Asthma–chronic obstructive pulmonary disease overlap syndrome (ACOS), characterized by coexisting symptoms of asthma and chronic obstructive pulmonary disease, is considered a separate disease entity. Previously, we investigated the effects of AD on pulmonary function in adult patients with asthma. Here, we present the findings of our further research on the differences in the effects of AD exposure on pulmonary function between patients with asthma alone and those with ACOS.Methods: Between March and May 2012, we conducted a panel study wherein we monitored daily peak expiratory flow (PEF) values in 231 adult patients with asthma. These patients were divided into 190 patients with asthma alone and 41 patients with ACOS in this study. Daily AD particle levels were measured using light detection and ranging systems. Two heavy AD days (April 23 and 24) were determined according to the Japan Meteorological Agency definition. A linear mixed model was used to estimate the association between PEF and AD exposure.Results: Increments in the interquartile range of AD particles (0.018 km-1) led to PEF changes of –0.50 L/min (95% confidence interval, –0.98 to –0.02) in patients with asthma alone and –0.11 L/min (–0.11 to 0.85) in patients with ACOS. The PEF changes after exposure to heavy AD were –2.21 L/min (–4.28 to –0.15) in patients with asthma alone and –2.76 L/min (–6.86 to 1.35) in patients with ACOS. In patients with asthma alone, the highest decrease in PEF values was observed on the heavy AD day, with a subsequent gradual increase over time.Conclusion: Our results suggest that the effects of AD exposure on pulmonary function differ between patients with asthma alone and ACOS, with the former exhibiting a greater likelihood of decreased pulmonary function after AD exposure. The origin of successive phase transitions observed in the layered perovskite $\alpha$-Sr$_2$CrO$_4$ is studied by the density-functional-theory-based electronic structure calculation and mean-field analysis of the proposed low-energy effective model. We find that, despite the fact that the CrO$_6$ octahedron is elongated along the $c$-axis of the crystal structure, the crystal-field level of nondegenerate $3d_{xy}$ orbitals of the Cr ion is lower in energy than that of doubly degenerate $3d_{yz}$ and $3d_{xz}$ orbitals, giving rise to the orbital degrees of freedom in the system with a $3d^2$ electron configuration. Density-functional-theory-based electronic structure calculations are made to consider the novel electronic states of Ru-pnictides RuP and RuAs where the intriguing phase transitions and superconductivity under doping of Rh have been reported. We find that there appear nearly degenerate flat bands just at the Fermi level in the high-temperature metallic phase of RuP and RuAs; the flat-band states come mainly from the $4d_{xy}$ orbitals of Ru ions and the Rh doping shifts the Fermi level just above the flat bands. The splitting of the flat bands caused by their electronic instability may then be responsible for the observed phase transition to the nonmagnetic insulating phase at low temperatures. We also find that the band structure calculated for RuSb resembles that of the doped RuP and RuAs, which is consistent with experiment where superconductivity occurs in RuSb without Rh doping., Comment: 9 pages, 5 figures, to appear in Physics Procedia (Proceedings of ICM2015-Barcelona)

We show that finite temperature variational cluster approximation (VCA) calculations on an extended Falicov-Kimball model can reproduce angle-resolved photoemission spectroscopy (ARPES) results on Ta2NiSe5 across a semiconductor-to-semiconductor structural phase transition at 325 K. We demonstrate that the characteristic temperature dependence of the flat-top valence band observed by ARPES is reproduced by the VCA calculation on the realistic model for an excitonic insulator only when the strong excitonic fluctuation is taken into account. The present calculations indicate that Ta2NiSe5 falls in the Bose-Einstein condensation regime of the excitonic insulator state., Comment: 21 pages(5 figures)

- Abstract
Background: The optimal treatment of anastomotic leak after rectal cancer resection is unclear. This worldwide cohort study aimed to provide an overview of four treatment strategies applied. Methods: Patients from 216 centres and 45 countries with anastomotic leak after rectal cancer resection between 2014 and 2018 were included. Treatment was categorized as salvage surgery, faecal diversion with passive or active (vacuum) drainage, and no primary/secondary faecal diversion. The primary outcome was 1-year stoma-free survival. In addition, passive and active drainage were compared using propensity score matching (2: 1). Results: Of 2470 evaluable patients, 388 (16.0 per cent) underwent salvage surgery, 1524 (62.0 per cent) passive drainage, 278 (11.0 per cent) active drainage, and 280 (11.0 per cent) had no faecal diversion. One-year stoma-free survival rates were 13.7, 48.3, 48.2, and 65.4 per cent respectively. Propensity score matching resulted in 556 patients with passive and 278 with active drainage. There was no statistically significant difference between these groups in 1-year stoma-free survival (OR 0.95, 95 per cent c.i. 0.66 to 1.33), with a risk difference of -1.1 (95 per cent c.i. -9.0 to 7.0) per cent. After active drainage, more patients required secondary salvage surgery (OR 2.32, 1.49 to 3.59), prolonged hospital admission (an additional 6 (95 per cent c.i. 2 to 10) days), and ICU admission (OR 1.41, 1.02 to 1.94). Mean duration of leak healing did not differ significantly (an additional 12 (-28 to 52) days). Conclusion: Primary salvage surgery or omission of faecal diversion likely correspond to the most severe and least severe leaks respectively. In patients with diverted leaks, stoma-free survival did not differ statistically between passive and active drainage, although the increased risk of secondary salvage surgery and ICU admission suggests residual confounding.

We have studied the nature of the three-dimensional multi-band electronic structure in the twodimensional triangular lattice Ir1-xPtxTe2 (x=0.05) superconductor using angle-resolved photoemission spectroscopy (ARPES), x-ray photoemission spectroscopy (XPS) and band structure calculation. ARPES results clearly show a cylindrical (almost two-dimensional) Fermi surface around the zone center. Near the zone boundary, the cylindrical Fermi surface is truncated into several pieces in a complicated manner with strong three-dimensionality. The XPS result and the band structure calculation indicate that the strong Te 5p-Te 5p hybridization between the IrTe2 triangular lattice layers is responsible for the three-dimensionality of the Fermi surfaces and the intervening of the Fermi surfaces observed by ARPES., Comment: 5 pages, 4 figures

We report an angle-resolved photoemission spectroscopy (ARPES) study on a triangular lattice superconductor Ir$_{1-x}$Pt$_{x}$Te$_2$ in which the Ir-Ir or Te-Te bond formation, the band Jahn-Teller effect, and the spin-orbit interaction are cooperating and competing with one another. The Fermi surfaces of the substituted system are qualitatively similar to the band structure calculations for the undistorted IrTe$_2$ with an upward chemical potential shift due to electron doping. A combination of the ARPES and the band structure calculations indicates that the Te $5p$ spin-orbit interaction removes the $p_x/p_y$ orbital degeneracy and induces $p_x \pm ip_y$ type spin-orbit coupling near the A point. The inner and outer Fermi surfaces are entangled by the Te $5p$ and Ir $5d$ spin-orbit interactions which may provide exotic superconductivity with singlet-triplet mixing., Comment: 10 pages, 4 figures

Using the band structure calculation and mean-field analysis of the derived three-chain Hubbard model with phonon degrees of freedom, we discuss the origin of the orthorhombic-to-monoclinic phase transition of the layered chalcogenide Ta$_2$NiSe$_5$. We show that the Bose-Einstein condensation of excitonic electron-hole pairs cooperatively induces the instability of the phonon mode at momentum $q\rightarrow 0$ in the quasi-one-dimensional Ta-NiSe-Ta chain, resulting in the structural phase transition of the system. The calculated single-particle spectra reproduce the deformation of the band structure observed in the angle-resolved photoemission spectroscopy experiment., Comment: 5 pages, 4 figures

Synchrotron X-ray diffraction experiment shows that the metal-insulator transition occurring in a ferromagnetic state of a hollandite K$_2$Cr$_8$O$_{16}$ is accompanied by a structural distortion from the tetragonal $I4/m$ to monoclinic $P112_{1}/a$ phase with a $\sqrt{2}\times\sqrt{2}\times 1$ supercell. Detailed electronic structure calculations demonstrate that the metal-insulator transition is caused by a Peierls instability in the quasi-one-dimensional column structure made of four coupled Cr-O chains running in the $c$-direction, leading to the formation of tetramers of Cr ions below the transition temperature. Based on the first-principles electronic structure calculations, we predict that a chromium oxide K$_2$Cr$_8$O$_{16}$ of hollandite type should be a half-metallic ferromagnet where the Fermi level crosses only the majority-spin band, whereas the minority-spin band has a semiconducting gap. We show that the double-exchange mechanism is responsible for the observed saturated ferromagnetism. We discuss possible scenarios of the metal-insulator transition observed at low temperature and we argue that the formation of the incommensurate, long-wavelength density wave of spinless fermions caused by the Fermi-surface nesting may be the origin of the opening of the charge gap., Comment: 7 pages, 7 figures; minor modifications; Phys. An \textit{ab initio} electronic structure calculation based on the generalized gradient approximation in the density functional theory is carried out to study the basic electronic states of hollandite vanadate K$_2$V$_8$O$_{16}$. We find that the states near the Fermi energy consist predominantly of the three $t_{2g}$-orbital components and the hybridization with oxygen $2p$ orbitals is small. The $d_{yz}$ and $d_{zx}$ orbitals are exactly degenerate and are lifted from the $d_{xy}$ orbital. The calculated band dispersion and Fermi surface indicate that the system is not purely one-dimensional but the coupling between the VO double chains is important. Comparison with available experimental data suggests the importance of electron correlations in this system., Comment: 5 pages, 5 figures

We report on results from high-energy spectroscopic measurements on CeFe2, a system of particular interest due to its anomalous ferromagnetism with an unusually low Curie temperature and small magnetization compared to the other rare earth-iron Laves phase compounds. Our experimental results indicate very strong hybridization of the Ce 4f states with the delocalized band states, mainly the Fe 3d states. In the interpretation and analysis of our measured spectra, we have made use of two different theoretical approaches: The first one is based on the Anderson impurity model, with surface contributions explicitly taken into account. The second method consists of band-structure calculations for bulk CeFe2. The analysis based on the Anderson impurity model gives calculated spectra in good agreement with the whole range of measured spectra, and reveals that the Ce 4f -- Fe 3d hybridization is considerably reduced at the surface, resulting in even stronger hybridization in the bulk than previously thought. The band-structure calculations are ab initio full-potential linear muffin-tin orbital calculations within the local-spin-density approximation of the density functional. The Ce 4f electrons were treated as itinerant band electrons. Interestingly, the Ce 4f partial density of states obtained from the band-structure calculations also agree well with the experimental spectra concerning both the 4f peak position and the 4f bandwidth, if the surface effects are properly taken into account. In addition, results, notably the partial spin magnetic moments, from the band-structure calculations are discussed in some detail and compared to experimental findings and earlier calculations., Comment: 10 pages, 8 figures, to appear in Phys. We report an experimental quantum key distribution that utilizes balanced homodyne detection, instead of photon counting, to detect weak pulses of coherent light. Although our scheme inherently has a finite error rate, it allows high-efficiency detection and quantum state measurement of the transmitted light using only conventional devices at room temperature. Observational and theoretical results indicate that low-redshift BL Lacertae objects are the most likely extragalactic sources to be detectable at TeV energies. In this paper we present the results of observations of 4 BL Lacertae objects (PKS0521-365, EXO0423.4-0840, PKS2005-489 and PKS2316-423) made between 1993 and 1996 with the CANGAROO 3.8m imaging Cherenkov telescope. During the period of these observations the gamma-ray energy threshold of the 3.8m telescope was ~2TeV. Searches for steady long-term emission have been made, and, inspired by the TeV flares detected from Mkn421 and Mkn501, a search on a night-by-night timescale has also been performed for each source. Comprehensive Monte Carlo simulations are used to estimate upper limits for both steady and short timescale emission., Comment: 6 pages including 2 figures

This paper reports the first discovery of TeV gamma-ray emission from a supernova remnant made with the CANGAROO 3.8 m Telescope. TeV gamma rays were detected at the sky position and extension coincident with the north-east (NE) rim of shell-type Supernova remnant (SNR) SN1006 (Type Ia). SN1006 has been a most likely candidate for an extended TeV Gamma-ray source, since the clear synchrotron X-ray emission from the rims was recently observed by ASCA (Koyama et al. 1995), which is a strong evidence of the existence of very high energy electrons up to hundreds of TeV in the SNR. The observed TeV gamma-ray flux was $(2.4\pm 0.5(statistical) \pm 0.7(systematic)) \times 10^{-12}$ cm$^{-2}$ s$^{-1}$ ($\ge 3.0\pm 0.9$ TeV) and $ (4.6\pm 0.6 \pm 1.4) \times 10^{-12}$ cm$^{-2}$ s$^{-1}$ ($\ge 1.7\pm 0.5$ TeV) from the 1996 and 1997 observations, respectively. Also we set an upper limit on the TeV gamma-ray emission from the SW rim, estimated to be $ 1.1 \times 10^{-12}$ cm$^{-2}$ s$^{-1}$ ($\ge 1.7\pm 0.5$ TeV, 95% CL) in the 1997 data. The TeV gamma rays can be attributed to the 2.7 K cosmic background photons up-scattered by electrons of energies up to about 10$^{14}$ eV by the inverse Compton (IC) process. The observed flux of the TeV gamma rays, together with that of the non-thermal X-rays, gives firm constraints on the acceleration process in the SNR shell; a magnetic field of $6.5\pm2$ $\mu$G is inferred from both the synchrotron X-rays and inverse Compton TeV gamma-rays, which gives entirely consistent mechanisms that electrons of energies up to 10$^{14}$ eV are produced via the shock acceleration in SN1006., Comment: 17 pages, 3 figures, LaTeX2.09 with AASTeX 4.0 maros, to appear in Astrophys. Astrophysics - Abstract
Gamma rays with energies greater than 7 TeV from the Crab pulsar/nebula have been observed at large zenith angles, using the Imaging Atmospheric Technique from Woomera, South Australia. CANGAROO data taken in 1992, 1993 and 1995 indicate that the energy spectrum extends up to at least 50 TeV, without a change of the index of the power law spectrum. The observed differential spectrum is \noindent $(2.01\pm 0.36)\times 10^{-13}(E/{7 TeV})^{-2.53 \pm 0.18} TeV^{-1}cm^{-2}s^{-1}$ between 7 TeV and 50 TeV. There is no apparent cut-off. The spectrum for photon energies above $\sim$10 TeV allows the maximum particle acceleration energy to be inferred, and implies that this unpulsed emission does not originate near the light cylinder of the pulsar, but in the nebula where the magnetic field is not strong enough to allow pair creation from the TeV photons. We have observed the Vela pulsar region at TeV energies using the 3.8 m imaging Cherenkov telescope near Woomera, South Australia between January 1993 and March 1995. Evidence of an unpulsed gamma-ray signal has been detected at the 5.8 sigma level. The detected gamma-ray flux is (2.9 +/- 0.5 +/- 0.4) x 10^-12 photons cm^-2 sec^-1 above 2.5 +/- 1.0 TeV and the signal is consistent with steady emission over the two years. The gamma-ray emission region is offset from the Vela pulsar position to the southeast by about 0.13 deg. The data for VHE (~TeV) gamma rays from young gamma-ray pulsar PSR1509-58 observed in 1996 with the CANGAROO 3.8m Cerenkov imaging telescope are presented, as well as the additional data from March to June of 1997. The high spin-down luminosity of the pulsar and the plerionic feature around the pulsar observed with radio and X-rays suggest that VHE gamma-ray emission is quite likely above the sensitivity of the CANGAROO telescope. The CANGAROO results on other pulsars, such as PSR1259-63, are also presented. We have observed the Vela pulsar region at TeV energies using the 3.8 m imaging Cherenkov telescope near Woomera, South Australia every year since 1992. This is the first concerted search for pulsed and unpulsed emission from the Vela region, and the imaging technique also allows the location of the emission within the field of view to be examined. A significant excess of gamma-ray-like events is found offset from the Vela pulsar to the southeast by about 0.13deg. The excess shows the behavior expected of gamma-ray images when the asymmetry cut is applied to the data. We report recent results of the CANGAROO Collaboration on very high energy gamma ray emission from pulsars, their nebulae, SNR and AGN in the southern sky. Observations are made in South Australia using the imaging technique of detecting atmospheric Cerenkov light from gamma rays higher than about 1 TeV. The detected gamma rays are most likely produced by the inverse Compton process by electrons which also radiate synchrotron X-rays. Together with information from longer wavelengths, our results can be used to infer the strength of magnetic field in the emission region of gamma rays as well as the energy of the progenitor electrons. The CANGAROO experiment has observed gamma-ray above 7TeV from the Crab pulsar/nebula at large zenith angle in Woomera, South Australia. We report the CANGAROO data taken in 1992, 1993 and 1995, from which it appears that the energy spectrum extends at least up to 50 TeV. The observed integral spectrum is (8.4+-1.0) x 10^{-13}(E/7 TeV)^(-1.53+-0.15)cm^{-2}s^{-1} between 7 TeV and 50 TeV. In November 1996, the 3.8m mirror was recoated in Australia, and its reflectivity was improved to be about 90% as twice as before. Due to this recoating, the threshold energy of ~4 TeV for gamma rays has been attained in the observation of the Crab at large zenith angle. This worldwide cohort study aimed to provide an overview of four treatment strategies applied. METHODS: Patients from 216 centres and 45 countries with anastomotic leak after rectal cancer resection between 2014 and 2018 were included. Treatment was categorized as salvage surgery, faecal diversion with passive or active (vacuum) drainage, and no primary/secondary faecal diversion. The primary outcome was 1-year stoma-free survival. In addition, passive and active drainage were compared using propensity score matching (2 : 1). RESULTS: Of 2470 evaluable patients, 388 (16.0 per cent) underwent salvage surgery, 1524 (62.0 per cent) passive drainage, 278 (11.0 per cent) active drainage, and 280 (11.0 per cent) had no faecal diversion. One-year stoma-free survival rates were 13.7, 48.3, 48.2, and 65.4 per cent respectively. Propensity score matching resulted in 556 patients with passive and 278 with active drainage. There was no statistically significant difference between these groups in 1-year stoma-free survival (OR 0.95, 95 per cent c.i. 0.66 to 1.33), with a risk difference of -1.1 (95 per cent c.i. -9.0 to 7.0) per cent. After active drainage, more patients required secondary salvage surgery (OR 2.32, 1.49 to 3.59), prolonged hospital admission (an additional 6 (95 per cent c.i. 2 to 10) days), and ICU admission (OR 1.41, 1.02 to 1.94). Mean duration of leak healing did not differ significantly (an additional 12 (-28 to 52) days). CONCLUSION: Primary salvage surgery or omission of faecal diversion likely correspond to the most severe and least severe leaks respectively. In patients with diverted leaks, stoma-free survival did not differ statistically between passive and active drainage, although the increased risk of secondary salvage surgery and ICU admission suggests residual confounding.

Ship maneuvering is a complex operation with inherent uncertainties. To express this complexity in system performance during the navigation process, an analysis model has been developed using Functional Resonance Analysis Method (FRAM) and Dynamic Bayesian Network (DBN). The functional level of dynamic work onboard is assessed and modeled using FRAM qualitatively, in which a key function and the function's potential coupling for specific instantiation are identified. Further analysis is done by integrating the FRAM analysis with DBN for quantification. The evolution of system performance over time is determined through changes in the probability of function's mode, namely strategic, tactical opportunistic, and scrambled. The model presented in this study concerns the fluctuation of ship officer performance to overcome the obstacles during the encounter event. As a result, the integration of FRAM-DBN shows promising usability to evaluate human performance. The essence of human adaptive capacity is also highlighted through system resilience potency, that is, the potency to learn, respond, monitor, and anticipate. We also discuss how this finding contributes to enhance safety analysis, in specific, to provide explicit representation of the dynamic in human performance in ship navigation based on Safety-II idea. - Abstract
Background: In a surgical setting, COVID-19 patients may trigger in-hospital outbreaks and have worse postoperative outcomes. Despite these risks, there have been no consistent statements on surgical guidelines regarding the perioperative screening or management of COVID-19 patients, and we do not have objective global data that describe the current conditions surrounding this issue. This study aimed to clarify the current global surgical practice including COVID-19 screening, preventive measures and in-hospital infection under the COVID-19 pandemic, and to clarify the international gaps on infection control policies among countries worldwide. Methods: During April 2-8, 2020, a cross-sectional online survey on surgical practice was distributed to surgeons worldwide through international surgical societies, social media and personal contacts. Main outcome and measures included preventive measures and screening policies of COVID-19 in surgical practice and centers' experiences of in-hospital COVID-19 infection. Data were analyzed by country's cumulative deaths number by April 8, 2020 (high risk, >5000; intermediate risk, 100-5000; low risk, <100). Results: A total of 936 centers in 71 countries responded to the survey (high risk, 330 centers; intermediate risk, 242 centers; low risk, 364 centers). In the majority (71.9%) of the centers, local guidelines recommended preoperative testing based on symptoms or suspicious radiologic findings. Universal testing for every surgical patient was recommended in only 18.4% of the centers. In-hospital COVID-19 infection was reported from 31.5% of the centers, with higher rates in higher risk countries (high risk, 53.6%; intermediate risk, 26.4%; low risk, 14.8%; P < 0.001). Of the 295 centers that experienced in-hospital COVID-19 infection, 122 (41.4%) failed to trace it and 58 (19.7%) reported the infection originating from asymptomatic patients/staff members.
