To evaluate the coronavirus disease 2019 pandemic's impact on pregnancy outcomes in a Japanese rural area.This retrospective study focused on the periods between March 1, 2020, and February 28, 2021 (during the coronavirus disease 2019 pandemic), and January 1, 2017, and December 31, 2019. Singleton pregnancies delivered at or after 22 gestational weeks were included. Preterm delivery, low-birth-weight, and small-for-gestational-age infant rates during the pandemic were compared to those in the preceding 3 years.In the pandemic and control groups, 1650 and 5762 pregnant women were included, respectively. Two pregnant women with coronavirus disease 2019 were identified (0.1%). There were no significant intergroup differences in preterm delivery rates (control, 4% vs. pandemic, 3.3%; difference: -0.7% [95% confidence interval: -1.7%-0.3%], p = 0.22). The low-birth-weight rate tended to decrease; however, the difference was insignificant (7.9% vs. 6.5%; difference: -1.4% [95% confidence interval: -2.8-0%], p = 0.06). The small-for-gestational-age infant rate was significantly lower in the pandemic than in the control group (7.3% vs. 5.2%; difference: -2.1% [95% confidence interval: -3.3-0.8%], p 0.01). However, the interrupted time series analysis showed no significant trend.There were no significant changes in the rates of preterm delivery, low-birth-weight infants, and small-for-gestational-age infants during the pandemic's first year compared to those in the preceding 3 years. Behavioral changes, such as "stay-at-home" measures, may not improve pregnancy outcomes in Japan.

We present a case of uterine trauma and intrauterine fetal death caused by seatbelt injury. A 37-year-old primigravida at gestational week 24 was involved in a single-car accident when traveling as a front-seat passenger and wearing a three-point seatbelt. Fetal heart rate monitoring patterns revealed fetal demise, and computed tomography revealed intraperitoneal bleeding due to damage to the uterine vessels and placental lacerations across the seatbelt-compressed region. Intensive treatment, including transfusion therapy and surgical laparotomy, prevented the loss of her life but not that of the fetus. Seatbelt use can reduce the overall mortality associated with motor vehicle crashes. Pregnant women must be educated on the proper use of restraints while traveling in motor vehicles.

Prevention of chemotherapy-induced nausea and vomiting (CINV) is crucial for maintaining the quality of life of cancer patients. Female patients have been underrepresented in previous clinical studies of aprepitant or palonosetron. We performed a prospective multicenter study to investigate the efficacy and safety of triple therapy comprising these two agents and dexamethasone in female cancer patients receiving chemotherapy that included cisplatin (≥ 50 mg/m(2)).Aprepitant was administered at a dose of 125 mg before chemotherapy on day 1 and at 80 mg on days 2 and 3. Palonosetron (0.75 mg) was given before chemotherapy on day 1. Dexamethasone was administered at a dose of 9.9 mg before chemotherapy on day 1 and at 6.6 mg on days 2-4. The primary endpoint was the the proportion of patients with a complete response (CR no vomiting and no use of rescue medication) throughout the overall period (0-120 h post-chemotherapy).Ninety-six women (median age 55 years) were enrolled. The overall CR rate was 54.2 %. CR was obtained during the acute phase (0-24 h post-chemotherapy) and the delayed phase (24-120 h post-chemotherapy) in 87.5 and 56.3 % of the patients, respectively. The most common adverse reactions were constipation and fatigue (reported by three patients each).Exhibition of a favorable overall CR rate over existing two-drug combinations suggests that the triple therapy regimen used in the present study is effective and tolerable in patients with gynecological malignancies receiving cisplatin-based chemotherapy. Female patients may have a higher risk of developing CINV.

Mouse fetal livers of 12 days gestation were transplanted beneath the kidney capsule of syngeneic castrated male hosts. Recipients received a single intraperitoneal injection of 5 mg estriol (E3) immediately after transplantation and were sacrificed 4, 7 and 14 days later. Control mice received injections of solvent only. The fetal liver grafts of the E3 groups showed remarkable growth compared with the control grafts at each corresponding time. In the grafted fetal livers of the E3 groups, many more basophilic hepatocytes appeared for much longer periods, and the early formation of wide sinusoids was noted. Although hemopoietic activity had almost ceased a short time after transplantation in the control grafts, many prominent extrasinusoidal erythroid, granulocytic and megakaryocytic hemopoietic foci developed and persisted to the end of the experiments in the grafted fetal livers of the E3 groups. In the next experiment, the recipients received three injections of 0.5-5 mg E3 at 5-day intervals, and were sacrificed 21 days after transplantation. Control animals received solvent only in the same manner. The fetal liver grafts of the E3 groups also showed remarkable, dose dependent growth. The growth was particularly striking at doses of 3-5 mg E3. In the grafts of these groups, basophilic hepatocytes were predominant, and hepatic cords and sinusoids were well formed. Moreover, sinusoidal erythropoiesis was intense. These results provide evidence for the possible role of E3, secreted from the placenta into the fetal circulation, in the development of the fetal liver.

A case is reported of diffuse laminar endocervical glandular hyperplasia, which is very rare among pseudoneoplastic endocervical glandular lesions. The patient was a 54 year old woman who had been suffering from watery vaginal discharge for about 7 years, and the lesion was found in the cervix of the hysterectomy specimen. Microscopically, the lesion showed a diffuse, laminar proliferation of hyperplastic endocervical glands, sharply demarcated from the underlying stroma. The significant nuclear atypia was absent. Inflammatory response was mild. Immunohistochemically, carcinoembryonic antigen was negative. Differential diagnosis from other non-neoplastic conditions and well differentiated adenocarcinoma is discussed.

Objective To report a case of a very rare ectopic cervical intramural pregnancy. Design Case report. Setting Prefectural hospital. Patient(s) A 22-year-old woman, gravida 1, para 0, was referred to our hospital with the suspicion of a cervical ectopic pregnancy (EP). Pelvic examination revealed an enlarged uterine cervix with no genital bleeding. We found a clear gestational sac (GS) and fetal heart beat in the anterior muscular layer of the uterine cervix by ultrasonography, and confirmed these findings by magnetic resonance imaging (MRI). Intervention(s) We injected methotrexate (MTX) into the GS cavity and around the GS. One week later, the GS was removed surgically without massive bleeding. Main Outcome Measure(s) On the 11th postoperative day, she recovered and was discharged from our hospital. Her menstruation restarted on the 35th postoperative day. Result(s) We have shown a case of a very rare ectopic cervical intramural pregnancy with successful treatment. Conclusion(s) We have explained a case and successful treatment of a very rare ectopic cervical intramural pregnancy with clear GS and fetal heart beat. Our strategy was injecting MTX into the GS cavity and around the GS, then performing an operation to remove the GS.

A single intraperitoneal injection of 10 mg estriol (E3) rapidly induced a transient decrease in the serum albumin (ALB) level in 3 to 5 days in adult mice. At a dose of 5 mg, E3 showed a remarkable threshold effect on the decline of serum ALB. By immunohistochemistry, two types of ALB-positive cells were found in the liver. Type 1 cells were stained intensely and almost homogeneously throughout all or nearly all their entire cytoplasm, and type 2 cells showed only granular or vesicular cytoplasmic deposits. After administration of 10 mg E3, the type 1 cells diminished rapidly and markedly in direct proportion to the fall in serum ALB from the centro- and mediolobular zones, where they had been normally localized either singly or in small groups. Simultaneously, the granular or vesicular deposits that had been normally stained very weakly in the type 2 cells in the centro- and mediolobular zones became intensely stained along with cells in the perilobular zone. Alpha-fetoprotein (AFP)-containing cells in the liver appeared in maximal numbers at 5 to 7 days after E3 administration. These serological and cytological changes after E3 administration are here discussed on the basis of a possible inverse relationship between ALB and AFP gene expression.

A single intraperitoneal injection with a 10 mg estriol (E3) in aqueous suspension induced a large and prolonged elevation of serum alpha-fetoprotein (AFP) in adult mice. E3 also raised the mitotic activity of hepatocytes in the absence of liver injury. Although both the AFP concentration and hepatocyte proliferation reached the peak on day 5 after E3 administration, a high level (about 12,500 ng/ml) of serum AFP persisted for a long period after hepatocyte proliferation declined. Five mg E3 showed a remarkable threshold effect on AFP elevation and 3 mg E3 on hepatocyte proliferation. Immunohistochemical studies indicated AFP production by hepatocytes in adult mice after the E3 administration.

Mixed mesodermal tumor of the uterus are not uncommon; however, their histogenesis has remained unsolved. A case of endocervical mixed mesodermal tumor consisting of a well-differentiated squamous carcinoma and a rhabdom-osarcoma was studied histologicall-. The sections were stained b- specific keratin staining methods, Martinotti's keratin staining and the peroxidase-antibod- method, as well as b- the usual histologic stainings. The specific stainirigs revealed keratin-like substances within the c-toplasm of the squamous cartinoma cells and also that of a few sarcomatous cells scattered in the area of the sarcoma. These cells, which were functionall- epithelial and morphologicall- non-epithelial, seemed to be intermediate forms between carcinoma and sarcoma. This finding suggested that the carcinomatous lesion might have undergone transformation into the sarcomatous one. The authors therefore believe that mixed mesodermal tumor originates from a single strain of endometrial or endocervical cells.
