Objectives To evaluate the incidence of adverse drug reactions (ADRs), including hypersensitivity reactions (HSRs) and post-contrast acute kidney injury (PC-AKI), after intravenous (IV) administration of ioversol. Materials and methods A systematic literature search (1980–2021) of studies documenting IV use of ioversol and presence or absence of ADRs, HSRs, or PC-AKI was performed. Key information including patients’ characteristics, indication and dose of ioversol, safety outcome incidence, intensity and seriousness were extracted. Results Thirty-one studies (> 57,000 patients) were selected, including 4 pediatric studies. The incidence of ADRs in adults was reported in 12 studies from ioversol clinical development with a median (range) of 1.65% (0–33.3%), and 3 other studies with an incidence between 0.13 and 0.28%. The incidence of HSRs (reported in 2 studies) ranged from 0.20 to 0.66%, and acute events (4 studies) from 0.23 to 1.80%. Severe reactions were rare with a median (range) of 0 (0–4%), and none were reported among pediatric patients. The incidence of ADRs and HSRs with ioversol, especially those of severe intensity, was among the lowest in studies comparing different iodinated contrast media (ICM) of the same class. PC-AKI incidence was variable (1–42% in 5 studies); however, ioversol exposure per se did not increase the incidence. Conclusions When administered by the IV route, ioversol has a good safety profile comparable to that of other ICM within the same class, with a low incidence of severe/serious ADRs overall, and particularly HSRs. PC-AKI incidence does not seem to be increased compared to patients who did not receive ioversol. Further well-designed studies are warranted to confirm these results. Key Points • Ioversol has a good safety profile in adult and pediatric patients when IV administered. • ADR and HSR incidence with ioversol, especially those of severe intensity, was among the lowest compared to other ICM. • IV administration of ioversol per se did not increase PC-AKI incidence.

Objectives To evaluate the incidence of adverse drug reactions (ADRs), including hypersensitivity reactions (HSRs) and post-contrast acute kidney injury (PC-AKI), after intra-arterial (IA) administration of ioversol. Methods and materials A systematic literature search was performed (1980–2021) and studies documenting IA use of ioversol, and reporting safety outcomes were selected. Key information on study design, patients’ characteristics, indication, dose, and type of safety outcome were extracted. Results Twenty-eight studies (including two pediatric studies) with 8373 patients exposed to IA ioversol were selected. Studies were highly heterogenous in terms of design, PC-AKI definition, and studied population. PC-AKI incidence after coronary angiography was 7.5–21.9% in a general population, 4.0-26.4% in diabetic patients, and 5.5–28.9% in patients with chronic kidney disease (CKD). PC-AKI requiring dialysis was rare and reported mainly in patients with severe CKD. No significant differences in PC-AKI rates were shown in studies comparing different iodinated contrast media (ICM). Based on seven studies of ioversol clinical development, the overall ADR incidence was 1.6%, comparable to that reported with other non-ionic ICM. Pediatric data were scarce with only one study reporting on PC-AKI incidence (12%), and one reporting on ADR incidence (0.09%), both after coronary angiography. Conclusions After ioversol IA administration, PC-AKI incidence was highly variable between studies, likely reflecting the heterogeneity of the included study populations, and appeared comparable to that reported with other ICM. The rate of other ADRs appears to be low. Well-designed studies are needed for a better comparison with other ICM. Key Points • PC-AKI incidence after IA administration of ioversol appears to be comparable to that of other ICM, despite the high variability between studies. • The need for dialysis after IA administration of ioversol is rare. • No obvious difference was found regarding the safety profile of ioversol between IA and IV administration.

- Abstract
BACKGROUND AND PURPOSE: Chronic kidney disease with reduced estimated glomerular filtration rate or elevated albuminuria increases risk for ischemic and hemorrhagic stroke. This study assessed the effects of sodium glucose cotransporter 2 inhibitors (SGLT2i) on stroke and atrial fibrillation/flutter (AF/AFL) from CREDENCE (Canagliflozin and Renal Events in Diabetes With Established Nephropathy Clinical Evaluation) and a meta-Analysis of large cardiovascular outcome trials (CVOTs) of SGLT2i in type 2 diabetes mellitus. METHODS: CREDENCE randomized 4401 participants with type 2 diabetes mellitus and chronic kidney disease to canagliflozin or placebo. Post hoc, we estimated effects on fatal or nonfatal stroke, stroke subtypes, and intermediate markers of stroke risk including AF/AFL. Stroke and AF/AFL data from 3 other completed large CVOTs and CREDENCE were pooled using random-effects meta-Analysis. RESULTS: In CREDENCE, 142 participants experienced a stroke during follow-up (10.9/1000 patient-years with canagliflozin, 14.2/1000 patient-years with placebo; hazard ratio [HR], 0.77 [95% CI, 0.55-1.08]). Effects by stroke subtypes were: ischemic (HR, 0.88 [95% CI, 0.61-1.28]; n=111), hemorrhagic (HR, 0.50 [95% CI, 0.19-1.32]; n=18), and undetermined (HR, 0.54 [95% CI, 0.20-1.46]; n=17). There was no clear effect on AF/AFL (HR, 0.76 [95% CI, 0.53-1.10]; n=115). The overall effects in the 4 CVOTs combined were: Total stroke (HRpooled, 0.96 [95% CI, 0.82-1.12]), ischemic stroke (HRpooled, 1.01 [95% CI, 0.89-1.14]), hemorrhagic stroke (HRpooled, 0.50 [95% CI, 0.30-0.83]), undetermined stroke (HRpooled, 0.86 [95% CI, 0.49-1.51]), and AF/AFL (HRpooled, 0.81 [95% CI, 0.71-0.93]). There was evidence that SGLT2i effects on total stroke varied by baseline estimated glomerular filtration rate (P=0.01), with protection in the lowest estimated glomerular filtration rate (45 mL/min/1.73 m2]) subgroup (HRpooled, 0.50 [95% CI, 0.31-0.79]). CONCLUSIONS: Although we found no clear effect

Supplemental Digital Content is available in the text., Background and Purpose: Chronic kidney disease with reduced estimated glomerular filtration rate or elevated albuminuria increases risk for ischemic and hemorrhagic stroke. Results: In CREDENCE, 142 participants experienced a stroke during follow-up (10.9/1000 patient-years with canagliflozin, 14.2/1000 patient-years with placebo; hazard ratio [HR], 0.77 [95% CI, 0.55–1.08]). Effects by stroke subtypes were: ischemic (HR, 0.88 [95% CI, 0.61–1.28]; n=111), hemorrhagic (HR, 0.50 [95% CI, 0.19–1.32]; n=18), and undetermined (HR, 0.54 [95% CI, 0.20–1.46]; n=17). There was no clear effect on AF/AFL (HR, 0.76 [95% CI, 0.53–1.10]; n=115). The overall effects in the 4 CVOTs combined were: total stroke (HRpooled, 0.96 [95% CI, 0.82–1.12]), ischemic stroke (HRpooled, 1.01 [95% CI, 0.89–1.14]), hemorrhagic stroke (HRpooled, 0.50 [95% CI, 0.30–0.83]), undetermined stroke (HRpooled, 0.86 [95% CI, 0.49–1.51]), and AF/AFL (HRpooled, 0.81 [95% CI, 0.71–0.93]). There was evidence that SGLT2i effects on total stroke varied by baseline estimated glomerular filtration rate (P=0.01), with protection in the lowest estimated glomerular filtration rate (

The horizontal force on a continent, at the land/ocean boundary, is calculated according to the concept of reflected tide waves, in which there is a time rate of change (reversal) of the linear momentum brought in by the tide to the continent. From the text books, the Stokes drift for surface gravity waves propagating in arbitrary constant mean depth, which is directly related to the linear momentum of the wave, is adapted to the tide wave by using the approximation that the wavelength of the tide is very much larger than the vertical length of the water column. This horizontal force is a maximum at full and new moon when the tide amplitude is greatest. Evidence is cited for the correlation of earthquakes at full moons. The tendency of the reflected tide force on the continents is to cause compression of the solid material in the horizontal direction and it is suggested that this possibly could trigger earthquakes occasionally.

- Abstract
ICT hold significant potential to increase resource and energy efficiencies and contribute to a circular economy. Yet unresolved is whether the aggregated net effect of ICT overall mitigates or aggravates environmental burdens. While the savings potentials have been explored, drivers that prevent these and possible counter measures have not been researched thoroughly. The concept digital sufficiency constitutes a basis to understand how ICT can become part of the essential environmental transformation. Digital sufficiency consists of four dimensions, each suggesting a set of strategies and policy proposals: (a) hardware sufficiency, which aims for fewer devices needing to be produced and their absolute energy demand being kept to the lowest level possible to perform the desired tasks; (b) software sufficiency, which covers ensuring that data traffic and hardware utilization during application are kept as low as possible; (c) user sufficiency, which strives for users applying digital devices frugally and using ICT in a way that promotes sustainable lifestyles; and (d) economic sufficiency, which aspires to digitalization supporting a transition to an economy characterized not by economic growth as the primary goal but by sufficient production and consumption within planetary boundaries. The policies for hardware and software sufficiency are relatively easily conceivable and executable. Policies for user and economic sufficiency are politically more difficult to implement and relate strongly to policies for environmental transformation in general. This article argues for comprehensive policies for digital sufficiency, which are indispensible if ICT are to play a beneficial role in overall environmental transformation., QC 20230213

If it is accepted that thermal convection consistently takes place inside the North Pacific High, as proposed here, then the existence of the NPH, as well as its seasonal variation, will be explained simultaneously, building on an earlier attempt. More observations than available at present would help prove that thermal convection happens and pin down its characteristics, since it is not visible. Also the physics of how thermal convection produces relatively high pressure at sea level needs work.

Assuming thermal convection takes place continuously inside the North Pacific High, an application of physics to the rising sea-warmed air plumes increases understanding of the relatively high air pressure at the sea surface and at the bases of the plumes. Since the ocean is warmer than the air under the NPH, heat is transferred upward decreasing the air density, which should then accelerate upward when no other forces are around to stop it. By Newton’s 3rd law the upward accelerating air will cause an equal but opposite (downward) reaction resulting in high pressure in the air under the rising column. That is the central proposal offered, which is consistent with available observations. New data that confirm the upward acceleration of the heated air are especially needed. Perhaps dye or neutrally buoyant particles could be released from a ship or buoy to make the upward air motion visible.

A first attempt has been made to confirm experimentally a theoretical concept, recently published, involving a rigid cone rotating about its long axis under still water: it should tend to translate along that axis blunt end leading and apex trailing. Two identical hollow cones, neutrally buoyant, with equal weights attached to the apexes, were released simultaneously at the surface of a swimming pool. One cone had a thin light weight spiral vane vertically attached to the cone’s outside surface in order to cause it to rotate as it sank. Several trial runs were made in the shallow and deep ends of the pool, and in every case, the non-rotating cone without a vane hit the bottom of the pool first. These comparisons qualitatively and indirectly validate the prediction.

Fluid particles in translating surface gravity waves have an orbital motion which decreases in size with increasing mean depth. These wave characteristics came from observations and were not forecast theoretically. The classical potential flow model is incapable of explaining the particle movement due to the irrotational assumption and to a flaw in carrying out the method. When a wave passes by an observer from left to right, the particles move clockwise under a crest and a trough. This correct conclusion is consistent with what the incorrect standard theory implies but should not be considered to have been derived from it.

A recent formula for the lift force on a low speed wing of circular arc cross-section [1] is adapted to the upward pressure force on the crests of a surface gravity wave propagating in the wind. In both cases, the main feature is the utilization of the air’s compressibility. At and near a wave crest, it is predicted that the air density is increased over the ambient value and that the air density decreases inversely as the square of the upward distance from the radius of curvature of the crest. As a consequence, the air pressure also decreases upward inversely as the square of the same distance. Therefore, an upward pressure force on each crest occurs which presumably will make the crests grow. Growth rates are largest for small wavelengths and large mean slopes of the wave surface. Contrary winds should produce wave growth (not damping) as well as no wind at all.

A fluid flow model consisting of Bernoulli’s law in its normal form, the equation of state of air, and the cross-stream force balance between a downward pressure gradient and the upward centrifugal force on fluid particles moving along curved streamlines over the top circular wing surface involving three equations in three unknowns (pressure, density and velocity) are solved to show that both density and pressure decrease upward as the inverse square of the distance from the circle’s center, and the velocity is independent of that dis-tance. These derived characteristics are used to explain the lift force on the wing in what is believed to be a novel way.

Starting with a recent unconventional explanation of the lift force on a wing, featuring compressibility of the air, an application of the same concept is made to the lift force on the equatorial sea surface due to the Trade Winds, by greatly increasing the spatial scales. If the equatorial sea level does rise up, the northward slope to the sea level should facilitate the poleward flux of summer heat in both the North Pacific and North Atlantic Oceans, as two examples, in accordance with the heat budget requirements of these oceans. Compressed air dynamics consists of Bernoulli’s law applied to the streamlines of the Trade Winds, the force balance between the upward centrifugal force of the curved streamlines at the earth’s surface and a downward pressure force, and the perfect gas law for air.

A typical summer weather event in the mid-west starts with a cool dry wind moving south near the Rocky Mountains and an equal mass of warm moist air moving north to the east of it in a side by side arrangement. Arguments supporting this proposition include observations, theory and one assumption. Observations are an awareness of the predominately north/south wind directions at one point in northwest Iowa for more than 30 consecutive summers as well as knowledge of the eastward flourishing vegetation distribution throughout the mid-west starting at the Rockies. The theory is the expectation that in geophysical flows the configuration most likely to persist and be observed has the least amount of friction among all possibilities. One assumption is that only a single southward cold wind takes place at one time to initiate the weather “cycle”.

A physical explanation is given for the observations that ocean surface gravity waves can travel up to half way around the world from generation in a wind storm to dissipation on shore. Inherent in these waves is an orbital fluid particle motion, known from laboratory experiments, that has no friction according to the Navier-Stokes equations. The prediction is based on application of Bernoulli’s law to all the closed orbital paths of the fluid particles and the cross-stream force balance on the particles between a pressure gradient and the centrifugal force in each orbital loop.

When a solid cone with smooth side and base rotates about its long axis in a still fluid, theory says that the cone will advance along the direction of the axis, base first and apex last. Bernoulli’s law for closed streamline loops is combined with the cross-stream force balance between the centrifugal force and a pressure gradient in order to obtain the result, which is believed to be new. When a solid cone with smooth surfaces rotates at a constant rate about its long axis in still water, it will experience no friction, as announced earlier [1] and documented further here. However, if not restrained, it will translate along the axis base first and apex last, which is caused by a variation of pressure on its side: low near the base and high at the apex. This translation needs to be verified experimentally. Friction will occur during translation but an opposite reaction force will also take place related to the front to back asymmetry of the body [2]. Whether or not these two oppositely directed forces can cancel each other out by an appropriate choice of variable magnitudes is not known, but if true, it would lead to an extended translation. Also the translation path could be lengthened by slightly modifying the front face of the cone such that the translation itself produces rotation. One idea for such a modification is suggested. Observations in the future would be very beneficial.

Surface gravity waves continually come to the beach but rarely go back to sea. They bring excess mass to shore which must be returned offshore. Rip currents do that job because there is less overall friction in the nearshore region than there is in the only other imagined circulation, a two-layer scheme, which has in fact never been seen. An argument is presented to support this proposal. If correct, rip currents join a group of geophysical flow phenomena that persist, probably because there is zero friction associated with them, which include surface gravity waves, tornadoes and hurricanes, individually promoted recently.

One of Newton’s mathematical solutions to a hypothetical orbital problem, recently verified by an independent physics model, is applied to the fluid particle motion in shallow water surface gravity waves. What is the functional form of the central force, with origin at the ellipse’s center, which will keep a body in the orbit? Newton found out it is the spring force, which is linear. All fluid particles in shallow water waves move in ellipses. By a superposition of solutions in a linear problem, the application of Newton’s result to shallow water waves is combined with a feature not noticed by Newton: the orbital period is independent of the semi-major and semi-minor axes. Two conclusions reached are that the wave period of shoaling waves should be constant and that there is no friction in these waves.

Kinetic and potential energies of a propagating surface gravity wave are calculated for the orbiting fluid particles. These energies are then compared with the respective standard theoretical quantities. Based on earlier work, it is concluded that the two sets of energies will not agree. To substantiate the prediction observations are needed, or a new theory for the depth decay rate of the particle orbits would work. Considerably less algebraic effort is involved in the present analysis than what was done in the past.

Ocean semi-diurnal tides are suggested qualitatively to be more accurately explained by the vertical tide generating forces of the sun and moon, not by the historically standard horizontal components. It is proposed that the sea level elevations created sequentially at the eastern and then western ocean coastal boundaries propagate seaward as shallow water surface gravity waves and interact without mutual disturbance. In that manner two tides per day are created. Horizontal tide forces generate horizontal ocean currents with speeds of 5 mph at the most. Shallow water tide waves move at about 500 mph in open waters and therefore come much closer to being in tune with the sun and moon transiting across the sky.

An earlier fluid mechanical model of a tornado [1], that assumed constant air density, forecast a horizontal rotary wind speed that decreased inversely as the distance increased from the tornado’s central axis. Such a velocity function implies that the force of friction is zero. However, if compressibility of the air is included in the model, and everything else is kept the same, dynamics changes the velocity structure to being constant and independent of the radius. Friction is also zero in this case. Observations are needed to find out what the wind structure is doing.

Within the two poleward wide warm surface currents that cross mid-latitudes on the eastern sides of the North and South Pacific Oceans, a single prediction is made that they have a horizontal velocity shear such that their mean speeds monotonically increase from east to west. This prediction is inspired mainly by a re-examination of the east/west asymmetric observations previously documented: the longitudinal maximum in sea surface temperature in the eastern parts of both oceans at mid-latitudes is located significantly west of the region of maximum mixed layer depths, which occur in the middle of these currents. Also the application of recent dynamical considerations discussed below stimulated the prediction, which include the application of Bernoulli’s law to streamlines in the flows as well as the geostrophic balance in the cross-stream direction. These caused the rethinking of an earlier explanation, which upon reflection now seems less suitable than the present one.

One rainstorm over the North Pacific is studied and compared to a single dynamic fluid flow equation in order to see what measure of agreement might occur. Meteorological data for the rain storm come from the bridge of an oceanographic ship that sailed from San Diego to Japan along 35°N in the spring of 1976 [1]. A single dynamic equation origi-nated from combining Bernoulli’s law with the geostrophic relation and eliminating the pressure between them [2]: horizontal wind shear equals the Coriolis parameter. Wind speed measured on the ship every two hours is used to compute the mean wind shear over the 48 hours of the rain storm. That shear has the right sign and order of magnitude to agree with the Coriolis parameter at 35°N.

An earlier discussion of the summer northward heat flux at ground level at one location in northwest Iowa, based on observing the winds there over 30 consecutive summers, is extended geographically west to the Rocky Mountains and southward. Also the time-scale is stretched well past 30 years. Evidence cited is the eastward increase in plant size and greenery at constant latitude starting at the mountains, which is easily noticed from Texas to North Dakota. One dynamical element, the Coriolis force, acting on the north and south winds over long time periods helps explain the observations. Conservation of mass is another important ingredient.

Epistaxis is one of the most common ear, nose and throat emergencies. The management of epistaxis has evolved significantly in recent years, including the use of nasal cautery and packs. However, a correct treatment requires the knowledge of nasal anatomy, potential risks, and complications of treatment. Epistaxis is often a simple and readily treatable condition, even though a significant bleed may have potentially severe consequences. At present, there are very few guidelines concerning this topic. The current Survey explored the pragmatic approach in managing epistaxis. A questionnaire, including 7 practical questions has been used. The current International Survey on epistaxis management reported a relevant prevalence (21.7%), mainly during childhood and senescence, an important hospitalization rate (11.8%), the common use of anterior packing and electrocoagulation, and the popular prescription of a vitamin supplement and intranasal creams. Twenty years of ship-injection temperatures are averaged annually and plotted against longitude in the western tropics of the North Pacific. Centered at 150E at both 20N and 25N is a large-scale longitudinal maximum in sea surface temperature. The simplest in-terpretation of this permanent feature is that a continuous flow of warm surface water out of the south is required to maintain it, since some heat will escape into the atmosphere in transit. It is concluded that the northward flow is the Kuroshio.

Steady flow past a circular arc wing produces a vertical lift force, which has been calculated before, but it also causes two horizontal reaction forces, equal and opposite, due to Newton’s third law: the accelerated and decelerated flows act back on the top surfaces of the wing. Reaction forces are computed here and compared with the lift force. The magnitude of the two perpendicular forces are equal when the ratio of the maximum thickness of the wing is about 1/5 the length of the wing’s flat bottom surface. An example of an asymmetric wing is discussed and the net reaction force is calculated, which is always directed horizontally away from the top surface of the wing that has the greater mean slope. Based on these results,it is predicted that a surface ship should go more easily through the water if the bow were blunt and the stern pointed, just the opposite configuration of what is commonly found traversing lakes and oceans. Surface gravity waves are argued not to change this conclusion in general.

Combining two satellite cloud photographs of the eastern North Pacific separated by 18 hours in the spring of 1976 with concurrent weather measurements from the bridge of an oceanographic ship leads to the following propositions. Two clockwise curving long bands of clouds were each independently produced by cold air flowing south and pushing the warmer in situ air up and out of the way causing the clouds to form. The cloud bands are oriented roughly northeast/southwest with a separation of about 30 degrees of longitude at mid-latitudes. Curvature of the cloud bands is thought to be due to the Coriolis force acting on the southward flow. These conclusions could become more general if additional observations support them. A significant theoretical addition to an earlier discussion of the subject, regarding the time variability of approximately two days in the hypothesized circulation, is offered here along with a bit of confirming evidence from existing data. An old story with a new ending is presented.

There is a long and wide continuous trough of deep mixed layers connecting the tropical western North Pacific Ocean with the offshore waters of the coast of California. Relatively warm water that is nearly uniform vertically fills the trough, which is concluded here to be a northeastward flow joining the wide warm surface current at mid-latitudes off California documented earlier. Evi-dence for the trough comes from a North Pacific atlas based on very many indi-vidual mixed layer depth data points, taken over a 27-year period, compiled (av-eraged) in monthly mean charts with contours of constant mixed layer depth dis-played. BTs (bathythermographs) were used to record temperature versus depth continuously from which the mixed layer depths were determined. Centerline curves, connecting the deepest mixed layer depths, which approximate the mid-dle of the troughs, are constructed from the atlas and are presented for all twelve months. In going from west to east, these curves bend counterclockwise, gradu-ally most of the way then more markedly near California. The curves for the summer months come closer to California than any of the other ones do, suggest-ing that the warm current itself is nearest to California in summer. Confirmation of the prediction awaits future efforts.

Thirty five day records of wind speed and atmospheric pressure, measured and recorded every two hours, were obtained on a ship that sailed from California to Japan along latitude 35 N in the spring of 1976. Time variations of two days, which can be seen in the raw data of both variables, were brought out more clearly by a two-step smoothing process followed by a subtraction to reduce both the lower and higher frequencies. Comparing the two similarly processed records shows that when the wind speed is relatively strong, the pressure is relatively weak, and vice versa, at the two day time scale. An argument is given that Bernoulli’s law was operating in those weather systems.

Two independent SST atlases are compared for the western tropical North pacific by means of their monthly mean charts. Good agreement is found in three cases involving the 80 F isotherm in the ship-injection temperature atlas and the 25 C isotherm in the BT atlas. From winter to summer the area between the equator and the particular isotherms doubles in size while the SST variation inside the areas is small. Also the average northward speed of the isotherms is the same: about 15 cm/sec. Mixed layer depth charts in the BT atlas strengthen an earlier prediction that in the spring and summer of every year excess absorbed solar radiation is advected out of the tropics toward the sub-polar regions, pushed by a downward slope to the north in sea level set up by thermal expansion in the deep and long surface layer trough described earlier. This is the main result of the paper.

A preliminary attempt is made to address Stommel’s suggestion that the established offshore shear in the Gulf Stream’s surface velocity is large (up to 0.5 f, where f is the Coriolis parameter) and that this needs to be understood better. Using Bernoulli’s law in conjunction with the geostrophic relation leads to the prediction that the shear should equal f. If a certain sea level variation across the Stream is proposed within the warm Stream water, it is qualitatively possible to reduce the shear value below f, but more work in the future will confirm or refute this idea.

During reflection from a solid vertical wall, surface gravity waves produce a steady force on the wall, which was calculated earlier by the momentum method. Recently the linear momentum of these waves has been estimated to be twice as large as deduced from the classical Stokes drift formula. Therefore, the magnitude of the reflected wave force has been doubled here and the algebraic form of the equation is converted to be more understandable physically. Wave force equals fluid density times the square of the particle’s orbital speed and a numerical constant. Observations are welcomed to compare with the theory.
