12. Various clinical scenarios leading to development of the string sign of the internal thoracic artery after coronary bypass surgery: the role of competitive flow, a case series

Abstract

Abstract Background The left internal mammary artery (LIMA) is the choice for grafting of the left anterior descending coronary artery (LAD). One possible mechanism of the rare graft failure involve the presence of competitive flow. Method 105 patients who had undergone coronary bypass grafting between 1998 and 2000 were included in this observational study. The recatheterizations were performed 28 months after the operations. The rate of patency the LIMA grafts was determined, and the cases with graft failure were analyzed. Results The LIMA graft was patent in 99 patients (94%). Six patients (6%) exhibited diffuse involution of the graft (string sign). The string sign was always associated with competitive flow as the basis of the LIMA graft involution. In one case quantitative re-evaluation of the preoperative coronary angiography revealed merely less than 50% diameter stenosis on the LAD with a nonligated side-branch of the LIMA. At recatheterization in two patients the pressure wire measurements demonstrated only a non-significant decrease of the fractional flow reserve (0.83 and 0.89), despite the 53% and 57% diameter stenosis in the angiogram. Another patient displayeda significant regression of the LAD lesion between the pre- and postoperative coronary angiography (from 76% to 44%) as the cause of the development of the competitive flow. In one instance, a radial artery graft on the LAD during a redo bypass operation resulted in competitive flow in the radial graft due to the greater diameter than that of the LIMA. In a further patient, competitive flow developed from a short sequential part of the LIMA graft between the nonsignificantly stenosed diagonal branch and the LAD, with involution of the main part of the graft to the diagonal branch. Conclusions The most common cause of the development of the string sign of a LIMA graft due to competitive flow is overassessment of the lesion of the LAD. Regression of a previous lesion or some other neighboring graft can also cause the phenomenon.

18. Webbase Online Consulting System for quasi real time consultations of cardiac images.

Abstract

The authors have constructed a web base consulting program (Online Consulting System: OCS) for cardiac images especially for coronary angiograms. The DICOM movies from the PACS server of the catheterization laboratory are uploaded by secured 128 bit Virtual Private Network (VPN) connection without compression by a “send to DICOM node” command to a mini PACS hosting the OCS. After then the php modul checked the AETITLE + IP source, the communication is taking place on the TCP/IP 104 port. The uploads of the images of one patients takes 1–15 minutes depending on the bandwidth and the size of the DICOM files. After the upload, the movies can be downloaded by a real-time-like speed from any Internet connected place. The secured connection is performed by SSL protocol (https) which identifies the “receiver”. The viewer is based html5, therefore it runs in any browser program without the need for further application; however it is also possible to download the original DICOM files for further evaluations. Tablets and mobiles with the usual platforms can also be used for the consultations, the latter has the advantage in urgent cases, because sending an SMS or E-mail containing the link of the uploaded urgent coronary angiography enables the consultant invasive cardiologist to see immediately the movies on his/her smartphone or tablet or PC. The OCS can integrate the consultations of cardiac imaging modalities (echocardiography, CT, MR, IVUS, OCT) in DICOM forms. Using the OCS also for ECG trace transmission, the system is also useful for prehospital consultations during the treatment of acute coronary syndrome. [ABSTRACT FROM PUBLISHER]

Abstract

Objective: Saphenous venous grafts (SVGs) are established choices for coronary artery bypass grafting (CABG); however, their lumen patency is limited. Our goal was to investigate the risk factors of SVG degeneration., Methods: Seventy-five patients (mean age, 57.5±10.4 years) with 133 SVG conduits who had cardiac catheterization ≥1 year after CABG were selected; follow-up period was 67.6±36.8 months. Patients were divided into 3 groups according to angiographic status at follow up [intact: <20% (n=23); narrowed: 20-99% (n=24); and occluded (n=28)]. Baseline clinical conditions were evaluated in relation to follow-up angiography. As onset date of chronic total occlusions is usually uncertain, they arise typically from thrombotic lesions; thus, their value in evaluation is limited., Results: There were no significant differences between the 3 groups in clinical parameters. Linear correlation analysis found significant (p<0.01) positive connection of SVG disease (luminal diameter reduction 20-99%) with C-reactive protein (CRP) and homocysteine (Hcy), as well as between CRP and Hcy. Multiple regression analysis showed plasma Hcy level to be significantly related to graft diameter reduction normalized to time elapsed until angiography in narrowed grafts: 1 µmol/L increase of Hcy was associated with 0.053%/month decrease in lumen diameter (p<0.01; R2=0.428); extrapolating: +10 µmol/L higher Hcy level during 5 years is associated with 32.1% lumen reduction., Conclusion: Medium- to long-term SVG degeneration is related to elevated plasma total Hcy in patients with sub-occlusive graft stenosis, while in cases with intact SVGs, the beneficial local flow conditions may protect the grafts from degeneration.

Abstract

Unlabelled: There are only very few epidemiological data about homocysteine levels in patients suffering from cardiovascular (CV) disease in Hungary, however, homocysteine is a newly recognized, independent risk factor of CV diseases., Aim of the Study: Therefore, in the present study, data of 1010 East-Hungarian patients with signs of CV disease were analyzed retrospectively for correlation between the level of homocysteine and CV diseases, laboratory parameters, as well as genetic differences., Patients and Methods: From the studied patient population a control ("healthy") group has been selected according to the following criteria: lack of previous stroke or stenosis of the carotid arteries or the lower extremities, lack of coronary artery stenosis more than 50%, no previous coronary intervention or an angiography diagnosed progression of the coronary atherosclerosis., Results: The level of homocysteine showed statistically significant negative linear correlation with HDL-cholesterol and the anti-atherogenic ApoAI, and showed a positive correlation with CRP and FXIII activities in the entire patient population. When compared to the control group, homocysteine level was significantly higher in patients with previous stroke or acute myocardial infarction, coronary stenosis, progressive coronary disease, physical inactivity, MTHFR gene polymorphism, low folate or vitamin B12 level in both men and women. In patients with type II diabetes the level of homocysteine was significantly higher only in women., Conclusions: It can be concluded that the level of homocysteine in patients suffering from various CV diseases is high in Hungary. This may have a prognostic value, and shows that reduction of homocysteine level in these patients may be beneficial.

Abstract

Stents represent a thrombogenic surface where one of the earliest events is the deposition of platelets. Activated platelets express neoepitopes that enable them to form homo- and heterotypic aggregates and activate the coagulation system. In this study, we investigated the effects of ad hoc clopidogrel treatment compared to a clopidogrel pretreated group on platelet P-selectin expression. We found that after stent implantation in the ad hoc treated group, P-selectin expression decreased after 1 h, while the initial decrease in platelet count normalized after 4 h. Aspirin non-responsiveness did not influence the results. These data point to the beneficial effect of clopidogrel treatment in reducing platelet reactivity during stenting.
