As the newly appointed Editor-in-Chief (EIC) of Global Pediatric Health (GPH), it is both an honor and a privilege to lead this prestigious journal, building upon the strong foundation established by my predecessor, Dr. Aishat Akere. Under her leadership and that of her predecessors, GPH has grown into a globally recognized platform for clinical studies, case studies, public health research, and health services delivery practices focused on the pediatric population. Dr. Akere’s tenure was marked by significant achievements, including the expansion of the Editorial Board, which strengthened the journal’s editorial team and expanded its global reach. My appointment comes at a time when the landscape of academic publishing is rapidly evolving, particularly with the rise in the number of online journals. Despite these changes, GPH continues to stand out, receiving over 230 submissions and more than 350 000 downloads in the year 2023 alone. These numbers reflect not only the journal’s relevance but also the trust that researchers and clinicians worldwide place in GPH as a reliable source of high-quality pediatric health research. As I step into this role, my vision is to further elevate the journal’s standing and impact by focusing on several key goals that align with the needs of our authors, the global community, and the field of pediatric health at large.

Newborn screening (NBS) is hailed as a public health success, but little is known about the long-term outcomes following a positive newborn screen. There has been difficulty gathering long-term follow-up (LTFU) data consistently, reliably, and with minimal effort. Six programs developed and tested a core set of minimal LTFU data elements. After an iterative data collection process and the development of a data collection tool, the group agreed on the minimal LTFU data elements. The denominator captured all infants with an NBS diagnosis, accounting for children who moved or died prior to the follow-up year. They also agreed on three LTFU outcomes: if the child was still alive, had contact with a specialist, and received appropriate care specific to their diagnosis within the year. The six programs representing NBS public health programs, clinical providers, and research programs provided data across multiple NBS disorders. In 2022, 83.8% (563/672) of the children identified by the LTFU programs were alive and living in the jurisdiction; of those, 92.0% (518/563) saw a specialist, and 87.7% (494/563) received appropriate care. The core LTFU data elements can be applied as a foundation to address the impact of early diagnosis by NBS within and across jurisdictions.

In the United States and around the world, newborns are screened on a population basis for conditions benefiting from pre-symptomatic diagnosis and treatment. The number of screened conditions continues to expand as novel technologies for screening, diagnosing, treating, and managing disease are discovered. While screening all newborns facilitates early diagnosis and treatment, most screened conditions are treatable but not curable. Patients identified by newborn screening often require lifelong medical management and community support to achieve the best possible outcome. To advance the long-term follow-up of infants identified through newborn screening (NBS), the Long-Term Follow-up Cares and Check Initiative (LTFU-Cares and Check) designed, implemented, and evaluated a system of longitudinal data collection and annual reporting engaging parents, clinical providers, and state NBS programs. The LTFU-Cares and Check focused on newborns identified with spinal muscular atrophy (SMA) through NBS and the longitudinal health information prioritized by parents and families. Pediatric neurologists who care for newborns with SMA entered annual data, and data tracking and visualization tools were delivered to state NBS programs with a participating clinical center. In this publication, we report on the development, use of, and preliminary results from the LTFU-Cares and Check Initiative, which was designed as a comprehensive model of LTFU. We also propose next steps for achieving the goal of a national system of LTFU for individuals with identified conditions by meaningfully engaging public health agencies, clinicians, parents, families, and communities.

Rapid advances in the screening, diagnosis, and treatment of genetic disorders have increased the number of conditions that can be detected through universal newborn screening (NBS). However, the addition of conditions to the Recommended Uniform Screening Panel (RUSP) and the implementation of nationwide screening has been a slow process taking several years to accomplish for individual conditions. Here, we describe web-based tools and resources developed and implemented by the newborn screening translational research network (NBSTRN) to advance newborn screening research and support NBS stakeholders worldwide. The NBSTRN’s tools include the Longitudinal Pediatric Data Resource (LPDR), the NBS Condition Resource (NBS-CR), the NBS Virtual Repository (NBS-VR), and the Ethical, Legal, and Social Issues (ELSI) Advantage. Research programs, including the Inborn Errors of Metabolism Information System (IBEM-IS), BabySeq, EarlyCheck, and Family Narratives Use Cases, have utilized NBSTRN’s tools and, in turn, contributed research data to further expand and refine these resources. Additionally, we discuss ongoing tool development to facilitate the expansion of genetic disease screening in increasingly diverse populations. In conclusion, NBSTRN’s tools and resources provide a trusted platform to enable NBS stakeholders to advance NBS research and improve clinical care for patients and their families.

Rapid advances in genomic technologies to screen, diagnose, and treat newborns will significantly increase the number of conditions in newborn screening (NBS). We previously identified four factors that delay and/or complicate NBS expansion: 1) variability in screening panels persists; 2) the short duration of pilots limits information about interventions and health outcomes; 3) recent recommended uniform screening panel (RUSP) additions are expanding the definition of NBS; and 4) the RUSP nomination and evidence review process has capacity constraints. In this paper, we developed a use case for each factor and suggested how model(s) could be used to evaluate changes and improvements. The literature on models was reviewed from a range of disciplines including system sciences, management, artificial intelligence, and machine learning. The results from our analysis highlighted that there is at least one model which could be applied to each of the four factors that has delayed and/or complicate NBS expansion. In conclusion, our paper supports the use of modeling to address the four challenges in the expansion of NBS.

Each year, through population-based newborn screening (NBS), 1 in 294 newborns is identified with a condition leading to early treatment and, in some cases, life-saving interventions. Rapid advancements in genomic technologies to screen, diagnose, and treat newborns promise to significantly expand the number of diseases and individuals impacted by NBS. However, expansion of NBS occurs slowly in the United States (US) and almost always occurs condition by condition and state by state with the goal of screening for all conditions on a federally recommended uniform panel. The Newborn Screening Translational Research Network (NBSTRN) conducted the NBS Expansion Study to describe current practices, identify expansion challenges, outline areas for improvement in NBS, and suggest how models could be used to evaluate changes and improvements. The NBS Expansion Study included a workshop of experts, a survey of clinicians, an analysis of data from online repositories of state NBS programs, reports and publications of completed pilots, federal committee reports, and proceedings, and the development of models to address the study findings. This manuscript (Part One) reports on the design, execution, and results of the NBS Expansion Study. The Study found that the capacity to expand NBS is variable across the US and that nationwide adoption of a new condition averages 9.5 years. Four factors that delay and/or complicate NBS expansion were identified. A companion paper (Part Two) presents a use case for each of the four factors and highlights how modeling could address these challenges to NBS expansion.

Newborn screening (NBS) is a public health service provided for all babies born in the United States and in most countries of the developed world. A series of tests are applied to the blood taken from newborn babies to detect genetic and metabolic disorders that can be treated if identified early. With early treatment and therapy, the affected babies can usually live a normal, healthy life. Timing for sampling, testing, and reporting is vital for NBS to function as an effective system. In order to be an effective system, the evolution of science, technology, and policy gradually had to come into a synchronous partnership, where the discovery of new genetic disorders led to timely development of technology for screening, which is supported by policy and implemented into practice. The timely “dance” of these partnerships in an era of personalized health and medicine forms the integrated approach supporting NBS. This review will include a brief history of scientific development, policymaking, and the economic consideration in the expansion of the NBS.

The goal of newborn screening is to improve health outcomes by identifying and treating affected newborns. This manuscript provides an overview of a data tool to facilitate the longitudinal collection of health information on newborns diagnosed with a condition through NBS. The Newborn Screening Translational Research Network (NBSTRN) developed the Longitudinal Pediatric Data Resource (LPDR) to capture, store, analyze, visualize, and share genomic and phenotypic data over the lifespan of NBS identified newborns to facilitate understanding of genetic disease and to assess the impact of early identification and treatment. NBSTRN developed a consensus-based process using clinical care experts to create, maintain, and evolve question and answer sets organized into common data elements (CDEs). The LPDR contains 24,172 core and disease specific CDEs for 118 rare genetic diseases, and the CDEs are being made available through the NIH CDE Repository. The number of CDEs for each condition average of 2200 with a range from 69 to 7944. The LPDR is used by state NBS programs, clinical researchers, and community-based organizations. Case level, de-identified data sets are available for secondary research and data mining. The development of the LPDR for longitudinal data gathering, sharing, and analysis supports research and facilitates the translation of discoveries into clinical practice.

Abstract Background Clinical decision-making has been conceptualized as a sequence of two separate processes: assessment of patients’ functioning and application of a decision threshold to determine whether the evidence is sufficient to justify a given decision. A range of factors, including use of evidence-based screening instruments, has the potential to influence either or both processes. However, implementation studies seldom specify or assess the mechanism by which screening is hypothesized to influence clinical decision-making, thus limiting their ability to address unexpected findings regarding clinicians’ behavior. Building on prior theory and empirical evidence, we created a system dynamics (SD) model of how physicians’ clinical decisions are influenced by their assessments of patients and by factors that may influence decision thresholds, such as knowledge of past patient outcomes. Using developmental-behavioral disorders as a case example, we then explore how referral decisions may be influenced by changes in context. Specifically, we compare predictions from the SD model to published implementation trials of evidence-based screening to understand physicians’ management of positive screening results and changes in referral rates. We also conduct virtual experiments regarding the influence of a variety of interventions that may influence physicians’ thresholds, including improved access to co-located mental health care and improved feedback systems regarding patient outcomes. Results Results of the SD model were consistent with recent implementation trials. For example, the SD model suggests that if screening improves physicians’ accuracy of assessment without also influencing decision thresholds, then a significant proportion of children with positive screens will not be referred and the effect of screening implementation on referral rates will be modest—results that are consistent with a large proportion of published screening trials. Consistent with prior theory, virtual experiments suggest that physicians’ decision thresholds can be influenced and detection of disabilities improved by increasing access to referral sources and enhancing feedback regarding false negative cases. Conclusions The SD model of clinical decision-making offers a theoretically based framework to improve understanding of physicians’ behavior and the results of screening implementation trials. The SD model is also useful for initial testing of hypothesized strategies to increase detection of under-identified medical conditions.

A future nuclear energy system is being developed at Korea Atomic Energy Research Institute (KAERI), the system involves a Sodium Fast Reactor (SFR) linked with the pyro-process. The pyro-process produces a source material to fabricate a SFR fuel rod. Therefore, an isotopic fissile content assay is very important for fuel rod safety and SFR economics. A new technology for an analysis of isotopic fissile content has been proposed using a lead slowing down spectrometer (LSDS). The new technology has several features for a fissile analysis from spent fuel: direct isotopic fissile assay, no background interference, and no requirement from burnup history information. Several calculations were done on the designed spectrometer geometry: detection sensitivity, neutron energy spectrum analysis, neutron fission characteristics, self shielding analysis, and neutron production mechanism. The spectrum was well organized even at low neutron energy and the threshold fission chamber was a proper choice to get prompt fast fission neutrons. The characteristic fission signature was obtained in slowing down neutron energy from each fissile isotope. Another application of LSDS is for an optimum design of the spent fuel storage, maximization of the burnup credit and provision of the burnup code correction factor. Additionally, an isotopic fissile content assay will contribute to an increase in transparency and credibility for the utilization of spent fuel nuclear material, as internationally demanded.

The N-H stretching frequencies of tri-n-octylammonium salts are reported for a series of weakly basic anions (A(-)), many of which are the conjugate bases of known strong acids and superacids. Data have been collected primarily in carbon tetrachloride, where Oct(3)N(+)-H...A(-) contact ion pairs are formed. In the more polar solvent 1,2-dichloroethane, some salts form both contact and solvent-separated ion pairs. Salts have also been studied in crystalline form or as oils. In general, the nuNH frequencies decrease in the order fluoroanions > carboranes > oxyanions, reflecting the relative basicities of the anions. By inference, the data reflect differences in the acidity of the corresponding conjugate acids (HA). This qualitative indicator of acid strength is useful because it reflects acidity on an individual molecule basis rather than in bulk. In this respect, it provides a condensed-phase analogy to gas-phase ("intrinsic") acidity and gives insight into the aggregation phenomena that determine bulk acidity. The data also reveal the importance of the chemical stability of conjugate base anions in attaining high acidity and suggest where acids stronger than those presently known may be discovered.

Salts of the C(3v) symmetric hydronium ion, H(3)O(+), have been obtained in the weakly basic solvents benzene, dichloromethane, and 1,2-dichloroethane. This is made possible by using carborane counterions of the type CHB(11)R(5)X(6)(-) (R = H, Me, Cl; X = Cl, Br, I) because they combine the three required properties of a suitable counterion: very low basicity, low polarizability, and high chemical stability. The existence of the H(3)O(+) ion requires the formation of three more-or-less equivalent, medium-to-strong H-bonds with solvent or anion bases. With the least basic anions such as CHB(11)Cl(11)(-), IR spectroscopy indicates that C(3v) symmetric trisolvates of formulation [H(3)O(+) .3Solv] are formed with chlorocarbon solvents and benzene, the latter with the formation of pi bonds. When the solvents and anions have comparable basicity, contact ion pairs of the type [H(3)O(+).nSolv.Carborane] are formed and close to C(3v) symmetry is retained. The conditions for the existence of the H(3)O(+) ion are much more exacting than previously appreciated. Outside of the range of solvent basicity bounded at the lower end by dichloromethane and the upper end by tributyl phosphate, and with anions that do not meet the stringent requirements of weak basicity, low polarizability of high chemical stability, lower symmetry species are formed. One H-bond from H(3)O(+) to the surrounding bases becomes stronger than the other two. The distortion from C(3v) symmetry is minor for bases weaker than dichloromethane. For bases stronger than tributyl phosphate, H(2)O-H(+)-B type species are formed that are more closely related to the H(5)O(2)(+) ion than to H(3)O(+). IR data allow criteria to be defined for the existence of the symmetric H(3)O(+) ion. This includes a linear dependence between the frequencies of nu(max)(OH) and delta(OH(3)) within the ranges 3010-2536 cm(-1) for nu(max)(OH) and 1597-1710 cm(-1) for delta(OH(3)). This provides a simple way to assess the correctness of the formulation of the proton state in monohydrated acids. In particular, the 30-year-old citation classic of the IR spectrum believed to arise from H(3)O(+) SbCl(6)(-) is re-interpreted in terms of (H(2)O)(x)().HSbCl(6) hydrates. The correctness of the hydronium ion formulation in crystalline H(3)O(+)A(-) salts (A(-) = Cl(-), NO(3)(-)) is confirmed, although, when A(-) is a fluoroanion, distortions from C(3)(v)() symmetry are suggested.

Infrared, X-ray structural, 1H NMR, and computational evidence for pi-solvation of H3O+ by benzene molecules is presented. A salt with a discrete [H3O.3benzene]+ cation can be isolated using a very weakly interacting carborane counterion, CHB11Cl11-. pi-Arene solvation of H3O+ explains the solubility of this salt in benzene solution. Similar results are indicated for the "Zundel-type" H5O2+ ion. These findings suggest structures for the active protonating species when strong acids are used as catalysts in arene solvents containing trace water. They are also relevant to structures that may be present in biological proton transport.

Measure for measure: Several measures indicate that carborane acids of the type H(CHB11R5X6) (see scheme, green R, red X, gray H, pink B) for R = H, Cl and X = Cl, Br, I are the strongest pure Bronsted acids. Based on NMR and IR spectroscopy, H(CHB11Cl11) can lay claim to be the strongest isolable acid presently known.

The nature of solvated acids in benzene and chlorinated hydrocarbon solvents is sensitively revealed by IR spectroscopy. Two similarly strong, structurally related acids, triflic acid (CF 3SO 3H) and the N-H acid bis-(trifluoromethylsulfonyl) imide ((CF 3SO 2) 2NH), behave quite differently toward water in. these media. Triflic acid protonates water at the one-equivalent level to give the hydronium ion, H 3O +. By contrast, bis(trifluoromethylsulfonyl) imide forms a simple hydrate, (CF 3SO 2) 2NH-OH 2, even with water at the two-equivalent level. The difference arises from the differing abilities of the conjugate base anions to engage in H-bonding and ion pairing. It illustrates how acidity in low-dielectric media is critically dependent on the nature of the conjugate base anion. νO-H and νN-H stretching frequencies indicate that the H-bonding acceptor ability for solvents toward acidic protons increases in the order carbon tetrachloride < benzene ∼ 1, 2-dichoroethane < water. With benzene, a marked tendency for acidic protons to H-bond to the π-face of the arene ring is noted and an IR signature of this solvation mode is observed.

We detail the uniform-sign cross-peak double-quantum-filtered correlation spectroscopy (UC2QF COSY) experiment, a new through-bond correlation method for disordered solids. This experiment is a refocused version of the popular double-quantum-filtered correlation spectroscopy experiment in liquids. Its key feature is that it provides in-phase and doubly absorptive line shapes, which renders it robust for chemical shift correlation in solids. Both theory and experiment point to distinct advantages of this protocol, which are illustrated by several experiments under challenging conditions, including fast magic-angle spinning (30kHz), anisotropic molecular motion, and (13)C correlation spectroscopy at the natural abundance isotope level.

A heterofullerene isoelectronic to C60 is reported. The azafullerenium cation C59N+ can be isolated in good yield as a carborane salt via the two-electron oxidation of the C-C bond of (C59N)2 dimer. [C59N][Ag(CB11H6Cl6)2] has been characterized by electronic, IR, Raman, and 13C NMR spectroscopies, MALDI spectrometry, DFT calculations, and X-ray crystallography.

When partnered with carborane anions, arenium ions are remarkably stable. Previously investigated only at subambient temperatures in highly superacidic media, protonated benzene is readily isolated as a crystalline salt, thermally stable to >150 degrees C. Salts of the type [H(arene)][carborane] have been prepared by protonating benzene, toluene, m-xylene, mesitylene, and hexamethylbenzene with the carborane superacid H(CB(11)HR(5)X(6)) (R = H, Me; X = Cl, Br). They have been characterized by elemental analysis, X-ray crystallography, NMR and IR methods. Solid-state (13)C NMR spectra are similar to those observed earlier in solution, indicating that lattice interactions are comparable to solution solvation effects. The acidic proton(s) of the arenium cations interact weakly with the halide substituents of the anion via ion pairing. This is reflected in the dependence of the C-H stretching frequency on the basicity of the carborane anion. Bond lengths in the arenium ions are consistent with predominant cyclohexadienyl cation character, but charge distribution within the cation is less well represented by this resonance form. Structural and vibrational comparison to theory is made for the benzenium ion (C(6)H(7)(+)) with density functional theory at B3LYP/6-31G and B3P86/6-311+G(d,p) levels. The stability of these salts elevates arenium ions from the status of transients (Wheland intermediates) to reagents. They have been used to bracket the solution-phase basicity of C(60) between that of mesitylene and xylene.

Large, inert, weakly basic carborane anions of the icosahedral type CHB(11)R(5)X(6)(-) (R = H, Me; X = Cl, Br) allow ready isolation and structural characterization of discrete salts of the solvated proton, [H(solvent)(x)][CHB(11)R(5)X(6)], (solvent = common O-atom donor). These oxonium ion Brønsted acids are convenient reagents for the tuned delivery of protons to organic solvents with a specified number of donor solvent molecules and with acidities leveled to those of the chosen donor solvent. They have greater thermal stability than the popular [H(OEt(2))(2)][BAr(F)] acids based on fluorinated tetraphenylborate counterions because carborane anions can sustain much higher levels of acidity. When organic O-atom donors such as diethyl ether, tetrahydrofuran, benzophenone, and nitrobenzene are involved, the coordination number of the proton (x) in [H(solvent)(x)()](+) is two. A mixed species involving the [H(H(2)O)(diethyl ether)](+) ion has also been isolated. These solid-state structures provide expectations for the predominant molecular structures of solvated protons in solution and take into account that water is an inevitable impurity in organic solvents. The O.O distances are all short, lying within the range from 2.35 to 2.48 A. They are consistent with strong, linear O.H.O hydrogen bonding. Density functional theory calculations indicate that all H(solvent)(2)(+) cations have low barriers to movement of the proton within an interval along the O.H.O trajectory, i.e., they are examples of so-called SSLB H-bonds (short, strong, low-barrier). Unusually broadened IR bands, diagnostic of SSLB H-bonds, are observed in these H(solvent)(2)(+) cations.

Evidence for a three-coordinate silyl cation is provided by the crystal structure of [(Mes)3Si][H-CB11Me5Br6].C6H6 (where Mes is 2,4,6-trimethylphenyl). Free (Mes)3Si+ cations are well separated from the carborane anions and benzene solvate molecules. Ortho-methyl groups of the mesityl substituents shield the silicon atom from the close approach of nucleophiles, while remaining innocent as significant ligands themselves. The silicon center is three-coordinate and planar. The downfield 29Si nuclear magnetic resonance chemical shift in the solid state (226.7 parts per million) is almost identical to that in benzene solution and in "gas phase" calculations, indicating that three-coordination can be preserved in all phases.

We present a novel nuclear magnetic resonance experiment for establishing through-bond connectivity in solids using scalar coupling-driven correlation. This method, a variant of the popular double-quantum-filtered correlation spectroscopy experiment in liquids, is robust under fast magic-angle-spinning conditions and in the presence of dynamics. In HC(60)(+), where anisotropic molecular motion renders through-space dipolar-driven correlation ineffective, this through-bond correlation method answers a significant structural question by accurately identifying the direct bond between the protonated sp(3) hybridized carbon site and the sp(2) hybridized cationic site.

Inert weakly coordinating carborane anions, CB(11)H(6)X(6)(-) (X = Cl, Br), allow access to the long sought, highly electrophilic diethylaluminum moiety in Et(2)Al(CB(11)H(6)X(6)). X-ray crystallography reveals ion-like structural features reminiscent of the corresponding trialkylsilylium species. Et(2)Al(CB(11)H(6)X(6)) is a potent catalyst for the electrophilic ethenation of benzene, the polymerization of cyclohexene oxide, and the oligomerization of ethene to a low molecular weight, highly branched product.

Aspects of the electron paramagnetic resonance (EPR) spectra of C60n- fulleride ions (n = 2, 3) and the EPR signal observed in solid C60 are reinterpreted. Insufficient levels of reduction and the unrecognized presence of C120O, a ubiquitous and unavoidable impurity in air-exposed C60, have compromised most previously reported spectra of fullerides. Central narrow line width signals ("spikes") are ascribed to C120On- (n = odd). Signals arising from axial triplets (g approximately 2.0015, D = 26-29 G) in the spectrum of C602- are ascribed to C120On- (n = 2 or 4). Their D values are more realistic for C120O than C60. Less distinct signals from "powder" triplets (D approximately 11 G) are ascribed to aggregates of C120On- (n = odd) arising from freezing nonglassing solvents. In highly purified samples of C60, we find no evidence for a broad approximately 30 G signal previously assigned to a thermally accessible triplet of C60(2-). The C60(2-) ion is EPR-silent. Signals previously ascribed to a quartet state of the C60(3-) ion are ascribed to C120O4-. Uncomplicated, authentic spectra of C60- and C60(3-) become available when fully reduced samples are prepared under strictly anaerobic conditions from freshly HPLC-purified C60. Solid off-the-shelf C60 has an EPR signal (g approximately 2.0025, DeltaH(pp) approximately 1.5 G) that is commonly ascribed to the radical cation C60*+. This signal can be reproduced by exposing highly purified, EPR-silent C60 to oxygen in the dark. Doping C60 with an authentic C60*+ salt gives a signal with much greater line width (DeltaH(pp) = 6-8 G). It is suggested that the EPR signal in air-exposed samples of C60 arises from a peroxide-bridged diradical, *C60-O-O-C60* or its decomposition products rather than from C60*+. Solid-state C60 is more sensitive to oxygen than previously appreciated such that contamination with C120O is almost impossible to avoid.

A new superacid, H(CB11H6X6) (where X = chlorine or bromine), whose conjugate base is the exceptionally inert CB11H6X6- carborane anion, separates Bronsted acidity from oxidizing capacity and anion nucleophilicity in a manner not previously achieved. Reaction of this superacid with C60 gives HC60+ as a stable ion in solution and in the solid state. In a separate experiment, an oxidant was developed such that the long-sought C60.+ ion can be synthesized in solution. The preparation of these two fullerene carbocations is a notable departure from the prevalent chemistry of C60, which is dominated by the formation of anions or the addition of nucleophiles. The H(CB11H6X6) superacid overcomes the major limitations of presently known superacids and has potentially wide application.
