5. Evaluation and application of American College of Radiology Thyroid Imaging Reporting and Data System for improved malignancy detection in paediatric thyroid nodules.

Abstract

Objective: The American College of Radiology Thyroid Imaging Reporting and Data System (TI‐RADS) is a widely used method for the management of adult thyroid nodules. However, its use in paediatric patients is controversial because adult fine needle aspiration biopsy (FNAB) recommendations may lead to delayed diagnoses of cancer in children. The objectives of this study were to evaluate the performance of TI‐RADS in paediatric thyroid nodules and to tailor FNAB recommendations for children. Methods: Consecutive surgically resected paediatric thyroid nodules from two tertiary care centres between 2003 and 2021 were reviewed. Ultrasounds were blindly scored by radiologists according to TI‐RADS. Management recommendations based on TI‐RADS were evaluated. Various modelling methodologies were used to determine the optimal cutoff for FNAB in children. Results: Of the 96 patients, 79 (82%) were female and the median age at surgery was 16.1 years. Fifty (52%) nodules were malignant on surgical pathology. The area under the receiver operating characteristic curve of TI‐RADS for predicting malignancy was 0.78. Adult TI‐RADS recommendations would have resulted in 4% of cancerous nodules being lost to follow‐up. Modifications to TI‐RADS (FNAB of all TR3 nodules ≥1.5 cm, FNAB of TR4 and TR5 nodules ≥0.5 cm, surveillance of nodules ≥1 cm, consider surgery for nodules >4 cm) reduced this missed malignancy rate to 0%. Conclusions: TI‐RADS can risk‐stratify paediatric thyroid nodules. However, the system requires modifications to reduce the missed malignancy rate in paediatric thyroid nodules. Abstract

Sustainable wastewater management is a significant issue to overcome water shortage problem. In this context, it is very important to manage industrial wastewater properly. The textile industry is one of the most water consuming industry and huge amount of colored toxic wastewater is produced during the production of textile products. One of the most environmentally damaging stages in textile production is dyeing process. In some studies, it is stated that textile finishing and dyeing processes cause 20% of all freshwater pollution. Various treatment methods including physicochemical, chemical, and biological techniques are used for the color removal in the textile wastewater. Adsorption is one of the most effective methods of color removal and natural biopolymers like chitosan have great attractive as eco-friendly and low-cost sorbents. Within the scope of the study, an imidazolium based ionic liquid was synthesized and the sorbent obtained by interacting this synthesized ionic liquid with chitosan was used for the removal of Reactive Black 5 dye. Ionic liquids are known as green solvents and their applications in wastewater treatment are quite new. In this study, chitosan saturated with 0.25 g ionic liquid, which is 1,4-bis(3-methylimidazolium) butane di[bis(trifluoromethylsulfonyl)imide], was used as sorbent. Adsorption experiments were carried out using a batch system. Removal efficiencies were investigated at different pH (5, 6, 7, 8, and 9) and different mixing times (10, 30, 60, 90, and 120 minutes) by adding 250 mg ionic liquid-based sorbent to the 50 mL of wastewater sample containing dye into the beaker placed on a mixer. RES method was used for color measurements. As a result of the study, higher than 99% removal efficiencies were achieved at all operational conditions. Abstract

The textile industry has large impacts on the environment for reasons such as the high use of chemicals and water on production processes, the disposal of wastes generated during production by incineration or transferring them to landfills. Cotton is one of the most used fibers and cotton cultivation also have negative impact for the environment since cotton grown needs large agricultural land and the use of water and pesticides. Therefore, recycling is an issue that should be given importance for reducing the environmental impact of the cotton textile industry and for a more sustainable production. Obtaining recycled fibers from textile scraps and producing textile products from these recycled fibers is an important application in terms of sustainable textile production. In this study, it was aimed to determine the environmental effects in case of using grid electricity and solar energy in the phase of obtaining fiber from textile scraps. Within the scope of the study, environmental effects were determined by life cycle analyses approach. The required data was obtained from a recycling company in Usak. The company uses electricity in the production and the current environmental effects of the production was determined by using their operational input and output values. As a more environmentally friendly application, the use of solar energy instead of grid electricity has been evaluated as a scenario. GaBi software was used for LCA study and the results were evaluated according to the CML impact categories. The results reveal that production with solar energy has lower impacts compared to electricity. According to results, there has been about 95% reduction in global warming potential. At almost all CML impact categories except the categories of abiotic depletion and ozone layer depletion potential, solar energy results lower effects than electricity. [ABSTRACT FROM AUTHOR]

25. Renal Solitary Fibrous Tumor With Müllerian Cysts: A Novel Differential Diagnosis for Biphasic Renal Neoplasms.

Abstract

Solitary fibrous tumor is an uncommon mesenchymal neoplasm, initially described in the pleura and infrequently found in the kidney. It is characterized by haphazardly arranged spindle cells, staghorn vasculature, coexpression of CD34 and signal transducer and activator of transcription 6 (STAT6), and a NAB2::STAT6 gene fusion. We report a 64-year-old woman who presented with a 2.5 cm multilocular cystic renal mass. On microscopic examination, the tumor consisted of a spindle cell component closely intermingled with ciliated and hemorrhagic epithelial cysts. The spindle cell component was positive for CD34, B-cell lymphoma 2 (BCL2), and STAT6, confirming the diagnosis of a solitary fibrous tumor. The epithelial cysts expressed keratin 7, PAX8, BCL2, estrogen receptor 1, and progesterone receptor, indicative of Müllerian cysts. The occurrence of solitary fibrous tumor in the kidney is extremely rare, and its association with Müllerian cysts has not been previously reported in the kidney. This morphologic variant has a striking similarity with biphasic renal neoplasms, especially with mixed epithelial and stromal tumors and angiomyolipoma with epithelial cysts. This report describes a novel renal solitary fibrous tumor subtype within this differential and underscores clinical and pathological distinctions of biphasic renal neoplasms. [ABSTRACT FROM AUTHOR]

Our aim is to prospectively determine the effect of psychiatric treatment on scores for depression, anxiety, quality of life (QoL), and sexual dysfunction in an outpatient population diagnosed with IBD and also anxiety and/or depression disorder.Patients and methods: Patients who scored higher than the cutoff point on the Hospital Anxiety Depression Scale were referred for further structured psychiatric evaluation and determination of the need for psychiatric drug treatment. Patients who underwent drug therapy completed Short Form-36 (SF-36) and the Arizona Sexual Experience Scale at baseline and after 6 months of follow-up.Results: Major depressive disorder and generalized anxiety disorder were the most common diagnoses. After 6 months, 47 patients had completely adhered to drug treatment (group A), whereas 20 were nonadherent (group B). In group A, all domains of SF-36, Arizona Sexual Experience Scale, depression/anxiety scores, and Crohn’s disease activity index were statistically improved after treatment when compared with the baseline. In group B, the three domains of SF-36, platelet count, and mean corpuscular volume were worse between baseline and at 6 months.Conclusion: In IBD patients having any psychiatric disorder, 6 months of antidepressant drug treatment is associated with an improvement in depression, anxiety, QoL, and sexual functioning scores, as well as an improvement in Crohn’s disease activity index. 41. Renal Juxtaglomerular Cell Tumors Exhibit Distinct Genomic and Epigenomic Features and Lack Recurrent Gene Fusions

Abstract

Juxtaglomerular cell tumor (JxGCT) is a rare type of renal neoplasm demonstrating morphologic overlap with some mesenchymal tumors such as glomus tumor (GT) and solitary fibrous tumor (SFT). Its oncogenic drivers remain elusive, and only a few cases have been analyzed with modern molecular techniques. In prior studies, loss of chromosomes 9 and 11 appeared to be recurrent. Recently, whole-genome analysis identified alterations involving genes of MAPK-RAS pathway in a subset, but no major pathogenic alterations have been discovered in prior whole transcriptome analyses. Considering the limited understanding of the molecular features of JxGCTs, we sought to assess a collaborative series with a multiomic approach to further define the molecular characteristics of this entity. Fifteen tumors morphologically compatible with JxGCTs were evaluated using immunohistochemistry for renin, single-nucleotide polymorphism array (SNP), low-pass whole-genome sequencing, and RNA sequencing (fusion assay). In addition, methylation analysis comparing JxGCT, GT, and SFT was performed. All cases tested with renin (n=11) showed positive staining. Multiple chromosomal abnormalities were identified in all cases analyzed (n=8), with gains of chromosomes 1p, 10, 17, and 19 and losses of chromosomes 9, 11, and 21 being recurrent. A pathogenic HRASmutation was identified in one case as part of the SNP array analysis. Thirteen tumors were analyzed by RNA sequencing, with 2 revealing in-frame gene fusions: TFG::GPR128(interpreted as stochastic) and NAB2::STAT6. The latter, originally diagnosed as JxGCT, was reclassified as SFT and excluded from the series. No fusions were detected in the remaining 11 cases; of note, no case harbored NOTCHfusions previously described in GT. Genomic methylation analysis showed that JxGCT, GT, and SFT form separate clusters, confirming that JxGCT represents a distinct entity (ie, different from GT). The results of our study show that JxGCTs are a distinct tumor type with a recurrent pattern of chromosomal imbalances that may play a role in oncogenesis, with MAPK-RAS pathway activation being likely a driver in a relatively small subset.
