Abstract

Background: There continues to be a lack of evidence to support physiotherapeutic rehabilitation measures after total knee arthroplasty (TKA), especially in the area of functional test procedures, which are indispensable for assessing perioperative physiotherapeutic interventions, including their efficacy. Objective: A multi-stage randomized controlled study design is concerned with the establishment of an evidence-based perioperative physiotherapeutic treatment programme to evaluate physical functional test procedures with regard to their predictive value and the efficacy of physiotherapeutic interventions. Setting: The RCT will be realized in a medical clinic in the department of orthopaedics, sports medicine and rehabilitation.Participants: A first stage will include young, healthy subjects (Group A) and subjects who are due to undergo TKA and are then to be treated with a standard rehabilitation programme (Group B). Design: Selected promising physical functional test procedures will be tested for their reliability. Inter-tester reliability will be determined in Group A. The test-retest reliability at different perioperative timepoints will be determined in Group B. Measurements: The following test procedures will be investigated for reliability: 10-m walk test, timed up and go, three times sit to stand, the isometric maximal force of knee extension, stair climbing and the cumulative leg circumference. In addition, these test procedures will be correlated with each other and with the WOMAC and KSS for selected aspects of validity. In a second stage, the reliable test procedures will be applied to investigate selected promising physiotherapeutic interventions. Conclusions: The investigation could change perioperative assessments and physiotherapeutic treatment in patients who have undergone TKA.

Abstract

Introduction Rare diseases (RDs) collectively impact over 30 million people in Europe. Most individual conditions have a low prevalence which has resulted in a lack of research and expertise in this field, especially regarding genetic newborn screening (gNBS). There is increasing recognition of the importance of incorporating patients’ needs and general public perspectives into the shared decision-making process regarding gNBS. This study is part of the Innovative Medicine Initiative project Screen4Care which aims at shortening the diagnostic journey for RDs by accelerating diagnosis for patients living with RDs through gNBS and the use of digital technologies, such as artificial intelligence and machine learning. Our objective will be to assess expecting parent’s perspectives, attitudes and preferences regarding gNBS for RDs in Italy and Germany. Methods and analysis A mixed method approach will assess perspectives, attitudes and preferences of (1) expecting parents seeking genetic consultation and (2) ‘healthy’ expecting parents from the general population in two countries (Germany and Italy). Focus groups and interviews using the nominal group technique and ranking exercises will be performed (qualitative phase). The results will inform the treatment of attributes to be assessed via a survey and a discrete choice experiment (DCE). The total recruitment sample will be 2084 participants (approximatively 1000 participants in each country for the online survey). A combination of thematic qualitative and logit-based quantitative approaches will be used to analyse the results of the study. 3. Fine-tuning of canted magnetization in stepped Fe films through thickness variation, Au capping, and quantum confinement

Abstract

We present a joint experimental and theoretical study that demonstrates how to efficiently control a canted state of magnetization in Fe films grown on Ag(001) vicinal surface and precisely characterize it with the magneto-optical Kerr effect. It is shown that by employing different mechanisms to tune the magnetization tilting angle, any magnetization orientation within the plane perpendicular to the step edges can be achieved. In particular, increasing the Fe film thickness leads to continuous rotation of the magnetization easy axis toward the film surface and the sense of this rotation in uncovered films is opposite to that in films covered with Au. Another tuning mechanism is provided by oscillatory changes of the tilting angle at low temperatures due to formation of quantum well states in Fe films. The observed canting of magnetization is explained within a phenomenological model by an interplay of the shape anisotropy and two magnetocrystalline anisotropy terms, perpendicular and step-induced anisotropies, which results in an effective uniaxial magnetic anisotropy. The fitted thickness dependencies of the anisotropy constants accurately reproduce experimental variations of the tilting angle with both Fe and Au thicknesses as well as transient changes of the magnetization orientation in ultrathin Fe films upon submonolayer Au coverage, observed with spin-polarized low-energy electron microscopy.

Abstract

When an ordered spin system of a given dimensionality undergoes a second order phase transition the dependence of the order parameter i.e. magnetization on temperature can be well-described by thermal excitations of elementary collective spin excitations (magnons). However, the behavior of magnons themselves, as a function of temperature and across the transition temperature TC, is an unknown issue. Utilizing spin-polarized high resolution electron energy loss spectroscopy we monitor the high-energy (terahertz) magnons, excited in an ultrathin ferromagnet, as a function of temperature. We show that the magnons' energy and lifetime decrease with temperature. The temperature-induced renormalization of the magnons' energy and lifetime depends on the wave vector. We provide quantitative results on the temperature-induced damping and discuss the possible mechanism e.g., multi-magnon scattering. A careful investigation of physical quantities determining the magnons' propagation indicates that terahertz magnons sustain their propagating character even at temperatures far above TC., Comment: 5 Pages, 3 Figures

Abstract

In low-collisionality scenarios exhibiting mitigation of edge localized modes (ELMs), stable ideal kink modes at the edge are excited by externally applied magnetic perturbation (MP)-fields. In ASDEX Upgrade these modes can cause three-dimensional (3D) boundary displacements up to the centimeter range. These displacements have been measured using toroidally localized high resolution diagnostics and rigidly rotating n = 2 MP-fields with various applied poloidal mode spectra. These measurements are compared to non-linear 3D ideal magnetohydrodynamics (MHD) equilibria calculated by VMEC. Comprehensive comparisons have been conducted, which consider for instance plasma movements due to the position control system, attenuation due to internal conductors and changes in the edge pressure profiles. VMEC accurately reproduces the amplitude of the displacement and its dependencies on the applied poloidal mode spectra. Quantitative agreement is found around the low field side (LFS) midplane. The response at the plasma top is qualitatively compared. The measured and predicted displacements at the plasma top maximize when the applied spectra is optimized for ELM-mitigation. The predictions from the vacuum modeling generally fails to describe the displacement at the LFS midplane as well as at the plasma top. When the applied mode spectra is set to maximize the displacement, VMEC and the measurements clearly surpass the predictions from the vacuum modeling by a factor of four. Minor disagreements between VMEC and the measurements are discussed. Abstract

The influence of the Dzyaloshinskii-Moriya interaction on the spin-wave dispersion in an Fe double layer grown on W(110) is measured for the first time. It is demonstrated that the Dzyaloshinskii-Moriya interaction breaks the degeneracy of spin waves and leads to an asymmetric spin-wave dispersion relation. An extended Heisenberg spin Hamiltonian is employed to obtain the longitudinal component of the Dzyaloshinskii-Moriya vectors from the experimentally measured energy asymmetry.

9. The role of electron confinement in Pd films for the oscillatory magnetic anisotropy in an adjacent Co layer

Abstract

We demonstrate the interplay between quantum well states in Pd and the magnetic anisotropy in Pd/Co/Cu(001) by combined scanning tunneling spectroscopy (STS) and magneto optical Kerr effect (MOKE) measurements. Low temperature scanning tunneling spectroscopy reveals occupied and unoccupied quantum well states (QWS) in atomically flat Pd films on Co/Cu(001). These states give rise to sharp peaks in the differential conductance spectra. A quantitative analysis of the spectra reveals the electronic dispersion of the Pd(001) d-band ($\Delta$$_5$-type) along the $\Gamma$-X direction. \textit{In-situ} MOKE experiments on Pd/Co/Cu(1, 1, 13) uncover a periodic variation of the in-plane uniaxial magnetic anisotropy as a function of Pd thickness with a period of 6 atomic layers Pd. STS shows that QWS in Pd cross the Fermi level with the same periodicity of 6 atomic layers. Backed by previous theoretical work we ascribe the variation of the magnetic anisotropy in Co to QWS in the Pd overlayer. Our results suggest a novel venue towards tailoring uniaxial magnetic anisotropy of ferromagnetic films by exploiting QWS in an adjacent material with large spin-orbit coupling., Comment: 9 pages including 6 figures

13. Ferromagnetism of Pd(001) substrate induced by Antiferromagnetic CoO

Abstract

Our first-principles study has revealed unexpected spin polarization of the Pd(001) substrate in contact with antiferromagnetic CoO overlayers. We give an evidence that the ferromagnetism of Pd is caused by the zigzag positions of Co atoms with respect to the Pd interface, resulted from the lattice-mismatch driven structural relaxation. Thanks to the itinerant nature of its 4d electrons, we see that the ferromagnetic properties of Pd are highly sensitive to the local environment and can be enhanced further by increasing the thickness of CoO overlayer film or/and by applying an additional uniaxial pressure along c-axis exerted externally on the bottom layers of the Pd substrate. Our finding provides new functionality for the interfacial moments of the CoO/Pd system, which can be accessed experimentally, e.g., by the magneto-optical Kerr effect (MOKE) or/and by element-resolved X-ray magnetic circular dichroism (XMCD) measurement.

14. Canted stripe phase evolution due to a spin reorientation transition in Fe films grown on Ag(001) vicinal surface

Abstract

The evolution of the domain structure with the thickness of bcc Fe films deposited on the Ag(116) vicinal surface is studied by spin-polarized low-energy electron microscopy. We show that a spin reorientation transition proceeds via two mechanisms: continuous rotation of magnetization within the vertical plane perpendicular to the steps and discontinuous reorientation of the in-plane component of magnetization, leading to splitting of the domains. In contrast to previously investigated systems with stripe domains, we reveal that in the case of a vicinal ferromagnetic surface, the domain width increases while changing the orientation of the magnetization from a canted out-of-plane state into an in-plane state. A theoretical model developed in this work successfully describes the domain structure behavior observed in our experiments and can be equally applied to other ferromagnetic films grown on vicinal surfaces.

Abstract

We present an x-ray spectromicroscopic investigation of single-crystalline magnetic FeMn/Co bilayers on Cu(001), using X-ray magnetic circular (XMCD) and linear (XMLD) dichroism at the Co and Fe L3 absorption edges in combination with photoelectron emission microscopy (PEEM). Using the magnetic coupling between the ferromagnetic Co layer and the antiferromagnetic FeMn layer we are able to produce magnetic domains with two different crystallographic orientations of the magnetic easy axis within the same sample at the same time. We find a huge difference in the XMLD contrast between the two types of magnetic domains, which we discuss in terms of intrinsic magneto-crystalline anisotropy of XMLD of the Co layer. We also demonstrate that due to the high sensitivity of the method, the small number of induced ferromagnetic Fe moments at the FeMn-Co interface is sufficient to obtain magnetic contrast from XMLD in a metallic system.

Abstract

We have investigated three-dimensional magnetization structures in numerous mesoscopic Fe/Mo(110) islands by means of x-ray magnetic circular dichroism combined with photoemission electron microscopy (XMCD-PEEM). The particles are epitaxial islands with an elongated hexagonal shape with length of up to 2.5 micrometer and thickness of up to 250 nm. The XMCD-PEEM studies reveal asymmetric magnetization distributions at the surface of these particles. Micromagnetic simulations are in excellent agreement with the observed magnetic structures and provide information on the internal structure of the magnetization which is not accessible in the experiment. It is shown that the magnetization is influenced mostly by the particle size and thickness rather than by the details of its shape. Hence, these hexagonal samples can be regarded as model systems for the study of the magnetization in thick, mesoscopic ferromagnets., Comment: 12 pages, 11 figures

Abstract

The atomic structures of clean and hydrogen-adsorbed Pd(001) are investigated by low energy electron diffraction (LEED) I/V analysis. Clean Pd(001) shows little surface relaxation in sharp contrast to previous reports. Adsorbing 1 monolayer of hydrogen on Pd(001), we observe sizable expansion of the interlayer spacing of the first two surface layers, $d_{12}$ by 4.7% of the corresponding one of bulk Pd. Both experimental observations are in excellent agreement with the predictions of recent {\it ab initio} calculations. A series of experiments with varying coverages of hydrogen adsorbed on Pd(001), reveals that $d_{12}$ monotonically increases with the increasing coverage. Such an observation strongly supports the contention that the previous observation of expanded $d_{12}$ in clean Pd(001) results from contamination of the surface by residual hydrogen., Comment: 6 pages, 5figures

Abstract

We have measured the in-plane angular variation of nucleation and annihilation fields of a multi-domain magnetic single dot with a microsquid. The dots are Fe/Mo(110) self-assembled in UHV, with sub-micron size and a hexagonal shape. The angular variations were quantitatively reproduced by micromagnetic simulations. Abstract

Eteplirsen is FDA-approved for the treatment of Duchenne muscular dystrophy (DMD) in exon 51 skip-amenable patients. Previous studies in boys > 4 years of age indicate eteplirsen is well tolerated and attenuates pulmonary and ambulatory decline compared with matched natural history cohorts. Here the safety, tolerability and pharmacokinetics of eteplirsen in boys aged 6–48 months is evaluated. In this open-label, multicenter, dose-escalation study (NCT03218995), boys with a confirmed mutation of the DMD gene amenable to exon 51 skipping (Cohort 1: aged 24–48 months, n = 9; Cohort 2: aged 6 to < 24 months, n = 6) received ascending doses (2, 4, 10, 20, 30 mg/kg) of once-weekly eteplirsen intravenously over 10 weeks, continuing at 30 mg/kg up to 96 weeks. Endpoints included safety (primary) and pharmacokinetics (secondary). All 15 participants completed the study. Eteplirsen was well tolerated with no treatment-related discontinuations, deaths or evidence of kidney toxicity. Most treatment-emergent adverse events were mild; most common were pyrexia, cough, nasopharyngitis, vomiting, and diarrhea. Eteplirsen pharmacokinetics were consistent between both cohorts and with previous clinical experience in boys with DMD > 4 years of age. These data support the safety and tolerability of eteplirsen at the approved 30-mg/kg dose in boys as young as 6 months old.

Abstract

This article presents long-term data on the efficacy of gene replacement therapy (GRT) for spinal muscular atrophy (SMA) using onasemnogene abeparvovec (OA). The study includes patients who were older, heavier, or more severely affected than those in previous clinical trials, as well as a large cohort of presymptomatic SMA patients identified through newborn screening. The results show that younger patients treated with OA had better motor function outcomes compared to those treated at a later age. Abstract

This article, published in the journal Neuropediatrics, discusses a case of ocular myasthenia, a common presentation of myasthenia gravis (MG) in children. The case presented challenges in diagnosis and treatment, requiring specialized laboratory tests and high doses of pyridostigmine. The patient initially had right-sided ptosis and diplopia, which worsened and showed rapid alteration. Despite negative ACh-receptor antibodies and normal CSF, MRI, and electrophysiology results, the patient was successfully treated with pyridostigmine. The article highlights the importance of live cell-based assays in confirming the diagnosis of ocular MG when other tests are negative, and suggests that higher doses of pyridostigmine may be necessary for successful treatment. Abstract

BackgroundOral corticosteroid (OCS) dependence among patients with severe eosinophilic asthma can cause adverse outcomes, including adrenal insufficiency. PONENTE's OCS reduction phase showed that, following benralizumab initiation, 91.5% of patients eliminated corticosteroids or achieved a final dosage ≤5 mg·day−1(median (range) 0.0 (0.0–40.0) mg).MethodsThe maintenance phase assessed the durability of corticosteroid reduction and further adrenal function recovery. For ∼6 months, patients continued benralizumab 30 mg every 8 weeks without corticosteroids or with the final dosage achieved during the reduction phase. Investigators could prescribe corticosteroids for asthma exacerbations or increase daily dosages for asthma control deteriorations. Outcomes included changes in daily OCS dosage, Asthma Control Questionnaire (ACQ)-6 and St George's Respiratory Questionnaire (SGRQ), as well as adrenal status, asthma exacerbations and adverse events.Results598 patients entered PONENTE; 563 (94.1%) completed the reduction phase and entered the maintenance phase. From the end of reduction to the end of maintenance, the median (range) OCS dosage was unchanged (0.0 (0.0–40.0) mg), 3.2% (n=18/563) of patients experienced daily dosage increases, the mean ACQ-6 score decreased from 1.26 to 1.18 and 84.5% (n=476/563) of patients were exacerbation free. The mean SGRQ improvement (–19.65 points) from baseline to the end of maintenance indicated substantial quality-of-life improvements. Of patients entering the maintenance phase with adrenal insufficiency, 32.4% (n=104/321) demonstrated an improvement in adrenal function. Adverse events were consistent with previous reports.ConclusionsMost patients successfully maintained maximal OCS reduction while achieving improved asthma control with few exacerbations and maintaining or recovering adrenal function.
