2. Dental approaches in children with congenital heart disease treated under general anesthesia for oral rehabilitation.

Abstract

Background: Children with congenital heart disease (CHD) are at high risk of contracting oral diseases. The aim of this study is to investigate dental procedures to prevent the risk of infective endocarditis in children with CHD. Material and Methods: 146 patients aged 2-14 years, in need of prophylaxis before cardiovascular surgery and who had filled out anamnesis records, were considered. Dental caries in all the children with CHD was reported as the number of decayed, missing and filled teeth (DMFT). Results: There was a significant strong positive relationship between the pre-oral rehabilitation DMF-T/dmf-t scores and the number of caries patients (r=0.95, p=0.01). There was no significant correlation between the preoral rehabilitation DMF-T/dmf-t scores and both tooth loss (r=0.14, p=0.09) and the number of restorations (r=0.11, p=0.17). In addition, there was no significant correlation between the post-oral rehabilitation DMF-T/dmf-t scores and the prevalence of dental caries. A positive and moderately strong correlation was found between the post-oral rehabilitation DMF-T/dmf-t scores and the number of missing teeth (r=0.56, p=0.01), while there was a positive and strong relationship between the post-treatment DMF-T/dmf-t scores and the number of fillings (r=0.62, p=0.01). Conclusions: Extraction should be considered when providing oral rehabilitation, rather than endodontic and deep restorative treatments. [ABSTRACT FROM AUTHOR]

4. The Effect of Restoration Polymerization and Residual Dentine Thickness on Thermal Changes of Pulp Chamber of Immature Permanent Teeth.

Abstract

Objective: To evaluate the pulpal temperature changes due to the polymerisation of resin and glass ionomer based materials in dentine thicknesses in immature permanent teeth with open apices. Material and Methods: Forty extracted sound human third molar teeth with open apices were included. The width of the cavities prepared on the occlusal surface was 4×5 mm. The depth was 2 mm in the resin groups. 4 mm in the groups in which glass ionomer liner was applied before composite restoration. The coronal parts of the samples were then placed on an acrylic plate with three gaps for feeding-extraction needles and the thermocouple. The temperature changes were recorded. The data was analyzed by SPSS. Statistical significance was accepted as p<0.05. Results: The temperature increase in the group of 1 mm remaining dentin thickness revealed higher results than the values detected from the 2 mm group (1.01 °C) (p=0.00). The mean values (1.49 °C, 1mm) of temperature changes in only glass ionomer applied group were lower than the avarage values (2.210°C, 1mm) determined in the polymerization process of resin composites with light-emitting diode devices. Conclusion: In a remaining dentin thickness of 1 mm in teeth with open apices, using a glass ionomer liner might be a useful effort for protecting the pulp from the heat generated by polymerisation devices. Abstract

Çalışmanın amacı; klinik muayene ile hasta velisi ifadesine bakılarak nokturnal bruksizm teşhisi konulan çocuklarda sert ve yumuşak oklüzal splint tedavisinin bruksizm semptomları ile bruksizm şiddetini gösteren Bitestrip® skorları üzerindeki etkinliğinin karşılaştırılmasıdır. Çalışma diş sıkma ve gıcırdatma şikayetine sahip çocuklar arasından splint için uygun olan nokturnal bruksizm varlığı tespit edilen yaşları 6-11 arasında değişen 40 çocuk üzerinden gerçekleştirilmiştir. Öncelikle sert ve yumuşak oklüzal splint kullanacak gruplar arasında Thorpy kriterlerine göre standardizasyon sağlanmış ve/veya diş sıkma ve diş gıcırdatma alışkanlığı olan çocukların ısırma şiddetleri Bitestrip® ile ölçülmüştür. Bundan sonraki aşamada gruplar 3 ay süre ile geceleri oklüzal splint kullanmış ve bu 3 ayın sonunda ısırma şiddetleri tekrar Bitestrip® ile ölçülmüştür. Çalışma sonucunda yumuşak oklüzal splint; sert oklüzal splinte göre bruksizmin olumsuz etkilerini azaltmış fakat bu durum Bitestrip® sonuçlarına yansımadığı görülmüştür. Buna göre çocuklarda üzerinde çok az çalışma bulunan nokturnal bruksizm etiyolojini araştıran daha fazla çalışma yapılmasının gerekli olduğu ve bu çalışmalardan elde edilen bilgiler ışığında splint uygulamasının da içinde olduğu farklı tedavi yöntemleri ile geniş popülasyonda yapılacak çalışmalara ihtiyaç bulunduğu görülmüştür.Anahtar Kelimeler: Bruksizm, Diş gıcırdatma, Sert ve Yumuşak oklüzal splint, Bitestrip, Çocuk Aim of the study; It was aimed to compare the effect of bruxism symptoms of hard and soft occlusal splint treatment on the Bitestrip® scores indicating bruxism severity in children diagnosed with nocturnal bruxism according to clinical examination and parent expression. Nocturnal bruxism, which is suitable for splinting among children with complaints of working teeth grinding and tightening, has been performed on 40 children between 6-11 years of age. Bite severity of children who were first standardized according to Thorpy criteria and / or who had a habit of tightening teeth and grinding were measured with Bitestrip® among the groups using hard and soft occlusal splint. In the following steps, the groups used occlusal splints for 3 months and at the end of these 3 months the bite severity was measured again with Bitestrip®. As a result of the study, soft occlusal splint; reduced the adverse effects of bruxism on the hard occlusal splint but this did not reflect on the results of Bitestrip®.
