論文, Research Article, 特集２ : インクルージョンと共生, 本稿では、高齢者のソーシャル・インクルージョン（社会的包摂）について、食と健康のコホート研究から、特に孤食と共食を中心として論じる。高齢期の孤食は、心理的健康度の低さや摂取食品の多様性の乏しさに関連することが明らかになり、孤食であることは高齢者の健康上のリスク要因の一つであると考えられる。しかしながら、高齢者の独居が避けられない日本社会において、共に食べる場を家庭内にとどめるには限界があり、また、通所介護（デイサービス）などの制度でも包摂されない実態も浮かび上がってきた。2018 年の介護報酬改定を受けて、要支援高齢者（要支援1、2 の認定者）がデイサービスを利用できないという状況が起こっている。高齢者を支援するための社会保障制度は、そのサービスを通じて高齢者に居場所を与える目的もあるが、一方で、地域社会から切り離してしまうという危険性も孕んでいる。そこで本稿では、家族、制度に依存しない、地域における草の根的な共食の事例を紹介し、食を通じた高齢者のソーシャル・インクルージョンの可能性について考察する。, This paper will discuss social inclusion and how it relates to the elderly basedon dietary and health cohort studies, with a particular emphasis on solitary and group meal-taking. Solitary meals eaten by seniors have been found to be associated with poor psychological well-being and a lack of variety in diet, and taking meals alone is thought to be one of several health risk factors among the elderly. However, in Japanese society, in which elderly people often cannot avoid living alone, the places where such people can stay to eat meals with others are limited, and the fact that some designated Seniors Requiring Long-Term Care/Support that are not covered by social support systems—such as day nursing services—can become further isolated has emerged as a concern. The social systems in place to support the elderly serve to provide seniors with places to stay, but also carry the danger of disconnecting them from their communities. As such, this paper will introduce cases of grassroots efforts to support group meal-taking in the community that are not dependent on family or institutions, as well as consider the possibility of social inclusion of the elderly through food.

Abstract

Aim: Although the relationship between impaired glucose tolerance (IGT) and mortality has been investigated in diverse populations, few studies have focused on older populations. This study aimed to investigate the relationship between glucose tolerance and overall mortality among populations aged ≥75 years. Methods: Data were obtained from the Tosa Longitudinal Aging Study, a community‐based cohort survey conducted in Kochi, Japan. According to the results of a 75‐g oral glucose tolerance test conducted in 2006, the participants were classified into four categories: normal glucose tolerance (NGT), impaired fasting glucose (IFG)/IGT, newly diagnosed diabetes mellitus (NDM), and known diabetes mellitus (KDM). The primary endpoint was overall mortality. Differences in overall mortality among the four categories were evaluated using the Cox proportional hazards model. Results: During a median of 11.5 years of observation, 125 deaths of the 260 enrolled participants were recorded. The cumulative overall survival rate was 0.52, and the survival rates of NGT, IFG/IGT, NDM, and KDM were 0.48, 0.49, 0.49, and 0.25, respectively (log‐rank test, P = 0.139). Adjusted hazard ratios (HRs) for mortality in the IFG/IGT and NDM groups compared with the NGT group were 1.02 (95% confidence interval [CI], 0.66–1.58) and 1.11 (95% CI, 0.56–2.22), while mortality in the KDM group was significantly higher than that in the NGT group (HR, 2.43; 95% CI, 1.35–4.37). Conclusion: Mortality did not differ significantly between the IFG/IGT, NDM, and NGT groups, but was higher in the KDM group than in the NGT group. Geriatr Gerontol Int 2023; 23: 341–347. Biomolecules, 11, 2, 1. https://doi.org/10.3390/biom11020131., Malignant melanoma favors spreading to bone, resulting in a weakened bone with a high fracture risk. Here, we revealed the disorganized alignment of apatite crystals in the bone matrix associated with the homing of cancer cells by developing an artificially controlled ex vivo melanoma bone metastasis model. The ex vivo metastasis model reflects the progressive melanoma cell activation in vivo, resulting in decreased bone mineral density and expression of MMP1-positive cells. Moreover, less organized intercellular connections were observed in the neighboring osteoblasts in metastasized bone, indicating the abnormal and randomized organization of bone matrix secreted by disconnected osteoblasts. Our study revealed that the deteriorated microstructure associated with disorganized osteoblast arrangement was a determinant of malignant melanoma-related bone dysfunction.

41. Repatriation from Fukami Village, Karafuto : An Analysis of the Japanese Government Survey into Repatriates' Postwar Lives

Abstract

本稿では国立公文書館所蔵『引揚者在外事実調査票』を基本資料とし、戦後、樺太深海村から北海道への引揚げについて分析した。これにより、樺太南部の漁村である深海村から北海道への引揚げについて実態を明らかにし、「樺太－北海道の上陸地－最初の住所－昭和31（1956）年現在の住所」と長いスパンで引揚者の動向を分析し、職業等と関連付けて定着地への軌跡を検証した。深海村では樺太全体と比べて「公式引揚」以前の、「緊急疎開」と「脱出」で引揚げた者の割合が高かった。また引揚出発港が、「公式引揚」で使用された真岡ではなく、大泊が最多であったことも特徴といえる。北海道では引揚げの最初の上陸地である稚内、函館、枝幸に定着した者が多く、また炭鉱都市へ定着した者も多かった。引揚げから昭和31年までの間に、6割以上の世帯が転居をしていることも明らかとなった。引揚後は漁業から、農業、炭鉱、日雇、公務員への転職が見られ、無職となった者もあった。深海村の公務員の引揚げについては、個別の事例を挙げて明らかにした。公務員は優先的に「再就職」されるよう考慮されたが、全員が「再就職」できたわけではなく、郵便局員と教員は同じ職に「再就職」したが、役場吏員の「再就職」は1名だけであった。学校は、教員の「再就職」先としてだけでなく、「小使」や「学校事務員」として転職した者もみられ、引揚者の受入先としても大きな役割を果たした。

Abstract

【目的】 本研究は，統合失調症患者の家族支援に示唆を得るための基礎的研究である．統合失調症患者の家族の体験を記した文献から家族支援についての記述を整理する，そして混乱時期における家族の体験を明らかすることを目的とする．【方法】 統合失調症患者の家族の体験について記された51 文献の結果および考察から，地域と医療施設，そして集団的と個人的からなる4分割のマトリックスで整理した．また研究対象文献から家族が患者の変調に巻き込まれる時期（以下，混乱時期とする）の体験が記述されている29文献を抽出し，体験の記述を抽出し類似性の観点から質的帰納的に分析した．【結果】 4分割のマトリックスで整理した結果，地域における家族の個別的支援について述べられた調査が十分でないことが明らかになった．また，混乱時期における統合失調症患者の家族の体験を記した文献を分析した結果，【家族の変調に対する対処困難】【スティグマが招く憂い】【家族のきずなが崩壊する危機】【発症に対する自責の念】【当事者との生活が限界に達してからの援助の希求】【資源に対する渇求】【医療介入により感じる緊張からの解放】の7つに集約された．【結論】 統合失調症患者の家族の支援は精神保健福祉分野の中で十分な研究の蓄積や体系立てたケアが確立されているとは言い難い．混乱時期における家族の体験を十分に理解し，入院の早期から家族の心理的側面および身体的側面に対するアセスメントをおこない家族の支援に繋ぐ必要がある．また本研究によって明らかにされた当事者の家族が抱える混乱時期の体験をもとに，その体験がどのように変化し意味づけられ，当事者と共に生活する上で家族にどのように影響しているのかといった体験の理解を深め，家族にとってどのような体験がパーソナル・リカバリーを促進するのかを丁寧に紐解いていくことが，必要な支援の在り方を検討するための一助になると考える．

Abstract

Socioeconomic factors and hypoxia play major roles in the prevalence of glucose intolerance in highlanders. Hypoxemia and polycythemia were closely associated with glucose intolerance after adjustment for the effects of lifestyle changes in our study. Tibetan people may be vulnerable to glucose intolerance, with polycythemia as a sign of poor hypoxic adaptation. Epigenetics is also play a role in its vulnerability with quick lifestyle change. Prevention of lifestyle-related diseases and health education should be advocated, especially in high altitude dwellers with rapidly prevailing socioeconomic globalization.

Abstract

This paper focuses on changes in the diets and health of Himalayan highlanders living in Ladakh, India. In this region, where food resources have traditionally been limited by geographic factors, it was thought that lifestyle-related diseases were absent. However, according to research conducted by the "Highlands Civilizations" project at the Research Institute for Humanity and Nature in Kyoto, Japan, lifestyle-related diseases have been emerging in recent years as a problem. As a member of this research project, I conducted research on the relationships between changing ways of life, dietary habits, and lifestyle-related diseases in Ladakh and nearby communities in the Himalayas. I found that there were notable differences in the total calorie intake among people living in urban areas, due to individual differences in income, lifestyle, and other factors. This trend was also apparent in farming villages relatively near cities, where modernization and the increased flow of goods have affected residents' ways of life. However, among nomadic communities living on the Changtang Plateau in Ladakh, people continue to follow traditional patterns of eating, which means that there are not large differences in calorie intake among individuals of the same age group. In particular, there has been a striking shift in the staple food eaten by highlanders in cities and farming villages, as seen in a decrease in barley consumption and an increase in white rice consumption. These changes in diet and lifestyle have also had a noticeable impact on health. Meanwhile, in Western countries, barley has received more attention in recent years from researchers, as the health effects of this grain have become more widely known. Barley is said to have a high amount of dietary fiber and a relatively low glycemic index compared to other grains, such as rice and wheat. In particular, studies have shown that the water-soluble fiber contained in barley, which is high in β-glucan, can help to control and maintain healthy levels of cholesterol, blood sugar, and blood pressure. In the United States and Europe, it has now become trendy among health-conscious consumers to eat barley. Despite this barley "boom" in so-called developed countries, the opposite trend is occurring among villages that have traditionally eaten barley as a staple food. In Ladakh, the popularity of "tsampa, " a roasted barley flour, seems to be decreasing, although it remains one of the most important foods for Himalayan highlanders. In this article, I first summarize and review recent trends in the literature on barley, focusing on its nutritional value as well as its effects on health, from a nutritional science perspective. Next, I analyze data on the nutritional intake from barley and other grains, which is based on a research study conducted in Domkhar village in Ladakh, an area that is known for the cultivation of barley. Finally, I consider the reality of dietary changes in Domkhar village, drawing on quantitative data from recent years that tracks changes in nutritional intakes from grains, as well as interviews with the villagers on the broader political, economic, and historical context.
