The problem of surgical treatment of postoperative ventral hernias remains relevant today. Currently, a large number of methods of surgical interventions for hernias of the anterior abdominal wall have been proposed, the variety of which requires systematization and analysis of the immediate and distant results of treatment. Goal: to analyze the effectiveness of laparoscopic hernia repair in patients with postoperative ventral hernia with a body mass index of 30kg/m2. The results of treatment of 29 patients with postoperative ventral hernia were analyzed. Men – 8 (27.5%), women – 21 (72.5%). In 16 (55.2%) patients (main group) laparoscopic intraperitoneal onlay mesh – IPOM was performed, in 13 (44.8%) patients (comparison group) open hernia repair with fixation of the mesh graft in the Sublay position was performed. The analysis of the duration of the surgical intervention indicates that with open hernia repair, there is a significant increase in the duration of the surgical intervention, p˂0.0001. Comparison of the duration of operations in both groups depending on the body mass index also indicates the dependence of the duration of surgery on this indicator. Thus, in the comparison group, there was a tendency to an increase in the duration of the operation with an increase in the body weight of patients with a significant difference in the duration of the operation between the main group and the comparison. The duration of a patient's stay in the hospital and the occurrence of respiratory complications correlates with the level of postoperative pain and the duration of surgery. Postoperative complications in the form of fluid accumulations in the wound area of the abdominal wall depend on the method of surgery and the location of the mesh graft. The use of laparoscopic – IPOM in obese patients significantly improves the results of surgery by reducing the level of postoperative pain, shortening the duration of surgery, and the absence of complications in the early and late postoperative period. When choosing the method of surgical intervention in patients with postoperative ventral hernia, laparoscopic IPOM should be preferred.

The success of scattering muon tomography has prompted us to explore cosmic ray flux attenuation, electro-magnetic shower induction, and muouic X-ray emission as additional means for the non-destructive assay of small and medium size objects. We report on the value of the signatures of these processes as an adjunct to multiple scattering muon tomography. We discuss our experiments and the Monte-Carlo calculations of these processes, the type of information that can be drawn from their signatures, the cost and the value of this information. We discuss the general requirements for detector systems to make use of cosmic ray flux attenuation, induction of electro-magnetic showers, and muonic X-rays in muon tomography. We find that the attenuation of the cosmic ray flux allows for the inference of the density distribution for objects of automobile size and smaller with exposure times of order a few minutes. Our preliminary experiments and calculations have shown that using muonic X-rays to identify composition is exceedingly difficult, requiring months of exposure. Our experiments are not yet sensitive enough to detect muonic x-rays at the rates expected from previous accelerator experiments. Some patients with chronic pancreatitis shows ductal hypertension without significant ductal dilatation, which causes difficulties in the choice of treatment (conservative and surgical), and the method of surgery (parenchyma-preserving, resection).Material and methods. One hundred and ten patients with chronic pancreatitis were operated. Among them 38 (34.5 %) had severe ductal hypertension without significant ductal dilatation. Average age was 48 years old, 34 males, 4 females. Among chronic pancreatitis forms (by A.A. Shalimov) 8 patients had calculous pancreatitis (8/38; 21.0 %), 17 persons had pseudotumor one (17/38; 44.7 %), 5 patients had cystic fibrosis (5/38; 13.2 %), 8 persons had degenerative fibrosis involving annexa (8/38; 21.1 %). Alcohol genesis was identified in 33 (33/38; 86.8 %) patients, idiopathic in 5 (5/38; 13.2 %). The patients were undergone ultrasound observation, CT-scan, endoscopy, ERCP; there were defined tumor marker CA 19-9, endogenous insulin, glycated hemoglobin, C-peptide, feces pancreatic elastase-1. there was conducted hystochemical and immunohystochemical analysis of intraoperative sections of the head, body and tail of pancreas. All the patients were undergone parenchyma-preserving surgery: longitudinal total pancreatic wirsungoduodenopapillotomy with longitudinal Roux-en-Y pancreaticojejunoduodenostomy.Results and discussion. The clinical, intraoperative and morphological data in these patients were analyzed. It is shown that the phenomenon of «ductal hypertension without ductal dilatation» in chronic pancreatitis forms due to the development of total periductal stromal fibrosis, which limits the dilatation of the ducts from the primary acinar to the main pancreatic duct. It was found that α-SMA-positive stellate cells are activated in response to the progression of ductal hypertension, their number has increased around the large and medium-sized pancreatic ducts. They aggressively penetrate from the area of periductal fibrosis in the interlobular stroma of the pancreas to form interlobular fibrous septa-tubules, as if cementing the stromal structure of the body, giving it a maximum stiffness (syndrome of mutual aggravation). As a result, the development of a combined periductal stromal fibrosis is characterized by its high severity. This group of patients is indicated to parenchyma-preserving type of operation: a longitudinal total pancreatic wirsungoduodenopapillotomy with longitudinal Roux-en-Y pancreaticojejunoduodenostomy or isolated anastomosis. A long-term result of this type of surgery in patients with chronic pancreatitis shows high quality of life on all scales of the international questionnaire MOS SF-36, especially if surgical intervention was performed timely (before the development of the exocrine and endocrine insufficiency moderate severe).Conclusion. Prompt surgical treatment as the parenchyma-preserving operation — longitudinal total pancreatic wirsungoduodenopapillotomy with longitudinal Roux-en-Y pancreaticojejunoduodenostomy is indicated for the chronic pancreatitis patients with ductal hypertension without ductal dilatation., По поводу хронического панкреатита оперированы 110 больных, у 38 (34,5 %) из них наблюдалась выраженная протоковая гипертензия без существенной протоковой дилатации. Представлен анализ клинических, интраоперационных и морфологических данных у этой категории пациентов. Показано, что феномен протоковой гипертензии без протоковой дилатации при хроническом панкреатите обусловлен развитием жесткого тотального перидуктально-стромального фиброза, ограничивающего расширение протоков — от первичных ацинарных до главного панкреатического. Данной группе больных показан паренхимосохраняющий тип операции: продольная тотальная панкреатовирсунгодуоденопапиллотомия с продольной панкреатоеюнодуоденостомией по Ру или с вариантом изолированного анастомоза. Отдаленные результаты такого типа операции у больных с хроническим панкреатитом определяют высокое качество жизни по всем шкалам международного опросника MOS SF-36, особенно при условии своевременного оперативного вмешательства (до развития экзо- и эндокринной недостаточности средней/тяжелой степени)., З приводу хронічного панкреатиту оперовані 110 хворих, у 38 (34,5 %) із них спостерігалася виражена протокова гіпертензія без істотної протокової дилатації. Представлено аналіз клінічних, інтраопераційних і морфологічних даних у цієї категорії пацієнтів. Даній групі хворих показаний паренхімозберігаючий тип операції: поздовжня тотальна панкреатовірсунгодуоденопапілотомія з поздовжньою панкреатоєюнодуоденостомією за Ру або з варіантом ізольованого анастомозу. Віддалені результати такого типу операції у хворих на хронічний панкреатит визначають високу якість життя за всіма шкалами міжнародного опитувальника MOS SF-36, особливо за умови своєчасного оперативного втручання (до розвитку екзо- та ендокринної недостатності середнього/тяжкого ступеня).

To be submitted to Phys Rev C, 15 pages, 18 figures; International audience; Electroproduction of exclusive $\phi$ vector mesons has been studied with the CLAS detector in the kinematical range $1.6\leq Q^2\leq 3.8$ GeV$^{2}$, $0.0\leq t^{\prime}\leq 3.6$ GeV$^{2}$, and $2.0\leq W\leq 3.0$ GeV. The scaling exponent for the total cross section as $1/(Q^2+M_{\phi}^2)^n$ was determined to be $n=2.49\pm 0.33$. The slope of the four-momentum transfer $t'$ distribution is $b_{\phi}=0.98 \pm 0.17$ GeV$^{-2}$. The data are consistent with the assumption of s-channel helicity conservation (SCHC). Under this assumption, we determine the ratio of longitudinal to transverse cross sections to be $R=0.86 \pm 0.24$. A 2-gluon exchange model is able to reproduce the main features of the data.

The exclusive electroproduction process $\vec{e}p \to e^\prime n \pi^+$ was measured in the range of the photon virtuality $Q^2 = 1.7 - 4.5 \rm{GeV^2}$, and the invariant mass range for the $n\pi^+$ system of $W = 1.15 - 1.7 \rm{GeV}$ using the CEBAF Large Acceptance Spectrometer. For the first time, these kinematics are probed in exclusive $\pi^+$ production from protons with nearly full coverage in the azimuthal and polar angles of the $n\pi^+$ center-of-mass system. The $n\pi^+$ channel has particular sensitivity to the isospin 1/2 excited nucleon states, and together with the $p\pi^0$ final state will serve to determine the transition form factors of a large number of resonances. The largest discrepancy between these results and present modes was seen in the $\sigma_{LT'}$ structure function. In this experiment, 31,295 cross section and 4,184 asymmetry data points were measured. Because of the large volume of data, only a reduced set of structure functions and Legendre polynomial moments can be presented that are obtained in model-independent fits to the differential cross sections.

Differential cross sections for the reaction $\gamma p \to K^{*0} \Sigma^+$ are presented at nine bins in photon energy in the range from 1.7 to 3.0 GeV. The \kstar was detected by its decay products, $K^+\pi^-$, in the CLAS detector at Jefferson Lab. These data are the first \kstar photoproduction cross sections ever published over a broad range of angles. Comparison with a theoretical model based on the vector and tensor $K^*$-quark couplings shows good agreement with the data in general, after adjusting the model's two parameters in a fit to our data. Disagreement between the data at forward angles and the global angle-energy fit to the model suggests that the role of scalar $\kappa$ meson exchange in $t$-channel diagrams should be investigated., Comment: 7 pages, 5 figures, submitted to Phys. The ep -> e'pi^+n reaction was studied in the first and second nucleon resonance regions in the 0.25 GeV^2 < Q^2 < 0.65 GeV^2 range using the CLAS detector at Thomas Jefferson National Accelerator Facility. For the first time the absolute cross sections were measured covering nearly the full angular range in the hadronic center-of-mass frame. The structure functions sigma_TL, sigma_TT and the linear combination sigma_T+epsilon*sigma_L were extracted by fitting the phi-dependence of the measured cross sections, and were compared to the MAID and Sato-Lee models., Comment: Accepted for publication in PRC

The reaction $\gamma p \to pK^+K^-$ was studied at Jefferson Lab with photon energies from 1.8 to 3.8 GeV using a tagged photon beam. The goal was to search for a $\Theta^{++}$ pentaquark, a narrow doubly charged baryon state having strangeness S=+1 and isospin I=1, in the $pK^+$ invariant mass spectrum. No statistically significant evidence of a $\Theta^{++}$ was found. Upper limits on the total and differential production cross section for the reaction $\gamma p \to K^-\Theta^{++}$ were obtained in the mass range from 1.5 to 2.0 GeV/c$^2$, with an upper limit of about 0.15 nb, 95% C.L. for a narrow resonance with a mass $M_{\Theta^{++}}=1.54$ GeV/c$^2$. This result places a very stringent upper limit on the $\Theta^{++}$ width., Comment: 6 pages, 4 figures

PHASE; Inclusive electron scattering off the deuteron has been measured to extract the deuteron structure function F2 with the CEBAF Large Acceptance Spectrometer (CLAS) at the Thomas Jefferson National Accelerator Facility. The measurement covers the entire resonance region from the quasielastic peak up to the invariant mass of the final-state hadronic system W~=2.7 GeV with four-momentum transfers Q2 from 0.4 to 6 (GeV/c)2. These data are complementary to previous measurements of the proton structure function F2 and cover a similar two-dimensional region of Q2 and Bjorken variable x. Determination of the deuteron F2 over a large x interval including the quasielastic peak as a function of Q2, together with the other world data, permit a direct evaluation of the structure function moments for the first time. By fitting the Q2 evolution of these moments with an OPE-based twist expansion we have obtained a separation of the leading twist and higher twist terms. The observed Q2 behavior of the higher twist contribution suggests a partial cancelation of different higher twists entering into the expansion with opposite signs. This cancelation, found also in the proton moments, is a manifestation of the "duality" phenomenon in the F2 structure function.

Over 120 million vehicles enter the United States each year. Many are capable of transporting hidden nuclear weapons or nuclear material. Currently deployed X-ray radiography systems are limited because they cannot be used on occupied vehicles and the energy and dose are too low to penetrate many cargos. We present a new technique that overcomes these limitations by obtaining tomographic images using the multiple scattering of cosmic radiation as it transits each vehicle. When coupled with passive radiation detection, muon interrogation could contribute to safe and robust border protection against nuclear devices or material in occupied vehicles and containers.
