10. Suspended particle and drug ingredient concentrations in hospital dispensaries and implications for pharmacists’ working environments

Abstract

The aim of this study was to assess the present status of working environments for pharmacists, including the concentrations of suspended particles and suspended drug ingredients in dispensaries. We conducted a survey on the work processes and working environment in 15 hospital dispensaries, and measured the concentrations of suspended particles and suspended drug ingredients using digital dust counter and high-performance liquid chromatography tandem mass spectrometry (LC–MS/MS), respectively. Of 25 types of powdered drugs that were frequently handled in the 15 dispensaries surveyed, 11 could be quantitatively determined. The amounts of suspended particles were relatively high, but below the reference value, in three dispensaries without dust collectors. The sedative-hypnotic drug zopiclone was detected in the suspended particles at one dispensary that was not equipped with dust collectors, and the antipyretic and analgesic drug acetaminophen was detected in two dispensaries equipped with dust collectors. There was no correlation between the daily number of prescriptions containing powdered drugs and the concentration of suspended particles in dispensaries. On the basis of the suspended particle concentrations measured, we concluded that dust collectors were effective in these dispensaries. However, suspended drug ingredients were detected also in dispensaries with dust collectors. These results suggest that the drug dust control systems of individual dispensaries should be properly installed and managed.

Abstract

Background: Pulmonary large cell neuroendocrine carcinoma (LCNEC) is a rare and aggressive tumor with a poor prognosis and standard therapy has not yet been established. Case: A 65-year-old male with a cough for 2 months presented to our hospital. He was clinically diagnosed with non small cell lung cancer cT3N1M0 stage IIIA and underwent right pneumonectomy. The final diagnosis was pulmonary LCNEC pT3N1M0 stage IIIA. Multiple subcutaneous masses were detected 4 months after surgery, and biopsy revealed postoperative recurrence and metastasis. Chemotherapy with carboplatin plus etoposide was initiated. Subcutaneous masses increased and multiple new brain metastases developed after two cycles. Additional tests revealed that epidermal growth factor receptor and anaplastic lymphoma kinase were negative, and the programmed death ligand 1 (PD-L1) expression rate in tumor cells was 40% (22C3 clones). The primary cells infiltrating the tumor were CD3-positive T cells and CD138-positive plasma cells. Second-line treatment with pembrolizumab was started. The shrinkage of subcutaneous masses was observed after one cycle, and the tumor had completely disappeared after six cycles. Treatment was continued for approximately 2 years. This response has been maintained for 4 years and is still ongoing. Conclusion: Pembrolizumab may be used as a treatment option for pulmonary LCNEC.

Abstract

The effect of an axial magnetic field on plasma pinch dynamics and on the extreme ultraviolet emission property of a compact Z-pinch device for ultraviolet sources is described. The time-resolved interferogram indicates that the axial magnetic field contributes by making the plasma compression smooth and by sustaining certain plasma conditions longer and hence it is concluded that applying an axial magnetic field is an effective method to improve ultraviolet emission.

Abstract

Immune checkpoint inhibitors (ICIs) have caused a paradigm shift in the treatment of lung cancer. Here, we encountered a case of inoperable locally advanced squamous cell carcinoma of the lung that became operable with pembrolizumab‐based immunochemotherapy and achieved a pathological complete response. An 82‐year‐old man suspected of having lung cancer was referred to our hospital. The patient was clinically diagnosed with left upper lobe squamous cell carcinoma cT2aN3M0 c‐stage IIIC. Immunostaining revealed the expression of programmed death‐ligand 1 in 60% of tumor cells. The cancer cells disappeared after two cycles of chemotherapy with carboplatin and nanoparticle albumin‐bound paclitaxel plus pembrolizumab. As the abnormal accumulation of 18F‐fluorodeoxyglucose (FDG) on FDG‐positron emission tomography/computed tomography before chemotherapy almost disappeared after pembrolizumab‐based immunochemotherapy, left upper lobectomy and lymph node dissection were performed. No cancer cells were pathologically detected from the resected tissue. Abstract

The creep of the newly developed Ni-based, Re free, single crystal superalloy NKH71 was examined at 1273 K. The creep properties were compared to those of CMSX-4, a second-generation commercial superalloy containing 3%Re. When the (applied) stress was higher than 160 MPa, the rupture lives of the present alloy were shorter than those of CMSX-4. However, when the stress was below 160 MPa, the rupture lives of the present alloy became longer than those of CMSX-4. It was noted that this lower stress level was more important when Ni-based superalloys were used for engineering land-based gas turbines. TEM and SEM observations showed that NKH71 formed a rafted structure in the early stage of creep. This was because a large number of dislocations existed in the γ/γ′ interfaces before creep. In contrast, CMSX-4 had very few dislocations before creep. This observation explained why NKH71 was inferior to CMSX-4 under high stress. However under lower stress, the γ/γ′ rafted structure changed to a more complex morphology in CMSX-4, while that of NKH71 tended to remain unchanged. This conservation of the rafted structure lead to the good creep resistance of NKH71 under a lower stress. In brief, the new alloy was shown to have good creep resistance even without Re element.

Abstract

The authors have developed an articulated medical instrument which assists with the movement of and maintenance of the position of such as a lung during surgery to assist providing improved viewability and providing the necessary space required to work efficiently and effectively. Conventionally holding such as, a lung in a given position is achieved by arranging gauze in an appropriate configuration with the aid of a simple shaft like surgical instrument, however for various reasons the gauze requires constant replacement and rearrangement resulting in significant surgical inefficiency. By using an articulated mechanism that mimics the functionality of fingers, we have developed a surgical device that inherently provides greater mechanical stability and does not require the use of gauze, thus enabling more efficient surgery. The prototyping process was assisted with the use of 3D-CAD to simulate static and dynamic conditions and a 3D-printer to confirm the concept and then a working prototype was created. In order to validate the dynamic functionality of this surgical instrument, it was tested on a porcine lung which closely approximates human lungs by a clinical doctor of thoracic surgery. The result was that the developed prototype articulated mechanism was able to stably move and hold the lung in position during surgery thus confirming the dynamic efficacy of the mechanism.
