Künstliche Intelligenz (KI) ist noch weit davon entfernt, eine „allgemeine künstliche Intelligenz“ zu erreichen, d.h. die Fähigkeit, eine Vielzahl von kognitiven Aufgaben zu bewältigen, die auch Menschen bewältigen können (Acemoglu & Restrepo, 2020; Malone, Rus & Laubacher, 2020). Dennoch gelingt es verschiedenen KI-Anwendungen, wie Machine Learning, Robotics, Computer Vision und Natural Language Processing, bereits, komplexe Problemlösungsaufgaben auszuführen; Sprachassistenz, Bilderkennung und algorithmische Personalauswahlverfahren.

Artificial Intelligence (AI) is yet nowhere near retrieving “artificial general intelligence”: the ability to undertake a wide variety of cognitive tasks that humans can (Acemoglu & Restrepo, 2020; Malone, Rus & Laubacher, 2020). Nevertheless, different fields of AI, such as machine learning, robotics, computervision, and natural language processing, are already succeeding in performing complex problem-solving tasks as humans would normally do; voice assistants,image recognition, and algorithmic hiring1 are just a few examples. Although the effects of AI implementation on labour yet remains unclear2, it is essential tounderstand how workers perceive its impact on their work and related outcomes.

Background: Trichophyton mentagrophytes (formerly Arthroderma vanbreuseghemii) and its clonal offshoot Trichophyton interdigitale, which are leading causes of dermatophytoses, have recently been recognized as two separate species. Over the last 20 years, several internal transcribed spacer (ITS) genotypes of Trichophyton mentagrophytes and Trichophyton interdigitale have been identified, some of which have specific characteristics and lead to typical clinical manifestations. Objectives: The aim of this study was to determine the current epidemiology of Trichophyton mentagrophytes and Trichophyton interdigitale genotypes in Switzerland, particularly in the Zurich area. Methods: Consecutive cases diagnosed by ITS sequencing between 2009 and 2019 were retrospectively analysed. Results: A total of 81 Trichophyton mentagrophytes and 81 Trichophyton interdigitale cases were investigated. T. mentagrophytes infections clearly differed from T. interdigitale infections by affecting younger and more frequently female patients, targeting almost exclusively head and body rather than feet and toenails, leading to inflammatory dermatophytosis and often requiring a combination of systemic and topical treatment. Seven different T. mentagrophytes genotypes (II*, III, III*, IV, VII, VIII and XXVI) were observed, with genotype XXVI being discovered in this study. Genotype III occurred most frequently (56% of all T. mentagrophytes cases) and affected predominantly children. Genotypes III* and VII led to inflammatory tinea in most cases. Four strains that proved to be terbinafine resistant belonged to the 'Indian genotype' VIII, which mostly caused tinea glutealis and inguinalis. Conclusion: Being able to distinguish between Trichophyton mentagrophytes and Trichophyton interdigitale is of paramount importance as the two species cause different clinical presentations. In addition, ITS genotyping allows recognizing sources of infection and potential terbinafine resistance. The latter needs to be confirmed by resistance testing or by sequencing part of the squalene epoxidase (SQLE) gene.

The study aims to evaluate the performance of selected, high-volume, highly specialized, Italian Breast Centers at the time of COVID-19 pandemic (year 2020), compared to pre-pandemic time (year 2019), highlighting differences in terms of clinical presentation of breast cancer (BC) and therapeutic strategies.Patients' data were provided by the Senonetwork data warehouse Senonet. In order to examine changes in the surgical and oncological management of BC patients during different phases of COVID-19 pandemic, we took advantage of a selection quality indicators (QIs). We performed the analyses in two time-frames, from July to September (Jul-Sep) (2019 versus 2020) and from October to December (Oct-Dec) (2019 versus 2020).Our analysis did not show any statistically significant difference in terms of diagnosis, surgical, oncological and radiation therapy procedures between the two trimesters 2019 and 2020. Nevertheless, we observed statistically significant differences, favoring 2020, when analyzing time-to surgery and time-to radiotherapy. On the other hand, we observed a significant reduction of neoadjuvant chemotherapy and we did not recollect any data on a major use of neoadjuvant endocrine therapy.In Italian Breast Centers, partners of Senonetwork, we could not observe any treatment delay or change in standard clinical practice for BC care during the 2020 pandemic year, compared to 2019 pre-pandemic year. This finding is in contrast with the globally reported decrease in the performance of the Italian Breast Centers due to the COVID-19 pandemic, and has to be linked to the sharp selection of Senonetwork Breast Centers.

Lung congestion is a risk factor for all-cause and cardiovascular mortality in patients on chronic hemodialysis, and its estimation by ultrasound may be useful to guide ultrafiltration and drug therapy in this population. In an international, multi-center randomized controlled trial (NCT02310061) we investigated whether a lung ultrasound-guided treatment strategy improved a composite end point (all-cause death, non-fatal myocardial infarction, decompensated heart failure) vs usual care in patients receiving chronic hemodialysis with high cardiovascular risk. Patient-Reported Outcomes (Depression and the Standard Form 36 Quality of Life Questionnaire, SF36) were assessed as secondary outcomes. A total of 367 patients were enrolled: 183 in the active arm and 180 in the control arm. In the active arm, the pre-dialysis lung scan was used to titrate ultrafiltration during dialysis and drug treatment. Three hundred and seven patients completed the study: 152 in the active arm and 155 in the control arm. During a mean follow-up of 1.49 years, lung congestion was significantly more frequently relieved in the active (78%) than in the control (56%) arm and the intervention was safe. The primary composite end point did not significantly differ between the two study arms (Hazard Ratio 0.88; 95% Confidence Interval: 0.63-1.24). The risk for all-cause and cardiovascular hospitalization and the changes of left ventricular mass and function did not differ among the two groups. A post hoc analysis for recurrent episodes of decompensated heart failure (0.37; 0.15-0.93) and cardiovascular events (0.63; 0.41-0.97) showed a risk reduction for these outcomes in the active arm. There were no differences in patientreported outcomes between groups. Thus, in patients on chronic hemodialysis with high cardiovascular risk, a treatment strategy guided by lung ultrasound effectively relieved lung congestion but was not more effective than usual care in improving the primary or secondary end points of the trial.

Extreme High-frequency BL~Lacs (EHBL) feature their synchrotron peak of the broadband spectral energy distribution (SED) at $\nu_{\rm s} \geq $10$^{17}$\,Hz. The BL~Lac object 1ES~2344+514 was included in the EHBL family because of its impressive shift of the synchrotron peak in 1996. During the following years, the source appeared to be in a low state without showing any extreme behaviours. In August 2016, 1ES~2344+514 was detected with the ground-based $\gamma$-ray telescope FACT during a high $\gamma$-ray state, triggering multi-wavelength (MWL) observations. We studied the MWL light curves of 1ES~2344+514 during the 2016 flaring state, using data from radio to VHE $\gamma$ rays taken with OVRO, KAIT, KVA, NOT, some telescopes of the GASP-WEBT collaboration at the Teide, Crimean, and St. Petersburg observatories, \textit{Swift}-UVOT, \textit{Swift}-XRT, \textit{Fermi}-LAT, FACT and MAGIC. With simultaneous observations of the flare, we built the broadband SED and studied it in the framework of a leptonic and an hadronic model. The VHE $\gamma$-ray observations show a flux level of 55\% of the Crab Nebula flux above 300\,GeV, similar to the historical maximum of 1995. The combination of MAGIC and \textit{Fermi}-LAT spectra provides an unprecedented characterization of the inverse-Compton peak for this object during a flaring episode. The $\Gamma$ index of the intrinsic spectrum in the VHE $\gamma$-ray band is $2.04\pm0.12_{\rm stat}\pm0.15_{\rm sys}$. We find the source in an extreme state with a shift of the position of the synchrotron peak to frequencies above or equal to $10^{18}$\,Hz, Comment: Accepted in MNRAS. Corresponding authors: M.
