Abstract

Simple Summary: Heart transplantation is the gold standard in selected patients with end-stage heart failure. However, immune responses to the transplanted heart can lead to major complications and graft failure, limiting long-term survival. It has been shown that antibodies targeting the heart are elevated in end-stage heart failure patients and before transplantation of the donor heart. We aimed to determine whether elevated antibody levels before heart transplantation are associated with a poor outcome after transplantation. We measured four general subtypes of antibodies (IgM, IgG1, IgG2, IgG3) in the blood of patients just before the transplantation was performed. We observe that patients with higher levels of IgG1 and IgG2 antibodies tend to reject the transplanted heart more often compared to patients with lower levels of these antibodies. In addition, we observe that IgG1 and IgG2 levels in these patients remain elevated after transplantation. Together our data suggest that a more active adaptive, antibody-mediated, immune system can lead to graft-related complications in cardiac transplant patients. This implies that a subpopulation of heart transplant patients might benefit from expanded immunosuppressive therapy, which includes medications targeting antibody-producing cells. Cardiac allograft vasculopathy (CAV) and antibody-mediated rejection are immune-mediated, long-term complications that jeopardize graft survival after heart transplantation (HTx). Interestingly, increased plasma levels of immunoglobulins have been found in end-stage heart failure (HF) patients prior to HTx. In this study, we aimed to determine whether increased circulating immunoglobulin levels prior to transplantation are associated with poor post-HTx survival. Pre-and post-HTx plasma samples of 36 cardiac transplant recipient patients were used to determine circulating immunoglobulin levels. In addition, epicardial tissue was collected to determine immunoglobulin deposition in cardiac tissue and assess signs and severity of graft rejection. High levels of IgG1 and IgG2 prior to HTx were associated with a shorter survival post-HTx. Immunoglobulin deposition in cardiac tissue was significantly elevated in patients with a survival of less than 3 years. Patients with high plasma IgG levels pre-HTx also had significantly higher plasma levels after HTx. Furthermore, high pre-HTX levels of IgG1 and IgG2 levels were also significantly increased in patients with inflammatory infiltrate in CAV lesions. Altogether the results of this proof-of-concept study suggest that an activated immune response prior to transplantation negatively affects graft survival. [ABSTRACT FROM AUTHOR]

Abstract

This position paper provides a review of the current European research vessel fleet, its capabilities and equipment, assessing its ability to support marine science across the globe now and into the future. It particularly looks at current and future capabilities in the context of deep sea and Polar research. It also takes a wider vision, assessing the importance of these vessels in the ocean and earth observing landscape. This review includes not only technological but also human capabilities, looking at training needs for crew and technicians to ensure they can continue to deliver on critical science needs. It also considers the ways in which the current European fleet is managed. This Position Paper sets out recommendations for how the fleet will need to develop in the future to ensure that it will continue to provide the same high level of support to science globally, as well as highlighting ways in which management could be made more efficient. It is aimed at national- and European-level policy makers and funders, as well as the marine science community and the research vessel operator community.

27. Dissection of a metastatic gene expression signature into distinct components

Abstract

Background Metastasis, the process whereby cancer cells spread, is in part caused by an incompletely understood interplay between cancer cells and the surrounding stroma. Gene expression studies typically analyze samples containing tumor cells and stroma. Samples with less than 50% tumor cells are generally excluded, thereby reducing the number of patients that can benefit from clinically relevant signatures.Results For a head-neck squamous cell carcinoma (HNSCC) primary tumor expression signature that predicts the presence of lymph node metastasis, we first show that reduced proportions of tumor cells results in decreased predictive accuracy. To determine the influence of stroma on the predictive signature and to investigate the interaction between tumor cells and the surrounding microenvironment, we used laser capture microdissection to divide the metastatic signature into six distinct components based on tumor versus stroma expression and on association with the metastatic phenotype. A strikingly skewed distribution of metastasis associated genes is revealed.Conclusion Dissection of predictive signatures into different components has implications for design of expression signatures and for our understanding of the metastatic process. Compared to primary tumors that have not formed metastases, primary HNSCC tumors that have metastasized are characterized by predominant down-regulation of tumor cell specific genes and exclusive up-regulation of stromal cell specific genes. The skewed distribution agrees with poor signature performance on samples that contain less than 50% tumor cells. Methods for reducing tumor composition bias that lead to greater predictive accuracy and an increase in the types of samples that can be included are presented.

It provides and analysis of the relevance of Research Vessels, the European Research Vessel fleet and the current EU policy towards Transnational access to these infrastructures. TheEUROFLEETS+ “Foresight: Legacy and Roadmap” - Objectives and Work Plan are described, with a detailed examination of possible legal entity models to advance towards a Eurofleets RI.
