- Abstract
BACKGROUND: Enteric fever is a serious public health concern. The causative agents, Salmonella enterica serovars Typhi and Paratyphi A, frequently have antimicrobial resistance (AMR), leading to limited treatment options and poorer clinical outcomes. We investigated the genomic epidemiology, resistance mechanisms, and transmission dynamics of these pathogens at three urban sites in Africa and Asia. METHODS: S Typhi and S Paratyphi A bacteria isolated from blood cultures of febrile children and adults at study sites in Dhaka (Bangladesh), Kathmandu (Nepal), and Blantyre (Malawi) during STRATAA surveillance were sequenced. Isolates were charactered in terms of their serotypes, genotypes (according to GenoTyphi and Paratype), molecular determinants of AMR, and population structure. We used phylogenomic analyses incorporating globally representative genomic data from previously published surveillance studies and ancestral state reconstruction to differentiate locally circulating from imported pathogen AMR variants. Clusters of sequences without any single-nucleotide variants in their core genome were identified and used to explore spatiotemporal patterns and transmission dynamics. FINDINGS: We sequenced 731 genomes from isolates obtained during surveillance across the three sites between Oct 1, 2016, and Aug 31, 2019 (24 months in Dhaka and Kathmandu and 34 months in Blantyre). S Paratyphi A was present in Dhaka and Kathmandu but not Blantyre. S Typhi genotype 4.3.1 (H58) was common in all sites, but with different dominant variants (4.3.1.1.EA1 in Blantyre, 4.3.1.1 in Dhaka, and 4.3.1.2 in Kathmandu). Multidrug resistance (ie, resistance to chloramphenicol, co-trimoxazole, and ampicillin) was common in Blantyre (138 [98%] of 141 cases) and Dhaka (143 [32%] of 452), but absent from Kathmandu. Quinolone-resistance mutations were common in Dhaka (451 [>99%] of 452) and Kathmandu (123 [89%] of 138), but not in Blantyre (three [2%] of 141). Azithromycin-resistance mutations

The T2K collaboration: reports evidence for electron neutrino appearance at the atmospheric mass splitting, |\Delta m_{32}^2|=2.4x10^{-3} eV^2. An excess of electron neutrino interactions over background is observed from a muon neutrino beam with a peak energy of 0.6 GeV at the Super-Kamiokande (SK) detector 295 km from the beam's origin. Signal and background predictions are constrained by data from near detectors located 280 m from the neutrino production target. We observe 11 electron neutrino candidate events at the SK detector when a background of 3.3\pm0.4(syst.) events is expected. The background-only hypothesis is rejected with a p-value of 0.0009 (3.1\sigma), and a fit assuming \nu_{\mu}->\nu_e oscillations with sin^2(2\theta_{23})=1, \delta_{CP}=0 and |\Delta m_{32}^2|=2.4x10^{-3} eV^2 yields sin^2(2\theta_{13})=0.088^{+0.049}_{-0.039}(stat.+syst.)., Comment: 43 pages, 41 figures

High Energy Physics - Experiment - Abstract
T2K has performed the first measurement of \nu{\mu} inclusive charged current interactions on carbon at neutrino energies of ~1 GeV where the measurement is reported as a flux-averaged double differential cross section in muon momentum and angle. The flux is predicted by the beam Monte Carlo and external data, including the results from the NA61/SHINE experiment. The data used for this measurement were taken in 2010 and 2011, with a total of 10.8 x 10^{19} protons-on-target. The analysis is performed on 4485 inclusive charged current interaction candidates selected in the most upstream fine-grained scintillator detector of the near detector. The flux-averaged total cross section is <\sigma_CC>_\phi =(6.91 +/- 0.13 (stat) +/- 0.84 (syst)) x10^{-39} cm^2/nucleon for a mean neutrino energy of 0.85 GeV.

High Energy Physics - Experiment - Abstract
The Tokai-to-Kamioka (T2K) experiment studies neutrino oscillations using an off-axis muon neutrino beam with a peak energy of about 0.6 GeV that originates at the J-PARC accelerator facility. Interactions of the neutrinos are observed at near detectors placed at 280 m from the production target and at the far detector -- Super-Kamiokande (SK) -- located 295 km away. The flux prediction is an essential part of the successful prediction of neutrino interaction rates at the T2K detectors and is an important input to T2K neutrino oscillation and cross section measurements. A FLUKA and GEANT3 based simulation models the physical processes involved in the neutrino production, from the interaction of primary beam protons in the T2K target, to the decay of hadrons and muons that produce neutrinos. The simulation uses proton beam monitor measurements as inputs. The modeling of hadronic interactions is re-weighted using thin target hadron production data, including recent charged pion and kaon measurements from the NA61/SHINE experiment. For the first T2K analyses the uncertainties on the flux prediction are evaluated to be below 15% near the flux peak. The uncertainty on the ratio of the flux predictions at the far and near detectors is less than 2% near the flux peak., Comment: 36 pages

We have searched for a bound state of kaon and pion denoted by $X$. The $X$ was conjectured to explain the so-called $\Theta^+$ resonance as a bound state of kaon, pion and nucleon. This model explains almost all properties of the $\Theta^+$, however, the model works only if the $K \pi$ interaction is strongly attractive. It is so strong that it could make a bound state $X$. Here we report a result of the search for the $X$ by using the $K^+ + N \rightarrow X^+ + N$ reaction at P$_K\sim$ 1.2 GeV/c. The $X^+ \rightarrow K^+ \gamma \gamma$ decay produces $K^+$ in momentum region where other processes cannot fill. We observed signature of the $X^+$ with statistical significance of 2 $\sigma$. Production cross section of $X$ with respect to that of $\pi^0$ is 1$\pm$0.5% if we take it as an evidence and 1.5% if we set an upper limit., Comment: 7 pages, 3 figures

High Energy Physics - Experiment - Abstract
We report a measurement of muon-neutrino disappearance in the T2K experiment. The 295-km muon-neutrino beam from Tokai to Kamioka is the first implementation of the off-axis technique in a long-baseline neutrino oscillation experiment. With data corresponding to 1.43 10**20 protons on target, we observe 31 fully-contained single muon-like ring events in Super-Kamiokande, compared with an expectation of 104 +- 14 (syst) events without neutrino oscillations. The best-fit point for two-flavor nu_mu -> nu_tau oscillations is sin**2(2 theta_23) = 0.98 and |\Delta m**2_32| = 2.65 10**-3 eV**2. The boundary of the 90 % confidence region includes the points (sin**2(2 theta_23),|\Delta m**2_32|) = (1.0, 3.1 10**-3 eV**2), (0.84, 2.65 10**-3 eV**2) and (1.0, 2.2 10**-3 eV**2)., Comment: 7 pages, 4 figures

Physics - Instrumentation and Detectors - Abstract
Precise measurement of neutrino beam direction and intensity was achieved based on a new concept with modularized neutrino detectors. INGRID (Interactive Neutrino GRID) is an on-axis near detector for the T2K long baseline neutrino oscillation experiment. INGRID consists of 16 identical modules arranged in horizontal and vertical arrays around the beam center. The module has a sandwich structure of iron target plates and scintillator trackers. INGRID directly monitors the muon neutrino beam profile center and intensity using the number of observed neutrino events in each module. The neutrino beam direction is measured with accuracy better than 0.4 mrad from the measured profile center. The normalized event rate is measured with 4% precision., Comment: 32 pages, 27 figures, submitted to Nucl. Instr. and Meth. A

High Energy Physics - Experiment - Abstract
In early 2010, the Long-Baseline Neutrino Experiment (LBNE) science collaboration initiated a study to investigate the physics potential of the experiment with a broad set of different beam, near- and far-detector configurations. Nine initial topics were identified as scientific areas that motivate construction of a long-baseline neutrino experiment with a very large far detector. We summarize the scientific justification for each topic and the estimated performance for a set of far detector reference configurations. We report also on a study of optimized beam parameters and the physics capability of proposed Near Detector configurations. This document was presented to the collaboration in fall 2010 and updated with minor modifications in early 2011., Comment: Corresponding author R.J.Wilson (Bob.Wilson@colostate.edu); 113 pages, 90 figures

High Energy Physics - Experiment - Abstract
The T2K experiment observes indications of $\nu_\mu\rightarrow \nu_e$ appearance in data accumulated with $1.43\times10^{20}$ protons on target. Six events pass all selection criteria at the far detector. In a three-flavor neutrino oscillation scenario with $|\Delta m_{23}^2|=2.4\times10^{-3}$ eV$^2$, $\sin^2 2\theta_{23}=1$ and $\sin^2 2\theta_{13}=0$, the expected number of such events is 1.5$\pm$0.3(syst.). Under this hypothesis, the probability to observe six or more candidate events is 7$\times10^{-3}$, equivalent to 2.5$\sigma$ significance. At 90% C.L., the data are consistent with 0.03(0.04)$<\sin^2 2\theta_{13}<$ 0.28(0.34) for $\delta_{\rm CP}=0$ and a normal (inverted) hierarchy., Comment: 20 pages, 6 figures, version published in PRL

- Abstract
The current global initiative to end Cholera by 2030 emphasizes the use of oral cholera vaccine (OCV) combined with feasible household Water-Sanitation-Hygiene (WASH) interventions. However, little is known about how improved WASH practices and behaviors and OCV interact to reduce the risk of cholera. We reanalyzed two arms of a cluster-randomized trial in urban Bangladesh, to evaluate the effectiveness of OCV given as a 2-dose regimen. One arm (30 clusters, n = 94,675) was randomized to vaccination of persons aged one year and older with OCV, and the other arm (30 clusters, n = 80,056) to no intervention. We evaluated the prevention of cholera by household WASH, classified at baseline using a previously validated rule, and OCV over 2 years of follow-up. When analyzed by assignment to OCV clusters rather than receipt of OCV, in comparison to persons living in "Not Better WASH" households in the control clusters, reduction of severe cholera (the primary outcome) was similar for persons in "Not Better WASH" households in vaccine clusters (46%, 95% CI:24,62), for persons in "Better WASH" households in the control clusters (48%, 95% CI:25,64), and for persons in "Better WASH" households in the vaccine clusters (48%, 95% CI:16,67). In contrast, when analyzed by actual receipt of a complete OCV regimen, , in comparison to persons in "Not Better WASH" households in the control clusters, protection against severe cholera increased steadily from 39% (95% CI:13,58) in residents of "Better WASH" households in the control clusters to 57% (95% CI:35,72) in vaccinated persons in "Not Better WASH" households to 63% (95% CI:21,83) in vaccinated persons in "Better WASH" households. This analysis suggests that improved household WASH and OCV received may interact to provide greater protection against cholera. However, the divergence between findings related to intent to vaccinate versus those pertaining to actual receipt of OCV underscores the need for further research on this topic.

- Abstract
OBJECTIVE: The present study aims to measure the prevalence of non-disabled frailty and its associated factors among Bangladeshi older adults. METHODS: This cross-sectional study was conducted during September and October 2021 among 1,045 Bangladeshi older adults (≥60 years). Telephone interviews, using a semi-structured questionnaire, were undertaken to collect data on participants' characteristics and level of frailty. The non-disabled frailty was measured using the 'Frail Non-Disabled (FiND)' questionnaire. A multinomial logistic regression model assessed the factors associated with frailty among the participants. RESULTS: Around a quarter of the participants (24.8%) were frail. The multinomial regression analysis showed that older participants aged ≥80 years (RRR = 3.23, 95% CI: 1.41-7.37) were more likely to be frail compared to participants aged 60-69 years. Likewise, the participants living in a large family with ≥4 members (RRR = 1.39, 95% CI: 1.01-1.92) were more likely to be frail compared to those living in smaller families. Also, participants having memory or concentration problems (RRR = 1.56, 95% CI: 1.12-2.17) were more likely to be frail compared to those who were not suffering from these problems. Moreover, participants whose family members were non-responsive to their day-to-day assistance (RRR = 1.47, 95% CI: 1.06-2.03) were more likely to be frail compared to those whose family members were responsive. Furthermore, participants who were feeling lonely (RRR = 1.45, 95% CI: 1.07-1.98) were more likely to be frail than their counterparts who were not feeling lonely. CONCLUSIONS: The findings of the present study suggest developing tailored interventions to address the burden of frailty among the older populations in Bangladesh. In particular, providing long-term care and health promotion activities can be of value in preventing frailty and reducing adverse health outcomes among this vulnerable population group.

Nurses, Public health, health care service - Abstract
The nursing profession is undoubtedly a very important part of health care service. Public Health Nurses use their specialized knowledge, experience, and skill set to initiate life-saving measures, improve and promote the health and well-being. This study was aimed to find out the scopes of utilization of public health specialist nurses, their socio-demographic characteristics, expectations, training status and satisfaction in government health care service. The study was descriptive cross-sectional study. Data was collected though semi structured questionnaire by direct interviewing, telephone interviewing and e-mailing among 127 respondents who had completed their MPH degree from NIPSOM and engaged in government service for minimum 3 years. Among 127 of respondents 81% was female and 19% was male and highest number of respondents 34.6% belongs to the age among 41- 45 years. Majority (78%) of the respondents gave opinion that they had no professional utilization as PHN in the government health sector. About 73.2% of respondents agreed that can getting scope to utilize their MPH educational knowledge in their working area, and most of them (83.9%) opined that they can utilize their knowledge in health education. Maximum 58.3% respondents completed MPH with the hope of becoming nursing teacher. Though 58% of respondents were giving clinical nursing service and among them 40.9% of respondents remained in same position after completing higher education. The 76% of respondents expected from central authority for the specialization of public health nursing post (DPHN) and they also wanted to more working scope in public health sector like upazila level to central level. About 59.8% of respondents didn’t get any training for their professional skill development and 67.7% were not satisfied in their present working position. Without utilizing PHSN properly health sector will remain incomplete. So, the study will help to create an opportunity to think about utilizing public health specialist nurses and for achieving different post need to create in every tire of health.

MDR TB, Adherence to treatment, interruption of treatment - Abstract
The aim of this study is to assess the determinants of multi-drug resistant tuberculosis patients (MDR TB) for treatment adherence. A cross sectional study was carried out for one year in National Institute of Diseases of the Chest and Hospital (NIDCH), Dhaka. One hundred and twenty MDR TB patients were selected for this study. Data were collected by treatment record review and face to face interview by using pre-tested semi-structured questionnaire and analyzed by SPSS version 20. Data revealed that the disease was most prevalent (42.5%) among age group from 21 - 30 years (mean age 30.61 years). Among enrolled patients 90.8% had medication adherence according to treatment regimen. Only 9.2% respondents had interruption of their medication due to side effect of the drugs and delivery disruption. 81.7% had received regular follow up visit. Among patients 87.5% had regular follow up sputum test done according to guideline. Most of the respondents were informed about MDR TB from doctors. It was observed that mass media had no active role in MDR TB. However, the treatment adherence rate of the MDR TB patients was moderately satisfactory. Treatment supporters, oral medication use, previous pulmonary TB disease were identified as determinants of MDR TB treatment adherence. To improve MDR TB treatment adherence, a patient -centered approach should be considered at the programmatic level.

- Abstract
The present study aimed to assess the changes in the prevalence and determinants of self-reported hypertension among older adults during the COVID-19 pandemic in Bangladesh. This repeated cross-sectional study was conducted on two successive occasions (October 2020 and September 2021), overlapping the first and second waves of the COVID-19 pandemic in Bangladesh. The survey was conducted through telephone interviews among Bangladeshi older adults aged 60 years and above. The prevalence of hypertension was measured by asking a question about whether a doctor or health professional told the participants that they have hypertension or high blood pressure and/or whether they are currently using medication to control it. We also collected information on the socio-economic characteristics of the participants, their cognitive ability, and their COVID-19-related attributes. A total of 2077 older adults with a mean age of 66.7 ± 6.4 years participated in the study. The samples were randomly selected on two successive occasions from a pre-established registry developed by the ARCED Foundation. Thus, the sample in the 2021-survey (round two; n = 1045) was not the same as that in the 2020-survey (round one; n = 1031) but both were drawn from the same population. The findings revealed that the prevalence of hypertension significantly increased across the two periods (43.7% versus 56.3%; p = 0.006). The odds of hypertension were 1.34 times more likely in round two than in the round one cohort (AOR 1.34, 95% CI 1.06-1.70). We also found that having formal schooling, poorer memory or concentration, and having had received COVID-19 information were all associated with an increased risk of hypertension in both rounds (p < 0.05). The findings of the present study suggest providing immediate support to ensure proper screening, control, and treatment of hypertension among older adults in Bangladesh.

The burden of Salmonella Typhi shedding in stool and its contribution to transmission in endemic settings is unknown. During passive surveillance S. Typhi shedding was seen during convalescence in 332 bacteremic typhoid patients although none persisted at one-year follow-up. Anti-Vi-IgG titres were measured in age-stratified cohort of serosurveillance participants. Systematic stool sampling of 303 participants with high anti-Vi-IgG titres identified one asymptomatic carrier shedding. These findings suggest ongoing S. Typhi transmission in this setting is more likely to occur from acute convalescent cases although better approaches are needed to identify true chronic carriers in the community to enable typhoid elimination.

- Abstract
BACKGROUND: Enteric fever is a serious public health concern in many low-income and middle-income countries. Numerous data gaps exist concerning the epidemiology of Salmonella enterica serotype Typhi (S Typhi) and Salmonella enterica serotype Paratyphi (S Paratyphi), which are the causative agents of enteric fever. We aimed to determine the burden of enteric fever in three urban sites in Africa and Asia. METHODS: In this multicentre population-based study, we did a demographic census at three urban sites in Africa (Blantyre, Malawi) and Asia (Kathmandu, Nepal and Dhaka, Bangladesh) between June 1, 2016, and Sept 25, 2018. Households were selected randomly from the demographic census. Participants from within the geographical census area presenting to study health-care facilities were approached for recruitment if they had a history of fever for 72 h or more (later changed to >48 h) or temperature of 38·0°C or higher. Facility-based passive surveillance was done between Nov 11, 2016, and Dec 31, 2018, with blood-culture collection for febrile illness. We also did a community-based serological survey to obtain data on Vi-antibody defined infections. We calculated crude incidence for blood-culture-confirmed S Typhi and S Paratyphi infection, and calculated adjusted incidence and seroincidence of S Typhi blood-culture-confirmed infection. FINDINGS: 423 618 individuals were included in the demographic census, contributing 626 219 person-years of observation for febrile illness surveillance. 624 S Typhi and 108 S Paratyphi A isolates were collected from the blood of 12 082 febrile patients. Multidrug resistance was observed in 44% S Typhi isolates and fluoroquinolone resistance in 61% of S Typhi isolates. In Blantyre, the overall crude incidence of blood-culture confirmed S Typhi was 58 cases per 100 000 person-years of observation (95% CI 48-70); the adjusted incidence was 444 cases per 100 000 person-years of observation (95% credible interval [CrI] 347-717). Paratyphoid fever is one of the major causes of morbidity of febrile illnesses in endemic regions. We report a case of high-grade fever in an infant who was positive for Salmonella enterica serovar Paratyphi B (S. Paratyphi B) both in blood and stool cultures. The baby was enrolled in the passive surveillance of multicenter, multicomponent epidemiological study of enteric fever (Strategic Typhoid alliance across Africa and Asia; STRATAA) conducted in a population of 110,000 residents over 2 years in an urban slum, Dhaka, Bangladesh. This is the only patient who was positive for S. Paratyphi B in blood and stool among more than 6,000 febrile ill patients enrolled in the passive surveillance. The report shows the significance of surveillance to identify changes in the epidemiology of enteric fever.

- Abstract
BACKGROUND: Multi-drug resistant typhoid fever remains an enormous public health threat in low and middle-income countries. However, we still lack a detailed understanding of the epidemiology and genomics of S. Typhi in many regions. Here we have undertaken a detailed genomic analysis of typhoid in urban Dhaka, Bangladesh to unravel the population structure and antimicrobial resistance patterns in S. Typhi isolated between 2004-2016. PRINCIPAL FINDINGS: Whole genome sequencing of 202 S. Typhi isolates obtained from three study locations in urban Dhaka revealed a diverse range of S. Typhi genotypes and AMR profiles. The bacterial population within Dhaka were relatively homogenous with little stratification between different healthcare facilities or age groups. We also observed evidence of exchange of Bangladeshi genotypes with neighboring South Asian countries (India, Pakistan and Nepal) suggesting these are circulating throughout the region. This analysis revealed a decline in H58 (genotype 4.3.1) isolates from 2011 onwards, coinciding with a rise in a diverse range of non-H58 genotypes and a simultaneous rise in isolates with reduced susceptibility to fluoroquinolones, potentially reflecting a change in treatment practices. We identified a novel S. Typhi genotype, subclade 3.3.2 (previously defined only to clade level, 3.3), which formed two localized clusters (3.3.2.Bd1 and 3.3.2.Bd2) associated with different mutations in the Quinolone Resistance Determining Region (QRDR) of gene gyrA. SIGNIFICANCE: Our analysis of S. Typhi isolates from urban Dhaka, Bangladesh isolated over a twelve year period identified a diverse range of AMR profiles and genotypes. The observed increase in non-H58 genotypes associated with reduced fluoroquinolone susceptibility may reflect a change in treatment practice in this region and highlights the importance of continued molecular surveillance to monitor the ongoing evolution of AMR in Dhaka. We have defined new genotypes and lineages o

- Abstract
Ever rising prevalence of Cardiovascular Diseases (CVD) is a major challenge for the health sector in Bangladesh. This study aimed to explore the prevalence of CVD and sociodemographic and lifestyle factors associated with it in Bangladesh. The data were collected through a cross-sectional survey following a two-stage cluster random sampling procedure. The present analysis was performed among 12,338 respondents aged ≥35 years, selected from rural areas and urban slums. Information was gathered using a structured questionnaire, whereas measurements were taken using standardized procedures. Logistic regression with exchangeable correlation structure among clusters was executed to explore the association. About 30% of participants had hypertension, 5% diabetes, 20% obesity; 77% were either smokers or consumed smokeless tobacco, and 28% were physically inactive. The prevalence of CVD was 4.5% (stroke: 1.8% and heart diseases: 3.2%). After adjusting for potential confounders, hypertension, diabetes, body mass index, extra salt intake, daily sleep, tiredness, age, gender, occupation, administrative division, and wealth quintile were found to be significantly associated with CVD. The study highlighted that the prevalence of CVD is high in Bangladesh, and its associated risk factors such as hypertension and diabetes are on the rise, especially in the older population, women, and high-income groups. Therefore, immediate public health intervention is warranted to address the issue.

- Abstract
OBJECTIVE: Childhood stunting remains a major public health concern in Bangladesh. To accelerate the reduction rate of stunting, special focus is required during the first 23 months of a child's life when the bulk of growth takes place. Therefore the present study explored individual-, maternal- and household-level factors associated with stunting among children under 2 years of age in Bangladesh. DESIGN: Data were collected through a nationwide cross-sectional survey conducted between October 2015 and January 2016. A two-stage cluster random sampling procedure was applied to select 11 428 households. In the first stage, 210 enumerations areas (EA) were selected with probability proportional to EA size (180 EA from rural areas, thirty EA from urban slums). In the second stage, an average of fifty-four households were selected from each EA through systematic random sampling. SETTING: Rural areas and urban slums of Bangladesh.ParticipantsA total of 6539 children aged 0-23 months. RESULTS: Overall, 29·9 % of the children were stunted. After adjusting for all potential confounders in the modified Poisson regression model, child's gender, birth weight (individual level), maternal education, age at first pregnancy, nutrition (maternal level), administrative division, place of residence, socio-economic status, food security status, access to sanitary latrine and toilet hygiene condition (household level) were significantly associated with stunting. CONCLUSIONS: The study identified a number of potentially addressable multilevel risk factors for stunting among young children in Bangladesh that should be addressed through comprehensive multicomponent interventions.

- Abstract
BACKGROUND: Evidence suggests that daily supplementation of 1500 to 2000 mg of calcium during pregnancy reduces pregnancy-induced hypertension (PIH). However, the evidence on the efficacy of low-dose calcium supplementation on PIH is limited. This paper assesses the longitudinal correlation between low-dose calcium intake (500 mg daily) and change in blood pressure during pregnancy among a homogeneous population in terms of hypertension and pre-eclampsia. METHODS: The study followed a retrospective cohort study design, and was carried out among 11,387 pregnant women from 10 rural upazilas (sub-districts) of Bangladesh where maternal nutrition initiative (MNI), implemented by Building Resources Across Communities (BRAC), was ongoing. The modified Poisson regression model was used to estimate the association (risk ratio) between consumption of calcium tablets and PIH. RESULTS: The present research found that women who consumed 500 mg/d calcium tablets for more than 6 months during their pregnancy had a 45% lower risk of developing hypertension compared to those who consumed less calcium (RR = 0.55, 95% CI = 0.33-0.93). CONCLUSIONS: Daily supplementation of 500 mg oral calcium during pregnancy for at least 180 tablets is associated with a considerably reduced risk of PIH, but this study is unable to confirm whether this association is causal. The causal relationship needs to be confirmed through a large scale randomized controlled trial.

Introduction Invasive infections caused by Salmonella enterica serovar Typhi and Paratyphi A are estimated to account for 12–27 million febrile illness episodes worldwide annually. Determining the true burden of typhoidal Salmonellae infections is hindered by lack of population-based studies and adequate laboratory diagnostics.\ud \ud The Strategic Typhoid alliance across Africa and Asia study takes a systematic approach to measuring the age-stratified burden of clinical and subclinical disease caused by typhoidal Salmonellae infections at three high-incidence urban sites in Africa and Asia. We aim to explore the natural history of Salmonella transmission in endemic settings, addressing key uncertainties relating to the epidemiology of enteric fever identified through mathematical models, and enabling optimisation of vaccine strategies.\ud \ud Methods/design Using census-defined denominator populations of ≥100 000 individuals at sites in Malawi, Bangladesh and Nepal, the primary outcome is to characterise the burden of enteric fever in these populations over a 24-month period. - Abstract
BACKGROUND: Anemia is a significant wide spread public health threat especially among the adolescent girls who are more vulnerable towards low level of hemoglobin particularly of low and middle income countries (LMICs). We investigated the prevalence of anemia among the adolescent girls (10-19 years) in Bangladesh and its socio-demographics distribution. METHODS: We collected data digitally in ODK platform from a sub-sample of a nationwide cross-sectional survey of 1314 adolescent girls in 2015. Capillary blood hemoglobin level was estimated using HemoCue®; anthropometric measurements through standardized procedure and details socio-demographic information were captured and analyzed. Malnutrition was defined as BMI-for-age Z-score below -2SD (BAZ < -2SD), measured in WHO-AnthroPlus. Univariate analysis followed by multiple logistic regression were performed to examine the association between socio-demographic variables and anemia, while controlling the effect of potential confounding variables. RESULTS: Overall, 51.6% girls were suffering from any form of anemia (non-pregnant-Hb < 12 g/dl; pregnant-Hb < 11 g/dl) while 46% were mildly (non-pregnant-Hb: 10-11.9 g/dl; pregnant-Hb: 10-10.9 g/dl) and 5.4% were moderately (Hb: 7-9.9 g/dl) anemic while only 0.2% were severely anemic. After controlling for relevant covariates in multiple logistic regression model, malnutrition (AOR: 1.42, 95% CI = 1.0-2.10, p-value = 0.083), non-pregnancy (AOR: 6.10, 95% CI = 2.70-13.78, p-value < 0.001), and households with bottom wealth quintile (AOR: 1.54, 95% CI = 1.03-2.30, p-value = 0.037) were identified as significant risk factors of anemia among adolescent girls of Bangladesh. CONCLUSIONS: Higher number of adolescent girls are still suffering from anemia in Bangladesh and non-pregnant adolescent girls contributed the most. Immediate, long term and sustainable public health intervention would require to combat the situation.

This paper contains a multiband low profile Modified Inverted-FL (IFL) strip antenna for 5.2/5.8 GHz WLAN and 5.5 GHz WiMAX operations in a laptop computer. The simulation results are analyzed by means of Numerical Electromagnetic Code-2 (4NEC2) software. This antenna covers -10 dB return loss bandwidth of 900 MHz (5150 MHz ∼6050 MHz). The antenna provides very high peak gains with lower gain variation in its operating bands. The antenna has suitable omnidirectional radiation characteristics. The dimension of the antenna is 16×18.5 mm2, which is very compact in size. So, this low profile antenna is promising to be embedded within the different portable devices employing WiMAX and WLAN applications. © 2012 IEEE.

To increase the crop production to its potential, it is needed first to know the upper limit of dry-matter production. Solar radiation data in the region of Comilla district was analyzed to estimate the potential crop production. The region received annual atmospheric radiation of 12,549 MJ/m2. Of this, 30 % reached the earth's surface as global radiation. Monthly mean daily global radiation varied from 8.4 MJ/m2/day in December to 15.2 MJ/m2/day in April. The annual global radiation received was 3792 MJ/m2. Based on 6.6 % photosynthetic efficiency, the available radiation could produce 117.4 t/ha drymatter in three crop seasons under optimum conditions of nutrition and other growth factors. Actual maximum dry-matter production during these three seasons was 32.6 t/ha (only 28 % of the solar potential). Hence, there is scope to further increase in crop production (by 260 %) in this area. Breading of crop varieties, selection of suitable crop rotations and agronomic practices can boost up productivity to potential level through utilization of the available solar energy.

The 'Jamuna Bridge' is a success in terms of implementation of a dream. It portends an era of socioeconomic transformation, linking the more marginalized North with the rest portion of the country. It was opened to traffic on 23 June 1998. The present study was mainly designed to assess the socioeconomic imp-act of Jamuna Bridge on farming community. The study was conducted in three villages of Saidabad Union under* Sadar Upazila in Sirajganj district of Bangladesh. Farm survey method was followed in the study. Most of the members (58.61 per cent) of the selected households were found in age group between 16.01 and 45 years. Average family size of the study area was 5.93 and it was the highest in the case of small farms. The area had a very impressive literacy profile; about 71 per cent of the family members were literate. Agriculture was found to be the main occupation of the majority (79.50 per cent) of the people in the study area, followed by business (6.32 per cent) and service (4.89 per cent). Household income was remarkably changed after the construction of Jamuna Bridge. It was found that the highest increase in income accrued in the case of medium farms. Although-income from both farm and off-farm sources was found to have increased, the share of farm income to the total income decreased by 2.20 per cent and share of off-farm income increased by 6.28 per cent. Findings of the Cobb-Douglas production function suggested that all the selected variables, viz., farm size, family size, average age of the members, average years of schooling, and income from off-farm activities except proportion of female members in the household had positive and significant impact on household income of the selected farmers. Construction of Jamuna Multipurpose Bridge has created numerous employment opportunities, especially in the off-farm sector and thus found to have contributed to alleviate poverty in the Northern region of Bangladesh as well as in the study area. Cropping patterns of the study area has changed to a considerable extent due to implementation of JMBP. It was observed that the farmers preferred crops with lesser risk higher economic returns. There was a notable positive change in vegetables production in the study area. Finally, based on the findings of the study, some policy recommendations such as establishment of agroprocessing industries, partial mechanization for farm operations, price stabilization for agricultural product, provision of credit to the farmers with soft conditions and providing self employment training to the young people, etc., were made for the sound economic development of the study area and the country.

- Abstract
The normalized event rate is measured with 4% precision.

- Abstract
In early 2010, the Long-Baseline Neutrino Experiment (LBNE) science collaboration initiated a study to investigate the physics potential of the experiment with a broad set of different beam, near- and far-detector configurations. Nine initial topics were identified as scientific areas that motivate construction of a long-baseline neutrino experiment with a very large far detector. We summarize the scientific justification for each topic and the estimated performance for a set of far detector reference configurations. We report also on a study of optimized beam parameters and the physics capability of proposed Near Detector configurations.
