Abstract

PURPOSE: To elucidate which swept-source optical coherence tomography (OCT)-derived optic nerve head (ONH) parameters are associated with longer axial length (AXL) in healthy myopic eyes. DESIGN: Prospective cross-sectional observational study. METHODS: Two hundred eleven healthy eyes of 140 participants (96 emmetropic-mild myopic [AXL: 22.2-24.5 mm], 83 moderately myopic [24.5-26.0 mm], and 32 highly myopic [26.0-27.4 mm] eyes) were enrolled. Bruch membrane opening (BMO), anterior scleral canal opening (ASCO) area and ovality, minimum rim width, parameters defining misalignment between the BMO and ASCO planes, OCT-defined region of perineural canal retinal epithelium atrophy and externally oblique choroidal border tissue, circumpapillary retinal nerve fiber layer thickness (cpRNFLT), circumpapillary choroidal thickness (cpChT), lamina cribrosa parameters, and peripapillary scleral (PPS) angle were calculated from BMO-centered radial scans reconstructed from 3D raster scans. Multivariate linear mixed models were used to elucidate ONH parameters that are independently associated with AXL. RESULTS: Longer AXL was associated with a greater misalignment between ASCO and BMO planes, larger region of externally oblique choroidal border tissue, thinner cpChT, larger PPS angle, larger ASCO area, and thicker cpRNFLT (all P < .040 after Bonferronis correction for number of included explanatory variables). CONCLUSIONS: A greater misalignment between BMO and ASCO planes, thinner choroid, a more posteriorly bowed PPS, an enlargement of ASCO, and thicker cpRNFLT were each associated with longer AXL. An enhanced understanding of these AXL-associated configurations should provide essential information to improve our ability to detect glaucoma-induced ONH morphology in myopic eyes.

5. Adversarial Multi-Task Learning for Disentangling Timbre and Pitch in Singing Voice Synthesis

Abstract

Recently, deep learning-based generative models have been introduced to generate singing voices. One approach is to predict the parametric vocoder features consisting of explicit speech parameters. This approach has the advantage that the meaning of each feature is explicitly distinguished. Another approach is to predict mel-spectrograms for a neural vocoder. However, parametric vocoders have limitations of voice quality and the mel-spectrogram features are difficult to model because the timbre and pitch information are entangled. In this study, we propose a singing voice synthesis model with multi-task learning to use both approaches -- acoustic features for a parametric vocoder and mel-spectrograms for a neural vocoder. By using the parametric vocoder features as auxiliary features, the proposed model can efficiently disentangle and control the timbre and pitch components of the mel-spectrogram. Moreover, a generative adversarial network framework is applied to improve the quality of singing voices in a multi-singer model. Experimental results demonstrate that our proposed model can generate more natural singing voices than the single-task models, while performing better than the conventional parametric vocoder-based model., Comment: Accepted by Interspeech 2022

Abstract

Recently, end-to-end Korean singing voice systems have been designed to generate realistic singing voices. However, these systems still suffer from a lack of robustness in terms of pronunciation accuracy. In this paper, we propose N-Singer, a non-autoregressive Korean singing voice system, to synthesize accurate and pronounced Korean singing voices in parallel. N-Singer consists of a Transformer-based mel-generator, a convolutional network-based postnet, and voicing-aware discriminators. It can contribute in the following ways. First, for accurate pronunciation, N-Singer separately models linguistic and pitch information without other acoustic features. Second, to achieve improved mel-spectrograms, N-Singer uses a combination of Transformer-based modules and convolutional network-based modules. Third, in adversarial training, voicing-aware conditional discriminators are used to capture the harmonic features of voiced segments and noise components of unvoiced segments. Abstract

BackgroundArteriovenous fistulas (AVFs) are the preferred vascular access type in most hemodialysis patients. However, the optimal vascular access type in octogenarians and older (≥80 years) hemodialysis patients remains widely debated given their limited life expectancy and lower AVF maturation rates.MethodsAmong incident hemodialysis patients receiving care in a large national dialysis organization during 2007-2011, we examined patterns of vascular access type conversion in 1 year following dialysis initiation in patients 1 year, those with initial AVF/AVG use and placement of AVF from a central venous catheter (CVC) had lower mortality compared with patients with persistent CVC use. When the reference group was changed to patients who had AVF placement from a CVC in the first year of dialysis, those with initial AVF use had similar mortality. A longer duration of CVC use was associated with incrementally worse survival.ConclusionsAmong incident hemodialysis patients ≥80 years of age, placement of an AVF from a CVC within the first year of dialysis had similar mortality compared with initial AVF use. Our data suggest that initial CVC use with later placement of an AVF may be an acceptable option among elderly hemodialysis patients.
