Abstract Background Type 2 diabetes mellitus, one of the most prevalent metabolic disorders worldwide, is closely linked with an enhanced risk of atherosclerosis. However, the molecular mechanism of this linkage is not still clear. Genetic variations in the mTOR gene may increase the susceptibility of individuals to these diseases. Methods One hundred nine diabetic patients and 375 healthy subjects participated in this study. mTOR Single Nucleotide Polymorphism (SNP) rs2295080 was determined using Polymerase Chain Reaction-Restriction Fragment Length Polymorphism (PCR-RFLP). Results Comparison of genotypic, allelic, and genotypic combination frequencies between cases and controls revealed no significant result. Nevertheless, the frequency of rs2295080 GT + TT genotype was significantly more in diabetic women with atherosclerosis compared with those without atherosclerosis (p = 0.047). Besides, the rs2295080 G allele was more frequently detected in diabetic women without atherosclerosis compared to those with atherosclerosis (p = 0.046). Conclusion The rs2295080 GT + TT genotype predisposes Iranian diabetic women to atherosclerosis, while the rs2295080 G allele protects them against atherosclerosis. However, additional experiments using larger sample sizes are needed to verify this result.

Abstract Background More than 70% of thalassemia’s major mortality is due to the cardiac complications of this syndrome, mostly consequent to myocardial Iron overload; therefore, evaluation of such complications is of utmost importance. T2*MRI is used to assess hepatic and myocardial Iron load in thalassemia patients, which is not always available. Signal-Averaged Electrocardiography is a rather easy method of evaluating major thalassemia patients regarding their risk for sudden cardiac death. Methods and materials In this cross-sectional study, 48 patients with thalassemia major underwent evaluation with electrocardiography, signal-averaged electrocardiography, echocardiography, T2*MRI, and ferritin level. The association of the existence of ventricular late potentials in SAECG and other cardiac variables was evaluated. Moreover, the association between myocardial and hepatic Iron load and cardiac characteristics was assessed. Results 48 patients with a mean age of 30.31 ± 7.22 years old entered the study. 27 (56.3%) of the patients had ventricular late potentials, which were associated with myocardial dry Iron weight (P = 0.011). Nonspecific ST-T changes and premature atrial and ventricular contractions were seen more frequently in patients with late potentials (P = 0.002, 0.031, and 0.031, respectively). Patients with higher myocardial and hepatic Iron loads had longer QTc in their 12-lead surface electrocardiograms. Conclusion Patients with ventricular late potentials assessed by SAECG had a higher myocardial Iron load. Higher myocardial Iron load is associated with higher cardiac complications in patients with beta-thalassemia major; therefore, SAECG can be used as a screening test for cardiac complications in beta-thalassemia major patients.

Background and Objectives: Psychology needs to study ethics as an important dimension of human personality. The present study aims to critique the moral foundations of Freud's theory based on Islamic ethics. Methods: This is a review study. We used library and internet studies, and the resources related to the subject were extracted and studied. Results: According to Freud, ethics results from internalizing parental and social values and relative, masculine, illogical, and irrational matters. A human being is morally neutral. The criterion for moral behavior is to follow the moral standards of parents or the community. On the contrary, according to Islam, man is not morally neutral but has a spiritual morality. Part of morality is introduced by nature and intellect, and the other is introduced by religious teachings. Islamic morality is absolute and is not subject to the tastes of individuals or social conventions, and its audience is a kind of human being. Proximity to God is the criterion for evaluating moral behaviors, and to achieve this goal, both the appearance of the behavior and the motivation of the doer must be good and divine. Conclusion: The most important critique of Freud’s moral view from the perspective of Islamic morality is to ignore the innate moral capacities of man, consider moral matters as irrational, relativize moral propositions, limit morality to masculinity, and pay attention to the appearance of moral behaviors and ignore the role of intention in the formation of moral behaviors.

Right ventricular (RV) aneurysm is a very rare ventricular lesion. An aneurysm is formed mainly as a complication of myocardial infarction (MI). As an RV aneurysm is a potentially life-threatening occurrence, its appropriate diagnosis is of great significance. However, right-sided heart diseases, especially RV aneurysms, have been neglected for years. Recent studies in the literature have elucidated the role of the right side of the heart in patients’ prognosis and response to treatment. However, RV aneurysm has been scarcely investigated, and most of the attention has been given to the left ventricular aneurysm in patients with ischemic heart diseases (IHD). Herein, we investigated a total of 625 patients with IHD referred for two-dimensional transthoracic echocardiography (2D TTE), among whom 18 were diagnosed with RV aneurysms through precise examination of several TTE views. The characteristics of these cases, including demographics, medical history, and results of cardiac tests (which the patients underwent previously), were recorded and presented. This study emphasized the importance of performing a meticulous 2D TTE evaluation and a thorough examination of different views by an expert echocardiographer, with special attention to the presence of an RV aneurysm in a patient suffering from IHD who presented either with acute coronary syndrome, including MI, or chronic IHD. The scarcity of information, especially in terms of complications and the most appropriate diagnostic methods, calls for further studies in this regard.

Background and Aim: In recent years, special attention has been paid to the use of complementary drugs in the treatment of breast cancer. Previous studies have shown that lavender (Lavandula angustifolia) has antiproliferative properties. In the present study, the possible antiproliferative effects of methanolic extract of lavender in MCF-7 breast cancer cell line were investigated. Materials and Methods: In this experimental study, MCF-7 breast cancer cell line was treated with different concentrations of methanol extract of lavender. Cell survival was assessed using MTT assay. The cell cycle was analyzed using flow cytometry. The ability of breast cancer cells to form a colony after treatment with plant extract was also investigated. The main components of the methanol extract were identified by GC/MS method. The T-test, ANOVA and Tukey statistical methods were used for data analysis. Results: The results showed that the methanolic extract has significant cytotoxicity on MCF-7 cells, the G0/G1 phase arrest after treatment with methanolic extract in the breast cancer cell line increased significantly. The main contents of the MetOH extract were determined as Coumarin (59.44%), Tricosane (15.22%), 7-methoxy Coumarin (12.69%), and 2-Furancarboxaldehyde (6.7%). Conclusion: In conclusion, according to the results of this study, the lavender methanolic extract has a cytotoxic effect on MCF-7 cells; therefore, lavender can be considered as a potential therapeutic supplement in future studies of breast cancer.

Background and aim: Adjunctive therapy can increase periodontal treatments' success, and to date, various drugs had been suggested. This study aimed to test the efficacy of a new formulation of locally delivered- slow-releasing 10% doxycycline (DOX) gel as a thermogelating system by histologic evaluation in rats with experimental periodontitis. Materials and methods: Periodontal disease was induced with a ligature in maxillary left and right upper first molars of twenty male Sprague-Dawley rats in a seven-day duration. Left molars were treated with DOX gel while the right ones received vehicle drug in the subgingival area. Animals were randomly divided into two groups (each group contained ten rats). One group received the drug only once, while the second one was treated with twice the drug's application. After seven days, animals were killed and prepared for histologic evaluation. Background. Previous animal studies have shown a protective effect of 5-phosphodiesterase inhibitors on cancer therapeutics-related cardiac dysfunction (CTRCD) of anthracyclines. Aim. The aim of this study was to evaluate the clinical effect of sildenafil on the primary prevention of CTRCD in human. Materials and Methods. In this randomized double-blind clinical trial, the primary end point was efficacy in preventing the reduction of left ventricular ejection fraction (LVEF). The intervention group patients received sildenafil at a dose of 25 milligrams twice a day before starting the chemotherapeutic regimen, and the control group received placebo. All the patients at baseline and after the 6-month follow-up underwent 4D and speckle-tracking echocardiography and cardiac MRI, accompanied by hs-troponin I and NT-Pro-BNP measurement. Results. Sixty patients were enrolled in this study, and data from 52 patients (24 patients in the intervention group and 28 patients in the control group) were used in the final analysis. Our findings showed that in the intervention and control groups, LVEF was dropped from 61.28 ± 7.36 to 51.57 ± 7.67 (difference (D) = −9.71 ± 11.95, p=0.003) and from 57.9 ± 7.29 to 50.2 ± 7.02% (D = −7.7 ± 5.93; p=0.001), respectively (between-group difference = −2.01%, p=0.26). CTRCD was detected in 11 patients in the control group (42.8%) and 10 in the intervention group (41.6%, p=0.51). Conclusion. Consumption of sildenafil for primary prevention of anthracycline-induced cardiac toxicity seems to be unbeneficial. This trial is registered with IRCT20180506039554N1.

This study was conducted to evaluate the effects of adding cinnamon nanoparticles (NPs), Zinc oxide (ZnO) nanoparticles (NPs), and Copper oxide (CuO) NPs on the antibacterial property of a luting and lining glass ionomer cement (GIC) that was used for the cementation of orthodontic bands to the tooth. Cinnamon NPs, ZnO NPs, and CuO NPs were added into a luting and lining GIC in weight percentages of 1%, 2%, and 4%, respectively while a non-modified GIC was considered as the control group. Agar disc diffusion test was applied to assess the antimicrobial property of samples against Streptococcus mutans (S. mutans). The cytotoxicity of the nanoparticles was examined through the MTT assay for gingival fibroblasts. Data showed that GIC containing cinnamon and ZnO NPs displayed a larger inhibition zone diameter and greater antibacterial activity against S. mutans than CuO NPs. Meanwhile, there were no significant differences in the inhibition zone diameter of cinnamon NPs and ZnO NPs. The cytotoxicity assessment revealed the lower cytotoxicity of cinnamon NPs and the higher cytotoxicity of CuO NPs while the cytotoxicity of ZnO NPs was observed to be higher than cinnamon NPs and lower than CuO NPs. GIC containing cinnamon NPs exhibited noticeable antibacterial activity against S. mutans and cinnamon NPs revealed less cytotoxicity and it is can be labeled as a favorable option for further assessment to be applied in fixed orthodontic treatments for the cementation of bands to teeth.

The major purpose of this study was to examine the construct validity of English Language Teachers Competency Test (ELT-CT) designed by Pishghadam, Baghaei and Shahriari (2010). In this study, Exploratory Factor Analysis (EFA) was used to examine the construct validity of the test. The results have shown that the underlying factors of the test are what the test-designer claims to be. The results have revealed that ten factors represent the underlying structure of ELT-CT. Finally, statistical results were discussed, and implications were provided in the context of English language teaching.

Background: Human leukocyte antigen (HLA) complex is a gene family involved in antigen presentation associated with protection or susceptibility to inflammatory, infectious and autoimmune diseases. Atherosclerosis is a chronic inflammatory disease in which HLA molecules play a role in the initiation and development of the disease through presentation of self or foreign antigens to T cells. Objective: To investigate the association of HLA-DRB1 alleles with atherosclerosis in a sample of southwestern Iranians. Methods: We performed an analytical cross-sectional study involving 96 patients with atherosclerosis and 72 controls. HLA-DRB1 genotyping was performed by PCR-SSP method. Results: We observed a significantly lower frequency of DRB1*01 in patients with coronary artery atherosclerosis than in controls (4.68% vs. 13.1, P=0.0052, OR=3.09, CI 95%: 1.35-7.05). However, this allele showed a positive association with high blood pressure (P=0.009) in patients. Furthermore, DRB1*16 allele was associated with hyperlipidemia (P=0.008) in patients. Conclusion: Our results demonstrated that DRB1*01 may be a protective allele against atherosclerosis in individuals who live in southwest of Iran. The mechanism of this protection needs further investigation.

The relationship between high levels of anti-Varicella Zoster Virus (VZV) IgG in cerebrospinal fluid (CSF) and cerebrovascular atherosclerosis commends a possible similar association in other vessels. We aimed to investigate the association of VZV-seropositivity with coronary artery atherosclerosis. We recruited 88 newly diagnosed patients with more than 50% stenosis in at least one of the main coronary arteries. As the control group, 99 age-matched individuals with normal/insignificant coronary artery findings were included. Clinical, paraclinical, and demographical data were gathered at the time of sampling. High‐sensitivity C‐reactive protein (hsCRP) levels were measured by nephelometry. VZV-seropositivity was determined by measuring of anti-VZV IgG level in plasma. Multivariable logistic regression was used to evaluate the correlation of data with coronary vascular atherosclerosis. The frequency of VZV-seropositivity was significantly higher in the atherosclerosis group compared to the controls (OR=1.88; 95%CI=1.03-3.44). The plasma levels of anti-VZV IgG were significantly higher in patients with atherosclerosis (Median=2.70, IQR=1.53-4.30 AU/mL) than in the controls (Median=2.10, IQR=1.70-3.10 AU/mL, p=0.034). Philosophical hermeneutics which means ontology of understanding delves into the ontological foundations of understanding and its existential conditions from philosophical perspective. Gadamer as a representative of this school of thought developed hermeneutics from the realm of methodology into ontology. The present study entitled “A Comparative Study of the Hermeneutics of Mulla Sadra and Gadamer” revealed that many hermeneutic features of Gadamer such as essence of understanding, ontology of text and interpreter, multiplicity of understandings, and the commentator’s presumptions are evident in the perception of Mulla Sadra from the Holy Quran. However, their hermeneutics are basically different in each of the above points. More importantly, Mulla Sadra believes that the Quran is determined by God so he does not believe in relativism which is the outcome of Gadamer’s hermeneutics. In fact, Mulla Sadra believes in the cautious integration of meaning horizon between the commentator and the text in accordance with the author’s will, so he regards accurate understanding of the texts achievable relying on the key role of the commentator on the one hand, and the author’s knowledge on the other hand.

Statement of the Problem: A great challenge in periodontal therapy is the regeneration enhancement of osseous defects through applying osteoinductive materials. Demineralized freeze-dried bone allograft (DFDBA) has already been introduced as an allograft with osteoconductive and variable osteoinductive properties. Calcium hydroxide [Ca(OH)2] is an available well-known material in dentistry, which induces hard tissue formation. Purpose: This study evaluated the efficiency of combination of DFDBA and Ca(OH)2 in improving the quality of osteoinduction of DFDBA. Materials and Method: Human bone marrow-derived mesenchymal stem cells were taken from volunteers’ iliac crest. Cell proliferation was determined by MTT test at 18, 24 and 48 hours post-culture in 10 groups. The employed material were 0.5, 1.0 mg/ml Ca(OH)2 in two forms of suspension and pH-adjusted solution, 10mg/ml DFDBA per se and in combination with 0.5 and 1.0 mg/ml Ca(OH)2. Mineralization was assessed by Alizarin red staining in 10 mg/ml DFDBA, DFDBA+ 0.5 and 1 mg/ml Ca(OH)2 in solution and suspension forms. The data were statistically analyzed by using one-way ANOVA and Tukey’s post-hoc test (p< 0.05). Results: The pH-adjusted solutions exhibited better cell proliferation compared with the suspension groups. The combination of 0.5mg/ml Ca(OH)2 solution and DFDBA increased the cell proliferation and mineralization compared with DFDBA per se (p= 0.033). Conclusion: The combination of Ca(OH)2 with DFDBA improved the osteoinductivity of DFDBA.

Background: Exercise is a physiologic stress that helps the physicians to clarify the presence or absence of cardiovascular disease which may be obscure at rest. Although it is sensitive, its specificity is affected by several parameters, such as some metabolic conditions, some structural heart diseases, and some baseline electrocardiogram abnormalities. Currently, the relationship between coronary dominance and accuracy of EET is not examined. Therefore, this study was conducted to determine the potential impact of coronary dominance on the accuracy of EET. Methods: In this retrospective study, data were gathered from 720 patients from four medical centers. The pattern of dominancy was determined, and the coronary dominance pattern of the patients who had normal angiograms despite abnormal EETs was compared to that from all the patients. Results: Among the patients who had a normal angiogram despite an abnormal EET, 27% were left dominant while the frequency of left dominancy in the whole population of the study was only 10.9% (P = 0.013). There were no significant differences in baseline characteristics, such as age and sex, between the two studied groups. Conclusion: The results indicated that the presence of left dominance in patients who had normal angiograms despite an abnormal EET was significantly higher than general population. Therefore, left dominance may be considered a confounding factor for EET, producing false positive results.

Background: In the nursing profession, it is apparent that the definition of caring differs between various perspectives. This article compares the difference of caring in Islamic with the Parse and Watson theories. Materials and Methods: In this study, we use concept analyses of Walker–Avants and compare research methods. Material used is all Islamic documents. Results: According to Islamic documents, there are four major types of caring, namely, (1) God taking care of humans, (2) Humans taking care of themselves, (3) Other humans taking care of humans, and (4) The universe taking care of humans and vice versa. God caring for humans affects the three other types of caring. All three definitions of caring have humanistic and holistic view. According to Watson's and Parse's definition, the development of the caring theory is based on the person's experiences that result from human interactions with, and experiences of, their environment. In Islamic definition, although the caring process is affected by environmental experiences and interactions, human not developed only base the effect of environment; rather, it is developed on the basis of human nature and divine commands. God taking care of humans is specific to Islamic perspective and is not found in other definitions. Islamic perspective maintains that God is the creator of humanity and is in charge of guiding humans. A superior form of human can always be discovered. Conclusions: Thus, nursing implementation in Muslims must be done based on Islamic commands, and Islamic commands are superior to human experiences. However, Islamic commands interpreted with human wisdom and thought can be striving toward excellence.

Background: Atherosclerosis is a chronic inflammatory disease affecting large and medium arteries. CD4+ T cells are known to play a role in the progression of the disease. CD4+CD25+Foxp3+ natural Treg (nTreg) cells seem to have a protective role in the disease and their reduction in acute coronary syndrome is recently shown. Objective: To investigate the frequency of nTreg subsets in the peripheral blood of patients with atherosclerosis. Methods: Confirmation of atherosclerosis was done by angiography and 15 ml heparinized blood was obtained from each of the 13 nondiabetic patients and 13 non-diabetic, non-smoker individuals with normal/insignificant coronary artery disease which was also confirmed by angiography. Lipid profiles of the patients and controls were measured at the time of sampling. Mononuclear cells were used for both RNA extraction and immunophenotyping by real-time PCR and flowcytometry techniques, respectively. Results: In natural Treg subsets, the frequency of CD4+CD45RO-CD25+Foxp3lo T-cells (resting nTregs) was greater in controls than patients (p=0.02). The frequency of CD4+CD45RO+CD25hiFoxp3hi T-cells (activated nTregs) was significantly higher in controls compared with patients (p=0.02). However, the frequency of CD4+CD25+CD45RO+Foxp3- T-cells (effector/memory) increased in patients compared with controls (p=0.01). Both the MFI and gene expression of Foxp3 were higher in control group than in patients (p=0.015 and p=0.017, respectively). Moreover, the TGF-β gene expression showed a decrease in the peripheral blood mononuclear cells of patients compared with controls (p=0.03). Conclusion: Decrease in both subsets of resting and activated nTregs along with a decrease in the expression of Foxp3 and TGF-β genes in patients with atherosclerosis suggests phenotypic changes in these subsets, which may as well be correlated with a more inflammatory profile in their lymphocytes.

The next generation of Far-infrared and X-ray space observatories will require detector arrays with thousands of transition edge sensor (TES) pixel. It is extremely important to have a tool that is able to characterize all the pixels and that can give a clear picture of the performance of the devices. In particular, we refer to those aspects that can affect the global energy resolution of the array: logarithmic resistance sensitivity with respect to temperature and current (α and β parameters, respectively), uniformity of the TESs and the correct understanding of the detector thermal model. Complex impedance measurement of a TES is the only technique that can give all this information at once, but it has been established only for a single pixel under DC bias. We have developed a complex impedance measurement method for TESs that are AC biased since we are using a MHz frequency domain multiplexing (FDM) system to readout an array. The FDM readout demands for some modifications to the complex-impedance technique and extra considerations, e.g. how to modulate a small fraction of the bias carrier frequencies in order to get a proper excitation current through the TESs and how to perform an accurate demodulation and recombination of the output signals. Also, it requires careful calibration to remove the presence of parasitic impedances in the entire readout system. We perform a complete set of AC impedance measurements for different X-ray TES microcalorimeters based on superconducting TiAu bilayers with or without normal metal Au bar structures. We discuss the statistical analysis of the residual between impedance data and fitting model to determine the proper calorimeter thermal model for our detectors. Extracted parameters are used to improve our understanding of the differences and capabilities among the detectors and additionally the quality of the array. Moreover, we use the results to compare the calculated noise spectra with the measured data.

Herpes simplex viruses (HSV) are one of the most important groups of human pathogenic viruses. The prominent characteristic of this virus is latency and the ability to reactivate. One of the possible factors for reactivation of this virus is dental procedures. The aim of this study was to evaluate the salivary level of Herpes simplex viruses, before and after periodontal (crown lengthening) surgery and its relation with age and sex. 30 HSV seropositive patients, who needed the crown lengthening surgery and accepted to cooperate in this research, were included as experimental group of this study. Unstimulated Saliva samples of the patients were collected in 1.5 ml micro-tubes, before and 24 h after the surgery, and were analyzed by Premix EX taq probe qpcr, using PCR real-time method. No significant statistical differences were observed in the salivary level of HSV before and after crown lengthening procedure (p = 0.18). However, the level of HSV in saliva after surgery was significantly higher than its level before surgery in women as compared to men (p = 0.003). The differences in virus level did not have any significant relationship with patients' age (p = 0.9). It seems that periodontal (crown lengthening) surgery does not affect the level of HSV in saliva yet, but it could be one of the stimulators of increased HSV level after surgery in women as compared to men; but age does not play an important role in changes of level of virus before and after the surgery.
