IntroductionTuberculosis (TB) preventive treatment (TPT) is recommended for people living with HIV (PLHIV) in high TB burden settings. While 6 months of daily isoniazid remains widely used, shorter regimens are now available. However, little is known about preferences of PLHIV for key features of TPT regimens.MethodsFrom July to November 2022, we conducted a discrete choice experiment among adult PLHIV engaged in care at an urban HIV clinic in Kampala, Uganda. Participants chose between two hypothetical TPT regimens with five different features (pills per dose, frequency, duration, need for adjusted antiretroviral therapy [ART] dosage and side effects), organized across nine random choice tasks. We analysed preferences using hierarchical Bayesian estimation, latent class analysis and willingness-to-trade simulations.ResultsOf 400 PLHIV, 392 (median age 44, 72% female, 91% TPT-experienced) had high-quality choice task responses. Pills per dose was the most important attribute (relative importance 32.4%, 95% confidence interval [CI] 31.6-33.2), followed by frequency (20.5% [95% CI 19.7-21.3]), duration (19.5% [95% CI 18.6-20.5]) and need for ART dosage adjustment (18.2% [95% CI 17.2-19.2]). Latent class analysis identified three preference groups: one prioritized less frequent, weekly dosing (N = 222; 57%); another was averse to ART dosage adjustment (N = 107; 27%); and the last prioritized short regimens with fewer side effects (N = 63; 16%). All groups highly valued fewer pills per dose. Overall, participants were willing to accept a regimen of 2.8 months' additional duration [95% CI: 2.4-3.2] to reduce pills per dose from five to one, 3.6 [95% CI 2.4-4.8] months for weekly rather than daily dosing and 2.2 [95% CI 1.3-3.0] months to avoid ART dosage adjustment.ConclusionsTo align with preferences of PLHIV in Uganda, decision-makers should prioritize the development and implementation of TPT regimens with fewer pills, less frequent dosing and no need for ART dosage adjustment, rather than focus primarily on duration of treatment.

Abstract Blood vessel formation relies on biochemical and mechanical signals, particularly during sprouting angiogenesis when endothelial tip cells (TCs) guide sprouting through filopodia formation. The contribution of BMP receptors in defining tip-cell characteristics is poorly understood. Our study combines genetic, biochemical, and molecular methods together with 3D traction force microscopy, which reveals an essential role of BMPR2 for actin-driven filopodia formation and mechanical properties of endothelial cells (ECs). Targeting of Bmpr2 reduced sprouting angiogenesis in zebrafish and BMPR2-deficient human ECs formed fewer filopodia, affecting cell migration and actomyosin localization. Spheroid assays revealed a reduced sprouting of BMPR2-deficient ECs in fibrin gels. Even more strikingly, in mosaic spheroids, BMPR2-deficient ECs failed to acquire tip-cell positions. Yet, 3D traction force microscopy revealed that these distinct cell behaviors of BMPR2-deficient tip cells cannot be explained by differences in force-induced matrix deformations, even though these cells adopted distinct cone-shaped morphologies. Notably, BMPR2 positively regulates local CDC42 activity at the plasma membrane to promote filopodia formation. Our findings reveal that BMPR2 functions as a nexus integrating biochemical and biomechanical processes crucial for TCs during angiogenesis.

Introduction The historical focus of vaccines on child health coupled with the advent of novel vaccines targeting adult populations necessitates exploring strategies for adult vaccine implementation.Areas covered This scoping review extracts insights from the past decade’s experiences introducing adult vaccines in low- and middle-income countries. Among 25 papers reviewed, 19 focused on oral cholera vaccine, 2 on Meningococcal A vaccines, 2 on tetanus toxoid vaccine, 1 on typhoid vaccine, and 1 on Ebola vaccine. Aligned with WHO’s Global Framework for New TB Vaccines for Adults and Adolescents, our findings center on vaccine availability, accessibility, and acceptance.Expert Opinion Availability findings underscore the importance of understanding disease burden for prioritization, multi-sectoral collaboration during planning, and strategic resource allocation and coordination. Accessibility results highlight the benefits of leveraging existing health infrastructure and adequately training healthcare workers, and contextually tailoring vaccine delivery approaches to reach challenging sub-groups like working male adults. Central to fostering acceptance, resonant sensitization, and communication campaigns engaging the communities and utilizing trusted local leaders countered rumors and increased awareness and uptake. As we approach the introduction of a new adult TB vaccine, insights from this review equips decision-makers with key evidence-based recommendations to support successful and equitable vaccinations targeting adults.

Abstract Objective The aims of this study were to translate the SAPE questionnaire (Scales of the Attitudes toward People with Epilepsy) questionnaire developed in Germany that assesses attitudes toward people with epilepsy (PWE) into Japanese and to examine its reliability and validity. Methods We crafted the Japanese version of SAPE (SAPE‐J) by drawing upon the original German version. On May 22nd and 23rd, 2023, we surveyed the general public registered with an online research survey service (Cross Marketing Group Inc., Tokyo, Japan) using an online questionnaire. Inclusion criteria were an age of ≥18 years, sufficient reading and speaking skills in Japanese, and the ability to comprehend the Japanese questionnaires. In addition to the translated SAPE‐J, we asked about knowledge of epilepsy, personal experience with epilepsy, and gathered information about age, gender, employment status, education level, marital status, and household income in accordance with the validation of the German version of the SAPE. Results 400 adults from the general public who had heard or read about epilepsy were asked to participate, granted informed consent, and completed the study questionnaire. The SAPE‐J questionnaire has 6 scales: 1. Social Distance; 2. Stereotypes; 3. Concerns when encountering a person with epilepsy (PWE) and emotional reactions differentiated by 4.1 Fear, 4.2. Anger, and 4.3 Pity; with a total of 26 items. The reliability of these scales ranged between acceptable to high (Cronbach's alpha 0.74–0.92) and confirmatory factor analyses (CFA) confirmed the presumed six‐factor structure of the SAPE (factorial validity). Multivariate generalized linear models (GLM) were used to investigate the construct validity and showed that, depending on subscale, different variables such as age, gender, education level, personal contact to PWE, and epilepsy specific knowledge were significant predictors of attitudes. Significance The Japanese version of the SAPE proved reliable and valid for assessing attitudes toward PWE in the Japanese public. Increasing understanding about how to deal with and accept persons with epilepsy may help reduce negative attitudes and fears about the condition. Plain Language Summary The study translated the German SAPE questionnaire, which measures attitudes toward people with epilepsy (PWE), into Japanese and tested its reliability and validity. The Japanese version (SAPE‐J) was created and surveyed online among 400 adults in Japan. The SAPE‐J has 6 scales covering social distance, stereotypes, and emotional reactions like fear, anger, and pity. Reliability was high (Cronbach's alpha 0.74–0.92), and factor analyses confirmed its structure. The study found age, gender, education, and personal contact with PWE influenced attitudes. The SAPE‐J is reliable and valid for assessing attitudes toward PWE in Japan, potentially reducing negative views and fears about epilepsy.

Catastrophic fire events in the last decade have led to policy changes regarding the permitted use of fire in cropping systems in Indonesia. This has led to a multi-stakeholder approach to fire management in the extensive peatland systems of Sumatra and Kalimantan. This study seeks to identify key stakeholders and to map and identify their roles in peat fire management at the village level. Two case study villages were used: Kayu Labu in South Sumatra and Tumbang Nusa in Central Kalimantan. The study was conducted using a Responsible-Accountable-Consulted-Informed (RACI) matrix. Data collection was carried out through in-depth interviews and a focus group discussion (FGD) in each village. Our findings showed that the Village Head was the key actor in fire management at the village level. The other stakeholders had shared roles in a largely collaborative process that included the provision of additional funding for resources and training. However, stakeholder engagement in the restoration of burnt peatland was limited, and the engagement of villagers in fire prevention and control was compromised by the threat of sanctions associated with the prohibition of burning. The findings suggest that policy commitments need to be revised in a way that ensures better engagement between external stakeholders and villagers, and focuses on the need to protect livelihoods.

The purpose of this mixed methods, multi-year study was to explore aspects of a virtual international literacies project, the Global Read Aloud, that promoted students' multiple literacies. Guided by theories of critical, digital, and global literacies, we analyzed survey (N = 436) and interview (N = 21) data from K-12 literacy educators. Data revealed that educators perceived GRA-related benefits of virtual collaboration for digital literacies learning, of reading quality multicultural literature, and of teaching critical literacies as foundational to global meaning making. The results of this study contribute teachers' voices to discussions on the importance of developing critical, digital, and global literacies in this interconnected world. Data suggest that using shared readings and virtual collaboration can foster a community of readers, locally and globally, that create possibilities for students to experience and use literacies as a part of participating in authentic global meaning making.

Background Employment and relationship are crucial for social integration. However, individuals with major psychiatric disorders often face challenges in these domains. Aims We investigated employment and relationship status changes among patients across the affective and psychotic spectrum – in comparison with healthy controls, examining whether diagnostic groups or functional levels influence these transitions. Method The sample from the longitudinal multicentric PsyCourse Study comprised 1260 patients with affective and psychotic spectrum disorders and 441 controls (mean age ± s.d., 39.91 ± 12.65 years; 48.9% female). Multistate models (Markov) were used to analyse transitions in employment and relationship status, focusing on transition intensities. Analyses contained multiple multistate models adjusted for age, gender, job or partner, diagnostic group and Global Assessment of Functioning (GAF) in different combinations to analyse the impact of the covariates on the hazard ratio of changing employment or relationship status. Results The clinical group had a higher hazard ratio of losing partner (hazard ratio 1.46, P < 0.001) and job (hazard ratio 4.18, P < 0.001) than the control group (corrected for age/gender). Compared with controls, clinical groups had a higher hazard of losing partner (affective group, hazard ratio 2.69, P = 0.003; psychotic group, hazard ratio 3.06, P = 0.001) and job (affective group, hazard ratio 3.43, P < 0.001; psychotic group, hazard ratio 4.11, P < 0.001). Adjusting for GAF, the hazard ratio of losing partner and job decreased in both clinical groups compared with controls. Conclusion Patients face an increased hazard of job loss and relationship dissolution compared with healthy controls, and this is partially conditioned by the diagnosis and functional level. These findings underscore a high demand for destigmatisation and support for individuals in managing their functional limitations.

BackgroundIn high-burden settings, most tuberculosis (TB) transmission likely occurs outside the home. Our qualitative study in Zambia explored the acceptability and preferences for designing TB active case finding (ACF) strategies to reach non-household contacts of people with TB.MethodsWe conducted 56 in-depth interviews with persons with TB (n = 12), TB healthcare workers (HCWs) (n = 10), TB lay HCWs (n = 10), and leaders/owners (n = 12) and attendees (n = 12) of community venue types identified as potential TB transmission locations. Interviews explored TB attitudes and beliefs, and perceptions toward two ACF strategies targeting non-household contacts: (1) “social-network strategy”—persons with newly diagnosed TB reach out to their close non-household contacts and (2) “venue-based strategy”—HCWs conduct screening at community venues frequented by persons with newly diagnosed TB. We used the Consolidated Framework for Implementation Research (CFIR) framework to develop interview topic guides and analyze data using a rapid deductive approach.ResultsAll participants felt that TB was an important issue in their community and that new detection strategies were needed. A “social-network strategy” was perceived as acceptable and feasible, where participants noted it was a caring act and could facilitate early diagnosis. For a “venue-based strategy,” most participants suspected TB transmission occurred at bars/taverns due to heavy alcohol use and prolonged time spent in crowded spaces; churches and betting halls were also commonly mentioned locations. Nearly all owners/leaders and patrons/attendees of bars, churches, and betting halls expressed acceptance of a venue-based strategy. They also indicated an interest in participating, citing many benefits, including increased TB knowledge/awareness, early diagnosis, convenience, and possibly reduced transmission, and recommended that the strategy incorporate sensitization, consent, volunteerism, and respectful, confidential, private services. For both strategies, most participants preferred the use of and being approached by trained peer TB survivors to facilitate ACF, given their prior TB patient experience and trust among community members.ConclusionStakeholders found social-network and venue-based TB-ACF strategies highly acceptable, recognizing their potential benefits for individuals and the broader community. Future research should evaluate the feasibility and effectiveness of TB ACF strategies for reaching non-household contacts.

BackgroundThe World Health Organization (WHO) recommends that outpatient people living with HIV (PLHIV) undergo tuberculosis screening with the WHO four-symptom screen (W4SS) or C-reactive protein (CRP) (5 mg·L-1 cut-off) followed by confirmatory testing if screen positive. We conducted an individual participant data meta-analysis to determine the performance of WHO-recommended screening tools and two newly developed clinical prediction models (CPMs).MethodsFollowing a systematic review, we identified studies that recruited adult outpatient PLHIV irrespective of tuberculosis signs and symptoms or with a positive W4SS, evaluated CRP and collected sputum for culture. We used logistic regression to develop an extended CPM (which included CRP and other predictors) and a CRP-only CPM. We used internal-external cross-validation to evaluate performance.ResultsWe pooled data from eight cohorts (n=4315 participants). The extended CPM had excellent discrimination (C-statistic 0.81); the CRP-only CPM had similar discrimination. The C-statistics for WHO-recommended tools were lower. Both CPMs had equivalent or higher net benefit compared with the WHO-recommended tools. Compared with both CPMs, CRP (5 mg·L-1 cut-off) had equivalent net benefit across a clinically useful range of threshold probabilities, while the W4SS had a lower net benefit. The W4SS would capture 91% of tuberculosis cases and require confirmatory testing for 78% of participants. CRP (5 mg·L-1 cut-off), the extended CPM (4.2% threshold) and the CRP-only CPM (3.6% threshold) would capture similar percentages of cases but reduce confirmatory tests required by 24, 27 and 36%, respectively.ConclusionsCRP sets the standard for tuberculosis screening among outpatient PLHIV. The choice between using CRP at 5 mg·L-1 cut-off or in a CPM depends on available resources.

The importance of emotional awareness (EA) and emotion regulation (ER) for children's and adolescents' development has been suggested in numerous studies, but longitudinal trajectories of these aspects of emotional competence have rarely been examined. Therefore, the main aim of this study was to investigate developmental trends of EA and ER in early and middle adolescence over a 1-year period. Longitudinal increases of different aspects of EA (emotion differentiation, bodily unawareness, attention to others', and analyses of own emotions) and dysfunctional ER, and decreases of functional ER were expected. Furthermore, it was explored whether these trajectories as well as their initial levels were associated with gender and age. Three-wave longitudinal self-report data of N = 1,225 German adolescents (aged 10-15 years at initial assessment, 54% female) who had completed the Emotion Awareness Questionnaire and the Regulation of Emotions Questionnaire were analyzed. After establishing at least partial scalar measurement invariance for the longitudinal models, the second-order Latent Growth Curve Models (LGCMs) were computed. Regarding EA, increases of emotion differentiation and bodily unawareness, as well as adolescents' attention to others' emotions, but also a decrease of their willingness to analyze own emotions were found. For ER, results suggested stability of dysfunctional and decreases of functional strategies. Conditional LGCMs (including gender, age, and the interaction between both) indicated that the increase of emotion differentiation and the decrease in the use of selected functional ER strategies were more pronounced for younger participants. Gender differences were found only for baseline but not for developmental trends, and no significant interaction with age was found. Overall, this study illustrates the developmental trajectories of EA and ER over the course of 1 year and emphasizes that adolescents have difficulties in applying functional ER strategies with increasing age, despite improvements in EA.

Summary: Controlled myogenic differentiation is integral to the development, maintenance and repair of skeletal muscle, necessitating precise regulation of myogenic progenitors and resident stem cells. The transformation of proliferative muscle progenitors into multinuclear syncytia involves intricate cellular processes driven by cytoskeletal reorganization. While actin and microtubles have been extensively studied, we illuminate the role of septins, an essential yet still often overlooked cytoskeletal component, in myoblast architecture. Notably, Septin9 emerges as a critical regulator of myoblast differentiation during the initial commitment phase. Knock-down of Septin9 in C2C12 cells and primary mouse myoblasts accelerates the transition from proliferation to committed progenitor transcriptional programs. Furthermore, we unveil significant reorganization and downregulation of Septin9 during myogenic differentiation. Collectively, we propose that filmamentous septin structures and their orchestrated reorganization in myoblasts are part of a temporal regulatory mechanism governing the differentiation of myogenic progenitors. This study sheds light on the dynamic interplay between cytoskeletal components underlying controlled myogenic differentiation.

To reach the millions of people with tuberculosis (TB) undiagnosed each year, there is an important need to provide people-centered screening and testing services. Despite people-centered care being a key pillar of the WHO END-TB Strategy, there have been few attempts to formally characterize and integrate the preferences of people most affected by TB - including those who have increased exposure to TB, limited access to services, and/or are at increased risk for TB - into new tools and strategies to improve screening and diagnosis. This perspective emphasizes the importance of preference research among people most affected by TB, provides an overview of qualitative preference exploration and quantitative preference elicitation research methods, and outlines how preferences can be applied to improve the acceptability, accessibility, and appropriateness of TB screening and testing services via four key opportunities. These include the following: (1) Defining the most preferred features of novel screening, triage, and diagnostic tools, (2) exploring and prioritizing setting-specific barriers and facilitators to screening and testing, (3) understanding what features of community- and facility-based strategies for improving TB detection and treatment are most valued, and (4) identifying the most relevant and resonant communication strategies to increase individual- and community-level awareness and demand. Preference research studies and translation of their findings into policy/guidance and operationalization have enormous potential to close the existing gaps in detection in high burden settings by enhancing the people-centeredness and reach of screening and diagnostic services to people most affected by TB who are currently being missed and left behind.

PURPOSE OF REVIEW: Despite the growing availability of effective HIV prevention and treatment interventions, there are large gaps in their uptake and sustained use across settings. It is crucial to elicit and apply patients and stakeholders preferences to maximize the impact of existing and future interventions. This review summarizes quantitative preference elicitation methods (PEM) and how they can be applied to improve the delivery and uptake of HIV prevention and treatment interventions. RECENT FINDINGS: PEM are increasingly applied in HIV implementation research; however, discrete choice experiments (DCEs) have predominated. Beyond DCEs, there are other underutilized PEM that may improve the reach and effectiveness of HIV prevention and treatment interventions among individuals by prioritizing their barriers to engagement and determining which attributes of interventions and delivery strategies are most valued. PEM can also enhance the adoption and sustained implementation of strategies to deliver HIV prevention and treatment interventions by assessing which attributes are the most acceptable and appropriate to key stakeholders. SUMMARY: Greater attention to and incorporation of patients and stakeholders preferences for HIV prevention and treatment interventions and their delivery has the potential to increase the number of persons accessing and retained in HIV prevention and treatment services.

This article focuses on moral judgements about the integrity of politicians expressed by various actors in public opinion in the wake of Dutch integrity scandals. The research assesses the link between a supposed act on the one hand and moral judgements of the actors involved on the other in an effort to improve our understanding of how political integrity is perceived and how it is used in public and political debate. This exploratory study takes three Dutch case studies of supposed conflicts of interest. Each case offers a view on judgements concerning political integrity from various media outlets, focused on three different levels: judgements of personal integrity, role integrity and political system integrity. This novel theoretical construct serves to see whether and, if so, how moral judgements about political integrity are linked to systemic (dis)satisfaction with government and politics. In addition—and focusing on the Dutch context only—the article assesses how people’s supposed place on a political ideological spectrum from left to right might influence their moral judgement. The article underscores the theoretical notion that the meaning of political integrity is inherently subject to varied interpretation and context of place and time. This also has important practical implications for managing and improving political integrity.

Statistics courses are challenging for many psychology students. We designed a 'Painting and Puzzles Exercise Book for Statistics' for first year psychology undergraduates, to repeat and deepen the course content and get prepared for the exam. We describe the development and characteristics of the book and report a longitudinal evaluation study. Users (N=72) rated the book positively and judged it as helpful, easy to use, and enjoyable. Students with initially higher skills, younger students and, those who graduated from school recently were more likely to use it. We matched users to non-users from the preceding and the evaluation cohort, respectively, on characteristics at study entry and found positive effects on achievement (exam grade and perceived management of course content) for users. We reason that the book is a valuable support that can be embedded in an ongoing course easily.

Early treatment of anal high-grade squamous intraepithelial lesions compared with active monitoring reduced the risk of anal cancer by 57% in persons with HIV in a landmark randomized trial of 4446 participants. In a multi- country randomized trial, an entirely oral combination regimen consisting of bedaquiline, pretomanid, linezolid, and moxifloxacin for 24 weeks outperformed the World Health Organization-recommended 36- to 96-week standard of care regimen for multidrug-resistant tuberculosis (TB), ushering in a new era of shorter multidrug-resistant TB treatment. These and other studies of TB and coinfections in persons with HIV presented at the 2022 Conference on Retroviruses and Opportunistic Infections pro vided new insights and are summarized herein.

O objetivo do presente estudo foi avaliar as características da carcaça e da carne de novilhos confinados no Brasil de acordo com o grupo genético por meio de uma revisão sistemática e meta-análise. Foi realizada busca eletrônica de artigos nas bases de dados Scielo e Google Acadêmico e os dados foram agrupados de acordo com a predominância genética. Os resultados foram testados quanto à heterogeneidade e submetidos ao teste de normalidade e teste F e as médias comparadas pelo teste de Tukey. Novilhos predominantemente britânicos foram terminados em idade mais jovem, necessitaram de tempos de confinamento mais longos e apresentaram menores pesos de carcaça quente e menores áreas de olho de lombo no abate. Os percentuais de carcaça muscular foram maiores nos novilhos Continentais e Sintéticos em relação aos Britânicos e Zebu, enquanto os percentuais de gordura foram maiores nos Britânicos e Zebu em comparação aos continentais e sintéticos. O marmoreio da carne foi maior nos novilhos britânicos, continentais e mestiços em relação aos novilhos sintéticos e zebuínos. A origem do novilho britânico permite menor idade de abate, embora leve à redução do peso da carcaça quente, enquanto a predominância genética do zebu pode produzir carcaças com alto percentual de gordura.

Myelodysplastic neoplasms (MDS) are clonal disorders of the myeloid lineage leading to peripheral blood cytopenias. Dysregulation of innate immunity is hypothesized to be a potent driver of MDS. A recent study revealed increased thrombomodulin (TM) expression on classical monocytes in MDS, which was associated with prolonged survival. TM is a receptor with immunoregulatory capacities, however, its exact role in MDS development remains to be elucidated. In this review we focus on normal monocyte biology and report on the involvement of monocytes in myeloid disease entities with a special focus on MDS. Furthermore, we delve into the current knowledge on TM and its function in monocytes in health and disease and explore the role of TM-expressing monocytes as driver, supporter or epiphenomenon in the MDS bone marrow environment.

IntroductionThis study investigated whether children with and without dyslexia differ in word learning and whether phonological cues to word class play a role. If children with dyslexia have difficulties with implicit learning, they might be less sensitive to such cues.MethodsA group of 89 Dutch primary school children from Grades 3 to 6 participated in a word learning experiment, consisting of children with dyslexia (n = 44) and typically developing children (n = 45). Test items were four monosyllabic ‘verb-like' nonwords (e.g., voek) and four bisyllabic ‘noun-like' nonwords (e.g., banijn). They were presented as novel verbs or nouns in a two word sentence frame (e.g., “I voek” or “a voek”), paired with pictures of unfamiliar actions or objects. Nonwords were either consistent (e.g., “I voek,” “a banijn”) or inconsistent (e.g., “I banijn,” “a voek”) with word class. The word learning experiment consisted of a repetition, identification, and naming phase.ResultsChildren with dyslexia showed lower word learning outcomes in the naming phase. However, phonological cues did not affect word learning in either group. Regression analyses indicated that phoneme awareness, receptive vocabulary, and nonword reading were predictors of word learning for all children.DiscussionThese findings indicate that Dutch children with dyslexia have more difficulty in recalling novel words, fitting in with their phonological difficulties. Phonological cues to word class did not contribute to word learning in either group.

Abstract Background Mantle cell lymphoma (MCL) is a type of B‐cell lymphoma that is currently incurable. Pirtobrutinib shows promising response rates in heavily pretreated MCL patients according to the approval study, but the real‐world data are scarce. Methods In this study, we retrospectively analyzed the efficacy and safety profile of pirtobrutinib in 10 relapsed/refractory MCL patients from compassionate use program (CUP). Results On average, the patients underwent three lines of systemic therapy prior to pirtobrutinib and were predominantly BTKi exposed (9/10). The best overall response rate (BORR) was 67%. In a median follow‐up of 8.6 months, the mean duration of response (DOR), progression‐free survival (PFS), and overall survival (OS) were not reached. No new safety signals were documented. Conclusions In summary, pirtobrutinib represented a safe and effective treatment option in a small real‐world population.

Background/settingIn San Francisco, HIV viral suppression is 71% among housed individuals but only 20% among unhoused individuals. We conducted a discrete choice experiment at a San Francisco public HIV clinic to evaluate care preferences among people living with HIV (PLH) experiencing homelessness/unstable housing during the COVID-19 pandemic.MethodsFrom July to November 2020, we conducted a discrete choice experiment among PLH experiencing homelessness/unstable housing who accessed care through (1) an incentivized, drop-in program (POP-UP) or (2) traditional primary care. We investigated 5 program features: single provider vs team of providers; visit incentives ($0, $10, and $20); location (current site vs current + additional site); drop-in vs scheduled visits; in-person only vs optional telehealth visits; and navigator assistance. We estimated relative preferences using mixed-effects logistic regression and conducted latent class analysis to evaluate preference heterogeneity.ResultsWe enrolled 115 PLH experiencing homelessness/unstable housing, 40% of whom lived outdoors. The strongest preferences were for the same provider (β = 0.94, 95% CI: 0.48 to 1.41), visit incentives (β = 0.56 per $5; 95% CI: 0.47 to 0.66), and drop-in visits (β = 0.47, 95% CI: 0.12 to 0.82). Telehealth was not preferred. Latent class analysis revealed 2 distinct groups: 78 (68%) preferred a flexible care model, whereas 37 (32%) preferred a single provider.ConclusionsWe identified heterogeneous care preferences among PLH experiencing homelessness/unstable housing during the COVID-19 pandemic, with two-thirds preferring greater flexibility and one-third preferring provider continuity. Telehealth was not preferred, even with navigator facilitation. Including patient choice in service delivery design can improve care engagement, particularly for marginalized populations, and is an essential tool for ending the HIV epidemic.
