistant bacteria. We recommend a community-focused training program to improve community awareness of the need to boil milk and the risk of raw milk consumption.

Dogs are reservoirs of many zoonotic diseases. In Ethiopia, the majority of owned dogs are semi-stray, freely roaming in the community. Studies reporting dog borne zoonotic diseases are scarce in Ethiopia. This study was conducted to assess Dipylidium caninum infection in dogs and in children with gastrointestinal complaints in Bishoftu Town, Oromia. We collected 384 fecal samples from dogs presented to veterinary teaching hospital and 259 stool samples from children presented to Bishoftu Hospital for clinical examination. Samples were first macroscopically examined for the presence of proglotids, followed by microscopic examination for the presence of eggs with the direct smear following flotation technique. The prevalence of D. caninum was 21% (95% CI: 16.6–24.9) in dogs. Although not statistically significant (p > 0.05), higher prevalence was detected in adult (11.9%), local breed (17.7%), and male (12.6%) dogs compared to young (8.59%), exotic breed (2.86%), and females (7.81%), respectively. Dipylidium caninum was detected in a stool sample obtained from a three year-old child (0.4%, 1/259). This study showed that the prevalence of D. caninum in the dogs is high while it is rare in children. Although the prevalence in children is negligible in this study, the high proportion of infected dogs can pose a significant risk of infection in the general human population. Public health risk can be reduced by eliminating the semi-roaming of owned dogs and proper management of dogs with regular deworming and prevention of environmental contamination with dog feces. Similarly, raising public awareness about dog borne zoonoses and avoiding contact with dog feces are important.
