Abstract

362 evaluable node-positive patients with stage II breast cancer were randomized, receiving either 6 cycles of conventional CMF or 6 cycles of the combination of cyclophosphamide (500 mg/m2), mitoxantrone (Novantrone 10 mg/ m2), and fluorouracil (500 mg/m2; CNF). After a median follow-up of 51 months, 64 (36%) patients relapsed in the CMF group and 60 (33%) in the CNF group (p = 0.8276). By Cox multivariate analysis, tumor size, menopausal status and number of involved nodes were retained as independently significant variables. Toxicities were remarkably similar in both groups. It appears that after a median follow-up of 51 months there is no significant difference in relapse-free survival between node-positive patients with breast cancer who received either 6 cycles of the conventional CMF or the CNF combination as adjuvant treatment. © 1996 S. Abstract

Background: Leptomeningeal carcinomatosis is a rare complication of solid tumors, e. g., breast, lung and gastrointestinal carcinomas. Clinical manifestations are variable with radicular pains with or without neurologic deficiencies as well as headache and hallucinations. Case Report: The rare case of a 57-year-old patient with neurologic symptoms caused by a leptomeningeal carcinomatosis and a spinal metastasis of an asymptomatic signet-ring cell gastric carcinoma is reported. In spite of combined radiochemotherapy the patient died already 4 weeks after discharge from hospital due to an intracerebral hemorrhage. Conclusion: Until today, prognosis of leptomeningeal carcinomatosis is poor with a median survival between 3–4 months independently of the primary tumor.

Abstract

The paper describes a new CT based 3D virtual simulator system called EXOMIO that has been developed at the Institut für Graphische Datenverarbeitung [Fraunhofer IGD] in collaboration with the Department of Radiation Oncology of the Klinikum Offenbach. The system is based on low cost and widely available hardware [PC]: unlike the other commercially available systems that depend on expensive workstations. It provides high quality and high performance visualisation tools. It can be connected via a network to any DICOM supporting CT or MR scanner and via DICOM-RT supplements it enables support for a treatment planning system and for a verification system for linear accelerators. Finally, a prostate cancer case history is given as an example of a clinical application of EXOMIO.

13. Application of the Monte Carlo integration (MCI) method for calculation of the anisotropy of 192Ir brachytherapy sources

Abstract

Source anisotropy is a very important factor in the brachytherapy quality assurance of high-dose rate (HDR) 192Ir after-loading stepping sources. If anisotropy is not taken into account then doses received by a brachytherapy patient in certain directions can be in error by a clinically significant amount. Experimental measurements of anisotropy are very labour intensive. We have shown that within acceptable limits of accuracy, Monte Carlo integration (MCI) of a modified Sievert integral (3D generalization) can provide the necessary data within a much shorter time scale than can experiments. Hence MCI can be used for routine quality assurance schedules whenever a new design of HDR or PDR 192Ir is used for brachytherapy after- loading. Our MCI calculation results are compared with published experimental data and Monte Carlo simulation data for microSelectron and VariSource 192Ir sources. We have shown not only that MCI offers advantages over alternative numerical integration methods, but also that treating filtration coefficients as radial distance-dependent functions improves Sievert integral accuracy at low energies. This paper also provides anisotropy data for three new 192Ir sources, one for the microSelectron-HDR and two for the microSelectron-PDR, for which data are currently not available. The information we have obtained in this study can be incorporated into clinical practice.

Abstract

The purpose of this study was to investigate whether the intelligence quotient (IQ) in children treated for leukemia decreases in the years following whole brain irradiation. Twenty-seven leukemic children were assessed following a mean time lapse between radiotherapy and IQ measurement of 9 years. The IQ test used was the Hamburg Weschsler Intelligence Test for Adults. The IQ results did not differ significantly, p > 0.05, from the IQs of the general population. It was found that age and dose were not predictors of a decrease in IQ. The only predictor was time lapse between irradiation and IQ measurement, which we found to be indicative of an IQ decrease even after 9 years. Time lapse between irradiation is a useful predictor of IQ. [ABSTRACT FROM AUTHOR]

Abstract

Aim: The aim of this protocol was to investigate breast conservation rates with and without flap-supported surgery after preoperative chemotherapy, radiotherapy and hyperthermia.Patients and Methods: One hundred and fifty-eight patients with stage IIA-IV breast cancers were initially treated with chemotherapy, radiotherapy and hyperthermia. Radiation treatment consisted of an interstitial boost of 10 Gy 192Ir-afterloading therapy and a course of external beam radiotherapy of 50 Gy, using 5 x 2 Gy/week. Local hyperthermia with 43.5-44.5 degrees C over 60 minutes was delivered immediately before interstitial radiotherapy.Results: One hundred and forty-two patients underwent salvage surgery. A breast-conserving approach was possible in 74 patients (52%). Fifty-three patients (37%) underwent flap-supported surgery. After a median follow-up of 20 months, one patient developed isolated local recurrence. In 14 cases, locoregional recurrences occurred in combination with distant metastases.Conclusion: In about 50%, breast conservation was achieved by chemotherapy, radiotherapy and hyperthermia. The low isolated local recurrence rate of 0.6% (1/158) has to be substantiated by further follow-up. [ABSTRACT FROM AUTHOR]

Abstract

Background: In recurrent head and neck cancer, therapeutic options are limited and high-quality evidence is rare. The purpose of this report was to present our experience of CT-guided interstitial high-dose rate (HDR) brachytherapy in 4 challenging cases of inoperable recurrent head and neck cancer., Methods: A 53-year-old man with mucosal melanoma of the paranasal sinuses, a 61-year-old man with myofibroblastic sarcoma of the nasal cavity, a 51-year-old woman with nasopharyngeal cancer, and a 44-year-old woman with orbital leiomyosarcoma were treated with reirradiation by HDR brachytherapy after previous comprehensive therapy, including full course external radiotherapy (RT)., Results: All patients showed response to brachytherapy with 1 patient experiencing locoregional failure. The mean lesion-specific disease control was 15 months. Currently, 1 patient is alive and 3 patients died after mean 20 months after treatment., Conclusion: In patients with inoperable recurrent head and neck cancer, CT-guided interstitial HDR brachytherapy can play a role in providing palliation and tumor control. © 2017 Wiley Periodicals, Inc. 49. Computed tomography-guided interstitial high dose rate brachytherapy for centrally located liver tumours: a single institution study.

Abstract

Objectives: To evaluate the clinical outcome of computed tomography (CT)-guided interstitial (IRT) high-dose-rate (HDR) brachytherapy (BRT) in the treatment of unresectable primary and secondary liver malignancies. This report updates and expands our previously described experience with this treatment technique., Methods: Forty-one patients with 50 tumours adjacent to the liver hilum and bile duct bifurcation were treated in 59 interventions of CT-guided IRT HDR BRT. The tumours were larger than 4 cm with a median volume of 84 cm(3) (38-1,348 cm(3)). The IRT HDR BRT delivered a median total physical dose of 20.0 Gy (7.0-32.0 Gy) in twice daily fractions of median 7.0 Gy (4.0-10.0 Gy) in 19 patients and in once daily fractions of median 8.0 Gy (7.0-14.0 Gy) in 22 patients., Results: With a median follow-up of 12.4 months, the local control for metastatic hepatic tumours was 89 %, 73 % and 63 % at 6, 12 and 18 months respectively. The local control for primary hepatic tumours was 90 %, 81 % and 50 % at 6, 12 and 18 months respectively. Severe side effects occurred in 5.0 % of interventions with no treatment-related deaths., Conclusions: CT-guided IRT HDR BRT is a promising procedure for the radiation treatment of centrally located liver malignancies., Key Points: • Interstitial high-dose-rate brachytherapy (IRT HDR BRT) is a promising treatment for central liver tumours • CT-guided IRT HDR BRT is safe for treating extensive tumours • CT-guided IRT HDR BRT could play a role in managing unresectable hepatic malignancies.

Abstract

Purpose: To report our results of computed tomography (CT)-guided interstitial high-dose-rate (HDR) brachytherapy (BRT) in the local treatment of inoperable primary and secondary liver malignancies., Methods and Materials: Between 2000 and 2009, 31 patients underwent a total of 42 BRT procedures for 36 hepatic lesions exceeding 4 cm and located adjacent to the liver hilum and bile duct bifurcation. The median tumor volume was 99 cm(3) (range, 46-1348 cm(3)). The median age was 64 years (range, 27-85 years). The HDR-BRT delivered a median total physical dose of 13.0 Gy (range, 7.0-32.0 Gy) in twice daily fractions of median 7.0 Gy (range, 4.0-10.0 Gy) in 14 patients and in once daily fractions of median 8.0 Gy (range, 7.0-14.0 Gy) in 17 patients., Results: The median followup was 13.3 months with an overall survival rate of 66% at 1 year. The local control rate for patients with metastatic lesions was 79%, 59%, and 59%, and for the subgroup with primary hepatic tumors 88%, 50%, and 50% at 1, 2, and 3 years, respectively. Severe side effects occurred in 4.7% of BRT procedures with no treatment-related deaths., Conclusions: Our results confirm CT-guided interstitial HDR-BRT to be a safe procedure for the local treatment of inoperable liver malignancies unsuitable for thermal ablation., (Copyright © 2012 American Brachytherapy Society.
