Son yıllarda minimal doku yaralanması ile gerçekleştirilebilecek bir cerrahi işlem için çok çeşitli yöntemler geliştirilmiştir. Ayrıca bu cerrahi yöntemlerin postoperatif ağrı, analjezik ihtiyacı, kanama, ödem oluşumu, cerrahi operasyonun süresi, yara iyileşmesi, nüks ihtimali, çevre dokulara gelen zarar, cerrahi yöntemin kolaylığı ve maliyeti gibi avantaj ve dezavantajları vardır. Bu çalışmada lazer cerrahisi ve elektrocerrahinin birbirlerinden farklarını ve birbirlerine karşı üstünlüklerini açığa kavuşturmak hedeflenmiştir. Çalışmamızda oral yumuşak dokunun reaktif bir lezyonu olan epulis fissuratum eksizyonu yapılmıştır. 17 hasta lazer cerrahisi ile 17 hasta da elektrocerrahi yöntemi ile tedavi edilmiştir. Çalışmaya toplam 34 hasta dahil edilmiştir. Hastalarda en çok rahatsızlık oluşturan şikayetler, parametrelerimiz olarak belirlenmiştir. Ağrı, gerginlik hissi, analjezik ihtiyacı ve reepitelizasyon süresi çalışmamızın parametreleridir.Hastaların postoperatif ağrı, gerginlik, analjezik ihtiyacı ve kullanılan toplam analjezik miktarları kendilerine verilen bir form aracılığıyla görsel ve sözsel skalalar yardımıyla doldurulmuştur. Hastalar, postoperatif 3., 7., 14., 21. ve 30. günlerde klinik kontrollere çağırılarak reepitelizasyonun tamamlandığı gün takip edilmiştir.Postoperatif ağrı açısından bu iki yöntem arasında anlamlı bir fark bulunamamıştır. Her iki cerrahi yöntem sonrasında da kabul edilebilir sınırlar içerisinde ve yaklaşık olarak eşit düzeyde ağrı şikayeti olmuştur.Her iki gruptaki hastalar da postoperatif dönemde hiç gerginlik hissetmediklerini kaydetmişlerdir. Reepitelizasyon modeli her iki yöntemde de tabandan tavana doğru geliştiği için vestibular sulkusta sığlaşma olmamıştır. Herhangi bir skar gelişimi gözlenmemiştir.Postoperatif dönemde ihtiyaç duyulan ve kullanılan toplam analjezik miktarları açısından da lazer cerrahisi ve elektrocerrahi yöntemleri arasında anlamlı bir fark bulunamamıştır.Klinik olarak takip edilen hastaların reepitelizasyon sürelerinin her iki grup için de ortalama olarak 21. günde tamamlandığı gözlenmiştir.Anahtar Sözcükler: Ağız içi, Elektrocerrahi, Lazer, Patoloji, Reepitelizasyon In recent years, a wide variety of methods have been developed for a surgical procedure that can be performed to a minimal tissue injury. These surgical methods have some advantages and disadvantages such as postoperative pain, analgesic requirement, bleeding, edema, wound healing, risk of infection, risk of recurrence, the damage to surrounding tissues, ease of surgery and cost of surgical procedure. In this study, the difference of laser surgery and electrosurgery and their superiority against each other was intended to be clarified. In our study, epulis fissuratum excision was carried out which is a reactive lesion of oral soft tissues. 17 patients were treated with laser surgery and 17 patients were treated with electrosurgery. 34 patients in total were included in the study.Most complaints that discomfort the patients was determined as the parameters of our study; pain, contraction, analgesic requirements and reepithelization time.An evaluation form on postoperative pain, contraction, analgesic requirement and total amount of analgesic used was filled by patients with the help of visual and verbal scales. Patients were monitored on the reepithelialization day by inviting them to clinical controls on 3., 7., 14., 21. and 30. postoperative days. No significant difference was found between laser surgery and electrosurgery. After both surgical procedures, there was approximately equal levels of pain and both within acceptable limits.During the postoperative period, both groups reported that they have no feeling of contraction. Since the reepithelialization model develops from basal layer to upper layer in both methods, there has been no shoaling in the vestibular sulcus. No scarring was observed.No significant difference was found between laser surgery and electrosurgery in terms of needed and used total amount of analgesic in the postoperative period.Reepithelization duration of clinically monitored patients was observed to be completed in 21. day on average for both groups.Keywords: Electrosurgery, Intraoral, Laser, Pathology, Reepithelization 125
