Irritable bowel syndrome (IBS) results from disordered brain-gut interactions. Identifying susceptibility genes could highlight the underlying pathophysiological mechanisms. We designed a digestive health questionnaire for UK Biobank and combined identified cases with IBS with independent cohorts. We conducted a genome-wide association study with 53,400 cases and 433,201 controls and replicated significant associations in a 23andMe panel (205,252 cases and 1,384,055 controls). Our study identified and confirmed six genetic susceptibility loci for IBS. Implicated genes included NCAM1, CADM2, PHF2/FAM120A, DOCK9, CKAP2/TPTE2P3 and BAG6. The first four are associated with mood and anxiety disorders, expressed in the nervous system, or both. Mirroring this, we also found strong genome-wide correlation between the risk of IBS and anxiety, neuroticism and depression (rg > 0.5). Additional analyses suggested this arises due to shared pathogenic pathways rather than, for example, anxiety causing abdominal symptoms. Implicated mechanisms require further exploration to help understand the altered brain-gut interactions underlying IBS.

Crohn's disease is a chronic immune-mediated disease which can cause inflammation in the gastrointestinal tract anywhere between mouth and anus. It is now understood to be the result of a complex interplay between genetic, microbial and environmental factors. The detail of these interactions remains an important focus of research. Clinicians and patients are faced with an increasing range of therapeutic options for Crohn's. Key decision points include first presentation to gastrointestinal services, assessment of risk of disease progression, escalation to biologic therapy, and withdrawal of immunomodulator or biologic therapies. Biomarkers are needed that help stratify patients at these times to facilitate shared decision-making. In this thesis, I have brought together seven of the first-author papers published from the work conducted during my period of study. I first investigated the associations between NOD2, the strongest genetic association for Crohn's disease, and the faecal microbiome. NOD2 encodes a bacterial pattern recognition receptor, and so disease-associated genetic variants might be expected to lead to an alteration in the microbiota. Although I demonstrated clear associations between the presence of inactive Crohn's and changes in the faecal microbial composition, I was unable to show an impact of NOD2 genotype in either patients with Crohn's or in volunteers without gastrointestinal disease. Calprotectin is the dominant protein in the cytosol of neutrophils. Its measurement in faecal samples has become well-established as a tool for non-invasive assessment of intestinal inflammation. I have investigated its use here for both diagnosis and prognosis in Crohn's disease. In the diagnosis paper, I have assessed the diagnostic accuracy in of faecal calprotectin in a large cohort of patients referred to secondary care with lower gastrointestinal symptoms. I demonstrated a high negative predictive value, with very few false negatives even after three years of follow-up. In patients with established Crohn's disease, I have then gone onto show that higher concentrations of faecal calprotectin, independent of symptoms, are associated with a greater risk of progressing to stricturing or penetrating disease, hospitalisation or resectional surgery. All of the therapies used in Crohn's disease carry long-term risks including an elevated risk of infection and certain cancers. For this reason, among others, clinicians and patients must regularly assess the risk-benefit of continued therapy versus withdrawal of medication. I conducted two national, multicentre studies to better understand the outcomes of treatment withdrawal; one for thiopurines, and the other for anti-tumour necrosis factor alpha (anti-TNF), two of the most widely used classes of maintenance medication for Crohn's disease. I demonstrated a relapse rate of 39% at two years following withdrawal of thiopurines for Crohn's disease, and for 56% two years following anti-TNF withdrawal. I identified factors predictive of an increased risk of relapse, and for anti-TNF performed a systematic review and meta-analysis of all of the available published data. Anti-TNF therapies were the first targeted biologic therapy licensed for Crohn's disease and they remain an important tool for the induction and maintenance of remission. Their introduction has been transformative for many patients, but secondary loss of response is common. In the final paper included in this thesis, I present the results from PANTS, a 120- site study of infliximab and adalimumab in 1,610 patients with Crohn's disease. I have demonstrated the importance of good early drug concentrations for outcome, and explored the relationship between patient factors, drug and anti-drug antibody concentrations and longer term outcomes. As part of the PANTS study, we have collected longitudinal biological samples, and building on the clinical data presented in this thesis, I have described some of the early downstream analyses of genetic associations with anti-drug antibody formation, as well as plans for analyses of DNA methylation, transcription, serum protein concentrations, immunoglobulin G glycosylation and variation in the microbiome. This thesis brings together work spanning a breadth of translational and clinical research into Crohn's disease. I have contributed to better understanding of pathogenesis, and to prediction of outcome at key decision points within the clinical disease course. These projects, particularly the PANTS study, provide a strong foundation for further research to better understand this important disease.

47. Whole Blood DNA Methylation Changes Are Associated with Anti-TNF Drug Concentration in Patients with Crohn's Disease.

- Abstract
Background and Aims Anti-tumour necrosis factor [TNF] treatment failure in patients with inflammatory bowel disease [IBD] is common and frequently related to low drug concentrations. In order to identify patients who may benefit from dose optimisation at the outset of anti-TNF therapy, we sought to define epigenetic biomarkers in whole blood at baseline associated with anti-TNF drug concentrations at week 14. Methods DNA methylation from 1104 whole blood samples from 385 patients in the Personalised Anti-TNF Therapy in Crohn's disease [PANTS] study were assessed using the Illumina EPIC Beadchip [v1.0] at baseline and weeks 14, 30, and 54. We compared DNA methylation profiles in anti-TNF-treated patients who experienced primary non-response at week 14 if they were assessed at subsequent time points and were not in remission at week 30 or 54 [infliximab n = 99, adalimumab n = 94], with patients who responded at week 14 and when assessed at subsequent time points were in remission at week 30 or 54 [infliximab n = 99, adalimumab n = 93]. Results Overall, between baseline and week 14, we observed 4999 differentially methylated positions [DMPs] annotated to 2376 genes following anti-TNF treatment. Pathway analysis identified 108 significant gene ontology terms enriched in biological processes related to immune system processes and responses. Epigenome-wide association [EWAS] analysis identified 323 DMPs annotated to 210 genes at baseline associated with higher anti-TNF drug concentrations at Week 14. Of these, 125 DMPs demonstrated shared associations with other common traits [proportion of shared CpGs compared with DMPs] including body mass index [23.2%], followed by C-reactive protein [CRP] [11.5%], smoking [7.4%], alcohol consumption per day [7.1%], and IBD type [6.8%]. EWAS of primary non-response to anti-TNF identified 20 DMPs that were associated with both anti-TNF drug concentration and primary non-response to anti-TNF with a strong correlation of the coefficients [Spearman's rho = -0.94, p <0.001]. Conclusion Baseline DNA methylation profiles may be used as a predictor for anti-TNF drug concentration at week 14 to identify patients who may benefit from dose optimisation at the outset of anti-TNF therapy. How is new information organized in memory? According to latent state theories, this is determined by the level of surprise, or prediction error, generated by the new information: a small prediction error leads to the updating of existing memory, large prediction error leads to encoding of a new memory. We tested this idea using a protocol in which rats were first conditioned to fear a stimulus paired with shock. The stimulus was then gradually extinguished by progressively reducing the shock intensity until the stimulus was presented alone. Consistent with latent state theories, this gradual extinction protocol (small prediction errors) was better than standard extinction (large prediction errors) in producing long-term suppression of fear responses, and the benefit of gradual extinction was due to updating of the conditioning memory with information about extinction. Thus, prediction error determines how new information is organized in memory, and latent state theories adequately describe the ways in which this occurs. [ABSTRACT FROM AUTHOR]

Background: Because of the increased prevalence of revision anterior cruciate ligament (ACL) reconstruction, there has been a desire to understand the role of posterior tibial slope on increased anterior tibial translation and increased ACL graft forces. One potential concern in supratubercle anterior closing wedge proximal tibial osteotomy (ACW-PTO) for decreasing the posterior tibial slope is the risk of altering the patellar height. Purpose: To radiographically assess changes in (1) patellar height, (2) anterior tibial translation, and (3) posterior tibial slope after supratubercle ACW-PTO. Study Design: Case series; Level of evidence, 4. Methods: Patients who underwent supratubercle ACW-PTO by a single surgeon between July 2019 and June 2023 were included. Standardized lateral knee weightbearing radiographs to assess patellar height (via the Caton-Deschamps index), anterior tibial translation of the lateral tibial plateau relative to the lateral femoral condyle, and posterior tibial slope were obtained at 4 time points (preoperatively and 1 day, 3 months, and 6 months postoperatively). Paired t test was used to compare differences between preoperative, 1-day, and 3- and 6-month values for patellar height as measured using the Caton-Deschamps index and for posterior tibial slope. Paired t test was also used to compare differences in the preoperative and 6-month postoperative values for anterior tibial translation. Results: In 20 patients after ACW-PTO, the Caton-Deschamps index demonstrated a significant increase in patellar height on postoperative day 1 (P <.001) but no significant differences at 3 (P =.057) and 6 (P =.176) months postoperatively. Anterior tibial translation on standing lateral knee radiographs was significantly decreased by a mean of 8.9 mm from preoperatively to 6 months postoperatively (P <.001). Posterior tibial slope was significantly decreased by a mean of 11.2° from preoperatively to 6 months postoperatively (P <.001). Conclusion: Supratubercle ACW-PTO performed for ACL reconstruction failure in the setting of an increased posterior tibial slope did not induce significant changes in patellar height postoperatively.
