Abstract

Coevolution of the human host and its associated microbiota has led to sophisticated interactions to maintain a delicate homeostasis. Emerging evidence suggests that in addition to small molecules, peptides, and proteins, small regulatory noncoding RNAs (sRNAs) might play an important role in cross-domain interactions. In this study, we revealed the presence of diverse host transfer RNA-derived small RNAs (tsRNAs) among human salivary sRNAs. We selected 2 tsRNAs (tsRNA-000794 and tsRNA-020498) for further study based on their high sequence similarity to specific tRNAs from a group of Gram-negative oral bacteria, including Fusobacterium nucleatum, a key oral commensal and opportunistic pathogen. We showed that the presence of F. nucleatum triggers exosome-mediated release of tsRNA-000794 and tsRNA-020498 by human normal oral keratinocyte cells. Furthermore, both tsRNA candidates exerted a growth inhibition effect on F. nucleatum, likely through interference with bacterial protein biosynthesis, but did not affect the growth of Streptococcus mitis, a health-associated oral Gram-positive bacterium whose genome does not carry sequences bearing high similarity to either tsRNA. Our data provide the first line of evidence for the modulatory role of host-derived tsRNAs in the microbial-host interaction.

Abstract

Neonates born very preterm (before 32 weeks’ gestational age), are a significant public health concern because of their high-risk of mortality and life-long disability. In addition, caring for very preterm neonates can be expensive, both during their initial hospitalization and their long-term cost of permanent impairments. To address these issues, national and regional neonatal networks around the world collect and analyse data from their constituents to identify trends in outcomes, and conduct benchmarking, audit and research. Improving neonatal outcomes and reducing health care costs is a global problem that can be addressed using collaborative approaches to assess practice variation between countries, conduct research and implement evidence-based practices. The International Network for Evaluating Outcomes (iNeo) of neonates was established in 2013 with the goal of improving outcomes for very preterm neonates through international collaboration and comparisons. To date, 10 national or regional population-based neonatal networks/datasets participate in iNeo collaboration. The initiative now includes data on >200,000 very preterm neonates and has conducted important epidemiological studies evaluating outcomes, variations and trends. The collaboration has also surveyed >320 neonatal units worldwide to learn about variations in practices, healthcare service delivery, and physical, environmental and manpower related factors and support services for parents. The iNeo collaboration serves as a strong international platform for Neonatal-Perinatal health services research that facilitates international data sharing, capacity building, and global efforts to improve very preterm neonate care.

Abstract

Analysis of the effect of the interaction of substance P (SP) and somatostatin (SOM) in cultured locus coeruleus neurons shows that SOM introduction increases inward rectifier K+ conductance in neurons loaded with guanosine 5'-(gamma-thio) triphosphate (GTP(gammaS)) and a following introduction of SP restricts this conductance. GTP(gammaS)-loaded cells initially exposed to SP remain limitedly affected by subsequent SOM application. SP introduction in GTP-loaded neurons transiently restricts the increase in conductance inducted by SOM.

41. Effects of implant diameter, insertion depth, and loading angle on stress/strain fields in implant/jawbone systems: finite element analysis.

Abstract

Purpose: To investigate the interactions of implant diameter, insertion depth, and loading angle on stress/strain fields in a three-dimensional finite element implant/jawbone system and to determine the influence of the loading angle on stress/strain fields while varying the implant diameter and insertion depth. Materials and Methods: Four finite element models were created, which corresponded to two implant diameters and two insertion depths. The jawbone was composed of cortical and cancellous bone and modeled as a linearly elastic medium; the implant had a detailed screw structure and was modeled as an elastic-plastic medium. Static loading was applied to the coronal surface of the implant with a maximum load of 200 N for all the models. Loading directions were varied, with buccolingually applied loading angles ranging from 0 to 85 degrees. Results: Increases in the angle of force application caused not only increased maximum stress/strain values but worsened stress/strain distribution patterns in the bone and implant. The maximum stress in the bone always occurred at the upper edge of the cortical bone on the lingual side adjacent to the implant. The use of a larger-diameter implant or an increased insertion depth significantly reduced the maximum stress/strain values, improved the stress/strain distribution patterns and, in particular, decreased the stress/strain sensitivity to loading angle. Conclusions: A narrow-diameter implant, when inserted into jawbone with a shallow insertion depth and loaded with an oblique loading angle, is most unfavorable for stress distribution in both bone and implant. An optimized design of the neck region of an implant, in combination with a carefully controlled implant insertion depth that sets the threads of the implant neck well below the upper edge of the cortical bone, should be especially effective in improving the biomechanical environment for the maintenance of bone in implant/bone systems. 42. Subjective assessment of chewing function of obturator prosthesis wearers.

Abstract

PURPOSE: Assessments of masticatory performance and occlusal force for wearers of obturator prostheses were performed as clinical objective assessments and reported in an earlier study. The purpose of the present study was to evaluate clinically the chewing function of obturator prosthesis wearers by self-evaluations and to examine their relationship to the objective assessments. MATERIALS AND METHODS: Twenty patients with maxillofacial obturator prostheses who were having a periodic checkup at the maxillofacial rehabilitation clinic in Kyushu University Hospital were recruited for this study. Chewing function was evaluated by 3 assessment tools: a self-assessment mastication scale, a chewing function score, and a mastication score. In addition, correlations among these assessments and objective tests--ie, masticatory performance and maximum occlusal force--were analyzed by the Spearman rank correlation coefficient. RESULTS: The mean self-assessment mastication scale was 63.2 (SD 31.8), the chewing function score was 54.0 (SD 30.2), and the mastication score was 51.4 (SD 33.3). There was no statistically significant relationship between the self-assessment mastication scale and each objective test. However, there were significant correlations between each semisubjective score--the chewing function score and the mastication score--and masticatory performance. There was no relationship between each semisubjective score and maximum occlusal force. CONCLUSION: A self-assessment mastication scale was not always in agreement with objective assessments, and assessments made by patients should be taken into consideration when arranging maxillofacial rehabilitation. Conversely, both chewing function and mastication scores corresponded with masticatory performance, and these would be useful as screening tests before performing objective tests. Abstract

PURPOSE: Eating, which includes chewing and swallowing, is an oral function that influences quality of life. Though the swallowing ability of maxillectomy patients was reported in our previous study, the chewing function has not been fully reported to date. Thus, the purpose of this study was to evaluate the chewing function of obturator prosthesis wearers by measurement of masticatory performance and occlusal force. The relationship of these 2 measurements was also investigated. MATERIALS AND METHODS: Twenty maxillofacial obturator prosthesis wearers undergoing periodic checkup at the maxillofacial rehabilitation clinic in Kyushu University Hospital were recruited for this study. Additionally, 20 young, healthy individuals were recruited as controls. Data on masticatory performance, which was measured by a sieve method using hydrocolloid material, and maximum occlusal force, which was measured by the Dental Prescale System (Fuji Film), were obtained for each participant. RESULTS: The mean of masticatory performance was 2.6 (SD 1.2) on a 1.40-mm mesh. There was no significant difference in masticatory performance between the patient group and the controls. The mean maximum occlusal force of the patient group was 625.9 N (SD 299.1 N), which was significantly lower than that of the control group. There was no significant correlation between masticatory performance and maximum occlusal force for the patient group in this study (P = .3726). CONCLUSION: Masticatory performance of obturator prosthesis wearers with dentate or partially edentulous maxillae was not different from that of young, healthy individuals, though maximum occlusal force of these patients was lower than that of controls. 44. Objective clinical assessment of change in swallowing ability of maxillectomy patients when wearing obturator prostheses.

Abstract

PURPOSE: Evaluation of treatment outcome is important in maxillofacial rehabilitation. Although eating is one of the oral functions that most strongly influences patients' quality of life, only a few reports exist on the objective assessment of swallowing for maxillectomy patients. The purpose of this study was to identify changes in the swallowing ability of maxillectomy patients when wearing obturator prostheses through the use of an objective clinical assessment. MATERIALS AND METHODS: The swallowing ability of 38 postmaxillectomy patients consecutively treated with obturator prostheses was objectively evaluated with the 'water-drinking test' that was developed for the assessment of dysphagia patients after cerebrovascular disease. In this test, the subjects were instructed to drink 30 mL of water in one swallow. The profile was evaluated with the combination of the time required for drinking the water and the incidence of cough reflex. Statistical analysis was performed using the Wilcoxon signed-rank test, the paired t test, and the Chi-square test with StatView 5.0 for the Macintosh. RESULTS: Performance improved significantly when the patients wore prostheses (P = .0026, Wilcoxon signed-rank test). The mean drinking times without and with prostheses were 8.2 +/- 6.3 s and 5.0 +/- 3.5 s, respectively. Drinking time was shortened significantly when the prosthesis was worn (P = .0002, paired t test). The assessment of behavior and episodes revealed that the swallowing ability of the maxillectomy patients was significantly improved when a prosthesis was worn (P = .0002, Chi-square test).
