Abstract

BackgroundNationwide audits facilitate quality and outcome assessment of pancreatoduodenectomy. Differences may exist between countries but studies comparing nationwide outcomes of pancreatoduodenectomy based on audits are lacking. This study aimed to compare the German and Dutch audits for external data validation.MethodsAnonymized data from patients undergoing pancreatoduodenectomy between 2014 and 2016 were extracted from the German Society for General and Visceral Surgery StuDoQ|Pancreas and Dutch Pancreatic Cancer Audit, and compared using descriptive statistics. Univariable and multivariable risk analyses were undertaken.ResultsOverall, 4495 patients were included, 2489 in Germany and 2006 in the Netherlands. Adenocarcinoma was a more frequent indication for pancreatoduodenectomy in the Netherlands. German patients had worse ASA fitness grades, but Dutch patients had more pulmonary co-morbidity. Dutch patients underwent more minimally invasive surgery and venous resections, but fewer multivisceral resections. No difference was found in rates of grade B/C postoperative pancreatic fistula, grade C postpancreatectomy haemorrhage and in-hospital mortality. There was more centralization in the Netherlands (1·3 versus 13·3 per cent of pancreatoduodenectomies in very low-volume centres; P < 0·001). In multivariable analysis, both hospital stay (difference 2·49 (95 per cent c.i. 1·18 to 3·80) days) and risk of reoperation (odds ratio (OR) 1·55, 95 per cent c.i. 1·22 to 1·97) were higher in the German audit, whereas risk of postoperative pneumonia (OR 0·57, 0·37 to 0·88) and readmission (OR 0·38, 0·30 to 0·49) were lower. Several baseline and surgical characteristics, including hospital volume, but not country, predicted mortality.ConclusionThis comparison of the German and Dutch audits showed variation in case mix, surgical technique and centralization for pancreatoduodenectomy, but no difference in mortality and pancreas-specific complications.

Abstract

The effects of aging on the levels of the vasopressin V2 receptor messenger RNA and adenosine 3'5'-cyclic monophosphate (cAMP) accumulation were analyzed in rat medullary thick ascending limb of Henle's loop (MTAL). The vasopressin-induced accumulation of cAMP was proportional to the amount of MTAL in the presence of 3-isobutyl-1-methylxanthine (IBMX). Age-related changes in vasopressin response also reduced cAMP accumulation in response to the maximal stimulation of the V2 receptor.

Abstract

Apresento a análise de ementas de disciplinas da licenciautra em Matemática e Pedagogia, a qual constituiu uma das etapas da pesquisa que desenvolvi no doutorado. Os resultados tomam como fonte de produção de dados a análise documental de Projetos Pedagógicos de Cursos (PPC’s) de Universidades públicas e privadas do estado de Mato Grosso do Sul (MS) em relação as aspectos do conhecimento da docência. O referencial teórico explora as bases de conhecimento para o ensino com destaques para o conhecimento específico e pedagógico de conteúdo. Faço referência, na seção metodológica, às docentes egressas dos cursos, haja vista que este processo transcorreu em decorrência do trabalho com um grupo de professoras iniciantes e suas relações com a Matemática. Os resultados indicam que a formação de professores, em ambas licenciaturas, carece de uma maior articulação entre o específico e pedagógico, sinalizando a necessidade pensar o lugar da docência e das práticas pedagógicas.

Abstract

With increasing cultural diversity in North America, ensuring that healthcare professionals are able to provide appropriate care to individuals from a wide range of cultural backgrounds, commonly described as “cultural competency”, has become a priority issue within the healthcare system. More specifically, culturally competent end-of-life care is essential for clinicians to prioritize as the individualized needs of older patients and their families, regardless of cultural background or ethnicity, are unique and context driven. The aim of this study was to explore the usefulness of “cultural competency” as a way to improve the ability of healthcare practitioners to meet the healthcare needs of elderly Chinese patients at the end-of-life in Canada. Using a qualitative research design, 49 in-depth interviews with 23 participants were conducted. Our results are organized into six vignettes, which illustrate how “Chinese culture” is not a commonly understood or experienced phenomenon, but that the individual ways in which “culture” is interpreted by patients and healthcare providers around issues of death and dying. Furthermore, the results from this study challenge the assumption that all members of an ethnic or cultural group share similar views on end-of-life and palliative care. Finally, how and why cultural explanations are used to justify health care-related requests and decisions, at the end-of life are explored. Not all “Chinese patients” had the same “cultural” needs or sensitivities and underscore the need for good communication with patients and their family members to facilitate healthcare providers’ understanding of patients’ needs and values regardless of their ethnic background.
