回復期リハビリテーション病棟の脳卒中患者46名を対象に「家事再開予測モデル」の外的妥当性を検証した．回復期病棟退院時に，家事再開予測モデルを用いて退院後の家事6項目（食事の用意，食事の後片付け，洗濯，掃除や整頓，力仕事，買い物）の再開状況を予測した．退院3ヵ月後にフォローアップを行い，実際の家事再開状況を調査した．家事再開予測モデルは項目ごとに判別的中率を算出し，ROC曲線のAUC にてモデルの予測能を評価した．結果，各家事項目における判別的中率は75.0～82.2%，AUCは0.71～0.86であった．本研究により，家事再開予測モデルは中等度の予測能を有していることが示された．, We examined the external validation of the Prediction for Resuming Domestic Chores After Stroke (PRDCAS) models in forty-six stroke patients admitted to a recovery rehabilitation ward. First, we used the PRDCAS models to predict whether each stroke patient resumed the six items of domestic chores (preparing meals, washing dishes, washing clothes, light housework, heavy housework, and local shopping) after discharge. Next, we investigated resumption status of domestic chores three months after discharge. After that, the prediction accuracy of each model was calculated, and the discrimination was assessed using the Area Under the Curve of Receiver Operator Characteristic curve. As a result, the prediction accuracy for each item ranged from 75. 0 to 82. 2%, and the AUC ranged from 0. 71 to 0. 86. 本研究の目的は，一人暮らしをしている男性高齢者を対象とした社会参加プログラムを開発するために，人との交流に関する構造を明らかにし，ニーズを解明することである．A地区に在住の65歳以上の一人暮らし男性高齢者10名を対象とし，半構造化面接を行った．逐語録をKJ法にて構造化・図解化・叙述化した．その結果，一人暮らし男性高齢者は，人と【自然な関係の維持】をしたり，【自分のコミュニティ狭小化】を食い止め【つながりの保全】を得ることが課題と思われた．この課題を解決するために，人との交流に関する方略である【支援を受けるハードル】と【暮らし再建の保留】に，潜在的な支援のニーズがあると考えられる．, This qualitative study focused on how to determine the strategies for improving the social interactions of elderly males living alone and highlighted the significance of developing an effective social participation program. For these purposes, semi-structured interviews were conducted with 10 elderly males living alone. The collected data were subsequently analyzed according to the affinity diagram. Results revealed that the males had various tasks such as maintaining natural relationships, establishing and maintaining connections, and preventing one’s community from becoming too narrow. Subsequently, certain aspects were identified, including resistance to support and opposition to rebuilding their living situations. 本研究の目的は，わが国における地域在住高齢者の IADL 維持・低下に関連する要因をシステマティックレビューにより明らかにし，統合的な見解を得ることである。医中誌 Web および MEDLINE を用いてわが国における地域在住高齢者の IADL 維持・低下に関連する要因を観察研究により検討した論文を選定した。その後，選定基準を満たした論文から IADL 維持・低下に関連する要因を抽出しカテゴリ化を行った。結果，対象論文は 35 件であった。これらから 45 個の要因が抽出され，「基本属性」，「健康状態」，「身体・認知機能」，「心理面」，「活動能力」，「環境」，「ライフスタイル」，「社会参加」,「成人期の労働経験」といった 9 つのカテゴリに整理された。本レビューは地域在住高齢者のIADL 維持に向けた施策およびプログラム開発の一助になると考える。, The purpose of this study is to conduct a systematic review of factors associated with the maintenance and decline of IADL among community-dwelling elderly people in Japan to obtain an integrated view. We extracted literatures which examined factors associated with the maintenance and decline of IADL among community-dwelling elderly people using “Ichushi-Web” and “MEDLINE” and each factor was categorized according to those similarity. As a result, 45 factors were extracted from 35 literatures, and 9 categories such as demographics, health status, physical and cognitive functions, psychological aspects, activities, environment, lifestyle, social participation and experiences of adulthood were generated. 本研究は埼玉県さいたま市岩槻区で一人暮らしをしている要支援の女性高齢者が人との交流を求める構造を明らかにすることを目的とした．要支援の一人暮らし女性高齢者10名を対象に半構造化面接を行い，KJ 法に準拠して分析した．その結果，対象となった要支援の一人暮らし女性高齢者は心身に不安を持ちながら生活する中で【工夫できる・できないを一人で判断する】という方略を持ち，その方略が【自分一人のための価値ある毎日を過ごす】という要因と，【自分の役割を実感したい】という要因を促進・抑制する構造となっていた．この構造を踏まえ，要支援の一人暮らし女性高齢者が自身の役割を実感できるような活動や，人と交流するための方略に支援が必要であることが示唆された．, This research aimed to determine the structure that elderly females who live alone and require support need for social interaction. We included 10 elderly females who required support and lived alone in Iwatsuki-ward, Saitama Prefecture. Next, we conducted semistructured interviews and analyzed the collected data according to the KJ method. Results showed that these females had requirements such as “spending valuable time every day for themselves” and “wanting to realize their own role” for them to enjoy interacting with people. 要旨 本研究では，被災高齢者を対象に，活動と参加に焦点を当てた作業療法プログラムを実施し，プログラムへの参加が生活課題に及ぼす影響を検証することと，被災高齢者に対する活動と参加に焦点を当てた作業療法プログラムの実行可能性を検討することを目的とした．東日本大震災後に恒久住宅での生活を開始した高齢者を対象に週1回120分のプログラムを全14回実施した．その結果，プログラムの参加継続率は90％以上と高く，プログラムに対する感想からは参加者のニーズを満たす内容であったと伺える反応が得られた．また，プログラム前後で各参加者が認識する生活課題および健康関連QOLに肯定的な変化が認められ，中等度以上の効果量が確認された．, The purpose of this study was to clarify the effect of the program focused on activities/participation for the affected elderly, and the feasibility of conducting the large-scale program. A program of 120 minutes was conducted once a week for the elderly people who started living in permanent housing after the Great East Japan Earthquake, and 14 times in all. As a result, the participation rate of the program was as high as 90% or more, and responses were shown to meet the needs of the participants. Positive changes were recognized before and after the program in the agendas in daily living and HRQOL that the participants recognized, and the effect size was moderate to large and the accuracy was also high. We interviewed 20 stroke clients living in home, and used modified-grounded theory approach (M-GTA) to analyze the data. They were assigned randomly to: an experimental group receiving a 15–session MOHO program consisting of lectures and seminars about personal causation, values, interests, roles, habits, motor skills, process skills, communication and interaction skills, and environment; a control group which received a 15–session crafts program or no treatment. Quality of life was compared between groups using the MOS 36–Item Short–Form Health Survey (SF–36), WHO/QOL–26 (QOL26) and Life Satisfaction Index Z. The Fisher's exact test and the t-test were used to assess differences between the twogroups.Results The follow-up rates for the experimental and control groups were 71％ and 72％. The experimental group comprised 80 people (mean age±SD, 71.1±4.68 years) and the control group 79 (71.4±4.66 years). Mean change of the following items for the experimental group was significantly greaterfrom that of the control group: a BP of SF–36 (P＝0.05); and an environment score of QOL26 (P＝0.02).Conclusion The findings provide evidence that the MOHO program can meet coping and influence needs, thus improving QOL. We further conclude that the MOHO program is an effective interventional method for improvement of literacy about health and occupation.

作業療法領域の研究において,脳卒中者へインタビューを用いてどのようなテーマで何考察しているかを明らかにする目的で,脳卒中者を対象としてインタビューを用いた研究についてレビューを行った.その結果,7つの研究テーマと3つの作業療法実践への示唆を見出した.研究者らは,脳卒中者が発症後の生活をどのように経験し意味づけているかを時間経過の変化を含めて扱おうとし,それに対する作業療法の主観的効果に関心を持っていた.また,それらを対象者や介護者の語りから理解しようとし,彼らとともに個別的支援計画を進める作業療法の特色や専門職としての特徴が考察されていたが,同時に今後の課題も明らかにされた., The pupose of this study was to investigate the kinds of the research questions Occupational Therapy researchers deal with using interviews for stroke clients. We examined English and Japanese Journals of Occupational Therapy Using web searches and hit 33 literatures. We formed categories from the research questions and suggestions for Occupational Therapy clinical practice. As a result, we discovered seven categories of research questions and three of suggestions. The researchers had interests in the stroke clients' lived experiences and the value they estimate of Occupational Therapy.

わが国のOTにおいて,脳血管障害者の心理社会面に対して行われている支援内容を明らかにすることを目的として文献的研究を行った.過去10年分の論文,日本作業療法学会演題から,OTRが行った支援内容が記述された箇所をラベルとして抽出し,KJ法によってグループ編成を行った.1. 障害とともに生きていくための支援を行う,2. 作業を介した治療的関わりを行う,3. クライエント-OARの協業的関係を築く,4. クライエント・家族の生活環境づくりを行う,の4つの大カテゴリーが見出された.1が本支援内容のコアカテゴリーであると考えられた.脳血管障害者の心理社会面について,本結果の内容を組み込んだOTを計画し実践する必要がある., The purpose of this study was to investigate the kinds of psychosocial supports occupational therapists provide for stroke clients. We examined Japanese journals of occupational therapy from 1995 to 2004 and formed categories by using the KJ method. As a result, we discovered four typical categories: (1) support to live with disability, (2) use of therapeutic activities effectively, (3) establishment of client-therapist collaborative relationships, (4) providing comfortable environments for clients and families. We regard category (1) as the core category. In conclusion, it is necessary for occupational therapists to understand the psychosocial supports necessary for stroke clients.
