15. Urinary markers in the early stage of nephropathy in patients with childhood-onset type 1 diabetes

Abstract

Background The aim of this study was to evaluate the association of a urinary tubular marker, liver-type fatty acid binding protein (L-FABP) and an inflammatory marker, serum/urinary YKL-40, with albuminuria in patients with childhood-onset type 1 diabetes (T1D). Methods Twenty-nine patients with childhood-onset T1D and 32 controls were enrolled. Serum and urinary concentrations of YKL-40 and urinary concentrations of L-FABP were measured. Results The serum levels of YKL-40 were not significantly different between the control group and the patient groups. However, the levels of urinary YKL-40/creatinine (Cr) were higher in the patients, even those with normoalbuminuria than in the controls (p < 0.001). The levels of urinary L-FABP/Cr were not different between the control group and the patient groups. However, the level of urinary L-FABP/Cr in the microalbuminuria group was higher than that in the normoalbuminuria group (p = 0.03). There were no associations between the levels of urinary albumin-to-creatinine ratio and urinary L-FABP/Cr or YKL-40/Cr. However, the urinary L-FABP/Cr level was significantly correlated with the hemoglobin A1C level (p = 0.005) and the urinary YKL-40/Cr level (p = 0.043). Abstract

Purpose To analyze the relationship between body mass index change and bone age progression during gonadotropin-releasing hormone agonist(GnRHa) treatment in girls with idiopathic central precocious puberty Methods: 65 girls with idiopathic central precocious puberty who are treated with GnRHa more than 2 years were retrospectively reviewed. Height, weight, Tanner stage, bone age were measured every 6 months from the start of treatment. The degree of obesity was evaluated using the body mass index (BMI) standard deviation score (SDS), and the relationship between the change in obesity and the progression of bone age was analyzed. Overweight was defined as a BMI greater than or equal to the 85th percentile. The chronological age at the start and the end of treatment was 7. 00±0. 00 years and 10.63±0.50 years in the overweight group (16 patients), and 6.86±0.35 years and 10.71±0.54 years in the other group (49 patients), respectively. Results There was no difference in bone age between the normal weight group and the overweight group at the start of treatment (9.85±0.57 years vs 10. 04±0.73 years, P=0.263). Bone age was higher in the overweight group, but after 1 year of treatment, 2 years of treatment, and at the end of treatment, the overweight group had a higher bone age than the normal weight group (10.47±0.56 years vs 10.87±0.57 years, P=0. 02; 11. 06±0.47 years vs 11.46±0.51 years, P=0. 004; 11.70±0.46 years vs 12. 01±0.52 years, P=0. 03). Bone age at the end of treatment was related to overweight at the start of treatment (r=0.265, P=0. 03) and the increase in BMI SDS from normal to overweight during treatment (r=0.285, P=0. 02). There were 11 children (17%) whose BMI SDS increased from normal to overweight during treatment, compared with the other patients, there was no difference in bone age during 2 years of treatment, but bone age at the end of treatment was higher (12. 09±0.60 years vs 11.71±0.47 years, P=0. 02), and the difference in bone age before and after treatment was also greater (2.31±0.68 years vs 1.79±0.52 years, P=0. 01). Linear regression analysis demonstrated that the increase in BMI SDS from normal to overweight during treatment had a positive correlation with bone age at the end of treatment (β=0.31, R2=0. 07, P=0. 03). Conclusion In girls with central precocious puberty, an increase in obesity from normal to overweight during GnRHa treatment does not affect bone age progression until 2 years from the start of treatment, but might be a risk factor for bone age progression at the end of treatment. Abstract

Translation has become a metaphor for contemporary life, and thus translation theories demand a paradigm shift from binary opposition between 'self and other', or between 'original and target', towards negotiation and dialogic interaction. This article argues that translation as dialogue in the 'in-between space'--or what Mary Louise Pratt termed a 'contact zone'--involves a new model of language use that can incorporate both cultures and languages without prioritizing any. Anna Wierzbicka's concept of linguistic universals and Eva Hoffman's experiences of integrating bilingual and bicultural aspects are elicited to illustrate the tentative translation strategies for 'translation as a dialogue'. The article proposes the importance of understanding and empathy, or, at least, a will to believe in translatability. It also maintains, drawing on Edward Sapir's insight that language is the index to culture, that translation should be not only about content but also about relation between 'self and other', and that translators should realize the importance of learning vocabularies as a sensitive index to a culture. These requirements ask translators to be more attentive to what people are and what people do, thus proposing a more people-oriented translation theory. Abstract

Background and Aim: Carbon dioxide (CO2) insufflation during gastric endoscopic submucosal dissection (GESD) under sedation can be used instead of room air insufflation. Appropriate monitoring of the partial pressure of CO2 during GESD is necessary due to the impaired respiration. The aim of this study was to assess the safety and efficacy of CO2 insufflation during GESD compared with conventional room air insufflation. Methods: Patients with a gastric epithelial neoplasm or early gastric cancer were enrolled. A total of 76 consecutive patients were randomly assigned to the CO2 insufflation group (CO2 group) or the room air insufflation group (air group). The primary outcome was the mean difference of end‐tidal CO2 (EtCO2) between two groups. Results: The upper bound of the 95% CI for the mean EtCO2 difference between the two groups before the procedure and at 15, 30 and 45 min after insufflation met the criteria for noninferiority. In a subgroup analysis of patients 70 years and older, the mean difference of EtCO2 was not significantly different between two groups. However, the air group received more analgesics than the CO2 group after the procedure (67.6% vs 35.1%, P = 0.005). In addition, in terms of improvement of abdominal pain or bowel gas after 24 h of GESD, CO2 group showed better results than air group (both P < 0.05). Conclusions: CO2 insufflation during GESD is as safe as using room air, and patients, including elderly patients, receiving CO2 achieve more rapid relief of abdominal pain and intra‐abdominal residual gas during and after the procedure.
