Abstract

[english] Aims: There is a growing need to implement teamworking in medical education. In reality, interdisciplinary and interprofessional education is often absent. Here we describe a pilot trial developed jointly by the nursing college and the medical faculty in Erlangen (Germany) in which nursing students and undergraduate medical students formed interprofessional teams and were confronted with interprofessional cases using simulated patients (SPs) and phantoms. The elective was planned using standard curriculum planning instruments and finally evaluated with a novel formative team OSCE (Objective Structured Clinical Evaluation) instrument.Methods: During one year, 20 nursing students participated voluntarily in the project and 10 medical students took the course as an elective. Results: The pilot project was first performed with two tutors, one from the medical school and one from the nursing school, as well as one to two SPs. During the second course, there was only one tutor. Overall evaluation of the voluntary students was good, with some elements (acute stroke, factual knowledge, and general organizational problems) that needed to be improved. Performance of a four-station team OSCE was feasible, but raters reported problems in assessing individuals and the team at the same time. Interobserver agreement was satisfactory (kappa 0.35).Conclusions: Interdisciplinary and interprofessional education between a nursing school and a medical school is feasible within an elective, but requires substantial personnel resources. Design and performance of a team OSCE is possible. The validity of the test has to be shown on follow-up.[german] Zielsetzung: Das Lernziel „Fähigkeit zur interprofessionellen und interdisziplinären Teamarbeit“ wird sowohl in der aktuellen Approbationsordnung für Ärzte als auch im Krankenpflegegesetz gefordert. In der Realität gibt es in beiden Ausbildungen kaum gemeinsame Lehreinheiten. Wir beschreiben ein Pilotprojekt der Medizinischen Fakultät und der Berufsfachschule für Gesundheits- und Krankenpflege in Erlangen, in dem Pflegeschüler und Medizinstudenten interdisziplinär vor Aufgaben gestellt werden, die sie im interprofessionellen Team lösen sollen. Sie werden unter Einsatz von Simulationspatienten (SP) oder Phantomen mit Szenen aus dem Stationsalltag, interdisziplinären Notfällen und schwierigen Situationen konfrontiert, die interprofessionelles Handeln erfordern. Die Planung des Konzeptes wurde angelehnt an die von Kern beschriebenen Schritte zur Curriculumsentwicklung. Der Kurs wurde mit einem formativen Team-OSCE evaluiert.Methodik: An dem Projekt nahmen im Wintersemester 2008/09 und im Sommersemester 2009 insgesamt 20 Schüler der Berufsfachschule und 10 Medizinstudenten (Curriculäres Wahlpflichtfach nach Approbationsordnung im 5.-9. Semester) teil.Ergebnisse: Der Kurs wurde als Wahlpflichtfach für Studierende der Medizin und freiwillig für Pflegeschüler durchgeführt. Initial waren ein Dozent aus der medizinischen Fakultät und ein Dozent aus der Berufsfachschule anwesend, sowie meistens 1-2 SPs. Im zweiten Durchgang wurde der Kurs von einem Dozenten durchgeführt und einem Experten, der bei Bedarf hinzugezogen werden konnte. Anhand der Evaluationsergebnisse des ersten Kurses konnten konkrete Verbesserungsvorschläge (generell zur Organisation und speziell zum vorausgesetzten Fachwissen) in den Kurs des Folgesemesters implementiert werden. Die Durchführung eines 4-Stationen-Team-OSCE gelang organisatorisch. Im ersten Durchlauf zeigte sich, dass eine gleichzeitige Prüfung der Einzelleistung und der Teamleistung nicht möglich ist. Im Verlauf erfolgte nur eine Beurteilung der Teamleistung mit einem Inter-Observer Kappa-Quotienten von 0,35.Schlussfolgerung: Ein interdisziplinäres und interprofessionelles Ausbildungskonzept für Pflegeschüler und Medizinstudenten ist planbar und umsetzbar, braucht aber große personelle Ressourcen. Die Entwicklung und Umsetzung eines Team-OSCE ist möglich, seine Validität muss in Folgeuntersuchungen geprüft werden. Vermutlich ist eine simultane Beurteilung von Einzel- und Teamleistung im Team-OSCE nicht möglich.

Abstract

[english] Aims: Despite many efforts by support groups, mental health professionals and health care policy-makers, a stigmatization of psychiatry as a specialty and of mentally ill people is still prevalent in the general population. Earlier studies have shown that the stigma is prevalent even among psychiatric professionals and students of medicine and psychology. However, past studies on this issue often used questionnaires with a limited validity. Also questionnaires do not necessarily allow a neutral assessment of the underlying concepts of the student. Methods: In this study 133 students were asked to create mind maps on psychiatry. Results: In a systematic evaluation (based on a consensus of psychiatric concepts) of 1353 concepts we found non-specific associations (N = 431) and five main categories: (i) illness scirpts (n = 487); (ii) therapeutic concepts (n = 241), (iii) and (iv) stigma and forensic psychiatry (81 mentions each); and (v) psychopathology (n = 32). Stigma-associated negative concepts were more prevalent than positive connotations. Conclusions: Overall, it was shown that mind maps can be used with ease and quantified according to constructivist learning theory to explore disease concepts without the need for predefined or poorly validated questionnaires.[german] Zielsetzung: Trotz vieler Bemühungen, u. a. von Selbsthilfegruppen, der Fachgesellschaft und der Gesundheitspolitik gibt es in der Allgemeinbevölkerung immer noch eine Stigmatisierung der Psychiatrie als Fachgebiet. Frühere Untersuchungen zeigten, dass auch in der Psychiatrie tätige Menschen und Studierende der Medizin und Psychologie stigmatisierenden Äußerungen zustimmen. Zur genaueren Betrachtung wurden in der Vergangenheit jedoch meist Fragebögen verwendet, die eine begrenzte Validität haben und nicht zwangsläufig eine neutrale Beurteilung der zugrundeliegenden Konzepte der Studierenden erlauben.Methodik: In der vorliegenden Studie wurden 133 Studierende gebeten, Mind-Maps zum Thema Psychiatrie zu erstellen. Ergebnisse: In der systematischen Auswertung (auf der Basis vorher im Konsens bestimmter Konzepte) von 1353 Begriffen ergaben sich neben unspezifischen Assoziationen (n=431) fünf Oberkategorien: 1. Krankheitsbilder (n=487); 2. Therapieformen (n=241); 3. und 4. Stigma-assoziierte Themen bzw. Eigen- und Fremdgefährdung (jeweils 81 Nennungen) und 5. Psychopathologie (n=32). Negativ belegte Stigma-Themen waren wesentlich häufiger als positive Einstellungen zum Thema Stigma und betrafen meist Zwangsmaßnahmen. Schlussfolgerung: Insgesamt zeigte sich, dass Mind-Maps einfach eingesetzt und ausgewertet werden können, um Krankheitskonzepte und assoziierte Begriffe zu einem Fachgebiet im Sinne der konstruktivistischen Lerntheorie zu explorieren, ohne vordefinierte Fragebögen zu verwenden.

Abstract

Risk for late-onset Alzheimer's disease (LOAD), the most prevalent dementia, is partially driven by genetics. To identify LOAD risk loci, we performed a large genome-wide association meta-analysis of clinically diagnosed LOAD (94,437 individuals). We confirm 20 previous LOAD risk loci and identify five new genome-wide loci (IQCK, ACE, ADAM10, ADAMTS1, and WWOX), two of which (ADAM10, ACE) were identified in a recent genome-wide association (GWAS)-by-familial-proxy of Alzheimer's or dementia. Fine-mapping of the human leukocyte antigen (HLA) region confirms the neurological and immune-mediated disease haplotype HLA-DR15 as a risk factor for LOAD. Pathway analysis implicates immunity, lipid metabolism, tau binding proteins, and amyloid precursor protein (APP) metabolism, showing that genetic variants affecting APP and Aβ processing are associated not only with early-onset autosomal dominant Alzheimer's disease but also with LOAD. Analyses of risk genes and pathways show enrichment for rare variants (P = 1.32 × 10-7), indicating that additional rare variants remain to be identified. We also identify important genetic correlations between LOAD and traits such as family history of dementia and education.

Abstract

Disorders of the brain can exhibit considerable epidemiological comorbidity and often share symptoms, provoking debate about their etiologic overlap. We quantified the genetic sharing of 25 brain disorders from genome-wide association studies of 265,218 patients and 784,643 control participants and assessed their relationship to 17 phenotypes from 1,191,588 individuals. Psychiatric disorders share common variant risk, whereas neurological disorders appear more distinct from one another and from the psychiatric disorders. We also identified significant sharing between disorders and a number of brain phenotypes, including cognitive measures. Further, we conducted simulations to explore how statistical power, diagnostic misclassification, and phenotypic heterogeneity affect genetic correlations. These results highlight the importance of common genetic variation as a risk factor for brain disorders and the value of heritability-based methods in understanding their etiology.

Abstract

BackgroundAlzheimer's disease (AD) is a heterogeneous disorder.ObjectiveTo investigate whether cognitive AD subtypes are associated with different rates of disease progression.MethodsWe included 1,066 probable AD patients from the Amsterdam Dementia Cohort (n = 290), Alzheimer's Disease Neuroimaging Initiative (n = 268), Dementia Competence Network (n = 226), and University of California, San Francisco (n = 282) with available follow-up data. Patients were previously clustered into two subtypes based on their neuropsychological test results: one with most prominent memory impairment (n = 663) and one with most prominent non-memory impairment (n = 403). We examined associations between cognitive subtype and disease progression, as measured with repeated Mini-Mental State Examination (MMSE) and Clinical Dementia Rating scale sum of boxes (CDR sob), using linear mixed models. Furthermore, we investigated mortality risk associated with subtypes using Cox proportional hazard analyses.ResultsPatients were 71±9 years old; 541 (51%) were female. Abstract

ImportanceCerebral amyloid-β aggregation is an early event in Alzheimer disease (AD). Understanding the association between amyloid aggregation and cognitive manifestation in persons without dementia is important for a better understanding of the course of AD and for the design of prevention trials.ObjectiveTo investigate whether amyloid-β aggregation is associated with cognitive functioning in persons without dementia.Design, setting, and participantsThis cross-sectional study included 2908 participants with normal cognition and 4133 with mild cognitive impairment (MCI) from 53 studies in the multicenter Amyloid Biomarker Study. Normal cognition was defined as having no cognitive concerns for which medical help was sought and scores within the normal range on cognitive tests. Mild cognitive impairment was diagnosed according to published criteria. Study inclusion began in 2013 and is ongoing. Data analysis was performed in January 2017.Main outcomes and measuresGlobal cognitive performance as assessed by the Mini-Mental State Examination (MMSE) and episodic memory performance as assessed by a verbal word learning test. Amyloid aggregation was measured with positron emission tomography or cerebrospinal fluid biomarkers and dichotomized as negative (normal) or positive (abnormal) according to study-specific cutoffs. Generalized estimating equations were used to examine the association between amyloid aggregation and low cognitive scores (MMSE score ≤27 or memory z score≤-1.28) and to assess whether this association was moderated by age, sex, educational level, or apolipoprotein E genotype.ResultsAmong 2908 persons with normal cognition (mean [SD] age, 67.4 [12.8] years), amyloid positivity was associated with low memory scores after age 70 years (mean difference in amyloid positive vs negative, 4% [95% CI, 0%-7%] at 72 years and 21% [95% CI, 10%-33%] at 90 years) but was not associated with low MMSE scores (mean difference, 3% [95% CI, -1% to 6%], P = .16).
