The aim of this study was to assess the airway volume of patients with Class III skeletal malocclusion undergoing bimaxillary orthognathic surgery. The sample consisted of 10 patients with Class III skeletal malocclusion undergoing bimaxillary orthognathic surgery. Through computed tomography, the three-dimensional volumetric measurements of the airways were determined using the Dolphin Imaging® 11.7 program and were compared between the pre- and postoperative periods. Patients were divided into two groups, (1) airway narrowing and (2) airway enlargement. Group 1 showed a mean reduction of 66.16 ± 52.48 mm² in the two-dimensional assessment of the airways and 2311.16 ± 1653.71 mm³ in the three-dimensional assessment (p≤ 0.05). On the other hand, group 2 increased 166.5 ± 73.13 mm² and 4116 ± 1323.85 mm³. There was no correlation between the values obtained with bone movement, in mm (p≥0.05). Patients with class III deformities undergoing bimaxillary surgery, most with reduced airways, however, this decrease does not always occur to the same extent for all patients. El objetivo de este estudio fue evaluar el volumen de la vía aérea de pacientes con maloclusión esquelética Clase III sometidos a cirugía ortognática bimaxilar. La muestra estuvo constituida por 10 pacientes con maloclusión esquelética clase III sometidos a cirugía ortognática bimaxilar. Mediante tomografía computarizada se determinaron las medidas volumétricas tridimensionales de las vías respiratorias mediante el programa Dolphin Imaging® 11.7 y se compararon entre los periodos pre y posoperatorio. Los pacientes se dividieron en dos grupos, (1) estrechamiento de las vías respiratorias y (2) agrandamiento de las vías respiratorias. El grupo 1 mostró una reducción media de 66,16 ± 52,48 mm² en la evaluación bidimensional de las vías respiratorias y 2311,16 ± 1653,71 mm³ en la evaluación tridimensional (p≤ 0,05). Por otro lado, el grupo 2 aumentó 166,5 ± 73,13 mm² y 4116 ± 1323,85 mm³. No hubo correlación entre los valores obtenidos con el movimiento óseo, en mm (p≥0,05). Pacientes con deformidades de clase III sometidos a cirugía bimaxilar, la mayoría con vías respiratorias reducidas, sin embargo, esta disminución no siempre ocurre en la misma medida en todos los pacientes. O objetivo deste estudo foi avaliar o volume de vias aéreas de pacientes com má oclusão esquelética Classe III, submetidos à cirurgia ortognática bimaxilar. A amostra foi composta por 10 pacientes com má oclusão esquelética Classe III, submetidos à cirurgia ortognática bimaxilar. Pacientes com deformidade classe III submetidos a cirurgia bimaxilar, a maioria com diminuição das vias aéreas, no entanto, essa diminuição nem sempre ocorre na mesma magnitude para todos os pacientes.

Orientador: Renato Mazzonetto Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: O objetivo do presente estudo in vivo foi avaliar histologicamente, de forma comparativa, o emprego de parafusos absorvíveis baseados no polímero poli(L-co-D,L ácido lático) 70:30 sintetizado por um laboratório nacional, para fixação de enxertos ósseos autógenos em tíbias de coelhos. Como controle foram utilizados parafusos à base de liga de titânio (Ti-6Al-4V/Grau V) de mesmas dimensões. Para tanto, foram selecionados 15 coelhos adultos, albinos, da raça Nova Zelândia, machos, com idade aproximada de 6 meses e peso variando entre 3,8 e 4,5 kg no momento do procedimento cirúrgico. De cada animal foram removidos 2 enxertos ósseos em espessura total da calota craniana, sendo um deles fixado à tíbia com parafuso absorvível e o outro com parafuso metálico. Os animais foram divididos em 3 grupos, de acordo com os períodos de sacrifício: 3, 8 e 16 semanas pós-operatórias. Após o processamento histológico, as lâminas foram coradas com hematoxilina e eosina e submetidas à análise histológica descritiva em microscopia óptica. Como resultado encontrou-se que o sistema de fixação baseado em polímero apresentou um comportamento histológico bastante semelhante ao sistema metálico. Em ambos os grupos o enxerto encontrava-se incorporado, ocorrendo neoformação óssea em sua interface com o leito receptor. Em nenhum dos grupos foi evidenciado processo inflamatório indesejável ou reação a corpo estranho. Baseando-se nos achados histológicos para o modelo experimental e metodologia empregada, pode-se concluir que o sistema de fixação baseado no polímero poli(L-co-D,L ácido lático) 70:30 é efetivo para a fixação de enxertos ósseos autógenos, com resultados comparáveis ao materiais à base de liga de titânio. Abstract: The aim of the present in vivo study was to evaluate histologically, by comparative means, the use of resorbable screws based on poli(L-co-D,L lactide) 70:30, manufactured by a brazillian laboratory, used for fixation of autogenous bone grafts in rabbit tibiae. As a control, titanium (Ti-6Al-4V Grade V) screws were used. For this purpose, fifteen white New Zeland male rabbits, with age of 6 months and weight between 3.8 - 4.5 Kg were used. Of each animal, 2 total thickness bone grafts were removed from the cranial vault and one of them was stabilized with resorbable screw while the other was stabilized with the metallic one. Animals were divided in 3 groups, according to the sacrifice period: 3, 8 and 16 week postoperatively. After histological processing, cuts were corated with hematoxilin and eosin and submited to descriptive histological analysis under light microscopy. As a result it was found that the fixation system based on polimer showed a histological behavior similar to the metallic system. For both groups bone graft was incorporated, with the presence of bone neoformation between this and the receptor site. In none of the groups undesirable inflammatory process or foreign body reaction was observed. Based on histological findings and respecting this experimental model it is possible to conclude that that the internal fixation system based on the poli(L-co-D,L lactide) 70:30 polymer is effective for fixation of autogenous bone grafts, offering results that are comparable the titanium fixation systems. Introduction. The bifid mandibular condyle (BMC) is an unusual temporomandibular joint (TMJ) disorder with controversial etiology. The association of this entity with ankylosis is rare. Objective. The objective of the present study is to report a case of BMC with associated TMJ ankylosis in a patient with no history of trauma and/or infection. Case Report. A 17-year-old male patient sought care reporting pain on the right TMJ region and mastication difficulty due to a severe limitation of mouth opening. In the clinic and imaging examinations, a 15 mm mouth opening and BMC associated with ankylotic mass of the right TMJ were observed, besides a facial asymmetry with chin deviation to the right. The proposed treatment plan was condylectomy on the right side, bilateral coronectomy, and genioplasty, so the chin lateral deviation could be corrected, under general anesthesia. The patient remains under clinical and imaging follow-up of two years with functional stability and no signs of relapse of the ankylosis. Conclusion. The association of BMC with ankylosis is an atypical entity which must be diagnosed and treated early to prevent aesthetic and functional damages to the patient.

The aim of this study was to report two rare cases of ectopic tooth into the nasal cavity. The first case reports a 32-year-old female patient with the main complaint of having a tooth inside her nose. According to her, this condition causes pain and sporadic nosebleed. The patient had a facial trauma when she was 6 years old. The second case refers to an 8-year-old girl with left incomplete unilateral cleft lip and palate. The main complaint was left-sided nasal obstruction by a white hard mass. The treatment for both cases was surgical removal of the ectopic erupted tooth under general anesthesia. In conclusion, we can state that the surgical removal of intranasal tooth is a safe procedure and improves patient’s quality of life.

Introduction. Class II division I malocclusions are the most common dentofacial deformities seen in clinical practice. Severe cases or cases in which growth has ceased may require full correction combining orthodontic and surgical treatment. We report a case of a total mandibular subapical alveolar osteotomy, performed to correct a class II division I dentofacial deformity. Case Report. A 19-year-old female patient was referred to the oral and maxillofacial surgery department at the Federal University of Paraná with chin aesthetic complaints as well as class II malocclusion. The proposed treatment was total mandibular subapical alveolar osteotomy, retaining the chin position and eliminating the need for genioplasty, since, although the patient presented with a class II dentofacial deformity, the chin was well positioned. Under general anesthesia, a “V-shaped” incision was conducted from the right retromolar region to the left retromolar region. A ring of cortical bone was removed around the mental foramen, with the aim to create a space around the mental nerve. Fixation was conducted with plates and screws of the 2.0 system. The patient on six-year follow-up showed osteotomy stability, a better overall occlusion, and outcome satisfaction.

No. of bitstreams: 0 Previous issue date: 2012 Objetivo: Avaliar radiograficamente a distância da língula da mandíbula até o ponto onde ocorre a fusão das corticais ósseas vestibular e lingual no ramo mandibular. Materiais e Métodos: 54 mandíbulas secas, divididas em 3 subgrupos (SG1: dentada, SG2: parcialmente dentada e SG3: desdentada), foram utilizadas neste estudo. A posição da língula foi demarcada com esfera metálica, e tomadas radiográficas foram obtidas. As distâncias entre a incisura e a língula mandibular (I/L) e entre a língula mandibular e o ponto onde ocorre a fusão das corticais ósseas no ramo mandibular (L/FC) foram mensuradas. Análise estatística pertinente foi aplicada aos valores obtidos. Resultados: a média dos valores obtidos para L/FC foram de 8,18mm, 7,30mm e 8,98mm para SG1, SG2 e SG3, respectivamente. Entretanto, os valores de I/L foram de 14,02mm, 13,90mm e 12,34mm para SG1, SG 2 e SG3, respectivamente. Os resultados também demonstram que a fusão das corticais ocorreu na metade da distância I/L em 28,57% das mandíbulas em SG1, em 46,67% das mandíbulas em SG2 e em 9,09% das peças em SG3. Conclusões: não houve diferenças significativas nas alturas em que ocorreram as fusões nos 3 subgrupos. No SG3, a correlação do valor médio de L/FC com o valor médio observado para I/L (12,34 mm) sugere uma diminuição da densidade óssea e da massa óssea, que pode estar relacionada com uma idade avançada das mandíbulas avaliadas nesse subgrupo. Purpose: to radiographically evaluate the distance between mandibular lingula and the exact spot where buccal and lingual cortical bone plates merge in the mandibular ramus. Materials and Methods: 54 dry mandibles, divided into 3 subgroups (SG1: dentate, SG2: partially dentate and SG3: edentulous) were used in this study. Lingula position was marked with a metallic sphere and radiographs were taken. The distance between mandibular notch and lingula (I/L) and the distance between mandibular lingula and cortical bone plates fusion (L/FC) were measured. Statistical analysis was applied to the values obtained. Results: mean values for L/FC were 8,18mm, 7,30mm and 8,98mm for SG1, SG2 e SG3 respectively. Moreover, mean values for I/L were 14,02mm, 13,90mm and 12,34mm for SG1, SG 2 and SG3 respectively. The results also showed that cortical bone plates fusion took place in half I/L distance in 28,57% of the mandibles in SG1, in 46,67% of the mandibles in SG2 and in 9,09% of the pieces in SG3. Conclusions: there were no statistically significant differences in the height where cortical bone plates took place in all 3 subgroups. In SG3, the correlation between the mean value for L/FC and the mean value for I/L suggests a reduction in bone density and bone mass, which can correlate to the evaluation of older mandibles in this subgroup.
