Abstract

Carotid endarterectomy (CEA) ist the most most frequently performed vascular surgical procedure, and belongs to the type of preventive interventions. Reperfusion syndrome is relatively rare complications of this procedure, but with clinically significant and potentially fatal consequences that can seriously compromise the outcome of treatment after CEA. The focus of this study is on frecuency of development of RS, the factors that favor its occurrence, causes of hemodinamc instability, the modalities of early diagnosis, as well as the possibilities of preventing this complication. The research covered the period from 2018 to 2022 during wich 963 CEA were performed., Karotidna endarterektomija (CAE) je najčešće izvođeni operativni zahvat vaskularne kirurgije. Spada u niz preventivnih postupaka. Reperfuzijski sindrom je relativno rijetka komplikacija ovog operativnog zahvata, ali s klinički značajnim i potencijalno opasnim posljedicama koje mogu ozbiljno ugroziti ishod liječenja nakon CEA. Fokus ovog rada je na učestalosti razvoja cerebralnog reperfuzijskog sindroma (CRS), čimbenicima koji pogoduju njegovoj pojavi, uzrocima hemodinamske nestabilnosti perioperativno, te modalitetima rane dijagnoze i terapijskog pristupa, kao i mogućnostima prevencije razvoja ove komplikaciju. Istraživanje je obuhvatilo petogodišnje razdoblje (2018–2022) KB Banjaluka u kojem su urađene 963 karotidne endarterektomije.

Abstract

Symptomatic stenosis of the left renal vein in preaortic position is consequence of compression of a. mesenterica superior to the aorta, and in retroaortic position, by compresion of aorta to the spine. Symptoms arise from hypertension in left renal vein. Our patient had pains in pelvis and in the left lumbal region for two years, and recidivant hematuria. Hematuria in left urether was diagnosed with cystoscopy. Color Doppler and CT angiography showed diametar of left renal vein retroaortic of 2 mm, and in proximal part of 14 mm. We decided for surgery and underwnt preaortic transposition of left renal vein preaortic with interposition of autovenous panel vein graft of v.saphena magna. Abstract

Superior mesenteric artery syndrome is a set of general and gastrointestinal disorders of intestinal obstruction due to high compression of the horizontal part of the duodenum by the superior mesenteric artery and aorta. Diagnosis is difficult. Along with conservative treatment, surgical treatment is the gastrojejunal or duodenal and jejunal anastomosis, or resection of the ligamentum Treitz, as well as relaxation, mobilization and rotation of the duodenal and jejunal transition, with or without duodenal and jejunal anastomosis. The literature describes a small number of cases with surgical treatment. The prevalence is 0,013% to about 0,78% of all radiographic findings in patients with gastrointestinal complaints. We had a female patient with chronical, high intestinal obstruction, and angiographic findings of the horizontal compression of the duodenum of the superior mesenteric artery and aorta, and she was treated with medications and surgery, resection of the ligament of Treitz, mobilization, transposition and dista rotation of the duodenal and jejuna circuit. 4. ANEURYSMAL VEIN GRAFT DISEASE AFTER ARTERIAL GRAFT RECONSTRUCTION.

Abstract

Aneurysmal vein graft disease is rare; according to literature there are fewer than 200 cases in the world. It can occur anywhere but most commonly in the popliteal region. Most patients had hyperlipidemia and hypertension. It is complicated by the rupture, thrombosis and acute ischemia. The study showed that 8 patients had atherosclerotic changes, and 4 non- atherosclerotic changes. The early follow up to 30 days was 100% of gtraft patency. Postoperative evaluation and adequate disclosure of aneurysmal disease of the venous graft is crucial for the success of surgical treatment. [ABSTRACT FROM AUTHOR]
