In Cote d'Ivoire, sickle cell disease affects 14% of the population. It is responsible for significant morbidity and mortality. Transfusion is a significant element in the management of major sickle cell anemia, which exposes them to post-transfusion hemochromatosis. The biological diagnosis is based on the determination of serum iron and the transferrin saturation coefficient (CST). As the determination of the CST was not available in our exercise context in Cote d'Ivoire, we determined only the ferritinemia. The interest of this work lies in the therapeutic implication linked to the identification of patients at risk of hemochromatosis because chelators are difficult to access for most patients. This was a prospective, descriptive and analytical study, on polytransfused sickle cell patients, followed at the transfusion therapy unit (UTT) of the CNTS of Abidjan, from 2010 to 2018. We included 78 sickle cell patients, all ages and genders who have received at least ten transfusions. The ferritinemia assay was carried out by ELISA. Transfusion exchange, with 59% of cases, was the most used mode of transfusion. The mean ferritinemia was 1719.19 ng / ml. Hyperferritinemia was found in 63% of patients. Most of the patients were on a long-term transfusion program with an average of 27.5 bags of red blood cell concentrates. Thirty-two patients had received at least 20 bags of red blood cell concentrates. We noted 21 patients treated, including 3 with deferoxamine and 18 treated with oral deferasirox. We have identified 33 sickle cell anemia patients at risk for hemochromatosis. The determinants of the risk of hemochromatosis were the high number of blood bags and the method of transfusion.

The objective of this study is to review the activities of free delivery of blood bags to the regular blood donors at the National Blood Transfusion Center of Abidjan Treichville. Il was a descriptive cross-sectional study that took place at the National Blood Transfusion Center in Abidjan from 1 January to 31 March 2015. Were Included in the study all regular blood donors who presented themselves for free blood bags at the blood distribution service in Treichville during the defined period. 74 regular blood donors were included. The data has been saved and processed by Epi info 6.04Fr. The age group of 32-41 years predominated with 40.55% followed by that of 22-31 years with 27.03%. For the beneficiaries, to equal value, the free of blood bags for mother and child predominated with 22.97% followed by the husband with 21.62% of cases of free In 55.40% of cases, the justification for the transfusion was severe anemia. At the end of our study, we can retain for our study population, that they are young blood donors having generally benefited from a single case of free. Regarding the right-holders for free to regular blood donors, we noted a predominance with equal values for the mother and for the child. It would be interesting for the whole population to feel concerned about the donation of blood because each of us is a potential transfused patient.

To conduct a study project on the inactivation of pathogens in whole blood using acridine derivatives in Africa, in order to prevent the transmission of pathogens, it is important to look for possible immunization to acridine in the blood donor population. It is therefore important to undertake an estimate of the prevalence in the sub-Saharan population in order to guide the design of the clinical study that will use the INTERCEPT Red Blood System procedure on whole blood for transfusion. to define the starting point in the evaluation of the immunological safety and transfusion safety of the product treated by this process. The objective of this study is to determine the prevalence of AAA among blood donors in sub-Saharan Africa. We conducted a multicenter prospective descriptive study of 902 blood donors collected in Cote d'Ivoire, Benin and Cameroon over the period from June 2015 to January 2017. Blood samples were collected from voluntary blood donors, of any sex, aged between 18 and 65, having given their consent for the study and having participated in the medical consultation for the donation of blood. The samples were analyzed according to the technique of the RAI gel card of the company BIORAD after centrifugation and incubation using test red cells treated with S-303 and glutathione. In the case of positive RAI results, to confirm the presence of anti-acridine, the donor plasma should be incubated with S-300. S-300 is a degradation product of S-303. The donor serum and S-300 are then incubated with the same red test cells. S-300 binds to the antibody and produces a negative result in the gel map in the presence of anti-acridin antibodies. Of the 903 samples tested both at the Abidjan laboratory in Cote D'Ivoire and at the Frankfurt laboratory, we found 1 positive sample and 8 reactive samples (positive for anti-erythrocyte antibodies). Positive donor plasma was incubated with S-300 which is the degradation product of S-303. The result is always positive, whereas according to the instructions of the reference laboratory of Frankfurt, it should be negative in case of presence of Locustacan. The results on AAA testing among 903 donors in three sub-Saharan countries show the absence of AAA in the sample of subjects included in the study according to the hypothesis emitted from this study. This opens the door to the prospect of conducting a clinical study on the inactivation of pathogens by acridine derivatives.

The objective of this study is to establish the mapping of blood group and phenotype Rhesus and Kell and compare it to the population of blood donors. Methodology: It is a prospective study performed in transfusion therapy unit (UTT) of the national blood transfusion center (NBTC) of Abidjan on sickle cell desease patients multitransfused between February and October 2013. The blood group ABO and phenotype Rhesus and Kell of the patients was performed by the gel agglutination technique. The statistical analysis was performed using SPSS 15.0 software. Results: Among the 145 patients followed by the UTT, males predominated with (sex ratio 1.27). The median age was 14 years. Homozygous sickle cell disease (SS) was more frequent (57.25%). The blood group O was more represented (54.6%). Ag D was found in 96%. The Ag C (16.5%) and Ag E (14.5%) have lower frequencies than those founded in the population of blood donors (Ag C 21.6% and Ag E 22.2%). The most phenotype rhesus and kell frequently encountered to the patients was C- c+ E- e+ K- (49.7%). These frequencies are lower than those of the blood donors population. Conclusion: Blood group O is the most common to our patients. The Ag D is the most found. The phenotype Rhesus and kell C- c + D + E- e- K- predominates with but remains below the frequencies reported among blood donors. This proves that there are risks of alloimmunization to these patients; hence the necessity to transfuse according to the protocol phenotyped and compatibility.

Sickle cell disease is a condition with 3 parts of speech including infectious complications. They are essentially encapsulated bacterial organisms. There is a significant incidence of urinary infections to adults with sickle cell disease most particularly among women. The frequency and characteristics of pyelonephritis in women with sickle cell disease is poorly understood to this day, it seemed interesting to analyze over a period of one year, the profile of sickle cell patients followed in the genetic disease unit of red blood cell Hospital Henri Mondor, Creteil, and who took at least one episode of pyelonephritis. Patients and Methods: This study was conducted in the genetic disease unit of red blood cell (UMGGR) Hospital Henri Mondor in Creteil (France). This is a retrospective descriptive and analytical study on patients with homozygous sickle cell disease, females, aged 18 years or more. The selection was conducted over a period of one year from February 2007 to February 2008. Data were collected from medical records and from reports of record of each patient. We considered the clinical, paraclinical and therapeutic latest episode of pyelonephritis. Data processing was done using the software Statview. Results: The prevalence of pyelonephritis in women with sickle cell disease aged over 15 years was 6.12% annually. The median age at first episode of pyelonephritis was 23.5 years. The majority (71.42%) was the first episode after 20 years. Among the factors predisposing to pyelonephritis, pregnancy has been implicated in 16.7% of cases. The symptoms of cystitis, with 52.4%, were the fact favoring predominant. The average number episodes of pyelonephritis to our patients were 2. The risk of recurrent pyelonephritis was significantly correlated statistically with the presence of cholelithiasis (p = 0.04). Fever was the clinical symptoms that prompted the consultation in 95.24% cases. The consequences of sickle cell disease were important. We identified 16.7% cases of sepsis and 16.7% of worsening anemia requiring transfusion. The causative agent was E. coli in 85.7% of patients. 61.9% of patients received combination therapy. The intravenous therapy as first-line was found in 81%. Fluoroquinolones were the molecules widely used in 71.4% of cases. The rate of clinical failure at 3 days of antibiotics was 4.76%. The average time for obtaining apyrexia was 3 days. The average duration of treatment was 15 days. We observed no cases of death secondary to the episode of pyelonephritis. Conclusion: This preliminary work has highlighted the incidence of pyelonephritis in women adult with sickle cell and the the potential impact on sickle cell disease. Despite being underestimated because of the retrospective nature and short duration (1 year) of study, this frequency must motivate strategy to encourage more active educational preventive towards patients and early diagnosis of urinary tract infections to women with sickle cell disease.
