Introduction: The objective was to conduct a systematic review of economic assessment of interventional strategies, in the prevention of frailty in elderly of 60 years and over living at home. Methods: The keywords were searched in databases such as Pubmed, ScienceDirect, Google Scholar and Embase. Articles published in English and French between 31/10/2010 and 31/12/2021 were included. The CHEERS statement reading grid was used to assess the quality of the studies in terms of economic assessments. Results: The search had identified nine relevant research studies, including seven randomised controlled trials and two quasi-experimental studies. Of these studies, we classified them into three programs: five studies on frailty screening, three studies on falls prevention and one study on the analysis of drugs and treatments prescribed and delivered. According to the cost-effectiveness plan of these programs, four studies had no conclusion on economic results, two studies had a dominant strategy, less expensive and more efficient, and three studies had a dominated strategy, more expensive and not efficient. Only 40% of the studies were of good quality. Conclusion: Only two multidimensional and interdisciplinary intervention strategies were highly efficient, cost-effective and improved the quality of life of elderly. Economic results were more mixed. Many methodological weaknesses were present in these studies.

In this study, we examined separately the association between VI and each adverse health conditions (cognitive disorders, vision-related quality of life [VRQoL], and daily functioning interference [DFI]) among Congolese older people. We also explored whether VI had a significant effect on VRQoL components in our population. Method: We performed cross-sectional analyses on data from 660 Congolese people aged ≥65 years who participated in the 2013 survey of the EPIDEMCA population-based cohort study. VI was defined as having a near visual acuity Results: VI was not associated with cognitive disorders after adjustment for residence area (adjusted odds ratio = 1.7; 95% confidence interval [CI]: 0.6; 4.7), but it was associated with a low VRQoL score (adjusted β = −12.4; 95% CI: −17.5; −7.3) even after controlling for several covariates. An interaction between VI and age (p = 0.007) was identified, and VI was associated with DFI only among people aged >73 years (adjusted β = 0.5; 95% CI: 0.2; 0.8). Our exploratory analysis showed that all components of VRQoL decreased with a decrease in visual acuity (corrected p ≤ 0.05). Conclusion: VI was associated with poor VRQoL and high DFI. Residence area seems to play a confounding role in the association between VI and cognitive disorders. Our findings suggest that targeting interventions on vision could reduce DFI among older people and improve their well-being.

International audience; Objective:It is recognized that some cardiovascular risk factors (CVRF) are involved in poor ocular health. The deleterious effects are potentially more pronounced among older adults. However, it is unknown whether CVRF areassociated independently or not to Visual Impairment (VI) among African older people while both (CVRF and VI) are verycommon in sub-Saharan Africa. Therefore, we investigated the association between CVRF and near VI amongCongolese older people.Methods: Participants were Congolese older people aged ≥ 65 years included in EPIDEMCA-FU (Epidemiology ofDementia in Central Africa - Follow-up) population-based cohort carried out from 2011 to 2015. Visual acuity wasassessed at 30 cm using a Parinaud chart. Near VI was defined as having a visual acuity < 20/40. A cohort design wasused taking all CVRF and covariates at baseline and visual acuity at 1st follow-up. Associations were investigated usingmultivariate logistic regression.Results: 549 participants were included in our analyses. Participants median age was 72 years [interquartile range: 68-78 years] and 331 (60.3%) were females. In total, 378 had near VI (328 had mild/moderate near VI, 38 had severe VI and12 were blind). Prevalence of near VI was 68.8% [95% Confidence Interval: 64.9% - 72.7%] and higher in males (71.1%;95%CI: 65.1% - 77.1%) than females (67.4%; 95% CI: 62.3% - 72.4%). Of the CVRF explored, we found that having highBMI ≥ 25 kg/m2 (adjusted Odds Ratio = 2.31 [95%CI: 1.25 – 4.28]), diabetes (aOR=2.14 [95%CI: 1.07 – 4.29]) andhypertension (aOR=1.65 [95%CI: 1.03 – 2.66]) were independently associated with near VI. However, after adjustmenton cognitive status, hypertension (full-adjusted OR=1.58 [95%CI: 0.97 – 2.58]) was no longer significantly associated withVI. There was no interaction between these CVRF. We found no significant association for alcohol consumption(p=0.541), smoking status (p=0.88), and indirect marker of poor diet (0.33) in our sample.Conclusion: Prevalence of near VI was high among Congolese older people. High BMI, diabetes, and hypertension wereindependently associated to near VI in this population. Our study suggests that these CVRF could represent targets for VIprevention. However, further studies are required to clarify underling mechanisms because these associations may bemediated by the presence of ocular conditions/eye diseases.

International audience; Introduction : Le vieillissement correspond à l’ensemble des processus physiologiques modifiant la structure et les fonctions de l’organisme à partir de l’âge mûr. Plus les personnes vieillissent, plus il y a apparition de fragilité physique et de polypathologie, dont les troubles cognitifs. L’objectif de cette étude était d’analyser l’association entre les troubles cognitifs et la fragilité physique.Méthodes : L’étude multicentrique, Epidemiology of dementia in Central Africa (EPIDEMCA) faite d’enquêtes transversales porte-à-porte auprès des personnes âgées de 65 ans et plus, a été menée entre novembre 2011 et décembre 2012 dans deux pays d’Afrique centrale, République du Congo et République centrafricaine. Un questionnaire sur la fragilité, SOF (Study of Osteoporotic Fractures) index, était intégrée lors de ces enquêtes avec des critères suivants : robuste (0 composante), pré fragile (1 composante) et fragile (≥ 2 et 3 composantes).Résultats : 1551 personnes âgées de 65 ans et plus étaient inclues, réparties en 394 à Brazzaville et 376 à Gamboma en République du Congo, puis 407 à Bangui et 374 à Nola en République centrafricaine. En RCA, les personnes âgées de 65 ans et plus présentant des troubles cognitifs de type Mild Cognitive Impairment (MCI) avaient deux fois plus le risque d’être fragiles 2,0 (1,1-3,9) avec p = 0,03, alors que le risque était multiplié par trois en cas de démences 3,0 (1,5-6,0) avec p = 0,002 au cours de l’analyse multivariée. Par contre en République du Congo, au cours de l’analyse uni variée, le risque était presque doublé quand les troubles cognitifs étaient de type MCI 1,9 (1,0-3,4) avec p = 0,05 et en cas de démences 2,2 (1,0-5,2) avec p = 0,06, mais aucune différence n’était statistiquement significative en analyse multivariée.Discussion : Les troubles cognitifs font partie des critères de causalité du syndrome de fragilité, au regard du processus physiopathologique du vieillissement cérébral. Les mécanismes comme les diminutions du poids du cerveau d’environ 2 %, du débit cérébral et des enzymes responsables de la synthèse des neurotransmetteurs sont incriminés.Conclusion : Les personnes âgées atteintes des troubles cognitifs et fragiles doivent être dépistées, traitées et suivies au long terme. L’information, l’éducation et la communication des populations ainsi que l’expertise gériatrique auront un impact sur la prise en charge

International audience; Objectives: The great age was long regarded as synonymous with wisdom in Africa. Nowadays, it is increasingly synonymous to: body that breaks down and senile dementia. Dementia is a public health problem in high-income countries and in low-and middle-income-countries. In the latter, where more than half of people with dementia (PWD) live, the disease is considered as an enigma by informal caregivers (IC) who sometimes abuse them and limit their access to health care because of pejoratives socio-cultural representations. The objective of the study was to analyze these representations could influence their access to health care and quality of life. Methods: An ethnological study was carried out in three months immersion in urban and rural areas to Congo. The non-directive interviews were conducted in local languages, associated by participant observations to analyze behaviors, gestures, discourses, to record phenomena that an interviewee could intentionally or not, fail to say and to control his statements. All data collected were analyzed at the time of their compilations. All non-directive interviews and participant observations were annotated and then transcribed. Before classifying these interviews into themes, a critical reading was made of them. Results: 36 life stories were conducted among IC. In urban area, 68.4% and 26.4% of PWD were treated respectively by syncretic church leaders (SCL) and traditional healers (TH). In rural area, 58.8% and 29.4% of PWD were treated respectively by TH and SCL. Only 5.3% of PWD was treated by general practitioner in urban area versus 11.8% by nurses in rural area. PWD were considered as possessed of evil spirits or demons, justifying the solicitation of to SCL and TH. The disease isn’t medicalized, the hospital isn’t a priority. Conclusion: Chance and ignorance have no place in the emergence of mental illnesses in traditional societies. Metaphysical causes are the most evoked, hence the inaccessibility of PWD to health care.

The aging population is becoming a major political issue. It results in an increase in the prevalence of non-communicable diseases including dementia. Dementia is a major public health problem worldwide and in sub-Saharan Africa. It is assimilated to a real "Alzheimer tsunami", because of the growing number of people with dementia. In 50% of cases, they are taken care of in residential care facilities for dependent elderly people or by ambulatory professionals in high-income countries. By cons, in low and middle incomes countries, their taken care is informal, provided by informal caregivers who are neither trained nor encouraged, having links of kinship as filiation, alliance and germanic with people with dementia. In sub- Saharan Africa, mental illnesses are rich in pejorative and demeaning sociocultural representations. The main objective was to describe socio-economic and family factors in the process of stigmatization of people with dementia in urban and rural areas in the Republic of Congo. An ethnological study was conducted in three months’ immersion on the ground. Non-directive interviews conducted in the local (Lingala, Kituba, Mbochi, Teke and Lari) and official languages (French) and complemented by participant observation were used to collect all the data. They were literally transcribed before their classification into themes. Informal caregivers were mainly daughters-in-law and granddaughters. The latter were considered as purveyors of informal care to an estimated duration between 10 to 15 hours a day, against daughters-in-law considered abusers and available between 2 to 6 hours per day. The irregular income of informal caregivers associated with the socio-cultural representations of people with dementia first led to their care by the syncretic churches, with strict diets for exorcism sessions. The second resort was traditional healers using medicinal plants, prayers-incantations and scarifications. Some people with dementia were accused of witchcraft and condemned in public in front of a customary jurisdiction. This accusation of witchcraft was the major form of stigmatization. In the Republic of Congo as in sub-Saharan Africa, the disease is constructed from nuclei of symbols from culture.; Le vieillissement de la population est en train de devenir un enjeu de politique majeur. Il a pour conséquence une augmentation de la prévalence des maladies non transmissibles, dont les démences. La démence est un problème de santé publique dans le monde et en Afrique subsaharienne. Elle est assimilée à un véritable « tsunami Alzheimer », du fait du nombre croissant de personnes atteintes de démence. Dans 50% des cas, elles peuvent être prises en charge dans les établissements d’hébergement pour personnes âgées dépendantes ou par des soignants professionnels en ambulatoire dans les pays à revenus élevés. Dans les pays à revenus moyens ou faibles, leur prise en charge est dite informelle et est réalisée par les aidants informels ni formés ni encouragés. L’objectif principal était de décrire les facteurs socio-économiques et familiaux du processus de stigmatisation dont sont victimes les personnes atteintes de démence en zones urbaine et rurale en République du Congo. Une étude ethnologique a été menée grâce à une immersion de trois mois sur le terrain. Les entretiens non directifs menés en langues locales (Lingala, Kituba, Mbochi, Téké, Lari) et officielle (Français), complétés par des observations participantes, ont permis de recueillir toutes les données. Elles étaient transcrites littéralement avant leur classement en thèmes. Les aidants informels étaient essentiellement des belles-filles et petites-filles. Ces dernières ont été considérées comme des pourvoyeuses des soins informels d’une durée estimée entre 10 à 15 heures par jour, contre les belles-filles, considérées comme des maltraitantes, pour une disponibilité de 2 à 6 heures par jour. L’irrégularité de revenus des aidants informels associés aux représentations socioculturelles des personnes atteintes de démence entraînaient d’abord leur prise en charge par les églises syncrétiques, comportant des diètes strictes pour les séances d’exorcisme. Le deuxième recours était les guérisseurs traditionnels utilisant les plantes médicinales, les incantations-prières et les scarifications. Quelques personnes atteintes de démence ont été accusées de sorcellerie et condamnées en public devant une juridiction coutumière, cette accusation constituant la forme majeure de stigmatisation. En République du Congo comme en Afrique subsaharienne, la maladie se construit à partir de noyaux de symboles puisés dans la culture.

13. Cardiovascular Health and Near Visual Impairment Among Older Adults in the Republic of Congo: A Population-Based Study.

Background: Visual impairment (VI) and determinants of poor cardiovascular health are very common in Sub-Saharan Africa. However, we do not know whether these determinants are associated with VI among older adults in this region. This study aimed at investigating the association between the determinants of poor cardiovascular health and near VI among older adults living in Congo. Methods: Participants were Congolese adults aged 65 or older included in Epidemiology of Dementia in Central Africa—Follow-up population-based cohort. Near VI was defined as visual acuity less than 20/40 measured at 30 cm. Associations between determinants of poor cardiovascular health collected at baseline and near visual acuity measured at first follow-up were investigated using multivariable logistic regression models. Results: Among the 549 participants included, 378 (68.8%; 95% confidence interval [CI]: 64.9%–72.7%]) had near VI. Of the determinants of poor cardiovascular health explored, we found that having high body mass index of at least 25 kg/m
2 (odds ratio [OR] = 2.15; 95% CI: 1.25–3.68), diabetes (OR = 2.12; 95% CI: 1.06–4.25) and hypertension (OR = 1.65; 95% CI: 1.02–2.64) were independently associated with near VI. Conclusions: Several determinants of poor cardiovascular health were associated with near VI in this population. International audience; A multicenter population-based study was carried out in Central African Republic (CAR) and Republic of Congobetween 2011 and 2012 including both urban and rural sites in each country. Participants aged ≥65 years oldwere interviewed and participants in Congo were followed-up annually between 2012 and 2015 (EPIDEMCA-FUstudy). Qualitative interviews using an ethno-anthropological approach were also performed with peopleidentified with dementia and their relatives in order to investigate social representations, beliefs and stigma.ResultsOverall, 2,002 elderly were interviewed in both countries, including 473 in Nola (rural CAR), 500 in Bangui (urbanCAR), 529 in Gamboma (rural Congo) and 500 in Brazzaville (urban Congo). The crude prevalence of DSM-IVdementia was estimated at 8.5% (CI95%[6.1-11.3] in Nola, at 6.4% (CI95%[4.4-8.9]) in Bangui, 5.7% (CI95%[3.9-8.0] in Gamboma and at 6.6% (CI95%[4.6-9.1]) in Brazzaville. The crude incidence was estimated at 15.8(95% CI 10.2 – 23.3) per 1000 PY. Factors associated with either prevalent or incident dementia in thesestudies, as well as the results of the qualitative interviews, will be detailed during the presentation.ConclusionThe need for targeted social and health policies for older people, as well as strategies for dementia care, isevident and growing in sub-Saharan Africa (SSA).
