18. Centering Health Equity in the Implementation of the Hospital Incident Command System: A Qualitative Case Comparison Study

Abstract

AbstractObjective:Disasters exacerbate inequities in health care. Health systems use the Hospital Incident Command System (HICS) to plan and coordinate their disaster response. This study examines how 2 health systems prioritized equity in implementing the Hospital Incident Command System (HICS) during the coronavirus disease 2019 (COVID-19) pandemic and identifies factors that influenced implementation.Methods:This is a qualitative case comparison study, involving semi-structured interviews with 29 individuals from 2 US academic health systems. Strategies for promoting health equity were categorized by social determinants of health. The Consolidated Framework for Implementation Research (CFIR) guided analysis using a hybrid inductive-deductive approach.Results:The health systems used various strategies to incorporate health equity throughout implementation, addressing all 5 social determinants of health domains. Facilitators included HICS principles, external partnerships, community relationships, senior leadership, health equity experts and networks, champions, equity-stratified data, teaming, and a culture of health equity. Barriers encompassed clarity of the equity representative role, role ambiguity for equity representatives, tokenism, competing priorities, insufficient resource allocation, and lack of preparedness.Conclusions:These findings elucidate how health systems centered equity during HICS implementation. Health systems and regulatory bodies can use these findings as a foundation to revise the HICS and move toward a more equitable disaster response.

Abstract

The article presents literary criticism of the books "The House of Mirth" by Edith Wharton and "A Lost Lady" by Willa Cather. Particular focus is given to the role of gossip in the novels and on the ways in which their female characters use language against one another. According to the author, single women are more vulnerable to the negative effects of gossip than married women. Details on rumors surrounding female characters' romantic affairs are presented.

23. Teacher Perceptions of Supports that Promote Computer Self-Efficacy and Transformational Digital Pedagogy in One-to-One Learning Environments

Abstract

Although one-to-one technology programs are rapidly expanding in secondary schools, the literature about how to effectively leverage these programs to improve teaching and learning is relatively small. Little guidance is available for school leaders attempting to improve teachers' willingness and ability to incorporate technology effectively into their instruction. The purpose of this mixed-methods case study was to discovery what supports school leaders provide to promote high levels of computer self-efficacy and transformational digital pedagogies in one-to-one learning environments. Data were collected from English language arts, Social Studies, and science teachers in three high schools in a suburban school district in northeast Texas using an online survey, eight virtual semi-structured interviews, and two virtual focus group interviews. Data were analyzed using descriptive statistics and deductive and inductive analysis. The findings of the study reveal most teachers perceived their ability to effectively deliver digital instruction as strong, and most were able to incorporate technology into their lessons at transformational levels. The following themes emerged from data regarding teachers' perceptions of support: shared vision, realistic and supportive climate, collaboration, encouragement, job-embedded professional learning, continuous improvement, equity, and safe, legal, and ethical use. The findings of this study serve as a foundation for understanding how school leaders can best support teachers as they attempt to integrate one-to-one technology into their lessons.

26. Development and evaluation of a 3-day patient safety curriculum to advance knowledge, self-efficacy and system thinking among medical students.

Abstract

Purpose: To develop a patient safety curriculum and evaluate its impact on medical students' safety knowledge, self-efficacy and system thinking., Methods: This study reports on curriculum development and evaluation of a 3-day, clinically oriented patient safety intersession that was implemented at the Johns Hopkins School of Medicine in January 2011. Using simulation, skills demonstrations, small group exercises and case studies, this intersession focuses on improving students' teamwork and communication skills and system-based thinking while teaching on the causes of preventable harm and evidence-based strategies for harm prevention. One hundred and twenty students participated in this intersession as part of their required second year curriculum. A pre-post assessment of students' safety knowledge, self-efficacy in safety skills and system-based thinking was conducted. Student satisfaction data were also collected., Results: Students' safety knowledge scores significantly improved (mean +19% points; 95% CI 17.0 to 21.6; p<0.01). Composite system thinking scores increased from a mean pre-intersession score of 60.1 to a post-intersession score of 67.6 (p<0.01). Students had statistically significant increases in self-efficacy for all taught communication and safety skills. Participant satisfaction with the intersession was high., Conclusions: The patient safety intersession resulted in increased knowledge, system-based thinking, and self-efficacy scores among students. Similar intersessions can be implemented at medical, nursing, pharmacy and other allied health schools separately or jointly as part of required school curricula. Further study of the long-term impact of such education on knowledge, skills, attitudes and behaviours of students is warranted.
