© 2022 by the American College of Prosthodontists.Purpose: The main objective of this study was to investigate the influence of implant design, bone type, and abutment angulation on stress distribution around dental implants. Materials and methods: Two implant designs with different thread designs, but with the same length and brand were used. The three-dimensional geometry of the bone was simulated with four different bone types, for two different abutment angulations. A 30° oblique load of 200 N was applied to the implant abutments. Maximum principal stress and minimum principal stresses were obtained for bone and Von misses stresses were obtained for dental implants. Results: The distribution of the load was concentrated at the coronal portion of the bone and implants. The stress distributions to the D4 type bone were higher for implant models. Increased bone density and increased cortical bone thickness cause less stress on bone and implants. All implants showed a good distribution of forces for non-axial loads, with higher stresses concentrated at the crestal region of the bone-implant interface. In implant types using straight abutments there was a decrease in stress as the bone density decreased. The change in the abutment angle also caused an increase in stress. Conclusions: The use of different implant threads and angled abutments affects the stress on the surrounding bone and implant. In addition, it was observed that a decrease in density in trabecular bone and a decrease in cortical bone thickness increased stress.

Purpose: To compare the efficacy of low-dose laser therapy to that of conservative treatment usingtwo different occlusal splints (stabilization and anterior repositioning splints) in patients with internalderangements of the temporomandibular joint (TMJ). Materials and Methods: The study populationconsisted of patients with disc displacement with reduction diagnosed according to the Research DiagnosticCriteria for Temporomandibular Disorders (RDC/TMD) Axis I and Axis II. In addition, disc displacement withreduction was confirmed in all patients using TMJ ultrasonography. These tests were conducted for 6 monthswith an interim control in the third month. The “clicking” sound from the joint on opening and closing themouth and the extent to which the mouth opened vertically and laterally were assessed. In all, 20 patientsreceived low-dose laser therapy, 20 were treated with a stabilization splint, and 20 were treated with ananterior repositioning splint. In addition, 10 untreated patients were included as a patient control group,and a further 10 healthy subjects were included as a healthy control group. Changes in the condition wereassessed based on the results of the RDC/TMD Axis II and with the use of an algometer. Results: Theanterior repositioning splint group showed improvement in the “clicking” sound during mouth opening.Lateral movement improved significantly in patients who received laser therapy. In the patient control group,the click disappeared during mouth opening, the algometrically determined pain improved, and the lateralmovement increased. There were no statistically significant differences between groups in the improvementof vertical mouth movement or in the clicking sound during mouth closing. Conclusion: Each treatmentmodality applied in this study separately produced positive results for the clicking sound, restrictions in verticaland lateral movements, and reduction of the pressure pain threshold observed in cases of TMJ irregularity.In conclusion, the decision regarding which treatment modality should be employed can be made based onthe patient’s symptoms. However, this study also indicates that TMJ derangements can resolve spontaneouslywhen left untreated.

Purpose: To determine whether the use of implant-supported overdentures (IODs) with different attachments influences the Mini Nutritional Assessment (MNA) and Geriatric Oral Health Assessment Index (GOHAI) scores in edentulous patients > 65 years of age. Materials and Methods: The MNA and GOHAI were administered to 54 edentulous patients > 65 years of age (mean age = 68.35 +/- 4.1 years) before treatment (A) and 6 months after treatment (B): 10 with maxillary + mandibular conventional complete dentures (CDs); 10 with a maxillary conventional CD + mandibular magnetic-retained IOD; 12 with a maxillary conventional CD + mandibular ball-retained IOD; 12 with a maxillary conventional CD + mandibular Locator-retained IOD; and 10 with a maxillary conventional CD + mandibular bar-retained IOD. Statistical differences between treatment types were evaluated using one-way analysis of variance and paired-sample t tests. The correlation between MNA and GOHAI scores was determined using Pearson correlation analysis (alpha = .05). Results: The difference between mean GOHAI-A and GOHAI-B scores was statistically significant for each type of denture (P < .005). The difference between the mean MNA-A and MNA-B scores was statistically significant for all types of denture except for conventional CDs (P < .05). There was a statistically significant positive correlation between MNA and GOHAI scores (P< .01). Conclusion: Regardless of the type of denture used, treatment of edentulous geriatric patients is important for improving nutritional status and self-rated oral health.

Prosthetic rehabilitation in maxillectomy patients aims to separate the oral and nasal cavities to provide adequate articulation, restore the contour of the midface, and provide acceptable aesthetic results. This clinical case report describes prosthetic rehabilitation via placement of dental implants in the zygomatic bone in a patient who underwent partial maxillectomy because of osteosarcoma. The success of obturator placement after maxillectomy requires retention by the surrounding dental tissue or dental implants. When zygomatic implants cannot be used, dental implants placed in the zygomatic bone and attachments fastened to these implants can easily provide the retention and stability required by the obturator.

Aim: The aim of this study is to compare the diagnosis of patients with temporomandibular joint (TMJ) internal derangements which had been diagnosed using Research Diagnostic Criteria/Temporomandibular Disorders (RDC/TMD) with the dynamic high resolution sonography findings. Settings and Design: Axis I section of RDC/TMD form had been applied to participants. Participants were divided into three groups as healthy TMJ, disc displacement with reduction, and disc displacement without reduction. The diagnoses had been compared with the dynamic high-resolution sonography findings. Materials and Methods: Twelve of the patients had been treated with laser therapy, whereas 13 patients were treated with stabilization splint. Seventeen patients were treated with anterior repositioning splint (n = 42). After the application of different treatment modalities, the position of the articular disc had been determined with Axis I of RDC/ TMD form and dynamic high-resolution sonography. The findings were compared and statistically analyzed. Statistical Analysis Used: Statistical analyses of data were analyzed with Turcosa Cloud (Turcosa Ltd Co, Turkey). Results: For the right TMJ, pretreatment and posttreatment ultrasonography (USG) diagnoses and RDC/ TMD clinical diagnoses were found similar (κ = 0.125–0.008). No statistically significant relationship was found (P > 0.05). For the left TMJ, pretreatment USG diagnosis and RDC/TMD clinical diagnose were found similar (κ = 0.070). No statistically significant relationship was found (P > 0.05). For the left TMJ, posttreatment USG diagnosis and RDC/TMD clinical diagnose were compared. A statistically significant difference was found (κ = 0.256). A statistically significant relationship was found (P < 0.05). Conclusions: Axis 1 of RDC/TMD form which is used for the diagnosis of internal derangements and dynamic high resolution sonography was not found in the agreement.

The aim of this study was to examine the effect of the position of anterior implants on stress distribution in the implant, the bone around the implant, and prosthetic components in the resorbed maxilla using the all-on-four concept. All-on-four designs were prepared with 3 different anterior implant positions in a fully edentulous maxilla. Anterior implants were placed axially in the central incisor area in model 1, in the lateral incisors area in model 2, and in the canine area in model 3, forming 3 groups. The von Mises and principal stresses in the bone tissue and the von Mises stresses in the implant and prosthetic components were evaluated by 3-dimensional finite element analysis. There were more stresses on the cortical bone than trabecular bone. The stresses on the bone tissue and implant components were generally concentrated around the posterior implant, whereas the stresses on the prosthetic components were generally concentrated in the anterior region. Changing the anterior implant positions from the central tooth to the canine tooth reduced the stress on the bone around the implant. The highest von Mises stresses occurred in the prosthetic superstructure in all models, whereas the lowest stresses occurred in the trabecular bone. Changing the position of the anterior implants from the central tooth to the canine area in the maxillary all-on-four concept created a favorable stress distribution.

Aim: The aim of this study was to evaluate the effects of two geometrically different fiber post systems and two different placing lengths on the stress distribution of endodontically treated teeth. Methodology: Four different mandibular premolar tooth models were created. These models were restored using oval or circular fiber posts with two different placing lengths. An oblique force of 300 N was applied to the top of the tooth, and von Mises stress evaluations were carried out on the dentin tissue, luting cement, and fiber posts. Results: The maximum von Mises stresses were observed in the 10-mm long circular fiber post model, while the minimum stresses were seen in the 5-mm long oval fiber post model. In general, the oval fiber post models presented more homogeneous stresses than the circular fiber post models. Moreover, the 10-mm long fiber post models generated greater von Mises stresses than the 5-mm long fiber post models in the dentin tissue and luting cement. Conclusion: According to the study findings, the use of oval fiber posts at both placing lengths is suggested for oval shaped root canals due to the lower stress concentrations. Amaç: Temporomandibular düzensizlik (TMD), çiğneme kasları ve temporomandibular eklemin ağrı ve fonksiyon bozukluğu ile karakterize, birçok etyolojik koşulu içeren bir rahatsızlıktır. Çiğneme kaslarında ağrı, ağız açmada kısıtlılık, mandibular harekette deviasyon ve eklem sesi gibi semptomlar ile ortaya çıkan rahatsızlığın etyolojisinde bruksizm, maloklüzyonlar, dejeneratif eklem hastalıkları gibi iç etkenlerin yanı sıra emosyonel durumların etkili olduğu çevresel dış etkenler de rol oynamaktadır. Bu çalışmanın amacı; temporomandibular eklem iç düzensizliği olan hastaların yaş, cinsiyet, medeni durum, iş durumu ve eğitim durumu gibi demografik bilgilerinin incelenmesidir. Gereç ve Yöntemler: Araştırmaya dahil edilen hastalar Erciyes Üniversitesi Diş Hekimliği Fakültesi’ne temporomandibular eklemindeki problemden dolayı başvuran kişiler arasından randomize olarak seçilmiştir. Hastalardan bilgilendirilmiş onam formu alınmıştır. Çalışma toplamda 80 kişiyi kapsamaktadır. Bulgular: Araştırmadan elde edilen veriler incelendiğinde temporomandibular eklem iç düzensizliğinin kadınlarda erkeklere oranla (3.7/1) daha çok görüldüğü tespit edilmiştir. Yaş ortalamasının 26.90±10.33 olması, temporomandibular eklem iç düzensizliğinin gençlerde daha sık görüldüğünü ortaya koymuştur. Çalışmamızda medeni durum, eğitim durumu ve iş durumu ile hastalık arasında bir ilişki olmadığı saptanmıştır. Sonuç: Temporomandibular eklem iç düzensizliği kadınlarda erkeklere oranla daha çok görülmektedir. Yaş dağılımı hastalığa genç-orta yaş hastalığı kimliğini vermektedir. Çalışmamızda medeni durum, eğitim durumu ve iş durumu ile hastalık arasında bir ilişki olmadığı bildirilmiştir., Background: Temporomandibular disorder (TMD) is a disorder that includes many etiological conditions, characterized by chewing muscles and pain and dysfunction of the temporomandibular joint. In addition to internal factors such as bruxism, malocclusions, degenerative joint diseases, environmental external factors play an important role in the etiology of the discomfort that occurs with symptoms such as pain in the chewing muscles, restricted mouth opening, deviation in the mandibular movement and joint sound. The purpose of this study; It is the examination of demographic information of patients with temporomandibular joint internal derangement such as age, gender, marital status, job status and educational status. Methods: The patients included in the study were randomly selected among those who applied to Erciyes University Faculty of Dentistry due to the problem in the temporomandibular joint. Informed consent form was taken from the patients. The study covers 80 people in total. Results: When the data obtained from the study were examined, it was found that the temporomandibular joint internal derangement was more common in women than in men (3.7 / 1). The average age of 26.90 ± 10.33 revealed that temporomandibular joint internal derangement was more common in young people. In our study, it was found that there was no relationship between marital status, education status and job status and disease. Conclusion: Temporomandibular joint internal derangement is more common in women than in men. Age distribution gives the disease the identity of young-middle age disease. In our study, it was reported that there was no relationship between marital status, education status and job status and disease.

Aim: The effect of oral rehabilitation on the oral perception of implant-supported overdenture patients is a concern. This study evaluated the effects of the attachment type and palatal coverage on oral perception and patient satisfaction in maxillary implant-supported complete denture patients. Subjects and Methods: The correlation between oral perceptual ability (OPA) and patient satisfaction in three groups was investigated. Group I consisted of dentate individuals (n = 40), Group II consisted of maxillary implant-supported complete denture patients with bar attachments and palatal coverage (n = 12), and Group III of maxillary implant-supported complete denture patients with magnetic attachments and palatal coverage (n = 14). In addition, implant-supported maxillary overdentures with bar attachments, with (Group II; n = 12) and without (Group IV; n = 18) palatal coverage, were examined in terms of patient satisfaction and OPA. The relationship between OPA and patient satisfaction was assessed with the Turkish version of the Oral Health Impact Profile-14 (OHIP-TR-14) satisfaction survey. To compare oral sensory function among the groups, tactile awareness and pressure awareness were assessed. Results: There was no significant difference in OHIP-TR-14 scores between Groups II and III. In addition, there was no correlation between oral tactile function and patient satisfaction in Groups II and III. For patients with maxillary bar-retained implant-supported overdentures, palatal coverage did not affect the correlation between OPA and patient satisfaction, lateral pressure threshold, or tactile thickness threshold. Conclusion: According to the results of the study, whether maxillary implant-supported overdentures were made with a bar- or magnetic-type retainer, and whether bar-retained implant-supported overdentures had an open or closed palate did not affect the correlation between patient satisfaction and oral perception.

Objectives: One of the most common treatment modalities for patients with a diagnosis of reduced disc displacement is occlusal splinting. Stabilization splints (SS) and anterior repositioning splints (ARS) are frequently used in such cases.Materials and Methods: The effects of these two splint types on patient satisfaction were evaluated in patients with temporomandibular disorder (TMD). 25 healthy people with no pathology of the temporomandibular joint were included as a control group. ARS’s were used in 18 patients; all patients used the splints for 2 months. The splint was then converted into a stabilization splint for use for 1 further month. In the other 32 patients, a SS was used for 1 month. All the patients filled out the same temporomandibular questionnaire (TQ) again after the ARS was converted into a SS and had been used for 1 month.Results: The use of both types of splint significantly affected scores on the TQ (p < 0.05). There was a significant difference in questionnaire results among the control group, SS group, and ARS group before treatment (p < 0.05). There was no statistically significant difference in questionnaire results among the three groups after treatment (p > 0.05).Conclusion: These results indicate that the use of both ARS and SS significantly increased satisfaction among patients with TMD.

Introduction: Root canal perforation during endodontic therapy is one of the most commonly encountered clinical situations that necessitates interdisciplinary therapy.Case Presentation: This case report presents the use of periodontal and prosthodontic teamwork to treat severely deteriorated anterior esthetics caused by an iatrogenically induced marginal tissue recession defect.Conclusion: In the case of deterioration of anterior dental esthetics, an interdisciplinary approach can successfully restore both esthetics and function, as observed in this case report.

Purpose This study evaluated the effect of delayed bonding and antioxidant application (AA, 10% sodium ascorbate) after internal bleaching (35% carbamide peroxide) on the shear bond strength of an adhesive cement to enamel. Materials and Methods Eighty-four human maxillary central incisors were endodontically treated. The control group remained unbleached with no AA. Experimental groups were all internally bleached. The buccal enamel was finished and polished with metallographic paper to a refinement of #600, in order to obtain a 5-mm2 flat bonding area. An adhesive cement (Clearfil Esthetic) was placed into a plastic tube with internal diameter of 3 mm and a 3-mm height and cured on the enamel. Bonding occurred either immediately after bleaching (group Im), a 7-day delay (group 7), or a 14-day delay (group 14), and half the specimens were treated with antioxidant application (groups Im-AA, 7-AA, and 14-AA). Shear bond strength testing was performed on a universal testing machine, and data were analyzed with ANOVA and Fisher test (5%). Results Delaying of bonding is a useful factor for enhancing shear bond strength (p < 0.05), whereas AA only enhanced shear bond strength after 7 days delayed bleaching (p < 0.05). The highest bond strength was noted in groups 7-AA (20.51 ± 4.5 MPa), 14 (19.82 ± 4.6), 14-AA (20.27 ± 4.4), and control (20.51 ± 5.1), which were not significantly different from each other. Conclusions After internal bleaching, adhesive cementation to enamel is recommended only when delayed 14 days, or delayed 7 days with sodium ascorbate application.

PURPOSE. The aim of this study was to evaluate whether the push-out bond strength varies between oval and circular fiber posts, and to examine the effect on the resin cement thicknesses around the posts. MATERIALS AND METHODS. Eighteen mandibular premolar roots were separated into two groups for oval and circular fiber posts systems. Post spaces were prepared and fiber posts were luted to the post spaces. Roots were cut horizontally to produce 1-mm-thick specimens. Resin cement thicknesses were determined with a metallographic optical microscope and push-out tests were done. RESULTS. The study aimed to evaluate the effects of different post morphologies and placement lengths on the fracture resistance of teeth with oval canal morphology that had been restored with crowns. Extracted mandibular premolars with similar dimensions were decoronated. After the root canal treatment, the teeth were mounted on acrylic blocks. Samples were randomly divided into four groups (n = 10 each). In groups C-10 and C-5, 10-mm- and 5-mm-long circular post spaces were created. In groups O-10 and O-5, 10-mm- and 5-mm-long oval post spaces were ultrasonically created. After post cementation, all specimens were restored with composite cores and prepared at height of 6 mm. Thereafter, all teeth were restored with crowns. After thermocycling, all specimens underwent fracture resistance testing. Oval posts and placement at 10-mm depth showed higher fracture resistance than circular posts and placement at 5-mm depth (P < 0.001). Increased post length and use of oval posts enhanced the fracture strength of teeth with oval canal morphology. Based on the results of this study, although the fracture resistance of teeth restored with crowns was enhanced by deep fiber post placement, the use of oval fiber post is recommended in cases where deep placement is impossible. Purpose: The purpose was to evaluate temperature increases during dowel space preparations with oval and circular fiber dowel systems. Materials and Methods: This study included 42 single-rooted human mandibular premolars. Roots were scanned with cone-beam computerized tomography (CBCT) to determine the ovoid root canal morphology. Root canals were treated with Ni-Ti rotary instruments and obturated. A second CBCT was taken to determine the thinnest dentin thickness of each root. Roots were randomly divided into two groups (n = 21) according to the fiber dowel system used: group 1, circular fiber dowel system (D.T. Light-Post); group 2, oval fiber dowel system (Ellipson Post). Dowel spaces were prepared using a circular fiber dowel drill and a diamond-coated ultrasonic tip with an oval section under water cooling until 9 mm dowel spaces were obtained. Temperature changes were recorded from the thinnest root surfaces using a FLIR E60 thermal imaging camera. Results: Temperature increases were significantly greater with the circular fiber dowel system than with the oval fiber dowel system (p < 0.05). Conclusions: Although both dowel systems generated high temperature increases on root surfaces, the relatively lower temperature increase associated with the use of oval fiber dowels in ovoid canals makes it preferable to the use of circular fiber dowels.

Purpose: The aim of this study was to measure the in vitro retention force of double conical crowns fabricated using primary galvanoforming and secondary casting techniques and those fabricated using primary casting and secondary galvanoforming techniques under simulated clinical conditions before and after a wear test. Materials and Methods: Primary galvanoformed crowns (n = 10) with non-noble secondary crowns (n = 10; group A) and primary non-noble crowns (n = 10) with secondary galvanoformed crowns (n = 10; group B) were fabricated. Each primary and secondary crown was embedded in acrylic resin and weighed with a digital balance. Retention forces were then measured using a universal testing device. To simulate wear, specimens were inserted and separated horizonatally 3285 times in wear equipment with artificial saliva. Retention forces and weights of the double crowns were then remeasured. Data were analyzed using paired t-tests and Wilcoxon tests, and the groups were compared using Mann–Whitney U-tests. Results: In group A, the wear test had a significant influence on the retentive force (p 0.05). In group B, the wear test had a significant influence on the retentive force (p < 0.05), and wear produced a significant difference in weight (p < 0.05). Conclusions: The results of this study indicated that the use of different combinations of galvanoforming and casting techniques in the fabrication of conical crowns significantly affected retention force.

Farklı post boylarının ve farklı siman kullanımının kron ile restore edilmiş dişlerin kırılma dayanımına etkisi Amaç: Bu çalışmanın amacı post-kor ve kronla restore edilmiş dişlerde farklı simantasyon materyallerinin ve post uzunluklarının kırılma direncine etkisini değerlendirmektir. Gereç ve Yöntemler: Benzer boylara sahip 40 mandibular premolar dişin kronları uzaklaştırıldı. Kanal tedavileri yapıldıktan sonra dişler akrilik bloklara gömüldü. Örnekler ilk olarak ikiye ayrıldı. 10 mm ve 5 mm derinliklerinde sirküler post boşlukları, sırasıyla #0.5 ve #2 kodlu üretici frezleri ile hazırlandı. Post (D.T. Light-Post) simantasyonu aşamasında, her post grubundaki 10 örnek kendinden adezivli rezin siman (Rely X) ile simante edildi. Diğer 10 örnek kendinden asitli kompozit siman (Clearfill Esthetic Cement) ile simante edildi (n=10). Tüm örnekler kompozit korlar ile restore edildi ve toplamda yaklaşık 5°’lik konverjans açısıyla 6 mm boyunda (1 mm ferrule dahil) prepare edildi. Sonra, tüm dişler tam metal kronlarla, cam iyonomer simanla simante edilerek restore edildi. Termal döngü ile yorma sonrası, tüm örneklere kırılma dayanımı testi (0,5 mm/min) uygulandı. Methods: Forty mandibular premolars with similar dimensions were decoronated. After the root canal treatment, the teeth were embedded in acrylic blocks. Specimens were fistly divided into 2 groups. 10-mm- and 5-mm-long circular post spaces were prepared with using #0.5 and #2 coded manufacturer’s drills, respectively. At the post (D.T. Light-Post) cementation stage, 10 specimens in each post group were cemented with a selfadhesive resin cement (Rely X). The other 10 specimens were cemented with self-etch cement (Clearfill Esthetic Cement) (n=10). All specimens were restored with composite cores and prepared at a height of 6 mm (including 1 mm ferrule) and a convergence angle of approximately 5° in total. Then, all teeth were restored with full metal crowns and cemented with a glass ionomer cement. After thermocycling, all specimens were subjected to fracture resistance testing (0,5 mm/min). The aim of this study was to assess the prevalence of denture-related stomatitis (DRS) in different attachment-retained overdenture wearers and its association with particular colonizing Candida species. Thirty-seven edentulous patients with implant-supported maxillary or mandibular overdentures were enrolled. A full clinical history was obtained, including details of patients' oral hygiene practices and the levels of erythema based on Newton's classification scale. Swabs were taken from the palate and investigated mycologically to identify the yeast colonies. Quantitative and qualitative microbiological assessments were performed, which included recording the total numbers of colonies (cfu), their color, and their morphological characteristics. Significant differences were found in cfu values between the attachment and inner surfaces of locator- and bar-retained overdentures (P < .05). Candida albicans was the most common species in both evaluations, being isolated from 81.3% of bar-retained overdentures and 38.1% of locator-retained overdentures. DRS developed in all patients using bar-retained overdentures but in only 71.4% of those using locator-retained overdentures. No statistically significant relationship was found between bar and locator attachments according to smoking habit, overnight removal, or plaque and gingival indices (P > .05).

ÖZET Amaç: Bu çalışmanın amacı, farklı yüzey işlemlerinin alumina içerikli Turkom-Cera tam seramik materyalinin Panavia F 2.0 rezin simanla makaslama bağlantı direnci üzerine etkisinin araştırılmasıdır. Gereç ve Yöntem: Boyutları 10x10x2 mm olan toplam 40 adet Turkom-Cera tam seramik örnekler hazırlandı. Farklı yüzey hazırlıklarına tabi tutulmak üzere örnekler rastgele 8’erli 5 gruba ayrıldı; hiçbir yüzey işleminin uygulanmadığı kontrol grubu (grup C), kumlama grubu (grup SB), Rocatec uygulanan grup (grup R), hidroflorik asitle pürüzlendirilen grup (grup HF) ve Z Primer uygulanan grup (grup Z).Yüksekliği 2 mm ve çapı 8 mm olan rezin siman bloklar Panavia F 2.0 rezin simandan teflon kalıp ile üretildi ve Turkom-Cera tam seramik örneklere simante edildi. Hazırlanan örnekler 1 hafta süreyle oda sıcaklığındaki suda bekletildi ve universal test cihazında 0,5 mm/dak hızla makaslama testine tabi tutuldu. İstatistiksel değerlendirme tek yönlü varyans analizi ve Tukey çoklu karşılaştırma testi ile yapıldı. Bulgular: Grup SB ve grup R istatistiksel olarak diğer gruplara göre anlamlı derecede yüksek bağlantı dayanımı gösterirken (p0,05). Grup C, grup HF ve grup Z arasındaki makaslama bağlanma dayanımı farklılığı istatistiksel olarak anlamlı bulunmadı (p>0,05). Sonuçlar: Turkom-Cera tam seramiklerin adeziv simantasyonunda 50 µm’luk Al2O3 kumlama ve silika kaplama bağlanma dayanımı açısından yeterli ve tatmin edici sonuçlar vermektedir.

The widespread use of endosseous osseointegrated implants to replace missing natural teeth increases the chances of implant complications and failures, despite the high initial success rate reported in the literature. Implant and healing cap fracture are possible rare complications that can cause significant problems for both clinicians and patients. This article reports on 2 unique cases of implant and healing cap fracture, their possible causes, and how the cases were managed.

WOS: 000325843200003, Background/purpose: Several all-ceramic materials have been developed to meet the most challenging requirements in restorative dentistry. However, only limited reports have focused on the issue of biocompatibility of all-ceramics. The purpose of this study was to evaluate the effects of different types of all-ceramic substructural materials to determine their biological performance under experimental conditions. Materials and methods: We tested six all-ceramic systems [glass-infiltrated alumina-reinforced ceramic (In-Ceram Alumina, Turkom Cera), glass-infiltrated zirconia-toughened alumina ceramic (In-Ceram Zirconia), low-fusing ceramic (Finesse), yttria-stabilized tetragonal zirconia ceramic (Zirkonzahn), and lithium disilicate glass ceramic (IPS e.max)] using a tetrazolium assay prior to and 3 days after aging, to determine their ability to alter cellular mitochondrial dehydrogenase activity. Mann-Whitney U test and Wilcoxon t test were used for statistical analysis (alpha = 0.05). Results: According to the results of the 3-(4,5-dimethyl-thiazol-2yl-)-2,5-diphenyl tetrazolium bromide tests, the toxic effects of Finesse and Zirkonzahn were statistically insignificant (P > 0.05) compared with the negative control group. In contrast, In-Ceram Alumina, In-Ceram Zirconia, Turkom Cera, and IPS e.max demonstrated statistically significant toxic effects (P < 0.05) compared to the negative control group. When effects of aging on cytotoxic properties were evaluated, In-Ceram Zirconia and Turkom Cera showed increased cytotoxic effects on the 1st day following the aging process, whereas IPS e.max and Zirkonzahn displayed cytotoxic effects on the 2nd day and Day 7, respectively. The cytotoxic effect of;Zirkonzahn and IPS e.max was decreased on the 1st day and at the 2nd week, respectively. Conclusion: Several types of all-ceramic substructures did not cause the same in vitro. responses. Finesse and Zirkonzahn did not carry high biologic risk. However, our results suggest that In-Ceram Alumina, In-Ceram Zirconia, Turkom Cera, and IPS e.max should not be considered as entirely biocompatible materials. Aim: This case report describes the use of a surgical guide in a crown-lengthening procedure to improve the aesthetics of the anterior interdental papillae in an adult female patient. Methodology: A 40-year-old female underwent a crown-lengthening procedure with guidance provided by a surgical template to correct unsatisfactory aesthetics in the anterior gingiva and prosthesis. Results: The patient’s functional and aesthetic expectations were met successfully with interdisciplinary treatment, including surgical crown lengthening and the placement of all-ceramic restorations. Conclusions: This case showed that satisfactory anterior aesthetics can be achieved by comprehensive examination, careful planning of treatment employing a surgical guide, and prosthodontist–periodontist teamwork, even in the complete absence of interdental papillae. The purpose was to evaluate temperature increases during dowel space preparations with oval and circular fiber dowel systems.This study included 42 single-rooted human mandibular premolars. Roots were scanned with cone-beam computerized tomography (CBCT) to determine the ovoid root canal morphology. Root canals were treated with Ni-Ti rotary instruments and obturated. A second CBCT was taken to determine the thinnest dentin thickness of each root. Roots were randomly divided into two groups (n = 21) according to the fiber dowel system used: group 1, circular fiber dowel system (D.T. Light-Post); group 2, oval fiber dowel system (Ellipson Post). Dowel spaces were prepared using a circular fiber dowel drill and a diamond-coated ultrasonic tip with an oval section under water cooling until 9 mm dowel spaces were obtained. Temperature changes were recorded from the thinnest root surfaces using a FLIR E60 thermal imaging camera.Temperature increases were significantly greater with the circular fiber dowel system than with the oval fiber dowel system (p0.05).Although both dowel systems generated high temperature increases on root surfaces, the relatively lower temperature increase associated with the use of oval fiber dowels in ovoid canals makes it preferable to the use of circular fiber dowels.

The aim of this study was to measure the in vitro retention force of double conical crowns fabricated using primary galvanoforming and secondary casting techniques and those fabricated using primary casting and secondary galvanoforming techniques under simulated clinical conditions before and after a wear test.Primary galvanoformed crowns (n = 10) with non-noble secondary crowns (n = 10; group A) and primary non-noble crowns (n = 10) with secondary galvanoformed crowns (n = 10; group B) were fabricated. Each primary and secondary crown was embedded in acrylic resin and weighed with a digital balance. Retention forces were then measured using a universal testing device. To simulate wear, specimens were inserted and separated horizonatally 3285 times in wear equipment with artificial saliva. Retention forces and weights of the double crowns were then remeasured. Data were analyzed using paired t-tests and Wilcoxon tests, and the groups were compared using Mann-Whitney U-tests.In group A, the wear test had a significant influence on the retentive force (p0.05), but wear produced no significant difference in weight (p0.05). In group B, the wear test had a significant influence on the retentive force (p0.05), and wear produced a significant difference in weight (p0.05).The results of this study indicated that the use of different combinations of galvanoforming and casting techniques in the fabrication of conical crowns significantly affected retention force.

The effects of desensitizer, disinfectant, saliva, blood, and hydrogen peroxide on the tensile bond strength between adhesive and ceramic as well as between adhesive and dentin were examined. Sixty 7x3 mm pressed ceramic discs of IPS e.max were fabricated and randomly assigned to six groups of different dentin surface treatments (control, desensitizer, disinfectant, saliva, blood, and hydrogen peroxide). Representative samples of fractured specimens were observed by SEM (scanning electron microscopy). There were significant differences between the control group and saliva, blood, and hydrogen peroxide groups (p0.05). Results of this study suggested that only saliva, blood, and hydrogen peroxide influenced the tensile bond strength between dentin and ceramic.

Purpose: This study investigated the effects of different desensitizers and an adhesive material application on pulpal temperature rise during direct provisional restoration polymerization. Materials and Methods: Two dentin desensitizers (Systemp, Ivoclar; BisBlock, Bisco) and a dentin adhesive (Prime Bond, Dentsply) were applied to the dentin surfaces as single layer or double layer. One autopolymerizing methacrylate (Prevision, Heraeus Kulzer) and one light polymerizing composite (Revotek LC, GC) provisional restoration materials were used in this study. A total of 160 dentin disks were prepared from extracted molars. These dentin disks were placed on an apparatus developed to measure temperature rise. After the desensitizer or dentin adhesive application, provisional materials were polymerized according to the manufacturers' instructions. The temperature increase was measured under the dentin disk with a J-type thermocouple wire that was connected to a data logger. Differences between initial and highest temperature reading were taken, and 10 readings were averaged to determine the mean value in temperature rise. Statistical analysis was performed with three-way analysis of variance followed by Tukey HSD test (α = 0.05). Results: Temperature rise varied according to the provisional restoration material used (composite resin, methacrylate) (p < 0.001), the agent thickness (single, double) (p < 0.001), and the agent (desensitizer, adhesive) (p < 0.001). The composite resin-based provisional material induced higher thermal changes than methacrylate based provisional material (p < 0.01). Conclusion: The thickness of desensitizing agents affected the thermal changes; however, the desensitizers did not affect on the intrapulpal temperature although the type of provisional material used may be effective. © 2009 Wiley Periodicals, Inc. Objectives: The aim of this study was to investigate surface roughness in provisional crown acrylics, after polishing, reinforced with different concentrations of glass fibers.Methods: A total of 48 disk-shaped specimens were prepared using autopolymerizing acrylic resin. These specimens were divided into four groups according to the level of glass fiber added: Group A (no fiber), Group B (0.5%), Group C (1%) and Group D (2%). After polishing the specimens, an average surface roughness (Ra) value was calculated using a profilometer from four randomly selected points on the surface.Results: A significant difference was determined among the surface roughness values of provisional crown resins to which different concentrations of fiber had been added (PConclusions: The reinforcement of provisional crown and fixed partial denture resin with glass fibers increases surface roughness. PURPOSE. The aim of the present study was to evaluate the effects of posts with different morphologies on stress distribution in an endodontically treated mandibular premolar by using finite element models (FEMs). MATERIALS AND METHODS. A mandibular premolar was modeled using the ANSYS software program. Two models were created to represent circular and oval fiber posts in this tooth model. An oblique force of 300 N was applied at an angle of 45 to the occlusal plane and oriented toward the buccal side. von Mises stress was measured in three regions each for oval and circular fiber posts. RESULTS. FEM analysis showed that the von Mises stress of the circular fiber post (426.81 MPa) was greater than that of the oval fiber post (346.34 MPa). The maximum distribution of von Mises stress was in the luting agent in both groups. Additionally, von Mises stresses accumulated in the coronal third of root dentin, close to the post space in both groups. CONCLUSION.
