Abstract

The paper aims to show how the performance of paint coatings is affected by their composition. Samples of two paint coatings, which had delaminated in service, were compared with the same type of paint applied to samples for the purposes of certification. The particle size distribution of the filler materials (inert pigments) affected the flow of resin and wetting of the particles. In both failed samples, large particles sizes had agglomerated creating voids and crevices, resulting in poor wetting by the resin. This poor wetting and inherent lack of bonding had resulted in their premature failure. Both equivalent laboratory samples had a much better particle size distribution, with a lower maximum particle size, resulting in inert pigment being well distributed through the paint layer. Abstract

Objectives Autoantibodies against the extracellular domains of the voltage-gated potassium channel (VGKC) complex proteins, leucine-rich glioma-inactivated 1 (LGI1) and contactin-associated protein-2 (CASPR2), are found in patients with limbic encephalitis, faciobrachial dystonic seizures, Morvan’s syndrome and neuromyotonia. However, in routine testing, VGKC-complex-antibodies without LGI1- or CASPR2-reactivities (“double-negative”) are commoner than LGI1- or CASPR2-specificities. Therefore, the target(s) and clinical associations of double-negative antibodies need to be determined. Methods Sera (n=1131) from several clinically-defined cohorts were tested for IgG-radioimmunoprecipitation of 125I-DTX-labelled VGKC-complexes from mammalian brain extracts. Positive samples were systematically tested for live hippocampal neuron reactivity, IgG-precipitation of 125I-DTX and 125I-DTX-labelled Kv1-subunits, and by cell-based assays which expressed Kv1-subunits, LGI1 and CASPR2. Results VGKC-complex-antibodies were found in 162 of 1131 (14%) sera. Ninety of these (56%) had antibodies targeting the extracellular domains of LGI1 or CASPR2. Of the remaining 72 double-negative sera, ten (14%) immunoprecipitated 125I-DTX itself, and 27 (38%) bound to solubilized co-expressed Kv1.1/1.2/1.6 subunits and/or Kv1.2 subunits alone, at levels proportionate to VGKC-complex-antibody levels (r=0.57, p=0.0017). The sera with LGI1- and CASPR2-antibodies immunoprecipitated neither preparation. None of the 27 Kv1-precipitating samples bound live hippocampal neurons or Kv1-extracellular domains, but 16 (59%) bound to permeabilised Kv1-expressing HEK cells. These intracellular Kv1-antibodies mainly associated with non-immune disease aetiologies, poor longitudinal clinical-serological correlations, and a limited immunotherapy-response. Conclusions Double-negative VGKC-complex-antibodies are often directed against cytosolic epitopes of Kv1-subunits, and occasionally against non-mammalian DTX. These antibodies should no longer be classified as neuronal-surface antibodies. They consequently lack pathogenic potential, and do not in themselves support use of immunotherapies.

Abstract

IgG of type 1 anti-neuronal nuclear antibody (ANNA-1, anti-Hu) specificity is a serological marker of paraneoplastic neurological autoimmunity (including enteric/autonomic) usually related to small-cell lung carcinoma. We show here that IgG isolated from such sera and also affinity-purified anti-HuD label enteric neurons and cause an immediate spike discharge in enteric and visceral sensory neurons. Both labelling and activation of enteric neurons was prevented by preincubation with the HuD antigen. Activation of enteric neurons was inhibited by the nicotinic receptor antagonists hexamethonium and dihydro-β-erythroidine and reduced by the P2X antagonist pyridoxal phosphate-6-azo (benzene-2,4-disulfonic acid (PPADS) but not by the 5-HT3 antagonist tropisetron or the N-type Ca-channel blocker ω-Conotoxin GVIA. Ca(++) imaging experiments confirmed activation of enteric neurons but not enteric glia. These findings demonstrate a direct excitatory action of ANNA-1, in particular anti-HuD, on visceral sensory and enteric neurons, which involves nicotinic and P2X receptors. The results provide evidence for a novel link between nerve activation and symptom generation in patients with antibody-mediated gut dysfunction.

Abstract

Background Myasthenia gravis is an autoimmune neuromuscular disorder, which has only rarely been reported to co-manifest with myositis. The diagnosis of concomitant myositis in patients with myasthenia gravis is clinically challenging, and requires targeted investigations for the differential diagnosis, including EMG, autoantibody assays, muscle biopsy and, importantly, imaging of the mediastinum for thymoma screening. Case presentation This report presents a case-vignette of a 72-year-old woman with progressive proximal muscle weakness and myalgias, diagnosed with thymoma-associated myasthenia and bioptically verified granulomatous myositis, with positive autoantibody status for ryanodine receptor and titin antibodies. Conclusions The diagnosis of concurrent myositis and myasthenia gravis, especially in the presence of ryanodine receptor and titin antibodies, should lead neurologists to adopt different treatment strategies compared to those applied in myasthenia or myositis alone. Moreover, further evidence is warranted that titin and, particularly, ryanodine receptor antibodies may co-occur or be pathophysiologically involved in myasthenia-myositis cases.

Abstract

Summary: Background: Bullous pemphigoid (BP) is the most common autoimmune subepidermal blistering skin disease. Two antigens have been identified as targets of circulating autoantibodies (autoAbs) – BP180 and BP230 – with BP180 being a critical transmembrane adhesion protein of basal keratinocytes of the epidermis. The noncollagenous domain 16A (NC16A) of BP180 is the immunodominant epitope in patients with BP, and anti‐BP180‐NC16A IgG antibodies (Abs) correlate to disease activity. Routine serological testing and follow‐up of BP relies on indirect immunofluorescence (IIF) of serum Abs, commonly performed on monkey oesophagus (ME), and/or enzyme‐linked immunosorbent assay (ELISA) testing on recombinantly produced fragments of BP180 and BP230 (BP180‐NC16A, BP230‐C/N). Objectives: To determine if NC16A epitopes are well represented on ME substrate. Methods: Sera from different BP cohorts were tested by IIF on ME and normal human skin (NHS). To confirm findings, affinity‐purified anti‐BP180‐NC16A/BP230 polyclonal Abs and recombinant anti‐BP180‐NC16A/BP230 monoclonal antibodies (mAbs) were used. Results: For sensitive detection of BP180‐NC16A‐specific IgG Abs, sections of NHS are superior to the widely used ME. Confirmation comes from polyclonal affinity‐purified anti‐BP180‐NC16A/BP230 Abs, and by mAbs cloned from a patient with active BP. Conclusions: Use of NHS is preferable over ME in routine IIF testing for BP. These results are of clinical relevance because anti‐BP180‐NC16A IgG titres are correlated to disease activity and detecting them reliably is important for screening, diagnosis and follow‐up of patients with BP. Summary: Background: Antilaminin 332 mucous membrane pemphigoid (MMP) is an autoimmune subepidermal blistering disease with predominant mucosal involvement and autoantibodies against laminin 332. Malignancies have been associated with this disease; however, no standardized detection system for antilaminin 332 serum antibodies is widely available. Objectives: Development of a sensitive and specific assay for the detection of antilaminin 332 antibodies. Methods: An indirect immunofluorescence (IF) assay using recombinant laminin 332 was developed and probed with a large number of antilaminin 332 MMP patient sera (n = 93), as well as sera from patients with antilaminin 332‐negative MMP (n = 153), bullous pemphigoid (n = 20), pemphigus vulgaris (n = 20) and noninflammatory dermatoses (n = 22), and healthy blood donors (n = 100). Results: In the novel IF assay, sensitivities with the laminin 332 heterotrimer and the individual α3, β3 and γ2 chains were 77%, 43%, 41% and 13%, respectively, with specificities of 100% for each substrate. The sensitivity for the heterotrimer increased when an anti‐IgG4 enriched antitotal IgG conjugate was applied. Antilaminin 332 reactivity paralleled disease activity and was associated with malignancies in 25% of patients with antilaminin 332 MMP. Conclusions: The novel IF‐based assay will facilitate the serological diagnosis of antilaminin 332 MMP and may help to identify patients at risk of a malignancy. Item does not contain fulltext, BACKGROUND: Autoantibodies against the M-type phospholipase A2 receptor (PLA2R1) are specific markers for primary membranous nephropathy (pMN) and anti-PLA2R1 serum levels may be useful to monitor disease activity. So far, a recombinant cell-based indirect immunofluorescence assay (RC-IFA) using recombinant PLA2R1 as a substrate has been widely available but lacks a finely graduated assessment of antibody concentrations. METHODS: In order to setup a standardized ELISA, the extracellular domain of human PLA2R1 was expressed in HEK293. The purified protein was used to form the solid-phase in an ELISA which was then employed to analyze sera from 200 patients with primary MN, 27 patients with secondary MN, 230 patients with other glomerular diseases, 316 patients with systemic autoimmune diseases, and from 291 healthy blood donors. RESULTS: At a set specificity of 99.9% the sensitivity of the anti-PLA2R1 IgG ELISA was found to be 96.5%. A similar sensitivity (98.5%) was obtained when binding of only subclass IgG4 was analyzed. The calibrated assay showed a good class correlation with the results of the RC-IFA, was robust and could be stored for several months without any loss of quality. CONCLUSION: The results demonstrate that the new test system is qualified for routine use and that it has an almost perfect agreement with both, the clinical characterization of the patients and the results generated with RC-IFA.
